1.Effects of ankle-foot orthoses on muscle fatigue of the lower limbs

BACKGROUND:At present, there is no relevant systematic research about the effects of ankle-foot orthoses on lower limb muscles. OBJECTIVE:To extract the main features of the surface electromyography signals of the lower limbs from normal people with ankle-foot orthoses and to analyze the effects of the fixed ankle-foot orthoses on lower limb muscle fatigue. METHODS:Each of five male healthy young adults was respectively required to walk with and without special ankle-foot orthoses for three times. (1) Group 1, walking in a nature gait without orthoses. (2) Group 2, walking in a natural gait with ankle-foot orthoses (the ankle center was packaged over 1.0 cm by the orthoses that was recorded 1.0 cm); wearing the same orthoses but 1.0 cm in the ankle was removed and the ankle plastic edge was just through the ankle center to colect the data (recorded as 0 cm); after 1.0 cm was removed again in the ankle, walking in a natural gait (recorded as-1.0 cm). (3) Group 3, wearing 1.5 cm heel-height ankle-foot orthoses and walking in a natural gait; wearing the same orthoses but with 1.0 cm and 2.0 cm heel-height and walking in a natural gait. During walking, surface electromyography signals were colected from the biceps femoris muscle, rectus femoris muscle, tibialis anterior muscle, and gastrocnemius of the lower limbs. RESULTS AND CONCLUSION: (1) Without orthoses, the surface electromyography signals were the weakest. (2) For any muscle, the electrical activity of subjects without orthoses was weaker than that of subjects wearing orthoses at different hardness. (3) For the biceps femoris muscle, the integral electrical value without orthoses was very close to that wearing orthoses with normal heel height. Meanwhile, these two values were lower than that wearing orthoses with abnormal heel height. These findings indicate that the fixed plastic ankle-foot orthoses could cause the fatigue of the biceps femoris muscle, rectus femoris muscle, tibialis anterior muscle, and gastrocnemius; and the orthoses with abnormal heel height can further increase the fatigue of the biceps femoris muscle.
2.Analysis of Acquired Immunodeficiency Syndrome with Manifestation of Chronic Diarrhea in 7 Children

Objective To explore the clinical manifestation and laboratory examination of chronic diarrhea in children with acquired immunodeficiency syndrome(AIDS) and analyze the reasons that causes the chronic diarrhea in children.Methods Clinical data of 7 patients (male 4 cases,famale 3 cases)with AIDS with manifestation of chronic diarrhea among the 17 cases were diagnosed as AIDS in Beijing children′s hospital from Jan.1999 to Dec.2006.The time of diarrhea,degree of dinrrhea,characteristic of stool,accompaniment symptom,laboratory examination were retrospectively analyzed.Results The average onset age was 6 years old(from 2-9 years old).The average time of diarrhea was 6 months(from 1-16 months).Four cases main complained with chronic diarrhea,3 cases came to the hospital because of fever,cough and wasting;6 cases with diarrhea,7 cases with malnutrition and anemia,5 cases accompanied hypokalemia and metabolic acidosis,6 cases had delay in growth and development,2 cases with abnormal stool routine exam,1 case with positive stool culture of fungi.All patients were with lower CD4,5 cases with lower CD4/CD8.Conclusions Chronic diarrhea is a common symptom in children with AIDS,and usually accompanied by obvious delay in growth and development,malnutrition and anemia,the reasons that causes the chronic diarrhea are consi-dered to be related with both the opportunistic infection and HIV infection.

C erbB 2 is overexpressed in 20%~30% of human breast cancers,this oncoprotein plays an important role in predicting response to chemotherapy in breast cancer.Overexpression of c erbB 2 in breast cancer is associated with resistance or sensitivity to certain chemo therapeutic drugs, c erbB 2 can be expected to be a biological marker to direct chemotherapy in the treatment of breast cancer.
4.Antimicrobial Resistance Among Klebsiella .spp Isolated in 5 Consecutive Years in Our Hospital

OBJECTIVE:To investigate the variance of antimicrobial resistance of Klebsiella.spp isolated in our hospital during the recent 5 years.METHODS:Disk diffusion test was used to study the anti microbial resistance.The results were evaluated based on the criteria recommended by American Clinical and Laboratory Standards Institute.The data analysis was performed using WHONET-5software.RESULTS:Atotal of 840 strains of Klebsiella.spp were collected fromour hospital during2003~2007,of which,59.3% were fromrespiratory secretion speci men,12.0%fromblood speci men,and10.1% from secretions.The detection rate of ESBLs-producing Klebsiella.spp was from51.0% to65.1%.No I mipenem-and Meropenem-resistant Klebsiella.spp strain was detected.Susceptibility test showed that Klebsiella.spp-producing ESBLs was multiple resistant,and the anti microbial resistance of Klebsiella.spp has increased year on year.CONCLUSION:Monitoring of the drug resistance of Klebsiella.spp should be strengthened.The variation of the anti microbial resistance of Klebsiella.spp should be noted so as to direct rational drug use in the clinic and prevent spreading of drug resistant strains.
5.Role of A_3 adenosine receptor on vascular reactivity of superior mesenteric artery in hemorrhagic shock in rat

AIM:To explore whether A3 adenosine receptor plays a role in the modulation of vascular reactivity after hemorrhagic shock in rat,and to find out the prospective drug target to restore the decreased vascular reactivity following hemorrhagic shock. METHODS:The hemorrhagic shock (40 mmHg) model was established in rat,and the reactivity of superior mesenteric artery (SMA) to norepinephrine (NE) was observed. A3AR expression at protein level and mRNA level were measured by Western blotting and RT-PCR respectively. RESULTS:The vascular reactivity of SMA to NE after hemorrhagic shock (40 mmHg) was decreased significantly in a biphasic response manner. The expression of A3AR mRNA in SMA after hemorrhagic shock decreased without significant difference. The expression of A3AR protein has a slight increase without statistical difference after 30 min of hemorrhagic shock and then has a significant decrease (especially at 2 h and 4 h after hemorrhagic shock). The usage of IB-MECA,a selective A3AR agonist,significantly increased the responsiveness of SMA to NE in hemorrhagic shock in rat. MRS1523,the selective A3AR antagonist,significantly abolished the restoration of the vascular reactivity to NE by IB-MECA in hemorrhagic shock in rat. CONCLUSION:A3AR plays a role in the modulation of vascular responsiveness to NE in hemorrhagic shock in rat,and the selective agonist of A3AR could restore the reactivity of SMA to NE in hemorrhagic shock in rat.
6.Comprehensive treatment of complicated and giant carotid body tumor: a report of 1 case

Objective: To study the treatment of complicated and giant carotid body tumor. Methods: A giant carotid body tumor was dissected 2 years ago. The external carotid artery was embolized by means of catheterization before surgical intervention, and techniques of internal shunting and autogenous blood transfusion was used during the operation. Results: The tumor was resected completely without cerebral vessel disorders and major cephalic nerve injuries. Conclusion: The application of radial intervention of external carotid artery, shunting of internal carotid artery and autogenous blood transfusion are helpful to the treatment of giant and re operative carotid body tumor.
7.Study on the physiological changes of the late-onset depression during sleep

Objective To explore ihe physiological changes of the late-onset depressive(LOD) persons during sleep.Methods 15 cases of LOD patients formed a group and 10 healthy aging persons formed a control group.Hamilton Depression Scale (HAMD) was used to score the severity of depression and polysomnographic recorders were used to monitor the whole night long.The subjective feelings of the sleep and the daytime mental status were assessed in the morning.Results Compared with the controlde group, the LOD patients obviously possessed a disordered sleeping process: sleep efficiency was lower( ( 59.20 ± 2.90 ) %, ( 77.09 ± 1.55 ) %, P <0.01 ); their sleep latency was longer( (54.00 ± 4.97 ), ( 24.00 ± 2.91 ), P < 0.01 ); the numbers of rapid eye movement(REM) sleep phase and rapid eye movement latency(REML) were strikingly different(P<0.01 ).Compad with the control group, the LOD patients' awakening time and percent were significantly increased.Not only were the time and percentage of S2 sleep phase much higher, but the time of slow wave sleep was shorter.Besides,REM sleep activity, its density, and its intensity were raised (P < 0.05 ).Conclusion There is a disturbance of the objective physiological indexes happening among the LOD patients.The increase of S2 sleeping time and percentage and the disinhibition of REM sleep can be used as the diagnosis indexes of LOD patients' specificity.
8.Influence of age on hypnotic target plasma concentration of dexmedetomidine administered by TCI

ObjectiveTo investigate the influence of age on hypnotic target plasma concentration (CT) of dexmedetomidine (Dex) administered by TCI.MethodsSixty ASA Ⅰ - Ⅱ patients aged 18-85 yr undergoing lower abdominal or extremity operation under combined spinal-epidural anesthesia were divided into 2 age groups ( n =30each):18 yr<groupⅠ<55 yr (Y) and 65 yr<group Ⅱ<85 yr (E).Each group was further divided into 5subgroups (n =6 each) receiving Dex administered by TCI at CT of 0.54,0.64,0.76,0.90,1.07 ng/ml in group Y and 0.36,0.42,0.51,0.60,0.71 ng/ml in group E respectively.Hypnosis was defined as no response to verbal command (OAA/S score ≤2) and loss of eyelash reflex.A quantal response model (probit analysis) was used to calculate the concentration-effect curve and predict plasma EC50 and EC95 (95%CI) of Dex.ResultsEC50 (95%CI) was 0.478(0.424-0.536) ng/ml and EC95(95%CI) 0.641 (0.567-0.816) ng/ml in group E,significantly lower than EC50 (95 % CI) 0.738 (0.657-0.827) ng/ml and EC95 (95%CI) 0.990 (0.874-1.267 ) ng/ml in group Y.ConclusionThe EC50 and EC95 of dexmedetomidine for hypnosis were significantly lower in the elderly than in younger patients.

Schistosomiasis Control and Elimination(GB 15976-2015)and Diagnostic Criteria for Schistosomiasis(WS 261-2006)are the only two national health criteria related to schistosomiasis control program implemented in P. R. China. The roles of criteria to guide and accelerate the transition from schistosomiasis control to elimination are concluded ,based on this system?atic review how the criteria led the implementation of the medium?and long?term national plan and provided the guidance when drafting the thirteen?five years national plan for schistosomiasis,and the suggestion to draft more criteria related to schistosomia?sis elimination program and strengthening the implementation of current criteria,so as to precisely guide the schistosomiasis elimination program in P. R. China.
10.The application of matrix metalloproteinase-3 and 7 joints ultrasonic score in assessment of disease activity in patients with rheumatoid arthritis

Objective To evaluate the significance of serum matrix metalloproteinase-3 (MMP-3) and joint ultrasonography in assessing the activity of rheumatoid arthritis (RA) by comparing MMP-3 level and the ultrasonic 7 joints (US7) score in RA patients.Methods Serum MMP-3 level and US7 score were measured in 133 RA patients by immune turbidity and Doppler ultrasound.Synchronous 53 healthy subjects were recruited as controls.Clinical data were collected.Erythrocyte sedimentation rate (ESR),serum level of anti-cyclic citrullinated peptide (CCP) antibody,health assessment questionnaire (HAQ) and disease activity score 28 (DAS28) were measured.The level of disease activity is interpreted as remission(DAS28 <2.6),low(DAS 28≥2.6-<3.2),moderate(DAS 28≥3.2-<5.1),high(DAS28≥5.1).The discriminating validity of MMP-3 and US7 score in disease was evaluated using receiver operating characteristic (ROC) curve analysis with DAS28 as the reference standard.Results Compared with that in healthy controls [35.20(25.90,48.90) μg/L] and remission patients[33.40(22.60,678.40) μg/L],the MMP-3 level in moderate [105.1 (61.70,172.70) μg/L] and high [363.1 (161.50,475.90) μg/L]groups increased dramatically.US7 score in patients with high disease activity was significantly higher than that in other groups.The level of MMP-3 was significantly correlated with DAS28,HAQ,US7 score,yet did not have correlation with anti-CCP antibody.Serum level of MMP-3 was positively correlated with US7 score (r =0.566,P < 0.001).In evaluating the disease activity,US7 score combined with MMP-3 (AUC 0.863 2) was not superior to MMP-3 alone (AUC 0.854 3),but significantly better than single US7 score (AUC 0.7643,P < 0.05).Conclusions MMP-3 is an effective and simple index in evaluating RA disease activity.The combination of MMP-3 and US7 score does not further improve the efficacy to evaluate disease activity than MMP-3 alone in patients with RA.
