3.Analysis of influencing factors of hemoglobin A1c in type 2 diabetic patients

Objective To analyze the influencing factors of hemoglobin A1c in type 2 diabetic patients,including blood glucose,anti-diabetic therapeutic regiments,disease course,age,diabetic education,degree-educated,self-monitoring blood glucose.Methods Seventy-eight type 2 diabetic patients were divided into two groups by different degree of blood glucose,anti-diabetic therapeutic regiments,disease course,age,diabetic education,degree-educated,self-monitoring blood glucose.The levels of hemoglobin A1c in two groups were compared.Results There were significant changes in the levels of hemoglobin A1c between fasting blood glucose (FBG) ≤ 6.1 mmol/L group (30 cases) and FBG > 6.1 mmol/L group (48cases),2 h postprandial blood glucose (2hBG) ≤8.0 mmol/L group (32 cases) and 2hBG > 8.0 mmol/L group (46 cases ),insulin treatment group (27 cases) and no-insulin treatment group ( 51 cases ),combined drugs treatment group ( 36 cases ) and single drug treatment group ( 15 cases),age ≤ 65 years old group (41cases) and age >65 years old group (37 cases),disease course≤ 10 years group (39 cases) and disease course > 10 years group( 39 cases),degree-educated above high grade group( 34 cases) and degree-educated below high grade group (44 cases),diabetic education ≥2 times/month group (20 cases) and diabetic education < 2 times/month group (58 cases),self-monitoring blood glucose≥2 times/week group ( 19 cases)and self-monitoring blood glucose < 2 times/week group (59 cases)[ (6.7 ± 1.5 )% vs.(7.9 ± 1.3 )%,(6.8 ±1.1)% vs.(7.8 ± 1.2)%,(6.7 ± 1.5)% vs.(8.1 ± 1.4)%,(6.8 ± l.0)%vs.(8.0 ± 1.6)%,(6.9 ± 1.7)% vs.(7.4 ± 1.6)%,(6.5 ± 1.2)% vs.(8.2 ± 1.3)%,(6.9 ± 1.0)% vs.(7.6 ± 1.4)%,(6.1 ± 1.7)% vs.(8.0 ±1.1 ) %,(6.7 ± 1.1 )% vs.(7.6 ± 1.2)% ] (P < 0.01 or < 0.05 ).Conclusion Blood glucose,anti-diabetic therapeutic regiments,disease course,diabetic,degree-educated,self-monitoring blood glucose are the important factors influencing hemoglobin A1c.

Objective To investigate the implications of gene expression pattern of pancreatic endocrine differentiation from mouse embryonic stem cells (mES) in vitro.Methods Mouse ES cell line ES W9.5 was cultured and differentiated into pancreatic endoerine progenitor cells by using the stepwise differentiation strategy in vitro,then stage specific genes expression were tested by RT-PCR and immnunoeytochemistry.The Illumina Mouse Ref-8 vl.1 Expression BeadChips were used to compare the changes in the transcriptome of mES-derived differentiated cells on day4,8,15,20,22,25 after differentiation to undifferentiated mES cells.Results A total 86 differentially expressed genes,both up- and down-regulated,were found during the whole differentiation stage,6 gene clusters with the same developing patterns and differentially expressed genes in 6 groups were collected.By number,the most differentially expressed genes corresponded to posterior foregut stage( 201 genes) followed by definitive endoderm stage ( 17 genes).Conclusions The analysis of the genes with the same developing pattern and differentially expressed genes at each differentiation stage during pancreas development by microarray analysis provides some evidences of early embryonic and pancreatic development.
6.Endovascular treatment for peripheral vascular anomaly in children

Objective To investigate the endovascular treatment of peripheral vascular anomaly in children and its efficacy.Methods Eighty-three children with peripheral vascular anomaly were treated via endovascular means. Before and during the endovascular operation,all the cases were diagnosed as follows: arteriovenous fistula in 23 cases, cirsoid angioma 32 and cavernous hemangioma 28. The endovascular approaches included:①transcatheter embolization using coils or gelfoam sponge;② transcatheter embolization combined with local cirrhosis agents treatment.Results Immediate angiography after embolization showed all the arteriovenous fistulas and hemangiomas were efficiently occluded and the clinical presentation and symptoms improved greatly. Follow-up one to three years after operation demonstrated 100% of efficacy and 68.7% curative rate with no complications. Conclusions Endovascular approach for peripheral vascular anomaly in children is a micro-invasive, safe and effective way.

8.A study of Qishen Tongluo Zengzhi decoction for intervention of cognitive impairment in patients with acute phase of ischemic stroke

ObjectiveTo observe the effect of Qishen Tongluo Zengzhi decoction on cognitive impairment in patients with acute phase of ischemic stroke.Methods A prospective randomized controlled trial was conducted, and 130 patients with acute phase of ischemic stroke and cognitive impairment accompanied by Qi deficiency and blood stasis and stagnationof phlegm-dampness syndrome admitted into the Neurology and Rehabilitation Departments of Rizhao Hospital of Traditional Chinese Medicine(TCM) Affiliated to Shandong University of TCM were randomly divided into treatment group and control group, 65 cases in each group. In the two groups, conventional internal treatment was given to all patients, and in the treatment group, additionally the Qishen Tongluo Zengzhi decoction was administered orally(composition: astragalus membranaceus 30 g, radix pseudostellariae 30 g, notoginseng 10 g,spatholobus stem 25 g, hirudo 3 g, pberetima 10 g, radix paeoniae rubra 12 g, Chinese angelica 12 g, peach kernel 10 g, carthamus tinctorious 10 g, achyranthes 12 g, radix rhapontici 10 g, rhizoma alismatis 6 g, Acorus gramineus Soland 9 g, polygala root 9 g, rhizoma cyperi 10 g, herba siegesbeckiae 15 g),one dose a day. While in the control group, oxiracetam 4.0 g intravenous drip was given, once a day. The whole course was 21 days in both groups. Before and after treatment, the cognitive function of all the patients in two groups was assessed by Mini-Mental State Examination(MMSE)and Montreal Cognitive Assessment(MoCA) scores, and incubation period and amplitude of P300 wave were recorded.Results Finally 62 cases were in treatment group and 63 cases in control group. Before treatment, the comparisons of the MMSE score, MoCA score and P300 latent period and amplitude between the two groups had no statistically significant differences(allP>0.05). After treatment in the two groups, the MMSE score, MoCA score and P300 wave amplitude were elevated, P300 latency period was shortened compared with those before treatment, and the changes were more prominent in treatment group〔score of MMSE: 25.33±2.32 vs. 21.68±2.29, score of MoCA(score): 26.61±3.06 vs. 22.40±2.93, P300 wave incubation period(ms): 349.62±20.01 vs. 371.87±19.63, P300 wave amplitude(μV): 8.70±2.92 vs. 5.72±2.33,allP<0.01〕.ConclusionQishen Tongluo Zengzhi decoctioncan effectively intervene cognitive impairment in patients with acute phase of ischemic stroke, and improve their cognitive function.
9.Study on Regulation of Autophagy and Endoplasmic Reticulum Stress in Hypothalamus byZi-Bu Pi-Yin Recipe among Spleen-yin Deficiency Diabetes-associated Cognitive Decline in Rats

This study was aimed to explore the mechanism ofZi-Bu Pi-Yin Recipe (ZBPYR) on autophagy and endoplasmic reticulum stress (ERS) to improve spleen-yin deficiency diabetes-associated cognitive decline (DACD). Rats were randomly divided into the control (cont) group, the diabetes (DM) group, the spleen-yin deficiency (pi) group, the spleen-yin deficiency diabetes (piDM) group, and the spleen-yin deficiency diabetes + ZBPYR treatment (ZBPYR) group. The expression of microtubule-associated protein 1A/1B-light chain 3 (LC3Ⅱ), inositol-requiring enzymeα (IRE1α), c-Jun N-terminal kinase (JNK) were observed by western blot. The results showed that the expression of LC3Ⅱ in the DM group, pi group and piDM group decreased compared with the cont group (P < 0.05); and the expression of LC3Ⅱ of the ZBPYR group increased compared with the DM group and piDM group (P < 0.05). Compared with the cont group, the p-IRE1α of the DM group and piDM group, as well as p-JNK1 in the pi group and piDM group were increased (P < 0.05). The p-IRE1α and p-JNK1 of the ZBPYR group were decreased compared with the DM group and piDM group (P < 0.05). It was concluded that ZBPYR improved spleen-yin deficiency DACD by regulating autophagy and ERS.
10.Values of apparent diffusion coefficients in staging the lesions of multiple sclerosis

Objective To explore the values of apparent diffusion coefficients (ADC) in staging the lesions of multiple sclerosis (MS) through analysing the differences of ADC in different types of lesions.Methods 137 lesions identified in 29 patients with MS,were characterized by their enhancement pattern on contrast-enhanced T1-weighted MR images and whether they were changing in appearance, size or signal.The lesions were classified into four groups:homogeneously enhancing lesions (HELs), ring-enhancing lesions (RELs), active nonenhancing lesions (ANELs), and inactive nonenhancing lesions (INELs).The mean ADC values for each type of lesions were calculated and compared.Results The ADC values for HELs, RELs, ANELs and INELs were (91.09±13.63)× 10 5 · m2 · s-1, (105.66±9.92) × 10-5 · m2 · s-1, (136.80±20.31) × 10-5 · m2 · s-1 and (127.46±13.65) ×10-5 · m2 · s-1.There were significant differences between the ADC values of any two groups in the four groups except the ADC values between ANELs and INELs.Conclusion ADC values are useful to differentiate each type of lesion,which can help stage the lesions of MS.
