2.Changes of the ocular surface and tear film after the coaxial incision 2.5mm and 3.0mm in the phacoemulsification

AIM:To study the changes of tear film and ocular surface after the coaxial incision 2.5mm and 3.0mm in the phacoemulsification.METHODS: Seventy patients (92 eyes) from February 2015 to February 2016 in our hospital were enrolled.The patients were randomly divided into two groups.Thirty-four patients (46 eyes) in Group A: coaxial 2.5mm incision phacoemulsification cataract extraction and intraocular lens(IOL) implantation.Thirty-six patients (46 eyes) in Group B: the conventional coaxial 3.0mm small incision phacoemulsification cataract extraction and IOL implantation.The break up time (BUT),Schirmer`s Ⅰ test (SⅠt) and lid-parallel conjunctival folds (LIPCOF) were assessed preoperatively and postoperatively.RESULTS: At 1wk,1 and 3mo postoperatively,the BUT in two groups decreased after operations,and the BUT of Group B was significantly lower than those of Group A,the differences were statistically significant (t=3.089,4.497,4.322;all P<0.05).The SⅠt in two groups increased after operations,the SⅠt of Group B was significantly higher than those of Group A and the differences were statistically significant (t=-6.286,-3.992,-3.278;all P<0.05).The LIPCOF score in two groups increased after operations,the LIPCOF score of Group B was significantly higher than those of Group A and the differences were statistically significant (t=-6.552,-5.131,-3.652;all P<0.05).At 6mo postoperatively,compared with the preoperative data,the BUT,SⅠt and LIPCOF score in Group A showed no statistically significant differences (t=0.646,0.551,0.165;P>0.05).The BUT,SⅠt and LIPCOF score in Group B showed statistically significant differences (t=-4.059,3.629,4.211;all P<0.05).CONCLUSION: Ocular surface has less damage and tear film has little influence at early stage after the coaxial 2.5mm incision phacoemulsmcation,compared with the conventional coaxial 3.0mm incision phacoemulsification surgery.
3.Effects of Preventive-electroacupuncture and Preventive-moxibustion at Guanyuan (RN4) on Hypothalamus-Pituitary-Ovary Axis in Ovariectomized Rats

Objective To observe the regulatory effects of the preventive acupuncture and moxibustion at Guanyuan (RN4) points in ovariectomized rats, and discuss the mechanism of preventive acupuncture and moxibustion on hyoathalamus-pituitary-ovary (HPO) axis. Methods Totally 48 female SD rats were divided into 5 groups, including normal group, sham operation group, ovariectomized rats group, preventive acupuncture group and preventive moxibustion group. Except normal group and sham operation group, rats were ovariectomized after preventive acupuncture and preventive moxibustion for 30 d in corresponding groups. ELISA was used to determine the levels of E2, ER, GnRH, FSH and LH in the tissues including hypophysis, hypothalamus and uterus. Results Compared with the normal group, E2, ER and GnRH level in the tissue of the model group was significantly lower (P<0.01), the levels of FSH and LH increased obviously (P<0.01). Compared with the model group, E2, ER and GnRH level of preventive acupuncture group and preventive moxibustion group increased in different degree (P<0.05). FSH and LH level reduced in different degree (P <0.05). There was no obvious difference between preventive acupuncture and preventive moxibustion group (P>0.05). Conclusions The preventive acupuncture and preventive moxibustion at Guanyuan (RN4) has benign modulation to disordered HPO axis of ovariectomized rats. The ability of preventive acupuncture and preventive moxibustion are almost the same.
4.Clinical features and antimicrobial resistance of lower respiratory tract in-fection with Streptococcus pneumoniae isolated from children

Objective To explore clinical features and antimicrobial resistance of lower respiratory tract infection (LRTI)with Streptococcus pneumoniae (S .pneumoniae)isolated from children in Nanjing.Methods Clinical data of children with confirmed S .pneumoniae LRTI through sputum culture at a children’s hospital in Nanjing between July 2013 and June 2014 were analyzed retrospectively,S .pneumoniae strains were performed antimicrobial susceptibility testing through K-B method and minimum inhibitory concentration (MIC)testing.Results Among 197 children with S .pneumoniae infection,72.59% were <3 years old,63.96% occurred in autumn and winter, 57.87% had elevated leukocyte count,cough and fever were the most common clinical symptoms,complications of digestive and circulatory system were also common.The resistance rates of S .pneumoniae to azithromycin, penicillin,and erythromycin were 94.92%, 92.98%, and 88.83% respectively; the sensitivity rates to vancomycin,chloramphenicol,meropenem,ceftriaxone,and ofloxacin were all > 90%,vancomycin was up to 98.98%.Conclusion The percentage of S .pneumoniae LRTI is high in children < 3 years old,most occur in autumn and winter,resistance rates to azithromycin,penicillin,and erythromycin are all high,antimicrobial agents should be selected for the treatment of infection according to antimicrobial susceptibility testing.
6.Case-control study on treatment of distal tibiofibula fractures with modified anterolateral or traditional approach.

OBJECTIVETo explore therapeutic effects of modified anterolateral approach and traditional approach in treating distal tibiofibula fractures.
METHODSFrom September 2009 to December 2012,45 patients with distal tibiofibula fractures were performed reduction and interal fixation with modified anterolateral and traditional approaches. According to Ruedi-All-gower classification, 12 cases were type I ,26 cases were type II, 7 cases were type III. Among 45 patients,there were 16 males and 7 females with an average age of (36.3 +/- 7.2) years old (ranged from 23 to 47) years old in advanced group; while there were 14 males and 8 females with an average of (33.8 +/- 6.4) years old (ranged from 25 to 45) in tradtional group. Operation time, blood loss, complications of incision were compared, and AOFAS scores were evaluated.
RESULTSAll patients were followed up from 6 to 36 months with an average of 21.7 months. In traditional group, operation time was (74.7 +/- 9.9) min, blood loss was (94.4 +/- 10.4) ml, and (73.7 +/- 10.0) min, (100.8 +/- 12.1) ml in advanced group,there was no significant meaning between two groups (P > 0.05); complications of insicion of advanced group was better than that of traditional group (chi2 = 4.078, P < 0.05); AOFAS score in advanced group (86.1 +/- 9.4) was better than that of traditional group, and had significant meaning (t = 3.787, P < 0.05).
CONCLUSIONModified anterolateral group,which has advantages of rapid recovery, less complications, is a good choice for closed distal tibiofibula fractures.

7.Clinical study of endostar combined with FOLFOX as a second line chemotherapy for advanced colorectal cancer

Objective To observe prospectively and systematically the effect and safety of rhendostati injection (endostar) combined with FOLFOX as a second line chemotherapy for advanced/metastatic colorectal cancer. Methods 23 patients with histological confirmed advanced/metastatic colorectal cancer after first line chemotherapy failure were observed. The dosage of 15 mg/time of endostar solved in 500ml normal saline was slowly intravenously dropped 4 h from day 1 to day 14. Oxaliplatin 85 mg/m~2 iv 2-3 h dl, d15. CF 200 mg/m~2 iv 2 h followed by 5-Fu 400 mg iv bolus and 5-Fu 600 mg/m~2 iv 22 h dl-2, d15-16 were given, every 4 weeks as one cycle. Efficacy was evaluated after 2 cycles according to RECIST criteria. Results 23 cases had been completed totally 56 cycles. Among 23 cases, 8 cases were PR, 12 cases SD, and 3 cases PD. The objective response rate (RR) was 34.8 % (8/23), and the disease control rate (DCR) was 87.0 % (20/23). The median time to progression was 7 months. The 1-year survival rate were 50.0 %. The 2-year survival rate was 40.0 %. The occurrence rate of G3/4 toxicities was low, including neutropenia(21.7 %), anemia(4.3 %), thrombocytopenia (13.0 %). Those toxicities were mainly related with the chemotherapy agents. Meanwhile transient electrocardiogram changes mild ST-T of changes occurred in 3 cases. 2 cases were mild hypertension and were symptomatically controlled. Conclusion There are better efficacies of endostar combined with FOLFOX chemotherapy for advanced/metastatic colorectal cancer, and it is low toxic and tolerable. It is worth of further clinical observation. More experiences need to be accumulated.
8.Comparison of therapeutic efficacy of different chemotherapeutic regimens on combined small cell lung cancer

Objective:To compare the therapeutic and adverse effects of chemotherapeutic regimen based on three drugs (taxol+carboplatin/cisplatin+etoposide) and two drugs (carboplatin/cisplatin+etoposide) on the combined small cell lung cancer (CSCLC). Methods:A retrospective study was conducted based on the data of 62 CSCLC patients who were admitted to and treated at Tianjin Medical University Cancer Institute and Hospital between July 2000 and April 2013. Of the 62 patients, 19 received the three-drug regi-men and 43 received the two-drug regimen. All patients received at least two cycles of chemotherapy and completed follow-up proce-dures. For each patient, the therapeutic efficacy was evaluated every two cycles, and toxicity was evaluated every cycle. Results:The response rates between the three-drug and two-drug groups were statistically significant (90%vs. 53%, P=0.033). However, no statisti-cal differences were observed in the disease control rate between the two groups (100% vs. 86%, P=0.212). The three-drug regimen could induce a better median progression-free survival compared with the two-drug regimen, but with no statistical significance (10.5%vs. 9.8%, P=0.484). Similarly, no statistical differences were noted in the median overall survival between the three-drug and two-drug groups (24.0%vs. 17.5%, P=0.457). The incidence rates of grade IV bone marrow depression and the termination of the original regi-men owing to severe adverse reactions were both significantly higher in the three-drug group than in the two-drug group (26.3% vs. 7.0%, P=0.036;31.6%vs. 14.7%, P=0.004). Conclusion:The two-drug regimen had almost the same survival rate and lower toxicity compared with the three-drug regimen. When using the TEP/TCE regimen, a close attention should be focused on its adverse reactions. The findings of this work showed that the two-agent regimen should be one of the standard treatments for CSCLC.
9.Micro-PET Observation of the Effect of Electroacupuncture on Cerebral Hippocampus Glucose Metabolism in SAMP8 Mice with Senile Dementia

Objective To observe cerebral hippocampus glucose metabolism using 18F-FDG micro-PET as an imaging technique in a normal group of mice, and model and electroacupuncture groups of SAMP8 mice with senile dementia (rapid aging mice). Method Four 6-month-old SAMR1 mice were picked at random out of the normal group and every four 6-month-old SAMP8 mice, out of the model and electroacupuncture groups respectively. Each of them was anesthetized by inhalation of 2% isoflurane and then given a bolus injection of radioactive tracer 18F-FDG 14.8~16.5 MBq through the tail vein. After one hour of the uptake, a PET scan was performed for 10 min. Hippocampus 18F-FDG uptake rate per gram of brain was calculated in every group of mice and the uptake rates were compared between the groups.Result The 18F-FDG uptake rate per gram of brain tissue was higher in the electroacupuncture group of mice than in the normal and model groups.Conclusion Electroacupuncture can markedly increase cerebral 18F-FDG uptake and the uptake rate per gram of brain tissue in SAMP8 mice. It may play a neuroprotective role through its influence on cerebral glucose metabolism.
10.Curative effect of Ganciclovir combined with interferon-α1b inhalation in treating children with infectious mononucleosis

Objective To investigate the clinical efficacy and safety of Ganciclovir combined with interferon-α1 b inhalation for children with infectious mononucleosis(IM).Methods A total of 177 childhood cases of IM were selected,and they were divided into 3 groups,59 cases in each group according to the random number table.Three therapeutic methods were applied in different groups for 5-7 days in different groups:Ganciclovir (group A),Ganciclovir + interferon-α1 b inhalation (group B) and Ganciclovir + interferon-α1b intramuscularly (group C).The time of post-drug recovery from isthmitis,less than 0.05 of heterotypic lymphocytes,shrink of cervical lymph nodes shrink,liver retraction,spleen retraction among groups were compared.The Epstein-Barr virus (EBV)-DNA copy number and T lymphocyte subsets were compared before and after treatment.Adverse reactions were observed in each group.Results Compared with group A,the time to defervescence [(3.20 ± 1.81) d,(3.17 ± 1.76) d vs.(4.01 ± 2.34) d],duration of isthmitis was [(3.15 ± 1.33) d,(3.09 ± 1.37) d vs.(3.98 ± 1.31) d],and the time of heterotypic lymphocytes less than 0.05 [(3.12 ± 1.55) d,(3.10 ± 1.33) d vs.(3.95 ± 1.26) d] in group B and group C,were obvious shorter,and there were significant differences(F =4.150,4.580,4.060,all P < 0.05).EBV-DNA negative conversion rate of group B and group C were higher than that of group A [53 cases(89.8%),52 cases (88.1%) vs.41 cases (69.5%),x2 =10.403,P < 0.05],and the cellular immune function was improved significantly than that of group A after treatment for 7 days [CD3 +:(63.00 ±4.39)%,(62.75 ±4.84)% vs.(68.70 ± 7.70)%;CD4+:34.08(30.21,41.70)%,33.94(29.17,45.17)% vs.32.34(28.16,43.53)%;CD8+:30.59 (27.14,40.22)%,30.09(27.54,40.48)% vs.32.57(28.68,41.17)%;CD4+/CD8+:1.12(1.03,1.31),1.11 (0.99,1.64) vs.0.94 (0.87,1.59),F/x2 =11.020,1.217,1.121,6.728,all P < 0.05].The differences in indexes between B group and C group were not significant,and there was no statistical significance (all P > 0.05).There were 2 cases with fever in the group C,and 2 cases of granulocytopenia in all group.Conclusions Ganciclovir combined with interferon-α1 b inhalation or intramuscular injection is effective and safe in treating children with IM.It can improve clinical symptoms,cellular immune function and EBV-DNA negative conversion rate.Since inhalation is of less side effects and no pain,it can be accepted by children and their parents easily.Therefore,it is recommended that Ganciclovir be used together with interferon-α1 b inhalation in the treatment of children with IM.
