1.Effects of ankle-foot orthoses on muscle fatigue of the lower limbs

BACKGROUND:At present, there is no relevant systematic research about the effects of ankle-foot orthoses on lower limb muscles. OBJECTIVE:To extract the main features of the surface electromyography signals of the lower limbs from normal people with ankle-foot orthoses and to analyze the effects of the fixed ankle-foot orthoses on lower limb muscle fatigue. METHODS:Each of five male healthy young adults was respectively required to walk with and without special ankle-foot orthoses for three times. (1) Group 1, walking in a nature gait without orthoses. (2) Group 2, walking in a natural gait with ankle-foot orthoses (the ankle center was packaged over 1.0 cm by the orthoses that was recorded 1.0 cm); wearing the same orthoses but 1.0 cm in the ankle was removed and the ankle plastic edge was just through the ankle center to colect the data (recorded as 0 cm); after 1.0 cm was removed again in the ankle, walking in a natural gait (recorded as-1.0 cm). (3) Group 3, wearing 1.5 cm heel-height ankle-foot orthoses and walking in a natural gait; wearing the same orthoses but with 1.0 cm and 2.0 cm heel-height and walking in a natural gait. During walking, surface electromyography signals were colected from the biceps femoris muscle, rectus femoris muscle, tibialis anterior muscle, and gastrocnemius of the lower limbs. RESULTS AND CONCLUSION: (1) Without orthoses, the surface electromyography signals were the weakest. (2) For any muscle, the electrical activity of subjects without orthoses was weaker than that of subjects wearing orthoses at different hardness. (3) For the biceps femoris muscle, the integral electrical value without orthoses was very close to that wearing orthoses with normal heel height. Meanwhile, these two values were lower than that wearing orthoses with abnormal heel height. These findings indicate that the fixed plastic ankle-foot orthoses could cause the fatigue of the biceps femoris muscle, rectus femoris muscle, tibialis anterior muscle, and gastrocnemius; and the orthoses with abnormal heel height can further increase the fatigue of the biceps femoris muscle.
2.Significance of AKR1C3 for esophageal cancer patients under radiotherapy

Objective To evaluate the significance of the expression level of aldo-keto reductase family 1 member C3(AKR1C3) for esophageal carcinoma patients treated with radiotherapy.Methods The clinical data of 28 patients with locally advanced esophageal carcinoma treated with radiotherapy alone was retrospectively analyzed, and the expression level of AKR1C3 was detected by immunohistochemistry.Statistical analysis was performed using SAS 8 statistical software, P<0.05 was considered statistically significant difference.Results The expression level of AKR1C3 was different in esophageal carcinoma patients at different levels of differentiation.The level of AKR1C3 expression was negatively correlated with efficacy of radiotherapy (P=0.031, 95%CI 0.151-0.914).Conclusion AKR1C3 can serve as a promising biomarker of the efficacy of radiotherapy for esophageal cancer.

4.Metabonomic study on the anti-liver injury effect of Si-Ni-San on rats by using UPLC-MS/MS.

A UPLC-MS/MS method based on metabonomic skills was developed to study the serum metabolic changes of rats after acute liver injury induced by CCl4 and to evaluate the action mechanism of Si-Ni-San. The integrated data were exported for principal components analysis (PCA) by using SIMCA-P software, in order to find the potential biomarkers. It showed that clear separation of healthy control group, model group, silymarin group, Si-Ni-San group was achieved by using the PCA method. Nine significantly changed metabolites were identified as potential biomarkers of acute liver injury. Compared with the health control group, the model group rats showed higher levels of phenylalanine, tryptophan and GCDCA together with lower levels of LPC 16 : 0, LPC 18 : 0, LPC 18 : 1, LPC 16 : 1, LPC 20 : 4 and LPC 22 : 6. These changes of serum metabolites suggested that the disorders of amino acid metabolism, lipid metabolism, bile acid biosynthesis and anti-oxidative damage were related to acute liver injury induced by CCl4. Si-Ni-San might have the anti-liver injury effect on all these four metabolic pathways.
5.Detection of high-risk human papillomavirus by multiplex real time PCR

Objective To investigate 13 high-risk types of HPV (HR HPV) infection rates in women with different grades of cervical lesions.Methods A total of 350 women, who were hospitalized in the department of gynecology in Bao′an Maternity & Child health hospital, were enrolled for the study.TCT technology was used to evaluate the cervical epithelium.The group were divided according to the cytology results.Multiplex real time PCR (mRT PCR) was used to detect the viral loads.HR HPV infection rate of different groups were analyzed using χ2 test or Fisher exact test.HR HPV viral loads of patients in different grades of cervical lesion groups were compared using Kruskal-Wallis or Wilcoxon test, and the age distribution of HR HPV positive group and negative group was analyzed by using Wilcoxon test.Results The HR HPV infection rates of NILM, ASCUS, LSIL, HSIL were 3.4% (10/295), 20.0% (7/35), 78.6% (11/14) and 100.0% (6/6), respectively.HR HPV positivity in NILM was lower than ASCUS (χ2=14.43,P<0.01) and LSIL (χ2=107.69,P<0.01), HR HPV positivity in ASCUS was lower than LSIL (χ2=14.76,P<0.01). The median of HR HPV viral loads in NILM, ASCUS, LSIL and HSIL were 4.10 (3.38-6.27), 5.33 (3.63-6.66), 5.77 (4.01-7.01) and 5.58 (4.19-5.85) respectively (copies/ml,lg).Combined ASCUS, LSIL and HSIL groups into cervical lesion group, HR HPV viral load of which was higher than that of NILM (U=43.0, P<0.05).The median Ages of HR HPV positive group and negative group were 36 and 33, respectively.No statistical significance was found between them (U=4 544, P>0.05).Conclusions The present study revealed that HR HPV infection was related to cervical lesion, but there was no correlation between viral load and cervical lesion grade. In additional, no difference in age distribution was found between HR HPV positive group and negative group.
6.Clinical analysis of severe acute pancreatitis complicated with mental disorders

Objective To investigate the causes, characteristics, diagnosis and treatment of severe acute pancreatitis (SAP) complicated with mental disorders. Methods The clinical data of 86 patients with SAP complicated with mental disorders who had been admitted to Union Hospital of Tongji Medical College from 2000 to 2007 were retrospectively analyzed. The causes, clinical manifestations, treatment and prognosis were analyzed. Results Of the 86 patients, 38 were caused by internal milieu disorder, 9 by pancreatic encephalopathy, 23 by infectious-toxic encephalopathy, and 16 by Wernicke encephalopathy. The manifestation of the patients with SAP complicated with mental disorders was complex, varied and nonspecific. Individualized treatment was applied to all patients after the analysis of the causes, and most patients recovered except 14 who died of hemorrhage or multiple organ dysfunction syndrome. Conclusions Internal milieu disorder should be first considered when patients have mental disorders which occurred at the early phase of SAP, then pancreatic encephalopathy should be considered. The infectious-toxic encephalopathy and Wernicke encephalopathy should beware of when the mental disorders occur in the late phase of SAP. Individualized treatment according to the causation and comprehensive treatment are effective in preventing and treating mental disorders which occurred in the course of SAP.
7.Sky bone expander kyphoplasty for osteoporotic vertebral body compression fractures

Objective To investigate surgical technique and clinical efficacy of Sky bone ex- pander kyphoplasty in the treatment of osteoporotic vertebral body compression fractures.Methods Eighteen cases with osteoporotic vertebral body compression fractures were treated with Sky bone expander kyphoplasty from August 2004 to November 2005.Under the local anesthesia,3.5-5ml of bone cements were injected into each pathologic vertebral body through unipedicle approach after reduction procedure was done with Sky bone expander.Results The postoperative follow-up ranged from 3 to 11 months, with an average of 4.5 months.Back pain was effectively relieved after the operation in all cases.No complications occurred.Conclusion The Sky bone expander kyphoplasty has the advantages of safe- ty,easy operation,minimal invasion,effective restoration of the vertebral body height and fast relief of pain.

Objective Observe the effect and the indications of intermittent short veno-venuous hemofiltration(ISVVH) in severe acute pancreatitis(SAP). Objective To study the effect of single short (SS) veno-venous hemofiltration and intermittent short (IS) veno-venous hemofiltration on imbalance of pro- and anti-inflammatory cytokines in severe acute pancreatitis (SAP) patients. 10.Topical Retinoids for Acne Vulgaris: Efficacy and Safety: a Systematic Review

Objective To evaluate the efficacy and safety of topical retinoids in the treatment of acne vulgaris compared with placebo, antibiotics, benzoyl peroxide and sulfur preparation. Methods According to the Cochrane reviewer′s handbook, randomized controlled clinical trials were selected for the systematic review. Results Up to 2002, 15 clinical trials (2,439 patients) that met the inclusion criteria were selected. There were four clinical trials which showed that topical retinoids were more effective than that of placebo (RR=1.87, and 95% CI: 1.13 ~ 3.11),especially for noninflammatory lesions (RR=12.70,and 95% CI : 4.09 ~ 39.40). There were 3 clinical trials which showed that topical retinoids had better efficacy than that of sulfur preparations (RR=1.75, and 95% CI: 1.42 ~ 2.16). For 7 clinical trials of retinoids compared with benzoyl peroxide, and 3 clinical trials of retinoids compared with antibiotics, no conclusion could be drawn. All the clinical trials showed that there were local side effects, including erythema, and scaling etc in the patients using topical retinoids, but no systematic side effects were observed, however, pregnant women had to be very cautious. Conclusions Topical retinoids are effective for acne vulgaris, and has better efficacy than sulfur preparation does, but there is not enough evidence to clarify that the efficacy of topical retinoids is better than that of benzoyl peroxide and antibiotics.
