OBJECTIVE To compare the efficacy and safety of evolocumab and probucol in patients with ultra-high-risk atherosclerotic cardiovascular disease (ASCVD) following percutaneous coronary intervention (PCI). METHODS A retrospective analysis was conducted on 156 ultra-high-risk ASCVD patients who underwent PCI in our institution between January 1, 2023 and December 31, 2024. According to the lipid-lowering regimen, the patients were categorized into evolocumab group ( n =86) and probucol group ( n =70). Changes in lipid parameters [total cholesterol (TC), low-density lipoprot ein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), triglycerides, lipoprotein (a), and lipid goal achievement rate ] , inflammatory markers [interleukin-6 (IL-6) and C-reactive protein (CRP) ] , and cardiac function indices (left ventricular ejection fraction, left ventricular end-systolic diameter, left ventricular end-diastolic diameter, and N-terminal pro-B-type natriuretic peptide) were compared between two groups at baseline and after 6 months of treatment. The incidence of adverse clinical events during treatment, including acute myocardial infarction, in-stent restenosis, acute heart failure, cerebral hemorrhage, and stroke, was also evaluated. RESULTS No statistically significant differences were observed between the two groups at baseline ( P >0.05). After 6 months of treatment, both groups demonstrated significant improvements in lipid profiles (except HDL-C) and inflammatory markers compared to those at baseline ( P <0.05). The evolocumab group exhibited greater reductions in TC, LDL-C, IL-6, and CRP, along with a higher lipid target achievement rate, compared with the probucol group ( P <0.05). There were no statistically significant differences in the cardiac function-related indicators before and after treatment between the two groups, nor in the incidence of adverse events during the treatment ( P >0.05). CONCLUSIONS For ultra-high-risk ASCVD patients after PCI, both of the above treatment options are associated with improvements in blood lipid and inflammatory response, with good safety during short-term follow-up. Evolocumab shows superior efficacy in TC, LDL-C and inflammatory markers reduction and lipid target achievement, compared to probucol.

This article adopts Professor CHEN Chaozu′s " sanjiao composed by membrane-striae" theory as its foundation to explore the relationship between irritable bowel syndrome and functional/structural abnormalities of the membrane-striae. Sanjiao encompasses both the tangible membrane and the intangible striae. These striae permeate the entire body, and their pathological changes comprehensively reflect qi,body fluids, and fasciae. Based on the physiological function of the membrane-striae in regulating qi and fluids, the pathogenesis of irritable bowel syndrome is characterized by a disharmony of membrane-striae and an imbalance of the qi-fluid interactions. In the early stage, external pathogens, emotional factors,or dietary stimuli often cause membrane-striae constriction and disordered qi-fluid circulation. In the middle stage, stagnant fluids gradually transform into phlegm retention, leading to membrane-striae obstruction. In the late stage, deficiency of vital qi becomes predominant, manifesting as laxity of membrane-striae with impaired control or weakened conduction. The treatment of irritable bowel syndrome should adopt " unblocking" as the guiding principle. In the early stage, therapy should focus on eliminating pathogenic factors and soothing membrane-striae to promptly restore qi-fluid circulation, thereby attaining unblocking through spasm relief. In the middle stage, treatment should focus on resolving tangible obstructions in membrane-striae,achieving unblocking via dredging. In the late stage, the emphasis should shift to reinforcing healthy qi,particularly by strengthening spleen-kidney yang qi,and achieving unblocking through supplementation. Concurrently, throughout the entire treatment process, the regulation of mental state and easing of emotional tension should be integrated to alleviate patient′s anxiety, achieving the goal of holistic treatment of both body and mind.

ObjectiveTo study on the different therapeutic effects and potential mechanisms of Rhei Radix et Rhizoma(RH) before and after steaming with wine on constipation model mice. MethodsFifty-four male ICR mice were randomly divided into control group, model group, lactulose group(1.5 mg·kg-1), high, medium and low dose groups of RH and RH steaming with wine(PRH)(8, 4, 1 g·kg-1). Except for the control group, the constipation model was replicated by gavage of loperamide hydrochloride(6 mg·kg-1) in the other groups. After 2 weeks of modeling, each administration group was gavaged with the corresponding dose of drug solution, and the control and model groups were given an equal volume of normal saline, 1 time/d for 2 consecutive weeks. After administration, the feces were collected for 16S rRNA sequencing, the levels of gastrin(GAS), motilin(MTL), interleukin-6(IL-6), γ-interferon(IFN-γ) in the colonic tissue were detected by enzyme-linked immunosorbent assay(ELISA), the histopathological changes of colon were observed by hematoxylin-eosin(HE) staining, flow cytometry was used to detect the proportion changes of CD4+, CD8+ and regulatory T cell(Treg) in peripheral blood. ResultsCompared with the control group, the model group showed significantly decrease in fecal number in 24 h, fecal quality and fecal water rate(P<0.01), the colon was seen to have necrotic shedding of mucosal epithelium, localized intestinal glands in the lamina propria were degenerated, necrotic and atrophied, a few lymphocytes were seen to infiltrate in the necrotic area in a scattered manner, the contents of GAS and MTL, the proportions of CD4+, CD8+ and Treg were significantly reduced(P<0.01), the contents of IL-6 and IFN-γ were significantly elevated(P<0.05, P<0.01). Compared with the model group, the fecal number in 24 h, fecal quality and fecal water rate of high-dose groups of RH and PRH were significantly increased(P<0.05, P<0.01), the pathological damage of the colon was alleviated to varying degrees, the contents of GAS, MTL, IL-6 and IFN-γ were significantly regressed(P<0.05, P<0.01), and the proportions of CD4+ and CD8+ were significantly increased(P<0.01), although the proportion of Treg showed an upward trend, there was no significant difference. In addition, the results of intestinal flora showed that the number of amplicon sequence variant(ASV) and Alpha diversity were decreased in the model group compared with the control group, and there was a significant difference in Beta diversity, with a decrease in the relative abundance of Lactobacillus and an increase in the relative abundances of Bacillus and Helicobacter. Compared with the model group, the ASV number and Alpha diversity were increased in the high-dose groups of RH and PRH, and there was a trend of regression of Beta diversity to the control group, the relative abundance of Lactobacillus increased, and the relative abundances of Bacillus and Helicobacter decreased. ConclusionRH and PRH can improve dysbacteriosis, promote immune system activation, inhibit the release of inflammatory factors for enhancing the gastrointestinal function, which may be one of the potential mechanisms of their therapeutic effect on constipation.

Objective To investigate the effects of glutathione peroxidase 7(GPX7)on the proliferation and senescence of hepatocellular carcinoma(HCC)cells and its potential value as a therapeutic target. Methods Clinical and transcriptomic data from HCC patients in The Cancer Genome Atlas(TCGA)database were analyzed to evaluate the relationship between GPX7 expression and prognosis.Western blotting,qRT-PCR,CCK-8 assay,EdU staining, and SA-β-Gal staining were employed to compare the expression and function of GPX7 in HCC cell lines and normal liver cells.A stable GPX7-knockdown Hep3B cell line was established, and transcriptomic changes were analyzed using RNA-seq.The PI3K inhibitor LY294002 and its activator 740 Y-P were utilized to determine the role of GPX7 in regulating cell senescence via the PI3K-AKT pathway. An orthotopic xenograft model was constructed using 18 male nude mice(6 weeks old,18~22 g)to observe tumor growth.Tumor and adjacent tissue samples from 56 HCC patients(obtained from the Eighth Medical Center of Chinese PLA General Hospital between January 2012 and December 2022)were analyzed to assess the relationship between GPX7 expression and clinicopathological features. Results TCGA data analysis revealed that GPX7 was significantly highly expressed in HCC tumor tissues(P<0.01),and the patients with high GPX7 expression had obviously shorter overall survival(OS)(P=0.018).Western blotting and qRT-PCR showed that the expression level of GPX7 was notably higher in the HCC cell lines than the normal liver cells(P<0.01).After GPX7 knockdown, the CCK-8 absorbance values of HCC cells were significantly reduced(P<0.01),the proportion of EdU-positive cells was obviously decreased(P<0.01),and the proportion of SA-β-Gal positive cells was notably increased(P<0.01).RNA-seq analysis revealed that differentially expressed genes were significantly enriched in the PI3K-AKT signaling pathway(P<0.05).The PI3K inhibitor LY294002 significantly enhanced the GPX7 knockdown-induced cell senescence(P<0.01),while the activator 740 Y-P partially reversed this effect(P<0.05).In the xenograft model,GPX7 knockdown significantly inhibited tumor growth in nude mice(P<0.01),and the PI3K activator 740 Y-P partially reversed this effect(P<0.01).Clinical sample analysis displayed that high GPX7 expression was negatively correlated with tumor differentiation(P<0.05),and its high expression was associated with shortened OS(HR=2.61,95%CI:1.26~5.39,P=0.005).Conclusion GPX7 regulates cell proliferation and senescence through the PI3K-AKT pathway, and may serve as a potential therapeutic target for HCC treatment.
