To analyze and summarize the clinicopathological features of renal inflammatory lesions which were easily misdiagnosed as renal tumors. Methods The cases from March 2006 to August 2015 in the Department of Pathology at Xijing Hospital were analyzed, retrospectively.Due to the diagnosis made by clinicians that these were renal (including renal pelvic) tumors, all cases had undergone radical nephrectomy or tumor enucleation.Among them, several inflammatory lesions were confirmed, and were combined with the patients' clinicopathological data and literature review to investigate the diagnosis and differential diagnosis. Results Of the 1 195 cases, only 6 cases (less than 1%) were renal inflammatory lesions, named as xanthogranulomatous pyelonephritis (2 cases),renal malakoplakia (1 case),renal tuberculosis (1 case),IgG4-related tubulointerstitial nephritis (1 case),and renal Wegener's granulomatosis (1 case). These diseases occurred in two males and four females (aged 14 to 55 years). All patients underwent radical nephrectomy due to the detected unilateral solitary renal mass in the imaging examination. Conclusion Renal inflammatory lesions are very rare, which show masses that are easily misdiagnosed as neoplastic lesions. Some of them have similar histological features. As the differences of treatment,a sufficient understanding of the clinicopathological features of these diseases is important to make a correct diagnosis.

ObjectiveTo investigate the feasibitity of detecting S100A6 expression at mRNA level in endoscopic ultrasonography guided fine needle aspiration (EUS-FNA) pancreatic ductal adenocarcinoma (PDA) specimens and its diagnostic value in PDA.MethodsA total of 18 PDA specimens and 22 normal pancreatic specimens were collected. RNA was extracted for reverse transcription. The expression of S100A6 gene was examined by real-time polymerase chain reaction. The cut-off value of S100A6 expression at mRNA level in PDA diagnosis was established through receiver operating characteristic (ROC) analysis. 28 patients with pancreatic head masses were selected for EUS-FNA examination, and the value of S100A6 mRNA expression level in PDA diagnosis was prospectively evaluated. The expression of S100A6 protein in PDA tissue was determined by immunohistochemistry staining.ResultsS100A6 mRNA expression in EUS-FNA and surgical PDA specimens (0.05023±0.10120,0.02083 ± 0.02848) was significantly higher than that of normal pancreatic tissues (0.00164±0.00202),both P<0.01.The expression of S100A6 in 22 EUS-FNA PDA specimens was significantly higher than that of 6 pancreatic benign disease biopsy specimens (0.00193 ± 0.00278,P =0.0009). There was no significant difference in S100A6 expression between 6 pancreatic benign disease biopsy specimens and normal surgical pancreatic samples (P=0.6143).When S100A6 mRNA expression in EUS-FNA specimens over 0.00525 was taken as positive diagnostic value, the sensitivity, specificity and accuracy in prospective pancreatic cancer diagnosis were 90.01%,100 % and 92.85 %,respectively.ConclusionThe high expression of S100A6 mRNA in EUS-FNA specimens of PDA has good preoperative diagnostic value.

Objective To study the changes of Th17 cell, regulatory T cell (Treg) and interleukin (IL)-6 in the peripheral blood of patients with systemic lupus erythematosus (SLE) and their relationship with disease activity. Methods Percentage of Th17 and Treg in the peripheral blood of 103 patients with SLE and 28 healthy volunteers were detected by flow cytometry. The concentration of IL-6 in SLE patients and healthy volunteers was detected by cytometric bead array (CBA). The disease activity of SLE was measured by SLEDAI. SLE patients were divided into two groups: stable SLE (SLEDAI≤ 9, n=37) and active SLE (SLEDAI>9, n= 66). The change of Th17, Treg, IL -6 and their relationship with disease activity were analyzed. Nonparamentric tests, t -test and spearman correlation were used for statistical analysis. Results The percentage of Th17 cells and the concentration of IL-6 in the peripheral blood in patients with SLE was higher than that in normal controls [respectively for (1.2±1.1)%, (35±92) pg/ml and (0.6±0.4)%, (6±3) pg/ml, P<0.05]. However, the percentage of Treg in patients with SLE was lower than that in normal controls [respectively for (1.6±1.2)%,(2.6±1.8)%, P<0.05]. The percentage of Th17, Th17/Treg IL-6 level in active SLE patients was higher than those in inactive SLE and those in normal controls (P<0.05). However, the percentage of Treg in active SLE was lower than that in stable SLE patients and that in normal controls (P< 0.05). The percentage of Th17, Th17/Treg and concentration of IL-6 was positively correlated to disease activity(P<0.05). But the percentage of Treg had negative correlation with the percentage of Th17 and disease activity (P<0.05). Conclusion Th17, Treg and serum IL-6 in SLE patients are abnormal and they maybe contribute to the pathogenesis of SLE.
