1.The effect of TGF-?_1 on expression of paxillin in human renal proximal tubular epithelial cell line

Objective To explore the effect of transforming growth factor ?_1 (TGF-?_1) on expression of paxillin(Pax) in human kidney proximal tubular epithelial cell line (HKC cells) in different phases. Methods HKC cells cultured in vitro were divided into four groups at random. Control group(C group): HKC cells were cultured with serum free medium (FSM), and TGF-?_1-treated groups (T1, T2, T3 group): HKC cells were cultured with FSM containing 10ng/ml TGF-?_1. In the latter groups, duration of treatment varied as follows: T1 group for 24h, T2 group for 48h, T3 group for 72h. The proliferation of HKC cells induced by TGF-?_1 was assessed by MTT and the expression of Pax was determined. The gene expression of Pax was determined by reverse transcripton polymerase chain reaction (RT-PCR) and the protein expression of Pax was assessed by immunohistochemical staining and Western blot. Results TGF-?_1 could induce the proliferation of HKC cells in a time-dependent manner. In C group there was expression of Pax mRNA and protein in HKC cells. Objective: To compare the advantages and disadvantages between Prospective Payment System based on Diagnosis Related Group System ( DRGs-PPS) mode and project payment mode. Methods: Historical review method, sampling and comparison are used to compare and analyze DRGs-PPS mode and project payment mode. Results: Through comparative analysis, it is founded that the application of DRGs-PPS mode could make the three wins of government, medical institutions and patients, which has more advantages than project payment mode. Conclusion: DRGs-PPS mode has important influences in controlling the increase of social health expenditure, improving the quality of medical services and standardizing medical behaviors.

6.Immunosuppressive strategy of leflunomide combining with calcineurin inhibitor and prednisone in renal transplant recipients

Objective To evaluate the efficacy and safety of immunosuppressive strategy of leflunomide (LEF) combined with calcineurin inhibitor and prednisone in Chinese renal transplant recipients.Methods Prospective clinical study was performed in 16 stable renal transplant recipients treated with calcineurin inhibitor, prednisone and either mycophenolate mofetil (MMF) or azathioprine (Aza), and MMF or Aza was replaced by LEF afterward. The loading dose of LEF was 50 mg/day for 3-5 days, and the maintenance dose was 20 mg/day. Prednisone was maintained 10 mg/day. The doses of CsA were adjusted according to its blood concentration.Results One patient dropped due to economic reason 1 mouth later, and continued to take Aza. Another one patient switched back to MMF due to a slight increase of serum creatinine (from 130 ?mol/L to 143 ?mol/L) 6 months after conversion. The remaining 14 patients were still on LEF. The mean observation time under LEF was (8.7?7.2) months (1~18 month). The mean serum creatinine was (103?24) ?mol/L before conversion, (95?26) ?mol/L at 6th month, and (108?27) ?mol/L at one year post-conversion. There was one episode of biopsy proved reversible acute rejection. The observed side effect was tolerable alopecia in 2 cases and rash in 1 case, but there were no significant increases in liver function, decrease of blood WBC count, deterioration of anemia, or any serious infection in any of the patients.Conclusion Immunosuppressive strategy of LEF+calcineurin inhibitor and prednisone is effective and safe in stable Chinese renal transplant recipients.
7.Mid-term treatment results of drug-eluting stent implantation for patients with multivessel chronic total coronary occlusion lesions

Objective To evaluate mid-term clinical result of drug-eluting stent (DES) implantation for patients with multivessel chronic total coronary occlusion (CTO) lesions. Methods We analyzed 89 patients who underwent interventional revascularization in their multivessel CTO lesions with DES from November 2002 to June 2005 (DES group). A control group was composed of 70 consecutive patients with multivessel CTO lesions treated with bare metal stents (BMS) from November 2000 to June 2003 (BMS group). Results There were no significant differences in basic clinical manifestations and success rate of intervention procedures between DES and BMS group. At 6-month follow-up, the cumulative rate of major adverse cardiac events (MACE) was 14.6% in the DES group and 28.6% in the BMS group (P=0.031). The incidence of restenosis was 16.9% in the DES group and 34.3% in the BMS group (P=0.011). The need for target vessel revascularization and target lesion revascularization in the DES group was significantly lower (9.0% vs 21.4%, P=0.003, and 7.9% vs 18.6%, P=0.043, respectively) compared with BMS group. Conclusion It is possible to obtain an ideal mid-term clinical outcome in patients with multivessel CTO lesions by DES implantation.

Objective: To investigate the changes in serum CD44v6/v EGF(sCD44v6/sVEGF)levels after concurrent chemoradiotherapy in patients with inoperable non-small cell lung cancer(NSCLC)patients and to analyze the correlation of serum markers with patients'prognosis. Methods: The sCD44v6/v EGF levels were detected by ELISA in 50 inoperable NSCLC patients before and after concurrent chemorediotherapy. Results: The sCD44v6 and sVEGF levels before treatment was significantly higher than that in the control group.The pre-treatment sVEGF level was closely related to cTNM stage.metastasis,cell differentiation and primary tumor size,but was not correlated with histological classification,lymph node status,age or gender.The pre-treatment sCD44v6 level was closely correlated with cTNM stage,metastasis,cell differentiation,and primary tumor size,but was not correlated with histological classification,age or gender.Patients'sCD44v6 before treatment(570.89±63.30 ng/L)was significantly higher than that after treatment(281.44±74.28 ng/L).Patients'sVEGF before treatment(241.09±85.96 ng/L)was significantly higher than that after treatment(133.64±67.69 ng/L).PD patients had the highest level of sCD44v6 and sVEGF and CR patients had the lowest sCD44v6 and sVEGF level.The sVEGF level was highly correlated with sCD44v6(r=0.291,P<0.05).Conclusion:The sCD44v6 and sVEGF levels might be helpful for evaluation of the biological behavior and prognosis of NSCLC.
