Objective To investigate the prevalence of gastroesophageal reflux disease(GERD)in outpatients who visited department of internal medicine in hospitals of Zhejiang province and its risk fac- tors were analyzed.Methods The epidemiologic study was carried out on outpatients with reflex diagnos- tic questionnaire(RDQ).The data was analyzed with Logistic regression methods.Results The preva- lence of GERD in those patients was 7.28%(95% CI:6.87%-7.69%)in preliminary screening.There was significant correlation among gender,age,degree of labour intensity,status of marriage and the oc currence of GERD(P

5.Evaluation of curative effect of chronic Keshan disease treated by captopril and metoprolol

Objective To observe the curative effect of captopril and metoprolol in the treatment of chronic Keshan disease (CKD). Methods One hundred and ninty-five patients with CKD chosen from Juxian, Wulian, Yishui, Pingyi, Sishui and Zoucheng in Shandong Province were randomly assigned to control group, captopril group and metoprolol group according to NYHA cardiac functional grading. All cases were given diuretics, digitalis and vasodilating agents as routine treatment. On this basis, captopril and metoprolol was administered in captopril group and metoprolol group respectively. After 12 months of follow-up visit, the causes of cardiac death, hospitalization status and the changes of heart size, electrocardiogram, blood pressure and heart rate were all observed. Results It was found that the mortality of captopril group and metoprolo] group was 4.76% (3/63), 5.00% (3/60) respectively, both lower than the control group 10.61%(7/66). But this difference had no statistically significance(P=0.39). Besides, the hospitalization days of each year in captopril group and metoprolol group was respectively (19.12± 20.35) and(18.86±21.52)days, much more reduced than in the control group[(21.45±21.74)days, q=3.17, 3.38, P<0.05]. The detection rate of cardiothoracic ratio decreased in captopril group and metoprolol group [45% (27/60) and 40.4% (23/57)] After treatment showed more pronounced amelioration than the control group [18.6% (11/59), χ2=9.51,6.59, all P<0.0125], still the detection rate of cardiomegaly and invariability had no significant difference among three groups (χ2=2.50,4.75, all P>0.05). The elimination coefficient of ectopic rhythm in metoprolol group [56.5% (13/23)] was pronounced higher than the control group and captopril group [23.8% (5/21), 22.7% (5/22)], but differences had no statistically significance(P=0.0358,0.0331, all P>0.0125). Significant differences were found in systolic blood pressure(SBP), diastolic blood pressure(DBP) and heart rate(HR) in three groups prior and post-treatment(F=47.51,44.23,80.66, all P<0.01). The interaction of therapy and treatment time had influence on SBP and HR (F=3.19,37.44, all P<0.05), but had no influence on DBP(F=2.21, P> 0.05). There was no difference in SBP, DBP or HR among three groups before treatment(F=0.28,0.57,1.80, all P>0.05). After treatment, SBP and DBP in captopril group, metoprolol group and the control group[(109.0±10.9), (112.2±12.8), (114.7±13.2)mm Hg, (69.3±7.2), (72.1±9.5), (73.3±9.3)mm Hg] were all lowered compared with pre-treatment[ (117.1±13.4), (119.0±14.4), (117.6±14.1)mm Hg and (74.2±10.2), (76.3±10.8), (75.4±11.1)mm Hg, t=4.79,4.47,2.08,5.12, 4.32,2.15, all P<0.05]. HR was reduced in metoprolol group, being [(66.2±7.7), (75.9±11.5)times/min] before and after treatment(t=10.81, P<0.01), while it remained unchanged in captopril group and control group[(70.6±8.0), (72.6±10.5) times/min and (71.9±10.4), (73.8± 12.2)times/min, t=1.77,1.74, all P>0.05]. After treatment, both SBP and DBP of captopril group were significantly lower than that in the control group (q=3.52,3.56, all P<0.05); HR was reduced in metoprolol group, lower than that in captopril group and control group(q=5.44,3.73, all P<0.01). Conclusions Having a tendency of depressing mortality, captopril and metoprolol can reverse or delay myocardial remodeling and reduce admission rate in a safe,reliable and economic way, and are worth to be widely used in the treatment of chronic Keshan disease.
6.Genotypes of Plasmid-mediated AmpC Beta-lactamases in Klebsiella pneumoniae

OBJECTIVE To investigate the genotypes of plasmid-mediated AmpC beta-lactamases carried in Klebsiella pneumoniae,to provide reasonable use of antibacterial agents and to reduce the spread of these drug resistant genes.METHODS Two hundred and eighteen strains of K.pneumoniae were performed in Tongji Hospital in Wuhan.Plasmid-mediated AmpC beta-lactamases-producing strains were screened by improved three-dimensional method and identified by multiplex PCR.DNA sequencing method was used to confirm these drug resistant strains.The MIC of 15 kinds of antibacterial agents against the clinical isolates was detected by double agar dilution method.RESULTS Thirteen strains of K.pneumoniae were detected by improved three-dimensional method.The detection rates were 5.96%.There were seven of thirteen positive strains of K.pneumoniae through improved three-dimensional method harboring DHA-1 type plasmid-mediated AmpC beta-lactamase by multiplex PCR and confirmed by DNA sequencing.Plasmid-mediated AmpC beta-lactamase producers revealed a highly drug resistance to cephalosporins,monobactams,beta-lactam/beta-lactamase inhibitor combinations,aminoglycosides and fluoroquinolones.Only imipenem was susceptible to all of these detected strains.CONCLUSIONS DHA-1 plasmid-mediated AmpC beta-lactamase is detected in K.pneumoniae strains from Tongji Hospital.The detection rate is 3.2%.The pAmpC-producing strains reveal multi-drug resistance.Only imipenem is susceptible to them.
7.X-ray,CT and MRI findings of synovial tuberculosis in joints

Objective To analyze the X-ray,CT and MRI findings of synovial tuberculosis,and to evaluate the role of MRI in diagnosing synovial tuberculosis.Methods Fourteen eases of synovial tuberculosis comfirmed by operation and pathology were retrospectively analyzed and summarized. All patients were examined by MRI and X-ray,and CT scans were performed in 3 cases. Results X-ray showed joint swelling(8 cases),articular space narrowing(7 cases),marginal joint erosions(4 cases), and periarticular osteoporosis(9 cases).The joint swelling was detected on CT in all 3 cases,and bony erosion and speckled sequestra were seen in 2 cases.MRI in all of patients showed joint swelling and synovial proliferation in different drgees,demonstrated as heterogeneously low signal on T_1 WI and slight high signal(7 cases)and obvious high signal(6 cases)on T_2WI,and diffuse synovial proliferation was demonstrated as massive and nodular signal in 8 cases.Joint effusion was present in 7 cases as low signal on T_1WI and high signal on T_2 WI.Osseous erosion lesions were seen in 7 cases,and intra-artieular cartilage thinned,partly or mostly disappeared in 11 cases.Periarticular bone marrow edema was found in 7 cases. Conclusion MRI was superior to X-ray and CT in the diagnosis and differential diagnosis of synovial tuberculosis.
8.Study on effect of tetramethylpyrazine on proliferation and apoptosis of leukemic U937 cells and its mechanism.

OBJECTIVETo study the proliferation and apoptosis of tetramethylpyrazine (TMP) on leukemic U937 cells and its possible mechanism.
METHODThe inhibitory effect of TMP on the proliferation of U937 cells was detected by CCK-8 assay. The cell apoptosis and cycle distribution were examined by the flow cytometry. The mRNA expressions of bcl-2 and P27 were determined by the Real-time PCR. Western blot was carried out to detect bcl-2, caspase-3, cyclin E1, CDK2 and P27 expressions.
RESULTTMP inhibited the proliferation of U937 cells in a dose-and-time dependent manner, with IC50 value of 160 mg x L(-1) at 48 h. In addition, TMP could induce the apoptosis of U937 cells and block the cell cycle in G0/G1 phase. According to the results of Real-time PCR and Western blot, TMP could down-regulate the expression of apoptosis-related molecule bcl-2, cycle-related protein cyclin E1 and CDK2 and up-regulate caspase-3 and P27.
CONCLUSIONTMP shows the effects in inhibiting the proliferation of leukemic U937 cells and inducing the apoptosis. Its mechanism may be related to the impacts on the cell cycle distribution, down-regulation of the bcl-2 expression, which finally activates caspase-3, starts the apoptosis path and causes the cell apoptosis.

9.Effects of neural stem cell transplantation from GFP transgenic mice on rat neurocognitive function and NGF expression after brain trauma

Objective To establish contusion brain injury model in rats, and investigate the efficacy of intravenous administration of neural stem cells(NSC) on posttraumatic neurocognitive function recovery and NGF expression in rats. Methods Cerebral contusion model in motor-sensory cortex of the right parietal cortex in rat was established by a 50 g-weight hammer falling respectively from 30 cm height along guide stick to impact collision pole by improved trauma device for model of contusion brain injury based on Feeney method. And the NSC isolated from GFP transgenic mice were injected intravenously via the tail vein 24 h after the brain trauma, and 1 week later neurocognitive function scores and NGF immunostaining were performed to explore the efficacy of NSC transplant. Results The NSCs from the GFP transgenic mice gathered at the injury site 1 weeks after transplants.Neurocognitive function scores and NGF-positive cells measurement(226 ±27,23 ±4 ) in the treatment group revealed significant increase than in the brain trauma group(300 ±36;15 ±3 )(P<0.05). Conclusion The intravenous NSC injection in rats can survive and migrate to the injured brain region and promote the post-injury neurocognitive function restoration. The increase of NGF expression may underline one of most important mechanisms in NSC treatment' s rats after brain injury.
10.Effect of olfactory entheath cells transplantation on neurophysiological function and synaptic density in rats with traumatic brain injuries

Objective To investigate the effect of olfactory entheath cells (OECs) transplantation on neural function and synaptic density of rats with traumatic brain injuries (TBI) and the possible mechanism. Methods Rats were divided into sham operation group, TBI group and OECs engrafted group. The brains of the rats were injured by Feeney percussion device through free falling. After cultured and identified by using specific marker (known as P75), OECs were transplanted into the area around the injured brain. Cortical somatosensory evoked potential (CSEP) and motor evoked potential ( MEP) were evaluated at day 14 after cell transplantation to determine the neurophysiologic function following TBI. Moreover, the synaptic densities around the injured brain were determined by using immunohistochemical method. One-way ANOVA test was used for statistical analysis. Results The transplanted OECs could survive and migrate around the injury site in the host brain 14 days after OECs transplantation. In addition, rats subjected to OECs implantation showed a marked neurophysiologic improvement and a significant increase of synaptic densities compared with the control group. Conclusion OECs transplantation can improve the neurophysiologic function and increase the synaptic density, which provides experimental basis for treatment of TBI with OECs.
