2.Clinical effect of bicyclol tablet on hepatic lesions caused by Epstein-Barr virus infetc ion in children

Objective To evaluate the efficacy and safety of bicyclol tablet on hepatic lesions caused by Epstein-Barr virus( EBV) infection in children.Methods A single-center controlled retrospective study was conducted in 121 children with hepatic lesions caused by EBV infection for evaluating safety,tolerability, and efficacy of treatment with bicyclol tablets or Glycyrrhizin capsules.Childer n in bicyclol group ( n=63 ) were treatedw ith bicyclol at blets and cotn rol group ( n =58 ) were treated with Glycyrrhizin capsules.The course of the treatment were both 8 weeeks for two groups.The level of the EBV load pretreatment and plas-ma aminotransferase,blood routine,urine routine pretreatment and 1 week,4 weeks and 8 weeks after treat-mentw ere analyzed er trospectively.Rse ults (1) The pal smaA LT level sigin ficantly decreased in theb icy-clol group compared with that in the contro l group(P<0.01), se pecai lly the levle after 8 weeks treatm ent. (2) Bicycol l was more effce tive in the bicyclol group than Glycyrrhizin capslu se in the control group( P<0.01).(3) Both grousp had no significantlya dvesr e events.Conclusion Bicyclol tablet can derc ease plas-ma aminotransferase level,espce ially ALT,inc hildren caused by EBV infection with better efficiency and safety.
3.Apparent Diffusion Coefficient Histogram Analysis:Differentiation of Genetic Subtypes of Diffuse Lower-grade Gliomas.

To explore the utility of apparent diffusion coefficient(ADC)histogram analysis for differentiating genetic subtypes of diffuse lower-grade gliomas. A total of 55 patients with WHO grade Ⅱ/Ⅲ diffuse lower-grade gliomas who underwent preoperative routine brain magnetic resonance imaging and diffusion weighted imaging in our center were retrospectively evaluated.Among whom there were 14 patients with isocitrate dehydrogenase(IDH)wild-type gliomas(IDH group),19 patients with IDH-mutant 1p19q intact gliomas(IDH 1p19q group),and 22 patients with IDH-mutant 1p19q co-deleted gliomas(IDH 1p19q group).The whole-lesion ADC values derived from histogram analysis(including ADC,ADC,ADC5%,ADC10%,ADC25%,ADC50%,ADC75%,ADC90%,ADC95%,ADC,mode,range,skewness,kurtosis,standard deviation,inhomogeneity,and entrophy)were measured for each patient.All parameters between the different genetic subtypes were compared by using the Student's test or Mann-Whitney test.Receiver operating curve(ROC)analysis was used to assess the diagnostic performance of ADC histogram in distinguishing the different genetic subtypes. Compared with IDH group,the ADC75%(=0.021),ADC90%(=0.015),ADC95%(=0.014),ADC (=0.035),range(=0.009),standard deviation(=0.001)and inhomogeneity(=0.001)were significantly lower in IDH group;in contrast,the ADC (=0.031)and kurtosis(=0.020)of IDH group were significantly higher than those in IDH group.The ADC(=0.010),ADC5%(=0.016),ADC10%(=0.012),ADC25%(=0.007),ADC50%(=0.005),ADC75%(=0.015),and mode(=0.002)were significantly higher in IDH 1p19q group than in IDH 1p19q group.Inhomogeneity achieved the highest area under ROC(AUC)(0.811)in differentiating IDH gliomas and IDH gliomas,with a cutoff value of 0.229;the sensitivity and specificity were 85.7% and 73.2%.The mode achieved the highest AUC(0.744)in differentiating IDH 1p19q gliomas and IDH 1p19q gliomas,with a cutoff value was 1448.75×10 mm /s;the sensitivity and specificity were 57.9% and 90.9%. ADC histograms analysis may be helpful to differentiate genetic subtypes in lower-grade gliomas.

Objective To discuss the synergistic mechanism of compatibility of Tripterygium Wilfordii and Astragalus,and to determine wilforlide A before and after compatibility.Methods To research Tripterygium Wilfordii single medicinal materials,Tripterygium Wilfordii compatibility of Astragalus and Tripterygium Wilfordii reference substance using the HPLC method,so as to provide further verification by the change of wilforlide A.Results The content of wilforlide A was 0.00 324% by measuring Tripterygium Wilfordii single medicinal materials.The content of wilforlide A was 0.00 184% by measuring Tripterygium Wilfordii compatibility of Astragalus.Conclusion The content of wilforlide A was decreased before and after compatibility and toxicity can be reduced.The synergistic mech-anism of the compatibility of Tripterygium Wilfordii and Astragalus is conteract the toxicity of another drugin posse.
5.Influencing factors for complete resection and operation time in endoscopic submucosal dissection for colorectal tumors

Objective To explore influencing factors for complete resection and operation time of endoscopic submucosal dissection( ESD) for colorectal tumors. Methods This retrospective study included 95 consecutive colorectal tumors in 88 patients whose pathological diagnosis was adenoma and carcinoma, treated with ESD at the Department of Endoscopy of the First Hospital of Jilin University from January 2013 to December 2014. Multiple logistic regression analysis was conducted on the factors related to complete resection and operation time. Results Average tumor size was 28. 7±14. 1 mm(range,8?80 mm), and the average procedure time was 80. 72±63. 90 min. The rate of complete resection was 92. 6%(88/95),and the rate of incomplete resection was 7. 4%(7/95). Multivariate logistic regression analysis revealed that fibrosis (P=0. 012,OR=52. 473, 95%CI:2. 571?1140. 438) contributed to incomplete resection. Fibrosis ( P=0. 001, OR=0. 045, 95%CI:0. 007?0. 289) ,tumor size ( P=0. 035,OR=0. 170, 95%CI:0. 033?0. 884) ,granular?type laterally spreading tumor ( P=0. 013, OR=34. 432, 95%CI:2. 138?554. 476 ) , non?granular?type laterally spreading tumor(P=0. 044,OR=31. 715, 95%CI:1. 093?919. 904) were independent factors for extending operation time of colorectal ESD. Conclusion The severer fibrosis can induce higher rate of incomplete resection. The more severe fibrosis is, the larger tumor size is, and the longer operation time is.

Objective To investigate the differences in dosimetry between conformal radiotherapy (CRT),field-in-field intensity-modulated radiotherapy (FIF-IMRT),and intensity-modulated radiotherapy (IMRT) after breast-conserving surgery for left-sided breast cancer.Methods A total of 31 patients who underwent breast-conserving surgery for left-sided breast cancer were randomly selected,and the plans for CRT,FIF-IMRT,and IMRT were developed.The dose-volume histogram (DVH) was used for self-control study,and the non-parametric test was used to compare the differences in target volume and doses to organs at risk (OARs).Results All the three methods met the requirements of the prescribed doses.The CRT group had a higher V105 of the target volume and higher heart V30 and Dmax (P=0.000,0.000,0.000).The IMRT group had higher V5 and Dmean (P=0.000,0.000),as well as a higher lung V5 and a lower lung V40 (P=0.000,0.000).The FIF-IMRT group had the lowest Dmean (P=0.000),and the IMRT group had significantly higher Dmean and Dmax of the right lung and the spinal cord than the other two groups (P=0.000,0.000,0.000,0.000).The FIF-IMRT group had a significantly lower single hop count than the other two groups (P=0.000).Conclusions CRT has a good dose distribution in the target volume,but greatly injures the surrounding tissues.FIF-IMRT can well protect OARs and cause less damage to the device.
7.Observation and analysis of malposition of catheter during PICC at different body positions

Objective To study the incidence of malposition of catheter at different body positions during the operation of peripherally inserted central catheter (PICC),and then select the optimum position to increase the success rate of insertion. Methods 155 patients were randomly divided into the horizontal position group (55 cases),the semi-reclining position group (49 cases) and the sitting position group (51cases).They all used chest X-ray to conffirm the position of top end of the catheter after insertion and then comparing the incidence rate of malposition of catheter among three groups. Results After insertion,11 patients' catheters were found malposition.Among them,8 patients were in the semi-reclining position group (incidence rate 16.3%),2 patients were in the horizontal position group (incidence rate 3.6%) and only one patient was in the sitting position group (incidence rate 1.9% ).Incidence rate of catheter malposition in the semi-reclining position group was significantly higher than other two groups. Conclusions The best position for PICC is the horizontal position.If the patient has expiratory dyspnea or need to adjust the malposition catheter,sitting position can be used.Semi-reclining position should be avoided in the operation of PICC.
8.HRCT diagnosis of Goodpasture syndrome

Objective To analyze the imaging features and follow-up changes of high-resolution CT(HRCT) in Goodpasture syndrome.Methods HRCT imaging features and follow-up findings of 15 cases Goodpasture syndrome confirmed by clinical were analyzed retrospectrively.The imaging features included extent,forms and follow-up changes.Results The lung lesion of Goodpasture syndrome involved two lobes(n=1), three lobes(n=2), four lobes(n=5) and five lobes (n=7).Upper lobe of the right lung was the most common involved region.Centered on the hilum of lung consolidations confused ground glass opacity (GGO) were showed in 7 cases, GGO distribution of pulmonary leaflets in 5 cases.On follow-up observation, lobar or segmental consolidation could change into GGO,GGO could disappear in short times.Conclusion Multiple lobar or segmental consolidations confused GGO without the lung bottom and periphery involvement is the imaging characteristics of Goodpasture syndrome patients with anemia and hemoptysis.HRCT is a helpful method for the diagnosis and following up of Goodpasture syndrome.

Objective To explore the characteristic and clinical value of Somatosensory Evoked Potential SEP and event?related potential P300 in patients with Juvenile Myoclonic Epilepsy (JME), trying to provide neuroelectrophysiological evidence for the pathogenesis, diagnosis and antiepileptic drug efficacy of this disease . Methods 28 patients with JME and 30 healthy controls were enrolled in the research. They were applied the SEP and P300 before and after one year treatment. Results (1) The amplitude of SEP in patients with JME was significantly higher than those in the control group(P=0.000), the abnormal rate was 75%(21/28), while the latency of SEP was no significant difference (P>0.05); The latency of P300 in patients with JME was significantly longer (P = 0.000), 2 out of 28 cases were poorly differentiated, 9 cases were longer (11/28, 39%), while the P300 amplitude was no significant difference(P=0.110). (2) After treatment, the amplitude of SEP was significantly lower (P<0.05), the latency of P300 was significantly shorter (P=0.001). (3) The amplitude of SEP had a positively linear relationship with the latency of P300 (r = 0.818, P = 0.000). Conclusions The SEP amplitude was significantly higher in patients with JME, often accompanied by a huge SEP, so SEP can provide electrophysiological evidence for the pathogenesis and diagnosis of this disease.And P300 may give a chance to find the subclinical cognitive abnormalities and to intervent it early. The SEP had a positively linear relationship with the P300, simultaneously dynamic monitoring the change of SEP amplitude and P300 latency may become an objective evaluation for the antiepileptic drug efficacy.
