1.Differentiation of adult adipose tissue-derived mesenchymal stem cells into cardiocyte-like cells in vitro

BACKGROUND: At present, many problems deserve to be solved before clinical utilization of adipose tissue-derived mesenchymal stem cells (ADMSCs) such as complete differentiation from ADMSCs into cardiomyocytes and ADMSCs with or without specific function of cardiomyocytes. It is of significance to solve the problems including how to elevate the differentiation rate of ADMSCs into cardiomyocytes and how to elevate the homing and survival rate after transplantation for clinical utilization of stem cells.OBJECTIVE: To observe the differentiation of ADMSCs into cardiomyocytes after in vitro culture and induction.DESIGN: Randomized controlled observation.SETTING: Laboratory of Cardiology, General Hospital of Chinese PLA.MATERIALS: Adipose tissue was collected from abdominal operative patients at Department of General Surgery of General Hospital of Chinese PLA with the agreement of patients. Iscove's Modified Dulbecco's Medium (IMDM), type Ⅰ collagenase, 5-azacytidine (5-aza) and polyclonal antibody of specific anti-myocardial Troponin T (TnT) were purchased from Hyclon company, Gibco company, Sigma and Fujian Maixin Biotechnology Company, respectively.Monoclone antibodies of CD44, CD45, CD34, HLA2DR and factor-Ⅷ, Desmin, anti-α-striated muscle actin and anti-myosin heavy chain (MHC) were bought from Beijing Zhongshan Golden Bridge Biotechnology Limited Corporation. DAB stain, reverse transcription-polymerase chain reaction (RT-PCR) kit and atrial natriuretic peptide (ANP) radioimmunity kit were purchased from Beijing Zhongshan Golden Bridge Biotechnology Limited Corporation,Invitrogen and Radioimmunity Research Institute of Science and Technology Development Center of General Hospital of Chinese PLA, respectively.METHODS: The experiment was performed at the Laboratory of Cardiology, General Hospital of Chinese PLA from March 2005 to April 2006. ADMSCs were isolated and cultured by digestion and attachment culture method. The third generation of cells were determined by immunocytochemical method for surface molecule CD44, CD13, CD105, CD45,CD34, HLA-DR, factor Ⅷ and induced by addition of 5-aza into culture medium. On the 7th, 14th, 21st and 28th days, gene of GATA4 and Nkx2.5 were tested by reverse transcription-polymerase chain reaction (RT-PCR), and the concentration of atrial natriuretic polypeptide (ANP) were also examined by radioimmunoassay.MAIN OUTCOME MEASURES: Determination of cell surface marker, gene of GATA4 and Nkx2.5, and ANP concentration.and factor Ⅷ negative for the cultured cells. After induction for 7 days with 5-aza, no cells expressed Desmin,α-Sarcomeric Actin, Myocin heavy chain (MHC) and TnT, most cells were positive stained for CD44. After induction for 14 days, small amounts of cells displayed positive for Desmin,α-Sarcomeric Actin and MHC but still negative for TnT,while partial cells expressed positive CD44. After induction for 21 days, most cells were positive for Desmin,α-Sarcomeric after induction and no significant difference was found compared with that at the 7th day [(0.022±0.01) ng/L (P>0.05].ANP concentration was respectively (7.92±0.21) and (8.12±0.50) ng/L at the 21st day and the 28th day (P>0.05), but there was significant difference compared with that at the 7th day (P<0.05).
2.Rehabilitative therapy for the elderly chronic somatic disease complicated with depression

BACKGROUND: There is a complicated relation between somatic disease and mental disorder. Somatic disease may induce functional disorder of brain, and generate psychiatric symptoms due to the change of psychological and social environment. The proportion of depression in elder people based on the chronic somatic disease is much higher, and thus the fatality of them increases.OBJECTIVE: To investigate the influence of single antidepressant, and therapeutic alliance of psychological and drug treatment on the rehabilitation of patients with elderly chronic somatic disease complicated with depression. DESIGN: Randomized and controlled grouping experiment.SETTING: Department of Senile Disease of Tongji Hospital Affiliated to Huazhong University of Science and Technology.PARTICIPANTS: Totally 102 patients were hospitalized in the Department of Senile Disease of Tongji Hospital Affiliated to Huazhong University of Science and Technology from March 2000 to January 2003, and all the patients had sleep disorder to a different extent, and suffered from symptoms of heart and alimentary tract which could not be explained as organic diseases, and chronic pain or systemic symptom. Their score of Zung scale was > 40 points with average of (48.83±4.15)points. The patients were randomly divided into the drug group (n=46) and the combined treatment group (n=56). Each patient had more than two somatic symptoms in the two groups and more than 81.0% of them had three somatic symptoms. METHODS: According to the different primary diseases in patients of the two groups, regular treatment was given routinely. Oral thymoleptics fluoxetine of 20 mg/d 5-hydroxyltryptamine reuptake restrainer with 20 mg/d was administered, the dose of which could be added to 40 mg/d according to the pathogenetic conditions. At the beginning of the administration of oral antidepressant drugs, the patients in the combined treatment group were The focal point of depression and fear was identified, and the comfort and edge related to the disease was supplied, and the pathogenetic condition and the prognosis were told to the patients objectively. The family and society were encouraged to learn the rehabilitative skill and improve their possible, and the family and society were encouraged to give the patients adequate psychological support and life support. The patient and the family were assisted to establish favorable communication. The feeling of loneliness and isolation was reduced, and the sense of safety was increased. The depression status and the quality of life of the patients in the two groups were evaluated before the treatment and at 2 and 8 weeks after treatment with the Zung scale and the Spitzer quality of life index score (QLI), including 5 aspects: Activity, daily life, health, support and perspective. The highest score was 10 points. The lowest was 0, and the higher the score, the better the quality of life is.MAIN OUTCOME MEASURES: The results of the Zung scale and the QLI score evaluated before the treatment and at 2 and 8 weeks after the treatment.RESULTS: The treatment of the patients in the two groups was all finZung scale in the drug group and the combined treatment group at 2 weeks after the treatment indicated that there was significant difference in comparison with those before the treatment (41.12±2.60,48.83±4.15,t =10.67,P < 0.05); (40.88±3.43,49.03±4.21 ,t=11.24,P < 0.05). There was not significant difference in group comparison. The total points of Zung scale at 8 weeks were significantly decreased (36.11 ±2.87 ), (30.71±3.62),and there was significant difference in comparison with that before the treatment. The combined treatment group was better than the drug group in difference of QLI score at 2 weeks between the drug group and combined treatment group before and after the treatment (5.47 ± 1.33,5.32 ± 1.04,5.55±1.18,5.27±1.35). The QLI score of drug group and combined treat ment group at 8 weeks after treatment began to rise 7.21±0.13 and 8.83 ±0.32 ,respectively, and there was significant difference in comparison withthose before the treatment, and there was also significant difference in group comparison.CONCLUSION: The positive and effective combined therapy of anti-depression significantly improved the somatic symptoms of patients, no complications appeared, and most patients presented optimistic attitude to the prognosis of the primary disease. To identify the depression of elder people in earlier period is very important to promote rehabilitation of the primary disease.
3.Production and Preliminary Application of Monoclonal Antibodies Raised Against Aβ22-35

Objective To produce a hybridoma secreting stable monoclonal antibodies against Aβ22-35 and to develop a detection method for the assay of Aβ. Methods Spleen cells from Balb/cmice immunized with Aβ22-35-KLH were fused with mouse myeloma cells SP2/0. The techniques of immunoprecipitation and western blotting plus ECL were used to investigate the levels of Aβ in the rat brain. Results Two strains of hybridomas (3A8 and 3B2) secreting stable monoclonal antibodies raised against Aβ22-35 were obtained. The subtypes of Aβ22-35 were IgG3. The levels of Aβ in young and older rat brain were 9.8±2.8 and 13.36±2.65 (pmol/12mg brain tissues, x±s), respectively. Conclusion The Aβ22-35 mAb obtained had high titres and specificity. The levels of Aβ in the older rat brain were significantly increased as compared with the young one (P<0.05).

RNF180 is a novel membrane-bound E3 ubiquitin ligase that participates in cell development,proliferation and apoptosis.It is a tumor suppressor gene that inhibits cell proliferation and induces apoptosis and may inhibit gastric cancer cell lymph node metastasis.The study found that RNF180 gene methylation and gastric cancer is closely related to the occurrence and development.Therefore,RNF180 gene methylation is expected as a tumor marker of gastric cancer for early diagnosis and prognosis of gastric cancer.In this paper,RNF180 on the diagnosis of gastric cancer research progress made a review.
5.Clinical analysis of related risk factors in 558 hospitalized cases with atrial fibrillation

Objective To investigate the related risk factors in patients with atrial fibrillation in order to prevent and delay the occurrence of atrial fibrillation.Methods Five hundred and fifty-eight inpatients with atrial fibrillation were retrospectively analyzed from June 2005 to June 2008.They were divided into several groups according to the age and the characteristics of the elder patients with atrial fibrillation were analyzed.Results In the 558 cases with atrial fibrillation, there were 298 males (53.4%) and 260 females (46.6%) aged from 21 to 97 years.The average age was (72.8 ±10.1) years.There were 57 cases aged 21-59 years(10.2 %)and 501 cases aged 60-97 years(89.8 %).The total number of inpatients in our hospital was 11 869, and there were 4049 cases aged<60 years, 2527 cases aged 60-69 years, 3971 cases aged 70-79 years, 1244 cases aged 80-89 years and 78 cases aged>90 years.The proportions of the inpatients with atrial fibrillation in the above five age groups of inpatients were 1.4%(57 cases), 4.2%(107 cases), 6.6% (262 cases), 9.5%(118 cases)and 17.9% (14 cases), respectively.In 558 cases with atrial fibrillation, there were 230 cases (41.2%) with paroxysmal atrial fibrillation, 44 cases (7.9%) with persistent atrial fibrillation and 284 cases (50.9%) with permanent atrial fibrillation.The most common underlying disease was hypertension in the 558 cases, followed by coronary heart disease, heart failure, diabetes, rheumatic heart disease and so on.Conclusions The prevalence of atrial fibrillation is increased with aging.Hypertension, coronary heart disease, rheumatic heart disease, heart failure, hyperthyroidism,diabetes, chronic pulmonary disease and renal failure are all the risk factors for atrial fibrillation.
6.Mutation analysis of keratin 5 and keratin 14 genes in a family with epidermolysis bullosa simplex with mottled pigmentation.

OBJECTIVETo identify keratin 5 (K5) and keratin 14 (K14) gene mutations in a family affected with epidermolysis bullosa simplex with mottled pigmentation.
METHODSGenomic DNA was extracted from peripheral blood samples obtained from eleven patients from the family and controls. All the exons of K5 and K14 genes were amplified using polymerase chain reaction (PCR) and directly sequenced.
RESULTSBy DNA sequence analysis, a missense mutation in K5 gene (c.237C>T) was detected. The same mutation was not found in non-affected members from the family and normal controls.
CONCLUSIONMutation in K5 gene (c.237C>T) may be responsible for the development of disease in this family.

7.Treatment and economic burden of urine incontinence in the rural elderly

ObjectiveTo explore the treatment and economic burden of urine incontinence (UI) in the rural elderly,and provide evidences for preventive intervention.MethodsA cluster sampling was carried out in 743 people (aged 60 years and over) from 5 villages dominated by Yanliu and Dafengshang health centers,Jixian County,Tianjin from 2007 to 2008.The information about general state of health,treatment and economic burden of disease were analyzed. Results The prevalence of UI was 33.4% (248 cases) among the 743 elderly people.Only 12 patients (4.8%) with UI ever visited community health-care service centers or hospitals for diagnosis and treatment,among whom only 1 case visited hospital within 1 year,8 cases during 1 to 5 years,and 3 cases over 5 years.And the method of treatment was all drug therapy.Among 248 cases with UI,there were 227 cases (91.5%) without any treatment,9 cases (3.6%) buying medicines by themselves.The direct cost for treatment of UI was 80-12000 yuan RMB,including 2 cases below 100 yuan RMB,2 cases 100-1000 yuan RMB,7 cases 1001-5000 yuan RMB and 1 case over 10000 yuan RMB. Conclusions Prevalence of UI is high while hospital-visiting rate is very low,and the financial burden of disease is serious,thus intervention measures for UI should be taken for the elderly people in rural areas of Jixian County,Tianjin.

9.Comparison of robot assistant radical prostatectomy by transperitoneal versus extraperitoneal approach

Objective To compare the safety and oncological results of robot-assisted laparoscopic radical prostatectomy performed between transperitoneal and extraperitoneal approach.Methods 79 patients underwent transperitoneal robot-assisted laparoscopic radical prostatectomy (Tp-RLRP).The other 86 consecutive patients went through extraperitoneal robot-assisted laparoscopic radical prostatectomy (Ep-RLRP).Results All procedures were performed successfully without conversion.Total procedure time for Ep-RLRP was shorter than it for Tp-RLRP[(218.1 ± 54.4) min vs.(180.2 ± 47.1) min,P < 0.001].Docking time for Ep-RLRP was longer than Tp-RLRP [(21.5 ± 7.1) min vs.(19.5 ± 5.7) min,P < 0.001] Blood lost of Tp-RLRP was lesser than Ep-RLRP[(122.4 ± 71.0) ml vs.(140.4 ± 90.7) ml,P < 0.05].There were no significant differences between two groups in terms of bowel function recovery,drainage tube reserve,foley catheter duration,etc.There were 19 patients (24.1%)and 4 patients (4.7%)were suffered lymph leakage in two groups separately,P < 0.001.One delayed vesiclurethra hemorrhagic patient and one mesenteric torsion patient were happened in Tp-RLRP group.There were no significant differences between two groups in terms of surgical margin,biochemical recurrence and urine continence.Conclusion Compared to Tp-RLRP,Ep-RLRP can provide similar short-term oncological control,recovery of postoperative urine continence,shorter operation time and lower complication rates.
10.Analysis on pathogens composition and drug resistance in 1 829 cases of positive blood culture

Objective To analyze the detected pathogens composition in positive blood culture samples and drug resistance in our hospital from January 2005 to December 2012 in order to accumulate the data information of pathogenic bacteria distribution and drug resistance in bacteremia .Methods The BD9240 and BacT /Alert3D 240 blood culture systems were used to perform the blood culture .The identification of isolated bacteria and the drug susceptibility test were conducted by using Microscan walkaway 40 sys‐tem and the Vitec2 compact system .The Data were analyzed by adopting the Whonet5 .6 software .Results In 1 829 positive bacte‐rial strains by blood culture ,986 strains were Gram negative bacilli ,accounting for 53 .9% ;721 strains were Gram positive coccus , accounting for 39 .4% ;104 strains were fungi ,accounting for 5 .68% .The resistant rate of staphylococcus to vancomycin ,linezolid and teicoplanin was 0% ,which to amoxycillin/clavulanic acid ,rifampicin ,amikacin ,sulfamethoxazole compound and chloramphenicol was lower than 40% .The sensitive of enterococcus to linezolid and teicoplanin was 100% ,but enterococcus faecium was resistant to vancomycin(2 .6% ) .The penicillin resistant rate of Streptococcus was 21 .7% .The resistant rates of E .coli and K lebsiella pneumo‐nia were 0% to imipenem and meropenem ,and less than 22% to amikacin ,piperacillin/tazobactam and cefoxitin .The resistant rates of salmonella to CLSI recommended five kinds of detection drug were less than 6 .5% .The resistant rates of pseudomonas aerugino‐sa were more than 25% to imipenem and more than 25% to meropenem .Conclusion The pathogens spectrum detected by blood culture is widespread .The resistance rates of different bacteria vary widely .
