Objective To evaluate the therapeutic effects of metformin hydrochloride and rosiglitazone maleate tablets on insulin resistance and androgen concentration in obese and nonobese women with polycystic ovarian syndrome (PCOS).Methods Seventy women with PCOS were enrolled into this study according to the diagnostic criteria of Rotterdam conference (2003).The women with PCOS were divided into the over weight/obese group (body mass index ≥ 25 kg/m2,n =34) and the nonobese group (body mass index<25 kg/m2,n =36).All women with PCOS took metformin hydrochloride and rosiglitazone maleate tablets twice daily for 3 months.The following indexes were measured before and after the treatment:height,weight,waist circumference,hip circumference,fasting plasma glucose,fasting plasma insulin,luteinizing hormone (LH),follicle stimulating hormone (FSH),total testosterone,free testosterone,follicular number and ovarian area,body mass index,waist to hip ratio,and homeostasis model assessment for insulin resistance (HOMA-IR) were calculated.Results (1) Compared with the nonobese group,dramatic increases in HOMA-IR and follicle number were observed in over weight/obese group(P<0.05),while LH,LH/FSH,total testosterone,free testosterone showed no significant difference between two groups.(2) After the treatment with metformin hydrochloride and rosiglitazone maleate tablets,weight,body mass index,fasting plasma glucose,fasting plasma insulin,HOMA-IR significantly decreased in two groups (P < 0.05).There were also significant declines of LH,LH/FSH,free testosterone,ovarian area,follicular number,especially in the over weight/ obese women with PCOS (P<0.05),while total testosterone and FSH did not change significantly.(3) The incidence of gastrointestinal adverse reactions in nonobese and over weight/obese group was 9.3% and 6.3% respectively,and no liver toxicity,edema,hypoglycemia and other adverse reactions was observed during treatment.Conclusion Compared with the nonobese group,there were severer insulin resistance and more marked polycystic change of ovary in over weight/obese women with PCOS,while total testosterone and free testosterone showed no significant difference between two gruops.Metformin hydrochloride and rosiglitazone maleate tablets can ameliorate insulin resistance and polycystic ovary,and also reduce body mass index and free testosterone in women with PCOS particularly in the over weight/obese individuals.

Human immunodeficiency virus (HIV) is the infectious agent causing acquired immunodeficiency syndrome (AIDS), a deadliest scourge of human society. Hepatitis C virus (HCV) is a major causative agent of chronic liver disease and infects an estimated 170 million people worldwide,resulting in a serious public health burden. Due to shared routes of transmission, co-infection with HIV and HCV has become common among individuals who had high risks of blood exposures. Among hemophiliacs the co-infection rate accounts for 85%; while among injection drug users (IDU) the rate can be as high as 90%. HIV can accelerate the progression of HCV-related liver disease, particularly when immunodeficiency has developed. Although the effect of HCV on HIV infection is controversial,most studies showed an increase in mortality due to liver disease. HCV may act as a direct cofactor to fasten the progression of AIDS and decrease the tolerance of highly active antiretroviral therapy(HARRT). Conversely, HAART-related hepatotoxicity may enhance the progression of liver fibrosis.Due to above complications, co-infection with HCV and HIV-1 has imposed a critical challenge in the management of these patients. In this review, we focus on the epidemiology and transmission of HIV and HCV, the impact of the two viruses on each other, and their treatment.
7.Study on cytokine expression profiles of CIK cells in vitro

Objective:To investigate the cytokine profile expressed by CIK cells.Methods:CIK cells were induced from peripheral blood mononuclear cells in the presence of IFN-?,IL-1?,IL-2 and mAb against CD3. The phenotype and characterization of CIK cells were identified by flow cytometric analysis.Cytokine expression profiles were determined at mRNA level by semi-quantitative RT-PCR.Results:The percentage of CD3~+CD56~+ positive cells reached up to or higher than 50%.CIK cells couldn't express IFN-?、IFN-? and IFN-? under this kind of condition. But they could express IFNs principally on day 10 to 20. The level of ILs went down after steadily expressed on day 6 to 10(or 20).TNF-? and TRAIL constitutively expressed during the culture period in vitro. TNF-? maintained high expression on day 20.TGF-?_1 mainly expressed on day 30.Conclusion:CIK cells can widely express various kinds of cytokines in vitro.
8.The Effect of Rosiglitazone on Glycemia and ?-Cell Function in Type 2 Diabetic Patients

Objective To investigate the effects of rosiglitazone (RSG) on glycemia and ?-cell function in type 2 diabetic patients. Mothods Open-label and self-match trial was carried out. 31 cases of type 2 diabetic patients were divided into RSG monotherapy group (n=13), RSG plus metformin group (n=9), and RSG plus sulphonylureas group. All the patients received treatment for 12 weeks. Weight, body mass index, FBG, FINS, blood pressure, TG, Tch, HDL, LDL, ALT, AST and TBIL were determined before and after therapy. The insulin resistance (HOMA-IR) and ?-cell function (HOMA-?) of the patients were evaluated using homeostasis model assessment (HOMA). Results Compared with before treatment, there was a significant improvement in FBG, FINS, HOMA-? and HOMA-IR in all the patients after treatment (FBG 8.89?2.49mmol/L, FINS 17.89?5.78mU/L, HOMA-? 79.75?41.28,HOMA-IR 7.15?3.40 vs FBG 7.27?2.26mmol/L, FINS 14.33?3.82mU/L,HOMA-? 120.84?140.66,HOMA-IR 4.63?1.81, respectively,P
9.Investigation on cognition of hospital staff for consent of patients

ObjectiveTo investigate the condition of hospital staff in surgery and internal medicine department in a specified hospital for consent of patients. MethodsThis study included 50 hospital staff in surgery and internal medicine department in specified hospital at the second level in Chongqing city in May 2010. We also analyzed the dealership of children and psychopath, consent of patients or relatives for choosing hospitals. ResultsThe ratios of cognition of hospital staff for consent of patients were 68% and 46%, which had statistical difference ( P=0.026 ). The main ways were radio and television. About 97% hospital staff knew that it's hospital staff s duty to inform relative information to patients. ConclusionMedical legislation department and medical institutions should enhance the study of hospital staff on relative laws and regulations. Thus hospital staff have a in-depth understanding on information of consent of patients.
10.CPT-11 transarterial chemoembolization prolongs survival of patients with unresectable hepatocellular carcinoma

Objective To study the efficacy of CPT-11 TACE in the treatment of unresectable HCC.Methods A retrospective review was undertaken on unresectable HCC patients receiving doxorubicin transarterial chemoembolization (59 cases) and irinotecan(CPT-11) in 24 cases from May 2003 to November 2011.Survival analysis was performed with Kaplan-Meier statistics.Differences in survival between the two groups were assessed for statistical significance with the log-rank test.Results Overall survival time was significantly longer in patients treated with CPT-11 compared with doxorubicin treated group (21.7 vs 14.5 months,P =0.042).There was no significant difference in time to progression between the two groups,but time to progression was longer in CPT-11 group than doxorubicin treated group (11.42 vs 9.46 months,P =0.091).Subgroup analysis showed that for intermediate-stage HCC,CPT-11 resulted in a significantly longer time to progression and overall survival compared with doxorubicin treated group (P =0.029 and P =0.014,respectively).There were no significant differences in adverse events among the two groups (P > 0.05).Conclusions Chemotherapeutic agent CPT-11 in the form of TACE significantly improved overall survival when compared with doxorubicin for the treatment of unresectable HCC.
