1.Application of medical polymer carrier materials to anticancer drugs

OBJECTIVE: To introduce the type and biological properties of medical polymer vehicle materials, and to evaluate its application in anticancer drugs. METHODS: A computer-based on-line research was performed in China Journal Full-text Database and Pubmed database published from 1990 to 2009. The key words were "polymer, anticancer drug, carrier". Articles concerning biological properties of medical polymer vehicle materials and its application in anticancer drugs were included. Meta analysis and repetitive studies were excluded. RESULTS: Quality of articles was assessed, and a total of 24 articles were included. Biological properties of medical polymer vehicle materials and its application in anticancer drugs were summarized. Medical polymer vehicle materials are novel technique with the development of pharmacological study, biomaterial study and clinical medicine. Good biocompatibility, biodegradability, regulation of degradation rate and good workability of polymer materials provided convenience and possibility for innovation of pharmaceutical preparation. Structure of drug carrier material, elevation of drug-loading efficiency, in vivo distribution, biodegradation function and effects of degradation product on bodies deserved further investigations. The study focus of anticancer drug high polymer carrier lies in the search of carrier materials with strong choice and good outcomes. CONCLUSION: Medical polymer carrier materials can control drug release speed by dosage form changes, which induced a stable drug concentration in vivo. The medical polymer carrier materials also can send drugs to a certain part of the body by release system, which cannot affect other regions in the body.
2.The synergistic antitumor effects of berberine alpha-hydroxy-beta-decanoylethyl sulfonate with hydroxycamptothecine and its effect on topoisomerase.

Synergistic antitumor effects of HB (berberine alpha-hydroxy-beta-decanoylethyl sulfonate, houttuyn berberine) with HCPT (hydroxycamptothecine), and its correlative mechanism were studied in vitro. MTT assay was employed to determine the cytotoxicity of HB combined with HCPT in tumor cells culture in vitro, IC50 and combination index (CI value) were used to evaluate the synergistic effects. The supercoiled DNA relaxation mediated by topoisomerase I & II was measured by agarose gel electrophoresis assay, and influence of HB was detected. The results showed that HB could inhibit the proliferation of tumor cells (SGC-7901, SW1116 and SW480) in vitro, and the inhibition ratio was increased, IC50 was reduced when combining with HCPT. CI value of the two drugs was less than 1 in HepG2, SW480, SGC-7901 and SW1116 cells. The lowest value was 0.447, 0.626, 0.161 and 0.178 in these tumor cells, respectively, further indicating HB has synergistic action with HCPT on suppressing tumor proliferation. The agarose gel electrophoresis assay showed HB can inhibit topoisomerase I & II activity of SW480 cells at the concentration of 2.0-8.0 mg x L(-1). HCPT is a typical inhibitor of topoisomerase I , the synergistic action between HCPT and HB on suppressing tumor proliferation is perhaps related to the congenerous inhibition of topoisomerase.
3.Clinical efficacy and safety of Kangbusipu combined with leazu monoclonal antibody in the treatment of diabetic macular degeneration

Objective To investigate the clinical efficacy and safety of conbercept and lucentis in the treatment of age-related macular degeneration.Methods A total of 66 patients with age-related macular degeneration from our hospital were chosen,the patients or family members signed an agreement,were randomly divided into experiment group and control group with 33 cases in each group.Patients in the control group were treated by lucentis injection therapy; patients in the experiment group were treated by conbercept injection,determined serological indexes and ophthalmic indexes before and after treatment,and the clinical efficacy wascompared,the patients were followed up,the adverse reactions were recorded.Results After treatment,the vision restoration effective rate of the control group 48%was lower than the experiment group 80%,with statistical significance(P<0.05); the adverse reactions rate of the experiment group(20%)compared with the control group(16%),was no difference.Conclusion Conbercept injection in the treatment of age-related macular degeneration can decrease peripheral blood VEGF and CRP contents,reduce the CMT and CNV levels,reduce intraocular pressure,improve vision.
4.Advances in mechanisms of treatment for spinal cord injury with lithium.

Spinal cord injury is a serious disabling disease caused by a series of internal and external factors in the field of orthopaedics and neuroscience, which is a big problem for doctors all over the world. Lithium has been used to treat dipolar disorder for over 100 years. It has been reported that lithium is benefit for brain neuron. The treatment effect for spinal cord injury gets more and more attention. Researches indicate that lithium is benefit for spinal cord injury by protecting neuron,reducing after-injury inflammation increasing the produce and release of neurotrophins, stimulating neurogenesis, enhancing autophagy and inhibiting apoptosis. This article summaries advances in mechanism of treatment for spinal cord injury with lithium by reviewing and analyzing researches. Therapy combined with lithium has a good application prospect.

5.Advances and new ideas of neurobiology scaffold in repair of nerve injury.

Nerve injury including peripheral nerve injury and central nerve injury has been a global problem. With the development of technology, many innovative approaches for nerve repair have been tested and some of the results are meaningful. It becomes a hot point that repair nerve injured by biological scaffold (nerve conduit). This article reviewed and analyzed several kinds of biological scaffold materials and microenvironment with better effect in recent years. Some new ideas were raised from the three aspects: appropriate materials, microstructure, and bionic microenvironment. It is better to combine multiple measures and achieve the best effect. In addition, nerve scaffold have a bright future in repair of central nervous system.

6.Long-term effects on health-related quality of life after short-segment pedicle fixation of thoracolumbar fractures without neurologic injury.

OBJECTIVETo evaluate the effects on health-related quality of life after short-segment pedicle fixation of thoracolumbar fractures without neurologic injury.
METHODSA total of 55 patients suffered from thoracolumbar fractures without neurological deficit from January 2007 to October 2010 were included in this retrospective study. Of the 55 patients, 35 were male and 20 were female. The mean age was (44.0±10.5) years old (ranged, 7 to 75). Preoperatively, the main clinical symptoms in patients were thoracolumbar pain without neurologic injury and the imaging findings mainly showed that the injured vertebra became wedging or flat with the loss of height. Three questionnaires (ODI, EQ-5D and SF-36) were applied to follow-up in this study.
RESULTSAll operations were successful and incisions got primary healing. The function of thoracolumbar spine fully or basiclly restored and postoperative imagings showed fractures healed without loose screws, broken screws or broken rods. The completion rates of all the queasionnaires were 100% while the mean follow-up period was 4.2 years (ranged, 3 to 6). The ODI scores were 25.8±18.7, which were statistically higher than those of the general population in our country (P<0.05). The EQ-5D index scores were 0.70, which were statistically lower than those of the normal Chinese population (P<0.05). Compared with the normal scores in our country, the scores of 8 items in the SF-36 questionnaire were obviously lower with significant difference (P<0.05).
CONCLUSIONCompared with the normal Chinese population, it should be paid more attention to that the health-related quality of life in patients after short-segment pedicle fixation of thoracolumbar fractures without neurologic injury because of lower scores.

7.Extorsion traction and pushing manipulation with fingers for the treatment of anterior shoulder dislocation in elderly.

OBJECTIVETo investigate a manipulating therapy for treatment of anterior shoulder dislocation in elderly.
METHODSFrom October 2011 to June 2012,27 elderly patients with anterior shoulder dislocation were treated by extorsion traction and pushing manipulation with fingers, including 7 males and 20 females aged from 65 to 86 years old with an average of 77. The course of disease ranged from 1 h to 1 d. The shoulder manifested square deformity, Dugus signs showed positive, and X-ray displayed anterior shoulder dislocation. Dugus fixation was applied for and removed external fixation at 3 weeks after operation and carried out shoulder functional exercise. Functional evaluation standard on shoulder joint injuries was used for evaluate clinical outcomes.
RESULTSAll patients were gained reduction for the first time, and followed up at 3 months after operation, no dislocation occurred. According to functional evaluation standard on shoulder joint injuries, 22 cases got an excellent result,2 cases good,and 1 case moderate.
CONCLUSIONExtorsion traction and pushing manipulating therapy for treatment of anterior shoulder dislocation in elderly, which has advantages of simple, convenient, less painful, and can avoid iatrogenic injury, is feasible to widespread.

8.Clinical analysis of the CD_(117)-weak and CD_(117)-negative gastrointestinal stromal tumors:a report of 13 cases

Objective To explore the CT features in CD_(117)-weak and CD_(117)-negative gastrointestinal stromal tumors(GIST).Methods The CD_(117)-weak and CD_(117)-negative Gist proved by pathologically and sur- gically in 13 cases were retrospectively analyzed.Results The tumor was located in the stomach in 6 cases, small intestine in 4 cases,colorectum in 2 cases,greater omentum in one case.The most common clinical manifestations were abdominal pain or discomfort in 6 cases,hematemesis or melena in 3 cases.Malignant tu- mors occurred in 8 cases.Benign tumors occurred in 5 cases.A soft tissue was identified in all 13 cases in CT examination.The CT examinations revealed cystic regions and bleeding in 5 cases.Calcification was not noted within the masses in all cases.The mean diameter of benign tumors was 3.4 cm?2.8 cm while that of malig- nant tumor was 7.5 cm?6.2 cm.All the cases underwent operation,the resection rate was 100%.After follow up from 6 months to 4 years in 10 cases,1 case died of tumor recurrence and metastasis,the others survived. Conclusion CD_(117)-weak and CD_(117)-negative Gist occurred more common in the stomach.CT is useful in lo- cation and differentiation of CD_(117)-weak and CD_(117)-negative gastric stromal tumors with regard to benign or malignant.Surgery is the main method for final diagnosis and treatment.
9.Impact of overweight and obesity on flow mediated dilation in adult

Objective To explore the impact of overweight and obesity on flow mediated dilation (FMD) in adult people.Methods Intact clinical data of 183 healthy people, aged 30~65 in our hosipital were analyzed.According to BMI, all people were divided into obesity group (48 cases), overweight group(75 cases) and normal group (60 cases).FMD was detected by E tracing of ultrasonic.Weight, WC, BFP, baPWV, BP,FPG,CHO,HDL-C,LDL-C,TG and UC were observed at the same time.FMD of three groups were compared and the risk factors of FMD were analyzed.Results The level of FMD in the obesity group and overweight group was significantly higher than that in the normal group[(5.63±2.76)%,(6.45±2.54)% vs.(8.31±3.04)%](F=13.77,P<0.001);there was no statistically significant difference between the obesity group and overweight group (t=1.68,P>0.05).Age(β=0.078,95%CI: 1.004,1.164),BMI(β=0.465,95%CI 1.148,2.208),LDL-C(β=2.294,95%CI: 9.918,1.612) and UC(β=0.007,95%CI 0.986,1.000) were independent risk factors of decreased FMD(P<0.05).Conclusion Age, BMI, LDL-C and UC are independent risk factors of decreased FMD.
10.Application of oil red O staining in spinal cord injury of rats.

OBJECTIVETo explore the value of the application of oil red O staining in spinal cord injury (SCI) of rats.
METHODSWith simple randomization, 24 Spargue-Dawley male rats were divided into normal control group including 6, and SCI group including 18. Spinal cord was transected at spinal lever T10 to build SCI model. Six rats of SCI group were sacrificed randomly at 1, 2, 4 weeks after surgery. After the spinal cord tissue sections were made, oil red O staining methods were used to observe the changes at the end of transected spinal cord. Images were analyzed by Image-Pro Plus 6.0 and SPSS 20.0 software.
RESULTSThe oil red O staining of normal control group showed that white matter surrounded by myelin sheath was clear and obviously distinctive from grey matter. Uneven and strengthened staining in oil O was observed in grey matter of SCI group at 1, 2, 4 weeks post-SCI.
CONCLUSIONIt is a good method to label the myelin sheath in spinal cord and distinct white matter from grey matter by oil red O staining. Analysis of the images showed that lipid may become another target for drugs, which needs more researches.
