20 first mandibular premolars were randomly divided into 2 groups(n =10).The teeth in experimental group were treated by fi-ber main post in combination with auxiliary pile,in control group by single fiber main post,and then were restored by metal crown.They were fixed in universal testing machine.The fracture load(N)of experimental and control group was (846.50 ±40.33)and (437.90 ±41.15) respectively(P <0.01).
3.Comparative study between off-pump and on-pump of coronary artery bypass grafting

Objective To compare the clinical effect of off-pump coronary artery bypass grafting (eft-pump CABG)and on-pump coronary artery bypass grafting(on-pump CABG).Methods The clinical dates of 152 cases with coronary artery bypass grafting(off-pump CABG group 85 cases,on-pump CABG group 67 cases)were received from June 2005 to December 2008,the preoperative dates,postoperative mortality and the rate of complication were compared between two groups.Results The preoperative dates of two groups were no significant difference(P>0.05);2 cases(2.35%)were died in off-pump CABG group and 13 cases(15.29%)had complications in off-pump CABG group;9 cases(13.43%)were died in on-pump CABG group and 25 cases(37.31%)had complications in on-pump CABG group.The mortality and the rate of complication in off-pump CABG group was significantly less than that in on-pump CABG group.it was significant difference(P<0.01).Conclusion Off-pump CABG is safe therapy of mieosurgieal operation.

5.Correlative factors analysis of affecting anatomical reattachment and vision restoration after scleral buckling surgery

Objeetive To analyze the effect of treatment of rhegmatogenous retinal detachment(RRD) by scleral bucking as well as the relative risk factors affecting the anatomical reattachment and visual recovery.Methods One hundred and fortyeight patients (148 eyes) with RRD treated by sclera buckling surgery in our hospital during January 2012 to January 2016 were retrospectively analyzed.The rate of postoperative retinal anatomic reattachment,the best corrected visual acuity (BCVA) and complications were observed.Logistic regression analysis was performed to analyze the correlative factors affecting the anatomical reattachment and postoperative vision restoration.R~ults Retinal reattachment achieved in 91.9% after initial surgery and the final success rate for anatomic reattachment was 97.3% assessed with ophthalmoscope and fundus photography.But these two rates were assessed with the optical coherence tomography (OCT) were 60.1% and 80.4% respectively.Single factor Logistic regression analysis showed that retinal detachment was affected by multiple breaks and Grade C1 PVR(all P <0.05);Single factor Logistic regression analysis showed that preoperative BCVA,course of disease,retinal detachment range,macular involvement or not had an impact on the postoperative recovery of BCVA (all P < 0.05),preoperative age,refractive status,releasing retinal fluid or not,intravitreal gas injection,combined scleral buckling,and postoperative subretinal fluid,all of these factors had no effect on BCVA recovery after surgery (all P > 0.05).And through multiple factors Logistic regression analysis,preoperative BCVA was an independent risk factor for BCVA recovery after surgery (P < 0.05).Conclusion Scleral bucking is an effective technique for managing RRD,but multiple breaks and Grade C1 PVR are significant risk factors for anatomic.Preoperative BCVA,course of disease,retinal detachment range,macular involvement or not have the impact on the BCVA recovery after scleral buckling,and the preoperative BCVA is the key factor.Early diagnosis and early treatment as well as protecting the preoperative visual acuity can improve prognosis.

Objective To study the relationship between central obesity and cardiovascular disease risk factors and aggregation in Yangpu District, and to provide scientific basis for the prevention and control measures and strategies for cardiovascular disease. Objective To explore the relationship between left ventricular diastolic function and the dynamic changes of myocardial ultrastructure and argyrophilic fiber in diabetic rats on the different periods of lesions(week 4,12 and 24).Methods The diabetes mellitus(DM)in healthy male Sprague-Dawley rats was induced by a single injection of streptozotocin(STZ,Sigma)into intraperitoneal at a dose of 65 mg/kg body weight.The left ventricular diastolic function was measured by Color Doppler Flow Imaging-Pulsed Wave(CDFIPW)and Doppler Tissue Imaging(DTI)echocardiography.Heart tissue at the apex was obtained rapidly for transmission electron microscope study.Argyrophilic staining was used in the study of argyrophilic fiber volume fraction(APFVF)in heart interstitial tissue.Results Diastolic dysfunction of left ventricle was detected in STZinduced diabetic rats by CDFI-PW(E/A < 1)at week 4,and progressed gradually.Pseudonormal filling (E/A > 1)was found in diabetic rats at week 24,which could be identified by DTI(Ea/Aa < 1).Diastolic function of normal rats was not impaired(E/A > 1 and Ea/Aa > 1).Transmission electron microscopy revealed a spectrum of subcellular remodeling in myocardium which was characterized by myofibril content decrease,disorganization,mitochondrial degeneration,sarcoplasmic reticulum,structural disorder.Compared with the control group,APFVF in myocardium was increased significantly in diabetic rats(P < 0.05).Conclusion The diastolic dysfunction in STZ-induced diabetic rats correlates with damage of ultrastructure and increase of myocardial argyrophilic fiber.
8.The protective effect of polysaccharide extracted from Laminaria japonica Aresch on vessels endothelial cell injury inducing by adrenaline.

AIMTo study the protective effect of Polysaccharide of Laminaria L01 on endothelial cell injury inducing by adrenaline.
METHODSIn order to observe the influence of L01 on the release of vWF in endothelial injured rats and HUVEC stimulated by adrenaline, a rat model of endothelial injury was established via injecting adrenaline, the damaged degree of vascular endothelial was evaluated by aortic immunity histochemistry, HUVEC was cultured in vitro, the content of vWF in rat plasma and in supernatant was measured by ELISA.
RESULTSThe measure of intact endodermis lengths (microm) stained by immunohistochemistry demonstrated the length in L01 high-dose group and low-dose group was obviously longer than that of model group (P < 0.05) in the 4th and 5th day during the model made. The content of vWF in rat plasma of L01 high-dose group was lower than that of model group (P < 0.05) in the 4th day, there were significant differences between this two groups, and the content of vWF in rat plasma of both L01 high-dose group and low-dose group was lower than that of model group (P < 0.05) in the 4th and 5th days. In the study of cultured HUVEC, on the 24 h, L01 groups (0.01 mg/ml and 0.1 mg/ml) decreased the supernatant vWF level, and on the 48 h, high-dose group (0.1 mg/ml) also decreased the supernatant vWF level, with significant difference compared with adrenaline group (10 microg/ml, P < 0.05).
CONCLUSIONL01 presented the protective effect on vascular endothelial cell.

9.Considerations on the construction and reform of maternal and child health service supervision system

One of the important part for ensuring maternal and child safety is to strength maternal and child health service supervision.A comprehensive supervision system for that guarantees its implementation.However, at this stage, the work faces the problems of insufficient effectiveness of laws and regulations, insufficient professional capacity of health supervisors, and insufficient supervision of emerging issues in maternal and child health service.With the implementation of "double random" spot check model in post-event supervision, primary health supervisors are facing some difficulties with the professional tasks.Combined with the situation, Shanghai municipal health supervision department established a key control point monitoring model for maternal and child health services, improving the training efficiency of primary health supervisors, offering guidance for health supervisors, which provides useful exploration in building and reforming the maternal and child health service supervision system.
10.Risk factors for 302 hepatic malignancies cases with residual tumor after ultrasound-guided radiofrequency ablation treatment

Background and purpose: Radiofrequency ablation (RFA) is one of the effective treatment methods for primary liver cancer and metastatic liver cancer. The purpose of this study was to investigate the risk factors of residual tumor after RFA for hepatic malignancies. Methods:A total number of 302 hepatic malignancies cases with 691 tumors after ultrasound-guided RFA from Jan. 2010 to Mar. 2013 were retrospectively analyzed. Single factor and multi-factor Logistic regression model were used to analyze the risk factors of residual tumor after RFA. Results:Complete ablation cases were 90.07%(272/302) for patients and 91.46%(632/691) for tumors, and the ablation residual rate was 8.54%. Ablation residual rates for tumor ≤3 cm, 3-5 cm and >5 cm in diameter were 6.30%, 9.57% and 28.57%, for tumor close to the intrahepatic vascular and gallbladder were 17.14%and 18.52%, for with and without combination with other local treatments were 7.02%and 13.41%, respectively. Multivariate analysis showed that tumor size>5 cm (P=0.044), proximity to large vessel (P=0.039) and without combination with other local treatments (P=0.001) were independent risk factors for ablation residual. Multivariate analysis showed that tumor near the intrahepatic vascular (P=0.014), single needle RFA (P=0.047) and without combination with other local treatments (P=0.023) were independent ablation residua risk factors for tumors between 3-5 cm in maximum diameter. Conclusion:Ultrasound-guided RFA can achieve satisfactory ablation effect. Tumor close to the intrahepatic vascular, tumor diameter>5 cm and without combination with other local treatment act as the independent risk factors for ablation residual. For tumors between 3-5 cm in diameter, in addition to close to intrahepatic blood vessels and without combination with other local treatment, single needle RFA is also another independent risk factor for ablation residual, and double-needle or multi-needle treatment can improve the ablation efifciency and reduce residual rate.
