1.Epidemiology and characteristics of acute poisoning treated at an emergency center

BACKGROUND:Acute poisoning is frequently encountered at emergency department. This study was to investigate the epidemiology and characteristics of patients with acute poisoning who were treated at the Emergency Center, Fujian Provincial Hospital, China. METHODS:We retrospectively analyzed the gender, age, causes of poisoning, types of poisons, poisoning route, emergency diagnoses, outcomes, and prognoses of these patients. RESULTS:Altogether 2867 patients with acute poisoning were treated from January 2004 to December 2009. The ratio of male to female was 1:1.04, and their average age was 33.8 years. Of the 2867 patients, 76.39% were between 18 and 40 years old. The incidence of acute poisoning was as high as 11.33% in January each year. The incidence of poisoning was in a descending order:alcohol poisoning (54.55%), medication poisoning (25.95%), pesticide poisoning (5.65%), and drug poisoning (4.88%). Most (56.44%) of the patients with drug poisoning were under 25 years and their mean age was significantly lower than that of patients with medication poisoning or alcohol poisoning (P < 0.01). Approximately 69.54% of the patients were followed up after emergency treatment, 30.39% were hospitalized, and four patients died. CONCLUSIONS:Acute poisoning is largely alcohol poisoning and medication poisoning in a city. The emergency green channel "pre-hospital emergency care-emergency department-hospital treatment"can significantly improve the survival rate of patients with acute poisoning.

3.Changes of miRNA-21 in the peripheral blood in patients with chronic heart failure and its clinical significance

Objective To investigate the expression difference of microRNA -21 (miRNA-21) in patients with chronic cardiac function and analyze its clinical significance. Methods Patients with chronic heart failure (trial group,150 cases) and the subjects without chronic heart failure (control group,45 cases) were enrolled. Patients with chronic heart failure were divided into three subgroups according to NYHA: group A (heart function classⅡ,49 cases),group B (classⅢ, 51 cases) and group C (class Ⅳ, 50 cases). miRNA-21 levels were detected by qRT-PCR method. The levels of B type natriuretic peptide urea (BNP),left ventricular end diastole diameter (LVEDD) and left ventricular ejection fraction (LVEF) were determined. Results miRNA-21 expression in patients with heart failure were higher than that of control group (P < 0.01),and miRNA-21 expression in group C was higher than that of group A.Correlation analysis showed that there was a positive correlation between expression of BNP (r = 0.763,P < 0.01), LVEDD (r = 0.691,P<0.01), and the level of miRNA-21 in patients with chronic heart failure.And a negative correlation between LVEF value and the level of miRNA-21 (r = -0.918,P < 0.01). Conclusion miRNA-21 might be a potential marker for diagnosis of heart failure and could be a basis for reference about prognosis evaluation.

Objective Through the comparative study of the acute toxic effects of PM2.5 in nickel-contaminated area in rats to explore the possible mechanism of acute toxicity of PM2.5.Methods A nickel-contaminated area and a control area were selected,the air quality of the two areas was monitored and the concentration of nickel in the collected PM2.5 was determined. Forty two male Wistar rats(SPF Grade)were randomly divided into three groups,including one saline control group and two experimental groups,the nickel-polluted group and the non-polluted group,in each.There were three subgroups.PM2.5 were administered to rats by intratraeheal instillation at doses of 1.6 mg/kg,8.0 mg/kg and 40.0 mg/kg respectively.The rats were sacrificed 24 hours after treatment and the brochoalveolar tavage fluid(BALF)were collected,the phagocytic function of alveola macrophages of rats were tested.Results There was no significant difference between the level of environmental quality in the two areas,the concentration of Ni in PM2.5 in the nickel-polluted area was 48.75 times of that in the non-polluted control area.The phagocytic function of alveola macrophages of rats in the experimental groups decreased significantly(P
5.Effect of Electromyographic Biofeedback Therapy on Spastic Diplegia Children with Cerebral Palsy

Objective To investigate the curative effect of electromyographic(EMG) biofeedback therapy on spastic diplegia children with cerebral palsy.Methods42 children were divided into the treatment group(n=22) and control group(n=20) randomly.The treatment group was treated with rehabilitation training plus EMG biofeedback training,while the control group with rehabilitation training solely.Manual muscle test(MMT) was performed before and 3 months after treatment,and the treatment group was assessed by electromyographic apparatus additionally.ResultsAfter EMG biofeedback training,the muscular force of children in the treatment group was higher than those in the control group significantly(P<0.05) and the level of root mean square(RMS) and integrated electromyography(iEMG) assessed by surface electromyography(sEMG) increased significantly(P<0.05).ConclusionEMG biofeedback training can provide a satisfactory treatment for spastic diplegia children with cerebral palsy.Electromyographic apparatus is effective to assess muscular force.
6.Comparison of Producing Dendrolimus punctatus Cytoplasmic Polyhedrosis Virus in Substitutive Host Insect

Argyrogramma agnata has been selected as a substitutive host insect for producing Dendrolimus punctatus Cytoplasmic Polyhedrosis Virus (DpCPV). In our experiment, it is very susceptible to DpCPV. The DpCPV produced in A. agnata is designated Aa-DpCPV. The cytoplasmic polyhedra body (CPB), the virion size and the shape of Aa-DpCPV are same as that of its original DpCPV (DpCPV-W1984). The RNA bands of Aa-DpCPV and DpCPV-W1984 all have 10 RNA segments respectively in 3% PAGE, which molecular weights ranged in size from 2.98×106 to 0.66×106 Dalton. Aa-DpCPV has the same strong toxicity as that of DpCPV-W1984 (from D. punctatus) to D. punctatus (Walker) larva. So it can be applied to the pine caterpillar control. The DpCPV yield in A.agnata is 2.5×108CPB/larva.

Targeting induced local lesions in genomes (TILLING) is a reverse genetics method for functional genomics research.It is possible to screen for point mutations in the populations of EMS mutagenesis with highthroughput and lowcost. EcoTILLING a method based on TILLING ,was developed for detecting multiple types of polymorphisms in germplasm collections,such as single nucleotide polymorphism,small deletion and insertion etc.Rice is a very important food crop and a model plant for genome research also. There are complete genome sequence and a lot of other bioinformatics resources about it.So the markerassisted breeding is becoming more and more important in rice breeding. Some issues based on TILLING about identifying germplasm based on gene sequence,EMS mutagenesis breeding,developing functional marker in rice breeding in future were discussed.
8.Protective effect and action mechanism of petroleum ether extracts from Saussurea involucrate on brain tissues of hypoxia rats.

OBJECTIVETo investigate the protective effect and action mechanism of petroleum ether extracts from Saussurea involucrate on brain tissues of hypoxia rats under constant pressure and closed conditions.
METHODThe PESI dosage-dependent experiment for hypoxia rats was conducted under constant pressure and closed conditions by intraperitoneally injecting 125, 250, 500 mg x kg(-1) to finalize that the optimum dosage is the high dose of PESI. Afterwards, 90 Wistar rats were randomly divided into the hypoxic model group, the acetazolamide 250 mg x kg(-1) group and the PESI high dose group. Each group was further divided into three subgroups according to different hypoxia times, with 10 rats in each subgroup. Under the same hypoxia and administration conditions, the rats were sacrificed after 0, 3, 6 h respectively. Their brain samples were collected for common pathological observation and immunohistochemical staining of HIF-1alpha. Real-time RT-PCR was used to detect HIF-1alpha, EPO, HO-1 and Caspase-3 gene expressions. And the Western blot assay was adopted to detect HIF-1alpha protein expression.
RESULTThe brain tissues of the hypoxia model group were severely damaged with the increase in the hypoxia time. The acetazolamide group and the PESI high does group were damaged in a much lower degree. According to the gene expression and the Western blot assay, high dose of PESI could inhibit HIF-1alpha expression. According to the pure gene expression test, high dose of PESI could increase EPO and HO-1 mRNA expressions, but inhibit Caspase-3 mRNA expression.
CONCLUSIONPESI's protective mechanism for brain tissues of hypoxia rats under constant pressure and closed conditions may be related to its effects in inhibiting HIF-1alpha expression, increasing EPO expression and resisting cell apoptosis.

9.Study on the effect of treatment and the status of drug resistance among HIV/AIDS patients treated with highly active antiretroviral therapy in Shandong province.

OBJECTIVETo understand the effect of highly active antiretroviral therapy (HAART) on AIDS patients, and to explore the prevalence and the impact of HIV-1 drug resistance in Shandong province.
METHODS2 cross-sectional studies were carried on in 2004 and 2005, to collect data on clinical symptoms and compliance of the AIDS patients with HAART through questionnaire. Informed-consent principle was followed to test on immunological, viral and laboratory index of them. HIV-1 drug genotype resistance by sequencing the gene of HIV-POL after RT-PCR was performed and analyzed.
RESULTS31 AIDS cases with and. 27 AIDS cases without HAART, were studied. 83.3% and 64.5% of the AIDS patients with HAART showed that the CD4+ T cell count was rising to over 350/microl, in the first study (2004) and in the second (2005) study respectively but still 45.8% and 45.2% of AIDS patients under HAART in the 2 years showed a decreasing HIV load under the detected limit. However, these findings were showing remarkable difference when compared with the AIDS without HAART. 7 drug resistance gene sites were found in AIDS patients with HAART and in AIDS patients without HAART. The rate on high degree drug resistance mutation and total drug resistance rate of mutation of the former were higher remarkably than those of the latter.
CONCLUSIONMost of the AIDS patients with HAART met the purpose of rebuilding immunity and control of HIV,as well as alleviation of symptoms. Although the drug resistance stain appeared in Shandong,but had little effect on HAART. AIDS; Drug resistance; Highly active antiretroviral therapy

10.Compensation of diseased vessels and their collateral circulation in patients with ischemic stroke

Objective To observe the compensations of diseased arteries and their collateral circulation in patients with ischemic stroke and explore the relation between compensation of collateral circulation and classification of clinical symptoms (Oxfordshire Community Stroke Project [OCSP] classification) in total or severe infarction cases. Methods Based on OCSP classification, we performed digital subtraction angiography (DSA) on 211 patients with cerebral infarction to identify the existence of diseased arteries and their collateral circulations, and the appearance of their compensations. Results one hundred and ninety-eight patients were identified as having diseased arteries with a total number of 206, including 98 internal carotid arteries (ICAs), 54 middle cerebral arteries (MCAs), 27 vertebral arteries (VAs), 6 common carotid arteries (CCAs), 5 basilar arteries (BAs), 4 subclavian arteries (SCAs) and 4 other arteries. Leptomeningeal anastomose compensated 115 arteries, Willis circle compensated 98, and the other 46 were compensated by external-cranium arteries. Conclusions ICAs and MCAs are the firstly involved arteries in patients with cerebral infarction, followed by VAs. Anterior circulation arteries lesions arouse more cerebral infarctions than posterior circulation arteries lesions do. Compensation by Willis circle is the most sufficient type in ischemic stroke cases;while compensation by Leptomeningeal anastomose is the most frequent one. The classification of cerebral infarction is mainly affected by the compensations of diseased arteries and collateral circulation.
