1.Effects of β_2 agonist salbutamol aerosol on the uptake of sevoflurane in elderly patients with chronic obstructive pulmonary disease

Objective To investigate the effects of β_2 agonist salbutamol aerosol on the uptake of sevoflurane in elderly patients with chronic obstructive pulmonary disease (COPD). Methods A randomized, placebo-controlled and double-blinded trial was designed. Twenty-six patients were recruited and randomly allocated to salbutamol group (group E, n=13) and placebo group (group C, n=13). Eligible patients were elderly patients with ASA physical status Ⅱ-Ⅲ, a body mass index (BMI) between 18 and 30 kg/m~2, well-defined clinical diagnosis of COPD. Routine monitoring (consists of a three-lead ECG, pulse oximetry, noninvasive blood pressure and expired gas analysis) was instituted and 500 mL Ringer's lactate solution was administered. Bispectral index (BIS) monitoring was initiated prior to induction. All subjects were received inhaled aerosol 200 μg according to manufacturers' recommendations 30 minutes before induction of anesthesia. Controlled ventilation was applied after the trachea was intubated. When stable hemodynamics was maintained for 5 minutes, fresh gas flow was set to 2 L/min with 2% sevoflurane in admixture, then HR, invasive arterial blood pressure (IABP), SpO_2, P_(ET)CO_2, bispectral index (BIS), minimum alveolar concentration (MAC), concentrations of inhaled sevoflurane (F_I) and end-tidal (F_E) were recorded at 1, 2, 3, 4, 5, 7,10, and 15 minutes after inhalation of sevoflurne. The P_(peak) and P_(plat) were also measured in 1, 5, and 10 minutes after the successful endotracheal intubation. Results Compared to the placebo group, the F_E was significantly higher at 2, 3, 4, and 5 minutes in experiment group. Peak airway pressure and plateau pressure in experiment group were strikingly lower than control group (P<0.05). There was no statistical significance about BIS variations between these groups in respective time in spite of increased MAC (P>0.05). Conclusions The increasing rate of alveolar concentration of sevoflurane was accelerated after the administration of inhaled salbutamol aerosol (200 μg) 30 minutes before induction of anesthesia, it might increase the uptake of sevoflurane.
2.Mn(Ⅱ) Oxidation and Removal by a Manganese-oxidizing Bacterium Bacillus sp. MK3-1

Manganese-oxidizing microorganisms are able to oxidize soluble Mn(II) into insoluble Mn oxides. Such microorganisms are very useful in treatment of Mn-contaminated water. In this research, a Mn(II)- oxidizing bacterium Bacillus sp. MK3-1 was isolated from Mn-contaminated soil. This bacterium has high MnCl2 resistance with a MIC of 20 mmol/L. The results showed that it is able to oxidize and remove more than 96% of Mn(II) in the culture medium. The immobilized solid-embedding Bacillus sp. MK3-1can re- moved 87.12% of manganese contaminated water. The final concentration of MnCl2 after the treatment reached the national discharge standard level. Scan electron microscope observation showed that the pro- duced Mn oxides located on the cell surfaces of Bacillus sp. MK3-1. Energy dispersive spectrdmeter analy- sis indicated that the content of manganese of cell surfaces of Bacillus sp. MK3-1 was 19.60% (W/W). At last we amplified a 903 bp multicopper oxidase gene mnxG encoding the putative Mn(II)-oxidizing protein.The product of mnxG showed 86% identity to the reported multicopper oxidase.
3.Improvement Effect of Smart Drug Shelves on Service Process in Outpatient Pharmacy of West Hospital of Our Hospital

Objective:To analyze the improvement effect of smart drugs shelves on the service process in the outpatient pharmacy and to provide supporting data for the new service process. Methods:The dispensing time of pharmacists and the waiting time of pa-tients were studied and compared before and after the use of smart drugs shelves. Results: After the use of smart drugs shelves, 71. 52% of dispensing time was in 5min, and more than 95% was in 10min. In the 9 periods, the medians of patient waiting time on Oct 21, 2013 were all shorter than those on Oct 18, 2010. Conclusion:The use of smart drugs shelves can improve service efficiency, shorten patient waiting time and enhance service level of outpatient pharmacy.
4.Clinical Observation of Breviscapine Combined with Double Shock in the Treatment of Lupus Encephalopa-thy

OBJECTIVE:To observe the efficacy and safety of breviscapine combined with double shock in the treatment of lu-pus encephalopathy. METHODS:70 patients with lupus encephalopathy were randomly divided into control group and observation group. Control group was treated with Methylprednisolone sodium succinate for injection 500 mg for continuous 3 d,iv,once a day,Cyclophosphamide for injection 1.0 g in the 4th day,iv,and Methylprednisolone sodium succinate for injection 80 mg was used for maintaining treatment in the 5th day for continuous 15 d,once a day. Observation group was treated with Breviscapine for injection 50 mg for continuous 20 d based on the treatment of control group,iv,once a day. The SLE activity(SLEDAI) score, IL-1β,IL-6,IL-8 and IFN-γ were observed,cases of neurological and psychological symptoms,the fatality rate and incidence of adverse reactions were recorded. RESULTS:After treatment,the SLEDAI,IL-1β,IL-6,IL-8 and IFN-γ in 2 groups were signifi-cantly lower than before,and observation group was lower than control group(P<0.05);the cases of neurological symptoms(ex-cept for aseptic meningitis) and psychological symptoms (except for auditory hallucinations) were significantly less than control group(P<0.05). There was no significant difference in the mortality rate and incidence of adverse reactions(P>0.05). CONCLU-SIONS:Breviscapine combined with double shock has good efficacy and safety in the treatment of lupus encephalopathy,it can re-duce the SLEDAI score,improve neurological and psychological symptoms.

6.Preliminary study on induction of mouse embryonic stem cells into neural stem cells in vitro

AIM:To explore the feasibility of inducing mouse embryonic stem cells into neural stem cells in vitro. METHODS: Embryonic body induced by retinoic acid and retinal m?ller cells were selected in neural stem cell-defined medium for 7 days, and the morphological changes were observed. The selected cells were stained immunocytochemically with anti-nestin, anti-BrdU antibodies, and their ability of expansion and differentiation were analyzed. RESULTS:Large amounts of neurospheres were derived from embryonic body in the selected medium on the 7th day, which could be passaged and differentiated, stained positive with nestin and BrdU, and expressed nestin, glutaminase and Brn-3 genes. CONCLUSION:Neural stem cells could be derived from embryonic body induced by RA and m?ller cells in the selected medium, which would offer an alternative to treat neuropathy such as glaucoma and retinal degeneration in the future.
7.Cost-effectiveness Analysis of Xueshuantong for Injection versus Ginkgo Leaf Extract and Dipyridamole In-jection in the Treatment of Ischemic Stroke

OBJECTIVE:To compare pharmacoeconomic and effect of Xueshuantong for injection and Ginkgo leaf extract and dipyridamole injection in the treatment of ischemic stroke. METHODS:Retrospective study was conducted. Totally 404 inpatients with ischemic stroke were divided into Xueshuantong group(271 cases)and ginkgo leaf extract and dipyridamole group(133 cas-es) according to clinical treatment programs. Based on the conventional treatment,patients in 2 groups were given Xueshuantong for injection and ginkgo leaf extract and dipyridamole injection,respectively. The average treatment course was 10 d. Cost-minimi-zation analysis was performed with the determination index of total effective rate. RESULTS:The total effective rates in Xueshuan-tong group and ginkgo leaf extract and dipyridamole group were 90.77% and 88.72%,respectively,the difference was not statisti-cally significant(P>0.05). The costs in 2 groups were 12 860.21 yuan and 13 155.40 yuan,respectively,and xueshuantong group had lower than ginkgo leaf extract and dipyridamde group. CONCLUSIONS:Both Xueshuantong for injection and Ginkgo leaf ex-tract and dipyridamole injection are effective in the treatment of ischemic stroke. However,the economy of Xueshuantong for injec-tion is superior to the other one.
8.The anti-oxidative effect of deferiprone on ocular lens in diabetic rat

Background Oxidative stress is thought to be responsible to diabetes-complicated cataract.Our previous study demonstrated that as an iron chelator,deferiprone can protect lens from oxidative damage.Objective This further study aimed to investigate the role of deferiprone on the formation of diabetic-complicated cataract.Methods Forty 6-week-old Wistar rats were included in the study and randomized into 4 groups.Eight of them were used as the normal control group.Diabetes mellitus animal models were established in 22 rats by the carbonhydratediet and fat diet and the intraperitoneal injection of 40 mg/kg streptozocin (STZ).The deferiprone of 50 mg and 100 mg were intragastrically given in 8 model rats respectively after 3 days once a day for 8 weeks.The opacification of lenses was examined under the slit lamp weekly after treatment.The animals were sacrificed and the lenses were obtained at the eighth week of deferiprone injection.The concentrations of water-soluble protein ( WSP),urine-soluble protein (USP) and alkali-soluble protein (ASP) in rat lens suspension were detected by Bradford method.The super oxide dimutese (SOD),malondialdehyde (MDA) and glutathione (GSH) were determined spectrometically using xanthine oxidase,thiobarbituric acid,dithio bis-nitrobenzoic acid.Results No evidently differences were found in the content of the WSP,USP and ASP among the these groups( F=1.73,0.18,0.09,P>0.05).The contents of MDA in 50 mg deferiprone group and 100 mg deferiprone group were ( 1.05 ± 0.10 ) mmol/g and ( 1.05 ± 0.22 ) mmol/g respectively,showing a significant decline in comparison with diabetic model group (P<0.05).The SOD and GSH contents in lens were (321.29±16.57) U/mg,(322.07±22.16) U/mg and (7.83±0.65 ) mg/g,(7.70±0.77 ) mg/g respectively in 50 mg deferiprone group and 100 mg deferiprone group and were considerably elevated in comparison with ( 298.70± 14.69 ) U/mg and ( 5.47 ± 1.01 ) mg/g of diabetic model groups ( P<0.05 ).No significant differences were found in the indexes mentioned above between 50 mg and 100 mg deferiprone groups(P>0.05).Conclusions Deferiprone can reduce oxidative stress and improve the energy metabolism of the lens in diabetic rats.
9.Identification of phenylpropanoids in ciwujia injection by HPLC-MS.

There are reports about the chemical compounds of Ciwujia herbs, but with no study report about the chemical material basis of Ciwujia injection (CWJI). In this study, LC-MS(n) and LC-Q-TOF-MS techniques were adopted for a qualitative analysis on phenylpropanoids in CWJI. The Ultmate XB-C18 column (4.6 mm x 250 mm, 5 microm) was adopted and eluted with the mobile phase of 0.5% formic acid-water and acetonitrile, with the flow rate at 0.8 mL x min(-1) and the column temperature at 20 degrees C. Based on the data of high-resolution and multi-stage MS, control products and literatures, altogether 54 phenylpropanoids were identified in Ciwujia Injection, including 34 phenylpropanoids, 16 ligans and 4 coumarins. Among them, 28 were reported for the first time in Ciwujia, and 14 compound structures were identified in comparison with the control products. The method established in this study could be used to simply and rapidly identify phenylpropanoids in CWJI. The findings provide scientific data for defining the chemical material basis of CWJI.

10.Clinical and experimental study on separately decocted and mingly decocted jianweishu granule.

OBJECTIVETo observe the difference in therapeutic effect of Chinese herbal granule decocted separately and mingly.
METHODSOne hundred patients of functional dyspepsia of Spleen deficiency and Liver stagnancy type were treated with Jianweishu Granule (JWSG, a self-formulated recipe by the author). Half of the patients received JWSG decocted separately, half of them received that decocted mingly. The therapeutic effects between the two groups were compared. Experimental observation on the effects of differently decocted remedies in rats was also conducted.
RESULTSThe therapeutic effects obtained in the two groups were similar, with respective cure rate of 72% and 70% and the total effective rate 96% in both group. There was insignificant difference between the two groups (P > 0.05). Experimental study all showed no difference between the differently decocted remedies in inhibiting gastric acid, pepsin activity, gastric function regulation, small intestine movement improvement and pain alleviation in rats.
CONCLUSIONThe therapeutic effects of JWSG decocted separately or mingly are the same. This fact provides a scientific basis for clinical use of granule form of single Chinese herbs.
