2.Clinical Observation on 30 Stroke Patients with Obstructive Sleep Apnea Hypopnea Syndrome Treated by Tongsai Xiaoshuan Decoction

Objective To observe the effect of Tongsai Xiaoshuan Decoction on the treatment of 30 stroke patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods 30 patients were treated with Tongsai Xiaoshuan Decoction. 28 patients treated regularly were served as controls. Both group were performed CT/MRI and Polysomnography examination before and after treatment. Their apnea and hypoventilation index were recorded. Results The efficiency of Tongsai Xiaoshuan Decoction was evident. The apnea and hypoventilation index were decreased, the medial hypopnoea time decreased, and the medial time for SaO2 less than 90% decreased. 3.Effects of Responsible Pharmacists on Irrational Medical Order Intervention in Pharmacy Intravenous Ad-mixture Service

OBJECTIVE:To probe the role of responsible pharmacist on irrational medical order intervention in Pharmacy Intra-venous Admixture Service (PIVAS),and to improve rational drug use in our hospital. METHODS:501 115 medical orders from PIVAS of our hospital during Jan.-Jun. 2012 were analyzed retrospectively. Irrational medical orders of 499 189 medical orders dur-ing Jul.-Dec. 2013 were classified and summarized after the establishment of responsible pharmacists system and pharmacist inter-vention. RESULTS:The number of irrational medical orders was 918(0.183 2%),and decreased to 448(0.089 7%)after interven-tion. Irrational medical orders of solvent selection reduced from 134(0.026 7%)to 69(0.013 8%);irrational medical orders of sol-vent volume reduced from 435(0.086 8%) to 206(0.041 3%);irrational medical orders of drug dosage reduced from 241(0.048 1%)to 117(0.023 4%);irrational medical orders of drug compatibility reduced from 51(0.010 2%)to 28(0.005 6%);irrational medical orders of dosing frequency reduced from 17(0.003 4%)to 2(0.000 4%). CONCLUSIONS:What responsible pharmacists intervened in the irrational doctor’s orders of intravenous drip was effective and feasible in PIVAS,improved rational drug use and guaranteed the safety of drug use.
4.Effects of Capparis Spinosa Total Alkaloid on MMP-9 and TIMP-1 Levels in Systemic Sclerosis Mice

Objective To investigate the effects of capparis spinosa total alkaloid on type collagen (ColⅣ - ),Ⅳmatrix metalloproteinase-9 (MMP-9), tissue inhibitor of metalloproteinase-1 (TIMP-1) and plasminogen activator inhibitor-1 (PAI-1) in bleomycin-induced systemic sclerosis (SSc) mice; To explore the effective mechanism of capparis spinosa total alkaloid on fibrosis of SSc. Methods Totally 90 BALB/c mice were randomly divided into control group, model group, penicillamine group and capparis spinosa total alkaloid low-, medium- and high-dose group. Mice models with SSc were established by repeated local injections of bleomycin in mice back, except for the control group. Mice in medication groups received external application with capparis spinosa total alkaloid cream;mice in penicillamine group were given penicillamine for gavage; mice in the control and model group received external application without substance, one time a day, for 60 days. The contents of MMP-9, TIMP-1 and PAI-1 in serum and Col- in skin tissue were dⅣ etected respectively by ELISA after the last medication. Results Compared with the model group, the levels of MMP-9 and ratio of MMP-9/TIMP-1 markedly increased and the levels of Col-Ⅳand TIMP-1 markedly decreased in medium and high- dose of capparis spinosa total alkaloid group (P<0.05, P<0.01). But the level of PAI-1 was not influenced (P>0.05). Conclusion Capparis spinosa total alkaloid is effective in treating fibrosis of SSc by adjusting imbalance of MMP-9/TIMP-1 and decreasing expression of Col-Ⅳ.
5.A survey of student engagement of pharmaceutical students in medical colleges

Objective By means of value-added reflection of students' learning behavior, this article gives an objective description of the current situation of pharmaceutical undergraduate education and puts forward some suggestions to strengthen the connotation construction of pharmaceutical education.Method Using the Chinese College Student Survey (CCSS) developed by Tsinghua University and Indiana University (US), 1100 students participated in a one-to-one web-based anonymous questionnaire, and using SPSS19.0 to analyze the data.Five comparable indexes and students' recognition behaviors were discussed using descriptive analysis and independent sample t test.Result The five comparable index scores and learning behavior skill index scores of the students in Chinese traditional medicine specialty and pharmacy were higher than the national overall level of all kinds of colleges and universities of science categories.Compared with the national norm of medical colleges, except that the score of 43.36 of the five comparable indexes in pharmaceutical professional academic challenge (level of academic challenge, LAC) was less than 43.63 national medical colleges LAC norm level, the other index scores were higher.LAC and active collaborative learning scores of Chinese pharmaceutical majors were lower than those of pharmacy students, and the difference was statistically significant (P<0.05).In the study of behavioral skills, the scores of two major students' use, evaluation and comprehensive behavior were lower than the national standard of medical institutions, and there was a statistically significant difference in the scores of Chinese medicine and pharmaceutical majors (P< 0.05).Conclusion Teaching experience and learning support need to be optimized for pharmaceutical students.Learning scientificalness and comprehensive abilities in pharmaceutical education need to be improved.Mid and higher level of learning behavior needs to be enhanced for pharmaceutical students.
6.Fibercholedochoscopy for the management of residual stone after choledochostomy: a report of 220 cases

Objective To evaluate the efficacy of fibercholedochoscopy for the removal of residual stones after a surgical choledochostomy. Methods Two hundred and twenty cases of cholelithiasis underwent fibercholedochoscopy through a surgically formed T tube fistulae for residual stones from Sept. 1993 to Feb. 2002. Results A total of 572 times of fibercholedochoscopy was performed with residual stones totally evacuated in 201 cases (91.4%). Complications developed in 84 cases with no mortality. Conclusion Postoperative fibercholedochoscopy through a T tube fistulae is less traumatic and effective remedy for postoperatively retained common bile duct stones.
7.Roles of G-Protein β and γ Subunits.in the Interaction of Gβγ with Adenylyl Cyclases Ⅱ

To explore the individual roles of G protein β and γsubunit in the interactions with effectors. Methods We investigated the interactions of Gβ1 and Gβ1γ2 with adenylyl cyclase Ⅱ(ACⅡ) us ing the yeast two-hybrid and three-hybrid systems. Results When assayed for the abilities to activate the reporter gene, the interactions among AD-β1, γ2 and BD-AC ⅡQ in the three-hybrid system were more po tent than the interactions between AD-β1 and BD-AC ⅡQ in the two-hybrid system. The expressions of BD-AC ⅡQ and AD-β1 in transformants coexpressed AD-β1 and BD-AC ⅡQ, and transformants coexpressed AD-β1, γ2 and BD-AC ⅡQ were respectively detected. The comparisons between the reporter activity and the expression levels of BD-AC ⅡQ and AD-β1 in the yeast cells show there was no correlations, i.e.The difference in the reporter activity was not a reflection of differential expression level of the hybrid pro teins. Conclutions All these results suggest that G protein β1 subunit is sufficient to maintain the basic in teraction between Gβ1γ2 and AC ⅡQ, and γ2 subunit plays an important role in the high affinity interaction of Gβ1γ2 with ACⅡQ.
8.Recombinant Design of Fc Fusion Protein of Human ScFv Against BoNTa

Human ScFv against botulinum neurotoxin serotype A(BoNTa) was modified by fusing human IgG1 Fc to C terminal of ScFv. ScFv-Fc fusion protein was expressed at high level over 30% of total host cell proteins in E.coli. Recombinant protein existed in inclusion body form. Renatured ScFv-Fc was purified to 90%~95% by Protein G Sepharose column. In vitro ScFv-Fc could bind specific to toxiod BoNTa in ELISA. Recombinant ScFv-Fc had similar relative affinity to parent ScFv and had improved stability.

10.Predictive value of pediatrics end-stage liver disease or model for end-stage liver disease score in the prognosis of pediatric acute liver failure treated with artificial liver support system.

OBJECTIVETo investigate the predictive value of pediatrics end-stage liver disease (PELD) or the model for end-stage liver disease (MELD) in the prognosis of pediatric acute liver failure (PALF) treated with artificial liver support system (ALSS).
METHODThe clinical data of 47 children with acute liver failure seen from August 2008 to July 2013 treated in Children's Hospital, Fudan University were analyzed. Thirty children were treated with ALSS in addition to conventional comprehensive medical treatment (ALSS group). Seventeen children were treated with only conventional comprehensive medical treatment (control group). The main biochemical parameters and coagulation function parameters before and after treatment were compared in the ALSS group and the mortality rates were compared between the two groups. The patients were graded by PELD or MELD when they were hospitalized and the relationship of PELD or MELD scores and mortalities of child patients with the receiver operating characteristic curve (ROC) were analyzed.
RESULTThere were significant differences in total bilirubin (TB) ((302 ± 208) vs. (161 ± 129) µmol/L); alanine aminotransferase (ALT) ((161 ± 225) vs. (761 ± 834) U/L); aspartate aminotransferase ( AST) (66 (35, 123 ) vs. 447 (184, 1,129 ) U/L) ; international normalized ratio (INR) ((2.6 ± 1.6) vs. (5.1 ± 4.0)); prothrombin time activity percentage (PTA) ((42 ± 25)% vs. (22 ± 13)%); albumin( ALB) ((35 ± 5) vs. (33 ± 6) g/L) in the ALSS group after treatment. Through the ROC curve analysis, the best PELD/MELD threshold was 25 to predict the patients survival after ALSS therapy, with a sensitivity of 92. 3% , and a specificity of 94.1% at the cutoff point. The area under the ROC curve was 0. 912. The mortality of patients with PELD or MELD score below 25 in the ALSS group (1/13) was lower than the control group (3/4) (P = 0.022), and the mortality of patients with PELD or MELD score over 25 (16/17) was higher than that of the control group (10/13) (P = 0.290).
CONCLUSIONPELD or MELD score is a valid index in prognostic evaluation of ALSS therapy, which may provide an evidence for the therapeutic strategies of PALF. Patients with PELD or MELD score below 25 treated with ALSS obtained more benefit.
