1.Study on cytomegalovirus gene mutations conferring resistance to ganciclovir in heart and lung transplant patients

Objective To establish a rapid and convenient assay to screen for emerging ganciclovir(GCV) resistant mutations in the UL97 gene of human cytomegalovirus (HCMV) from transplant recipients. Methods Thirteen spefcific oligonucleotide probes were designed according to the target sequence encoding HCMV-UL97 gene. A nested polymerase chain reaction (PCR) amplifying UL97 gene was employed. Line probes assay (LiPA) was configured to detect relevant non-synonymous mutations at codon 460,520,594,595,603, and 607. In parallel, nested-PCR amplicons were subsequently sequenced directly. Results Four of 16 heart and lung transplant recipients had been infected with HCMV resistant strains during GCV therapy. The mutant codons of UL97 gene were at codon 520、595 and 603. All amplicons detected by LiPA were fully concordant with the nucleotide sequenced.Conclusions Long-term prophylaxis and treatment with GCV in heart and lung transplant recipients may develop HCMV resistant strains. HCMV-LiPA proves to be an alternative method for the resistance genotype analysis of HCMV.

METHODS: A total of 71 000 prescriptions were collected from Sept. 2006 to Dec. 2007 (before implementation) and from May to Dec. in 2007 (after implementation) for a statistical analysis and evaluation of the irrational prescriptions. 4.Microsurgical treatment for brain stem tumor

Objective To investigate the clinical manifestation,pathological characteristics,neurological imaging,indications,operative approach and microsurgieal technique of surgery for brain stem tumor.Methods 17 cases of brain stem tumor treated by applying microsurgical skills were retrospectively analyzed from Jan.2002 to Dec.2007.Ten cases were operated via the suboccipito-retromastoid approach,among which 5 situated in pons,3 in medulla oblongata and 2 invoved both pens and medulla oblongata;4 cases were performed via the posteromedian-suboccipital approach,among which 2 located in pens,1 in medulla oblongata and 1 involved both midbrain and pens;3 lesions situated in varying sites of mesencephalon were operated by the suboceipito-supratentorial,infratemporal approach and pterional approach respectively.Results In the series of the patients,lesion was totally excised in 6 cases,subtotally in one,without surgical mortality.All patients were followed up for years ranging from 5 months to 4 years.During the follow-up period,seven patients resumed work and normal life;4 patients could perform physical labor;3 patients obtained self care capability;2 patients needed to be taken care of,1 patient died from pneumonia after discharge.Conclusion Fine therapeutic efficacy and low disability rate could be obtained while applying microsurgical techniques to resect brain stem tumors with correct indication,appropriate approach and elaborate microneurosurgical skills.
5.Employing units' evaluation of the importance of the preventive medicine talents' competencies

Objective To investigate the employing units' evaluation of the importance of the pre-ventive medicine talents' competencies, Hainan province, in order to provide reference for the construction of competency model of Preventive Medicine and make suggestions for the cultivation of Preventive Medicine professionals in our province. Methods Random sampling methods were applied and a survey was conducted among 10 CDCs and Health Supervision Institutes with self-designed questionnaires through telephone interview, correspondence or field investigation. SPSS 20.0 software was used to analyze the data. Results Teamwork, responsibility, preventive medicine knowledge, ability to prevent and control the dis-eases, ability to accept new methods and new techniques were thought very important by 94.4%, 100%, 72.2%, 77.8%, 72.2%of the surveyed units, respectively. However, only 5.6%、5.6%、33.3%and 38.9%of the surveyed units, respectively, considered that the computer applications ability, English proficiency, research approval and innovation ability were very important. Conclusion Colleges should improve the work of cultivating preventive medicine specialists to meet the employing units' demands.
6.Expression of PirB in normal and injured spinal cord of rats.

The expression of paired immunoglobulin-like receptor B (PirB) in normal and injured spinal cord of rats was investigated. The SD rat hemi-sectioned spinal cord injury (SCI) model was established. Before and 1, 3, 7, 10 days after SCI, the spinal cord tissues were harvested, and Western blot and immunohistochemistry were used to examine the expression and location of PirB. The results showed that the expression level of PirB in the normal spinal cord of SD rats was low. At the first day after SCI, the expression of PirB was obviously increased, and that in the injured spinal cord from the first day to the 10th day was significantly higher than in the normal spinal cord. The positive expression of PirB in neurons from different regions of gray matter of the injured spinal cord was seen. It was concluded that the expression of PirB in the normal spinal cord of rats was low. The expression of PirB in SCI was significantly increased till at least the 10th day.
7.Corneal aberrations and visual performance after flap creation of LASIK with a femtosecond laser and a mechanical microkeratome

Background Laser in situ keratomileusis (LASIK) for myopia and/or myopic astigmatism still is one of primary surgeries.Femtosecond laser (FS)-LASIK offers several advantages over microkeratomes for flap creation including better safety,reproducibility,and predictability.However,the visual performance after FS-LASIK is of high clinical concern.Objective This study was to compare the visual performance between Intralase FS60 femtosecond laser flap and mechanical microkeratome Moria M2 flap in LASIK.Methods A prospective nonrandomized controlled study was performed.Two hundred and four myopic eyes of 102 patients were enrolled in this study and divided into two groups according to patients’ opinion,with matched demography between the two groups.LASIK with FS60 femtosecond laser flap was performed on 100 eyes of 50 myopia and/or myopic astigmatism in the FS group,and Moria M2 flap LASIK was performed on 104 eyes of 52 patients in the microkeratomes group.Zernike coefficients and the root-mean-square (RMS) of higher order aberrations were measured by Wavescan wavefront aberrometer before and 3,6 and 12 months after surgery,and the contrast sensitivity (CS) in different spatial frequencies under the scotopia and scotopia + glare background was detected using Optec 6500 visual function instrument before and 3,6,12 months after surgery.This study protocol was approved by Ethic Committec of Beijing Tongren Hospital,and written informed consent was obtained from each patient prior to surgery.Results The uncorrected visual acuity (UCVA) levels after surgery reached or was superior to the preoperative best corrected visual acuity (BCVA) in 95.1%,94.2% and 93.9% patients in the FS group and in 94.2%,93.8% and 93.2% patients in the microkeratomes group 3,6,12 months after surgery,respectively.The mean spherical equivalent (SE) was (-6.37 ±2.06)D,(-0.26±0.45)D and (-0.45± 0.51)) in the FS group,(-6.25± 2.43) D,(-0.44± 0.64) D and (-0.35±0.59)D in the microkeratomes group before surgery and 3,12 months after surgery,respectively.There were no significant differences in the SE in various time points between the two groups (u =1.194,1.429;both at P> 0.05).The root means square high order (RMSh) and RMS3-6 in the FS group were significantly lower than those in the microkeratomes group 3,6 and 12 months after surgery (all at P<0.05).Under the background of scotopia or scotopia+glare,the LogCS values under different spatial frequencies were significantly higher in the FS group than those in the microkeratomes group 3 and 6 months after surgery (all at P<0.05).In 12 months after surgery,no significant difference was found in the LogCS under the 1.5 c/d in the background of scotopia between FS group and microkeratomes group (P>0.05),and the LogCS values under the 12.0 c/d and 18.0 c/d in the background of scotopia or scotopia+glare in the FS group were better than those in the microkeratomes group (all at P<0.05).Conclusions Femtosecond laser assistant LASIK has better visual performance and lower higher-order aberrations than microkeratomes assistant LASIK.
9.Comprehensive treatment of complicated and giant carotid body tumor: a report of 1 case

Objective: To study the treatment of complicated and giant carotid body tumor. Methods: A giant carotid body tumor was dissected 2 years ago. The external carotid artery was embolized by means of catheterization before surgical intervention, and techniques of internal shunting and autogenous blood transfusion was used during the operation. Results: The tumor was resected completely without cerebral vessel disorders and major cephalic nerve injuries. Conclusion: The application of radial intervention of external carotid artery, shunting of internal carotid artery and autogenous blood transfusion are helpful to the treatment of giant and re operative carotid body tumor.
10.Antihypertensives Used in Our Hospital During 2004~2005:Utilization Analysis

OBJECTIVE:To investigate the utilization situation and tendency of antihypertensive drugs in our hospital during the period 2004~ 2005.METHODS:The consumption sum,DDDs and the defined daily cost of antihypertensives during 2004~ 2005 were analyzed statistically.RESULTS:The consumption sum of antihypertensive drugs increased year on year,dominating the first 4 places were calcium antagonists,ACEI,angiotensin receptor antagonists and ? adrenoceptor antagonists.Dominating the first 10 places in terms of consumption sum were chiefly newly marketed,expensive but safe and effective varieties.Leading the top 15 places in terms of DDDs were those drugs that proved to be of confirmed efficacy and lower prices.CONCLUSION:The utilization of antihypertensive drugs in our hospital is basically in line with the general utilization condition home and abroad,and it is up to the current medication principle of antihypertensive drugs
