1.Cognitive function and plasma somatostatin level in patients with leukoaraiosis

Objective To explore the relationship between the cognitive function and plasma somatostatin(SS) level in the patients with leukoaraiosis(LA).Methods According to the clinical findings and CT imaging,the subjects were divided into three groups:26 patients with LA and dementia, 47 patients with LA but without dementia, and the normal controls for 38 cases. The three groups were determined by means of Hasegawa's Dementia Scale(HDS)and SS levels in plasma.Results As compared with the control group,HDS score and plasma SS level in patients with LA (including dementia group and non dementia group) decreased remarkable(P0.05).Conclusion LA might be the cause of cognitive impairment,the concentrations of plasma SS had declined before dementia did not occur,the cognitive impairment in patients with LA might relate to the decrease of SS levels in the body.
2.The neurophysiological features of shoulder-hand syndrome after stroke

Objective T_O analyze the electrophysiological features of shoulder-hand syndrome(SHS)fol- lowing stroke and investigate the relationships between peripheral nerve damage and the factors causing SHS.Meth- ods Fifty-eight stroke patients were divided into an SHS group(39 patients with shoulder-hand syndrome)and a model group(19 patients without shoulder-hand syndrome).Standard sensory and motor nerve conduction studies were performed with 58 of the patients,including sensory nerve conduction velocity(SCV),amplitude of the sensory nerve action potential(SNAP),distal motor latency(DMI)and the amplitude of the compound muscle action poten- tial(CMAP)of the median nerve.A needle electromyogram(EMG)test was performed on the abductor pollicis of the affected side with both groups.Results The needle EMG showed abnormal insertion potential,fibrillation po- tential and positive sharp waves in all 39 cases of the SHS group,which was significantly higher than in the model group.The amplitude of the sensory nerve action potential(7.77?4.34 mV)and the amplitude of the compound muscle action potential(10.13?3.15 mV)were significantly lower than those of the model group.Abnormal ampli- tude was more severe in sensory nerves than in motor nerves.Conclusion Peripheral nerve damage was found in the shoulder-hand syndrome patients,and this damage was mainly the dystrophy of axonal neuropathy.The damage severity was more in sensory nerves than in motor nerves.The study could offer an useful clue of prevention and treat- ment of shoulder-hand syndrome.
3.Studies on the criteria of two-way referral between hospital and community health-care service centerfor patients with cerebral hemorrhage: an analysis of 91 cases

This study aimed to study the criteria of two-way referral between tertiary-care hospital and communityhealth-care service center for patients with cerebralhemorrhage and a protocol of implementation to promote its treatment and rehabilitation.The criteria of two-way referral between tertiarycare hospital and community health-care service center for patients with cerebral hemorrhage after surgical operation were formulated on a trial basis with mutual consultation by the department of neurosurgery,Shenzhou hospital affiliated to Shenyang Medical College and Hunhewan Community Health-Care Service Center,Heping district,Shenyang. Totally,91 patients of cerebral hemorrhage admitted to the hospital during October 2007 to September 2010 were recruited for the study,46 in the intervention group with two-way referral and 45 in the control group by self-referral determined by their family members.All the patients of the two groups were followed-up for three years.During the follow-up in the intervention group,40 of 46 (95%) patients could comply with taking medication ordered by physicians,ability of daily living remarkably improved by Barthel index in 21 (50%),physical function recovered well in 33 (79%),34 (81%) could insist in self-monitoring and 32 (76%) could visit physicians for regular consultation,all these being significantly better than those in the control group (64%,31%,28%,25% and 25%,respectively) (P <0.01 ).Medical and rehabilitative cost for patients with cerebral hemorrhage accounted for 575 RMB yuan per month in intervention group,as compared to 2246 RMB yuan for those in control group.Two-way referral for patients with cerebral hemorrhage can reduce their medical cost and promote their better rehabilitation.
4.Study on relationship between hyperlipidemia syndrome differentiation of traditional Chinese medicine and its correlation medical test index

Objective To investigate the relationship between TCM syndrome of hyperlipidemia and indexes such as age,sex,BMI,TC,TG,LDL-C,HDL-C,ApoA,ApoB,the vascular media,coagulation and inflammatory indices.Methods 200 patients of hyperlipidemia recruited from the cadre ward in hospital were made TCM syndrome differentiation according to Guiding principles for the clinical study of Chinese Medicines.SPSS16.0 statistical software were adopted for analyzing the relation between TCM syndrome and indexes such as age,sex,BMI,TC,TG,LDL-C,HDL-C,ApoA,ApoB,the vascular media,coagulation and inflammatory indices.Results ①)There are no differences TCM syndrome and the indexes of age,sex,TC,TG,HDL-C,LDL-C,VLDL-C,lipoproteins,the vascular media,coagulation and inflammatory indices.②Compared with the liver kidney yin deficiency syndrome group,apolipoprotein A was higher in the groups of qi stagnation and blood stasis,phlegm stagnation syndrome,and hyperactivity of Yang due to yin deficiency syndrome,the difference was statistically significant; while APOB showed no difference among the groups.Conclusion TCM syndrome of hyperlipidemia had no relation to indexes of age,sex,body mass index,blood lipid,and blood coagulation.The study found a correlation between hyperlipidemia and phlegm and Qi deficiency,which can provide a theoretical basis for syndrome differentiation of TCM.

6.Oral-appliance for erectile dysfunction induced by obstructive sleep apnea-hypopnea syndrome.

OBJECTIVETo evaluate the effect of the oral-appliance in the treatment of ED induced by obstructive sleep apnea-hypopnea syndrome (OSAHS).
METHODSThis study included 40 cases of OSAHS-induced ED. Based on the apnea-hyponea index (AHI) and lowest SaO2, we divided the patients into groups A (AHI < or = 30/h and lowest SaO2 > or = 80%) and B (AHI > 30/h and lowest SaO2 < 80%), and treated them with the oral-appliance for 3 months, followed by evaluation of the therapeutic effect using polysomnography, IIEF-5 questionnaire and Sexual Encounter Profile (SEP) diaries.
RESULTSThirty-seven of the patients accomplished the treatment, 16 in group A and 21 in group B. Compared with the baseline, the AHI was significantly decreased in both groups A (21.4 +/- 7.3 vs 14.2 +/- 6.6, P < 0.05) and B (51.4 +/- 9.5 vs 29.7 +/- 8.1, P < 0.05) after treatment, the IIEF-5 scores increased, even more remarkably in B than in A (3.36 +/- 2.48 vs 1.12 +/- 1.27, P < 0.05). The positive rates of SEP2 and SEP3 were markedly lower in A than in B (37.5% vs 76.2% and 25.0% vs 61.9%, P < 0.05).
CONCLUSIONThe oral-appliance can improve the erectile function of OSAHS-induced ED patients, particularly for those with severe OSAHS.

8.Characteristics of glucose tolerance in patients with nonalcoholic fatty liver disease and high blood pressure

Objective To investigate glucose metabolism , insulin secretion and serum lipid in patients of nonalcoholic fatty liver disease (NAFLD) with hypertension. Methods A total of 100 subjects including NAFLD with hypertension (n = 50) and NAFLD without hypertension (n = 50), in our hospital during Jul. 2010 to Jul. 2012, were enrolled in this study. Records the following information of the two groups: The clinical data , oral glucose tolerance and insulin releasing test , serum lipid , serum uric acid and ALT were recorded. Results Compared with the NAFLD without hypertension group, impaired glucose regulation was significantly increased in patients in the NAFLD with hypertension group(P < 0.05).The insulin releasing was significantly increased at 30 min and was increased at 180 min in patients in the NAFLD with hypertension group (P < 0.05).The insulin releasing during 30 min to 120 min was descend gradually in patients in the NAFLD with hypertension group,with no significant difference between these two groups. TG was markedly elevated in patients in the NAFLD with hypertension group (P < 0.05). Conclusion Prevalence of impaired glucose regulation was elevated obviously in NAFLD with hypertension group,and early-phase insulin secretion were reduced and delayed.
9.Study on the relationship between quality of life and metal stress of breast cancer patients' spouses during perioperative period

Objective To investigate the quality of life and metal stress of breast cancer patients'spouses in the perioperative period,and analyze the relationship between them.Methods Self-made General Information Questionnaire (WHOQOL-BREF) aud relative stress scale (RSS) were used to investigate 110 breast cancer patients' spouses during the perioperative period,in order to analyze the relationship betweeu quality of life and metal stress.Results A total of 110 copies of questionnaires were gave out,102 valid questionnaires,effective recovery was 92.73%.The scores of quality of life in physiology,psychology,social relationship and environment were (13.34 ± 1.88),(13.95 ± 2.63),(13.61 ± 2.45) and (12.22 ± 2.47)respectively and the score of RSS was (11.16 ±4.69).There was a negative correlation between the scores of WHOQOL-BREF and the RSS,the metal suffering as well as the confusion of life (P < 0.05).Only the scores of psychology and social relationship were negatively related to the negative emotions because of the patients (P < 0.05).Conclusions The quality of life of breast cancer patients' spouses during perioperative period is negatively affected by their metal stress.
10.MRI Features of Neuropsychiatric Abnormalities in Systemic Lupus Erythematosus

Objective To evaluate the MRI features of neuropsychiatric abnormalities in systemic lupus erythematosus(NPSLE).Methods Brain MRI images of 21 cases with NPSLE confirmed by clinic were analyzed retrospectively.ADC values of the lesions and normal brain tissue were measured.Results Brain abnormal MRI findings in 20/21 cases were found(95%)including:(1)High signal intensity on DWI in 14 cases,11 were diffuse lesions,3 were focal lesions,4 of 14 combined with brain atrophy.The lesions mostly localized in white matter.The ADC values of the lesions in gray matter were decreased(t=2.513,P=0.019),while the ADC values of the lesions in white matter were increased(t=2.877,P=0.007).(2)2 cases only showed brain atrophy.(3)Leukoaraiosis presented in 2 cases.(4)Encephalomalacie presented in 2 cases,1 accompanied with brain atrophy.(5)No enhancement(3/5)and little patchy enhancement(2/5)were observed at contrast MRI study.Conclusion MRI plays an important role in displaying cerebral lesions and the location of it,progression and succession of NPSLE.
