3.Study on depression in 212 patients with viral hepatitis

Objective To investigate the symptoms on depression in patients with viral hepatitis. Methods A cross-sectional study was conducted among the patients with viral hepatitis in infectious diseases Hospital of Taiyuan. The questionnaire included a Eysenck Personality Questionnaire, self-rating depression scale (SDS) , and a self-designed one related to information regarding general conditions of the disease and social support. Results (1)Depression symptom prevalence rate among chronic viral hepatitis patients was 54.7% (116/212). (2) Factors as age, occupation, education, confirmed time, number of recurrence and anti-virus treatment, self-confidence on recovery, satisfaction on the surrounding environment etc. that might be associated with depression. (3) The severity of depression was significantly negative correlation with social support scores, objective support scores, subjective support scores (r=-0.262, P=0.000;r=-0.228, P=0.001 ; r=-0.270, P=0.000). (4) There was positive correlation noticed between severity of the depressive disorder and Eysenck Personality two dimensions scores, while the scores of introversion and extroversion scores were negatively correlated (r=-0.330, P=0.000) but positively correlated to the emotional stability scores (r=0.309, P=0.000). Conclusion (1) Patients with hepatitis showed symptoms of depression to a certain degree. (2) Factors as age, occupation, education, economic situation, confirmed time of diagnosis, number of recurrence and anti-virus treatments, confidence on recovery, satisfaction on the surrounding environment might be associated with symptoms of depression. (3) There was positive correlation between severity of depressive and Eysenek Personality two dimensions scores but the scores of introversion and extroversion scores were negatively correlated.
4.Clinical observation on spleen-invigorating and qi-benefiting pediatric massage for treating recurrent respiratory tract infection in children with cerebral palsy due to qi deficiency of spleen and lung

Objective: To observe the clinical efficacy of spleen-invigorating and qi-benefiting pediatric massage (tuina) for treating recurrent respiratory tract infection in children with cerebral palsy due to qi deficiency of spleen and lung. Methods: A total of 70 children with cerebral palsy who suffered from recurrent respiratory tract infection due to qi deficiency of spleen and lung were randomized into an observation group and a control group by the random number table method, with 35 cases in each group. Both groups were treated with conventional rehabilitation training, while the observation group was given additional spleen-invigorating and qi-benefiting pediatric massage, and the control group additionally took oral Yu Ping Feng granule. The clinical efficacy was evaluated by the total effective rate, traditional Chinese medicine (TCM) symptom score, and serum levels of immunoglobulin (Ig) A, IgM and IgG. Results: The difference in total effective rate between the two groups after treatment was statistically significant (P<0.05). After treatment, the TCM symptom scores in both groups decreased to varying degrees than those before treatment, and the intra-group differences were statistically significant (all P<0.01); the differences in the scores of various TCM symptoms between the two groups were statistically significant (all P<0.05). After treatment, the levels of IgA, IgM and IgG of the children in both groups increased to varying degrees, and the intra-group differences were statistically significant (all P<0.05). The between-group differences in the IgA, IgM and IgG levels after treatment were statistically significant (all P<0.05). Conclusion: Spleen-invigorating and qi-benefiting pediatric massage can effectively treat recurrent respiratory tract infection due to qi deficiency of spleen and lung in children with cerebral palsy, relieve the clinical symptoms and improve immune function, and thus is worthy of clinical promotion and application.
5.Study on steroidal saponins from Dioscorea zingiberensis and their platelet aggregation activities.

Using the absorbent resin, silica gel and ODS column chromatography as well as semi-preparative HPLC, ten compounds were isolated from 70% ethanol extract of tubers of Dioscorea zingiberensis C. H. Wright, and their structures were elucidated as trigoneoside XIIIa (1), parvifloside (2), trigoneoside IVa (3), deltoside (4), protobioside (5), lilioglycoside k (6), zingiberensis newsaponin I (7), deltonin (8), prosapogenin A of dioscin (9), and trillin (10) on the basis of NMR and MS spectral data analysis. Among these compounds, 1, 3, 5 and 6 were isolated from this plant for the first time. In the screening test on platelet aggregation, compounds 7 and 8 exhibited induction effect on platelet aggregation, while compound 9 exhibited significant inhibitory effect on platelet aggregation in vitro.

6.Effects of cinnamyl aldehyde on cell cycle and relafeol proteins expression in NIH3T3 cells.

OBJECTIVETo observe the effects of Cinnamyl aldehyde (CA) on NIH3T3 cell cycle and explore the possible mechanism further.
METHODFlowcytometry was used for observing cell cycle distribution. Expressions of proliferation cell nuclear antigen (PCNA) and Cyclin D1 protein in NIH3T3 cells were assessed by immunocytochemistry.
RESULTAfter culture with CA for 24 hours, the percentage of populations of S phase was enhanced by 3% (P < 0.05) and cell proliferation index (PrI, S + G2/M) was increased by 3.5% (P < 0.01) , but G2/M phase had no obvious changes. The expressions of Cyclin D1 and PCNA proteins were improved markly by CA compared with controlgroup (P < 0.01).
CONCLUSIONCA could promote more cells in G0/G1 phase into S phase, which may be related to the regulation of the expressions of PCNA and Cyclin D1.

7.Attitude of patients for the PICC and its relevant factors for patients' indwelling selection

Objective To study the attitude of patients for the PICC and analyse its relevant factors for patients' indwelling selection,so as to provide evidence for the promotion of PICC and health educations for clinical nurse.Methods The questionnaire survey was used and108 patients that had indwelling indications in Beijing Chao-Yang Hospital,between April and May in 2010 were recruited.They were divided into acceptance group( n =50) and refuse group( n =58) and were investigated with self-designed questionnaire.Results Both of two groups had significantly differences in terms of gender,payment method for health care,potential risk of it,financial burden from it,impacts on daily life,and the trust from the medical personnel ( P < 0.05 ).The absolutely right percent of can avoid needling during transfusion period for the attitude on the PICC function,maybe pipe block of PICC for the attitude on the danger of PICC,such needle is very expensive for the attitude on the financial burden of PICC,influencing eating for the attitude on the influencing daily life of PICC of the refuse group respectively was 46.6%,50.0%,44.8%,22.4%,and that of acceptance group was 86.0%,12.0%,28.0%,6.0%,and the difference was statistically significant ( P < 0.01 or P < 0.05 ).There was significantly difference in the attitude of PICC can reducing the puncture times,alleviating pain was the necessary therapy condition between two groups ( P < 0.05 ),as well as in the attitude of the PICC has the danger of infection,taking off,forming thrombus,catheter misplacement,fracture,and the operation safety ( P < 0.05 ).there was no significantly difference on the attitude of existing dangerous in the PICC installing( P > 0.05 ),while the difference in the attitude of the financial burden influence of PICC on the daily life between two groups was significantly( P < 0.05 ).Conclusions From the view of patients with indwelling PICC indications,most patients agree that PICC can reduce the pain suffered from long-term infusion,but they comment that fees are high,and they have different attitude towards the risk of indwelling PICC and the influences of their daily lives.Most patients trust medical staff and not believe that indwelling PICC affect their psychological status and themselves image.Therefore,patients indwelling selection is correlate with the potential risk of indwelling,the impact of daily lifc,relevant economic burden,psychological and social factors.
8.Survey of patients with chronic hepatitis B to identify factors that influence quality of life.

OBJECTIVETo survey patients with chronic hepatitis B (CHB) to determine their perceptions of CHB-related quality of life (QOF) and to determine the factors influencing this measure.
METHODSA total of 268 patients with CHB (disease group) and 205 healthy individuals (control group) completed the World Health Organization (WHO)QOL-BREF life assessment survey and a self-designed questionnaire of health and QOL. The groups' responses were comparatively analyzed by the cluster sampling method and the independent samples t-test. The strength of influence of each factor on the patients' perceptions of QOL was determined by multiple stepwise regression and one-way ANOVA.
RESULTSThe disease group had significantly lower scores than the control group for overall QOL (62.88 ± 8.22 vs. 67.31 ± 5.82), the physiological area (PHYS: 64.71 ± 15.05 vs. 73.21 ± 11.26), the psychological area (PSYCH: 64.35 ± 14.71 vs. 68.94 ± 10.13), the social relations area (SOCIL: 67.20 ± 12.98 vs. 69.83 ± 8.65), the environmental area (ENVIR: 59.58 ± 13.23 vs. 63.97 ± 10.24), the QOL self-assessment (60.75 ± 21.54 vs. 66.90 ± 17.57) and the health self-assessment (58.13 ± 19.15 vs. 76.26+/-14.27) (all, P less than 0.05). Multiple stepwise regression analysis identified the following parameters as risk factors of PHYS: depression (P less than 0.001), perception of being seriously ill (P less than 0.001), self-payment for treatment (P = 0.003), CHB significant impact on income (P = 0.002), poor appetite (P = 0.002), langur (P less than 0.001), and fear of infecting others (P = 0.022). Confidence of treatment was a protective factor of PHYS (P = 0.001). The risk factors of PSYCH were depression (P less than 0.001) and recurrence (P less than 0.001), and the protective factors were confidence of treatment (P = 0.003) and male sex (P = 0.014). The risk factors of SOCIL were depression (P less than 0.001, dissatisfaction with the attitude of the people around (P = 0.001), recurrence (P = 0.008), and advanced age (P = 0.009), and the protective factors were social support (P less than 0.001) and confidence of treatment (P = 0.015); however, the scores were significantly different for different occupations (P = 0.008). The risk factors of ENVIR were depression (P less than 0.001), dissatisfaction with the attitude of the people around (P less than 0.001), living in rural area (P = 0.007), and recurrence (P = 0.016).
CONCLUSIONPatients should be monitored for depressive symptoms during the course of clinical medical care for CHB so that psychological care may be initiated in a timely manner. It is important to strengthen communication between healthcare professionals and patients in order to improve the patient's perception of social support and quality of life.

9.Xianling Gubao Capsule improves semen quality.

OBJECTIVETo observe the effect of the compound Chinese traditional drug Xianling Gubao Capsule on the semen quality of infertile males.
METHODSWe treated 66 infertile men with Xianling Gubao Capsule for 24 months, and analyzed the semen quality and sperm morphology before and after the treatment.
RESULTSTwo months after the medication, sperm concentration was increased by a small margin, but no statistically significant changes were observed in sperm vitality and motility (P > 0.05), the rate of morphologically normal sperm was significantly raised from 25.8% before treatment to 57.6% (P < 0.05) in those with the normal rate > or = 15%, but decreased from 53.0% to 25.8% (P < 0.05) in those with the normal rate < 9%. Among the 7 cases of oligospermia, the rate of morphologically normal sperm was elevated to an average of 10.9% after the 4-month medication, significantly different from the baseline rate of 5.8% (P < 0.05). Five spontaneous pregnancies and 1 successful IVF-ICSI were achieved during the treatment.
CONCLUSIONXianling Gubao Capsule can improve semen quality and significantly increase the percentage of morphologically normal sperm.

10.Efficacy and safety of lamivudine plus adefovir combination therapy and entecavir monotherapy for chronic hepatitis B patients.

To compare the efficacy and safety of Lamivudine (LAM) plus Adefovir dipivoxil (ADV) combination therapy and Entecavir (ETV) monotherapy for chronic hepatitis B patients. 120 patients with chronic hepatitis B managed in a single-centre clinical practice (median 96 weeks) were split into 2 cohorts, one was treated with de-novo combination Lamivudine (100 mg/day) plus Adefovir (10 mg/day) (LAM+ADV), the other with Entecavir (0.5 mg/day) monotherapy. Serum levels of ALT, creatinine, HBsAg, HBeAg and HBV viral load, together with genotypic resistence were analyzed at 0, 12, 24, 48, 96 weeks, respectively. HBV DNA was determined by real-time PCR. HBsAg and HBeAg were assessed by chemiluminescence. Serum levels of ALT and creatinine were detected by automatic biochemical analyzer. HBV genotypic resistence was tested by direct sequencing. (1) At the time point of 96 weeks, a total of 99 patients (51 cases in combination therapy cohort and 48 case in monotherapy cohort) were compared. The baseline characteristics as for HBV viral load, median age, serum levels of ALT and creatinine were compatible between combination therapy cohort and monotherapy cohort. (2) The rates of HBV DNA values is less than 300 copies/ml and HBV DNA values is less than 1000 copies/ml had no significant difference between LAM + ADV and ETV cohorts by the 12 and 24 weeks (P more than 0.05). (3) At the time point of 48 weeks, the rates of HBV DNA is less than 1000 copies/ml, HBeAg seroconversion, and ALT normalization were similar in both cohorts, though the rate of HBV DNA values is less than 300 copies/ml was obviously higher in combination therapy cohort than that of monotherapy cohort (90.7% vs 76%, P values is less than 0.05). (4) At the time point of 96 weeks, the rates of HBV DNA values is less than 300 copies/ml (96.1% vs 79.2%), HBV DNA values is less than 1000 copies/ml (98% vs 87.5%) and the HBeAg seroconversion (41.7% vs 16.7%) were markedly higher in combination therapy cohort than those of monotherapy cohort statistically (P values is less than 0.05 for all). The mean values of decreases for HBV viral loads and HBsAg levels were smilar in both cohorts at 48 and 96 weeks. (5) Elevated serum creatinine not be found in both cohorts at the end of treatment. (6) No virological breakthrough occurred in combination therapy cohort at the end of treatment. Four patients in monotherapy cohort were found with virological breakthrough at 96 weeks and three cases among were confirmed to be of variants associated with ETV resistance (rtL180M + T184L + M204V). Present study suggests that Lamivudine plus Adefovir dipivoxil de-novo combination therapy was more efficacious than Entecavir monotherapy for CHB patients and the tolerance is compatible.
