1.Efficacy and safety of permissive hypercapnia ventilation in mechanically ventilated newborns: a meta-analysis

Objective To evaluate the efficacy and safety of permissive hypercapnia ventilation in mechanically ventilated newborns.Methods PubMed,Embase,the Cochrane Library,China National Knowledge Infrastructure (CKNI),Wanfang Data and Chinese BioMedical Literature Database (CBM) were searched up until March 31,2013.Randomized controlled trials (RCTs) comparing permissive hypercapnia (PHC) group with normocapnia (NC) group in mechanically ventilated newborns were included.The primary outcomes included the incidence of ventilator associated lung injury (VALI),bronchopulmonary dysplasia (BPD),intraventricular hemorrhage (IVH),periventricular leukomalacia (PVL),patent ductus arteriosus (PDA),neonatal necrotizing enterocolitis (NEC),neurodevelopmental injury and the mortality rate.Secondary outcomes included the duration of ventilatory support and the length of hospital stay.The Cochrane Handbook 5.1.0 was used to evaluate the methodological quality and RevMan 5.1 software from Cochrane Collaboration was used for meta-analysis.The fixed effects model or the random effects model was adopted according to the result of heterogeneity.Results (1) A total of 325 articles were searched,and eight RCTs involving 605 newborns (302 newborns in PHC group while 300 newborns in NC group) which met the inclusion criteria were selected.In seven studies,the partial pressure of carbon dioxide (PCO2) was controlled at < 65 mmHg (1 mmHg=0.133 kPa) and pH at ≥ 7.2 in PHC group.In one study,PCO2 was > 52 mmHg and pH>7.2,without descripition of the upper limit of PCO2.(2) Four articles described the method of random allocation in detail; three described allocation concealment; all eight studies used blinding method for research subjects; two used blinding method for outcome assessment; all eight studies reported complete data; and three articles described the source of other bias.(3) All eight studies reported the incidence of VALI (I2=56%,P=0.03).The random effects model was used for the meta-analysis,and there was significant difference between PHC group and NC group (RR=0.52,95%CI:0.29-0.93,P=0.03).According to the gestational age,the eight studies were divided into two subgroups.One subgroup,including three studies with an average gestational age of 25 weeks (I2=0%,P=0.46),showed no significant difference in the incidence of VALI between PHC and NC group (RR=1.05,95%CI:0.72-1.54,P=0.78).The other subgroup,including five studies with gestational age of >27 weeks (I2=0%,P=0.68),showed significant difference in the incidence of VALI between the two groups (RR=0.27,95%CI:0.14-0.50,P<0.01).The in-hospital mortality and duration of ventilation showed significant difference between the two groups (in-hospital mortality:RR=0.40,95%CI:0.22-0.74,P<0.01; duration of ventilation:difference in means=-0.75,95%CI:-1.04--0.46,P<0.01).There was no significant difference in the incidence of BPD,IVH,PVL,PDA,NEC and neurodevelopmental impairment between the two groups (all P>0.05).Conclusions PHC ventilation in mechanically ventilated newborns can decrease the incidence of VALI,the in-hospital mortality and the duration of ventilation,while its protective efficacy against BPD is not remarkable.It does not increase the risk of IVH,PVL,PDA,NEC and neurodevelopmental injury,when the PCO2 is < 65 mmHg and pH ≥ 7.2.
2.Effect of calcium-sensing receptor on apoptosis of neonatal rat cardiomyocytes

ObjectiveTo observe the influence of calcium - sensing receptor (CaSR) on Fas/Fas ligand (Fas L) pathway during anoxia/reoxygenation (A/R)- induced cardiomyocytes apoptosis in neonatal rat. MethodsSingle cells were dissociated from minced hearts of 2 - day - old Wistar rats with a 0. 25% solution of crude trypsin and then cultured as monolayers at a density of 5 x 104cells/cm2 in DMEM medium equilibrated with humidified air containing 5% CO2 at 37C. Three days after the cells were seeded, the cultured cardiomyocytes were randomly divided into three groups. ( 1 ) control group: cardiomyocytes were continuously cultured for 26 hours in DMEM medium.(2) A/R group: cardiomyocytes underwent anoxia for 2 hours and reoxygenation for 24 hours.(3) GdCl3 group: 300μmol/L GdCl3 was added to the culture medium at the beginning of reoxygenation. Apoptosis of cardiomyocytes was assessed by flow cytometer and morphological alterations were observed with transmission electron microscope.The expression of CaSR, Fas and Fas L were analyzed by Western blot.Results The result of flow cytometer showed that cardiomyocytes apoptosis was 12. 18% ± 1.54% in A/R group,and was higher than that in the control group (P <0. 01 ). At the same time, mitochondrial cristae dissolution and disappearance could be detected. Compared with A/R group, GdCl3, a specific activator of CaSR, further enhanced cardiomyocytes apoptosis to 20. 25% + 2. 87% ( P < 0. 01 ), along with an increment in CaSR, Fas and Fas L expressions ( P < 0. 05 ). ConclusionCaSR is closely involved in cardiomyocytes apoptosia during anoxia/reoxygenation. CaSR could induce apoptosis of neonatal rat cardiomyocytes through Fas/Fas L receptor pathway.
3.The gender differences of the magnetic source imaging during recognizing dynamically sad facial expression in depressed patients

Objective To explore the gender differences of brain activation between patients with depression in recognition of dynamically sad expression with magnetoencephalography (MEG) technology and the paradigm of expressional experiment,and further to examine its neurological basis. Methods Brain activities were detected using MEG during the recognition of dynamic facial video with 12 female depressed patients, 11 male depressed patients, and their corresponding normal controls matched by age and education. ( p < 0. 05, corrected). Conclusion The data suggest that there may be significant gender differences of functional network in depressed patients during recognizing dynamical sad facial expression.
4.Effects of inhaled iloprost on pulmonary hypertension after cardiopulmonary bypass in patients with congenital heart disease undergoing surgical correction of anomaly

Objective To evaluate the effects of inhaled iloprost on pulmonary hypertension after cardiopulmonary bypass in patients with congenital heart disease undergoing surgical correction of anomaly. Methods Fifty-eight patients with congenital heart disease aged 14-60 yr undergoing surgical correction of anomaly under cardiopulmonary bypass (CPB) were enrolled in this study. Radial artery was cannulated before induction of anesthesia. A 6-lumen pulmonary catheter was placed via right internal jugular vein after tracheal intubation. Their mean pulmonary arterial pressure was still > 25 mm Hg after operation. ECG, HR, BP, CO, PAP and SpO_2 were continuously monitored. Aerosolized iloprost 10 fig was inhaled via nebulizer after CPB. Hemodynamics were measured before iloprost inhalation (baseline) and at 0, 5, 15, 30 and 60 min after the end of iloprost inhalation. Results There were 28 patients with pulmonary hypertension after CPB among the 58 patients with congenital heart disease. Inhalation of iloprost 10μg significantly decreased mPAP, pulmonary vascular resistance and intrapulmonary shunt as compared with the baseline. Iloprost inhalation could also improve significantly CO and the mixed venous oxygen saturation (SvO_2 ). Conclusion Iloprost inhaled after CPB can effectively reduce pulmonary arterial pressure and pulmonary vascular resistance in patients with congenital heart disease undergoing surgical correction of anomaly and help them wean from CPB.
5.The dynamic characteristics of the magnetic source imaging of recognizing expression in major depressive disorder

Objective To explore the features of functional networks about recognizing dynamically positive expression with using magnetoencephalography (MEG) technology and the paradigm of expressional experiment, and further to examine its neurological basis. Methods Twelve depressed patients and twelve age,education-matched healthy controls participated to recognize the dynamically positive expression in the MEG scans. Results In comparison with the healthy,the abnormally activities regions in depressed patients during different time periods were separately as follow;decreased activation in the right inferior parietal lobule(t = 3.94),the right su-pramarginal gyms(t = 3.59),the bilateral posterior cingulated(t = 5. 04, t = 4. 85) ,the bilateral precuneus(t = 3.84) ,the left cuneus(t = 3.58) and increased activation in the bilateral hippocampus(t = 2. 13, t = 2.56) ,the left amygdale(t = 2.41), the bilateral uncus gyrus(t = 2.33, t = 2.44) , the right anterior cingulated (t = 2. 15) , the left fusiform gyms (t = 3. 33) (P < 0. 05, unconnected). Conclusion The results indicate, compared with healthy controls,abnormal brain activities as the weakened function of the medial temporal cortex,parietal lobe and the enhanced function of limbic system were distributed extensively in depressed patients during recognizing dynamically positive expression. These abnormalities may prompt the dynamic characteristics of the magnetic source imaging about recognizing dynamically positive expression,and further reveals the mechanisms of emotional symptoms in depression.

BACKGROUND: A large number of cytokines produce in the periodontium on the tension side during orthodontic tooth movement, participate in periodontal remodeling, and make the tooth move to its normal position, thus achieving alveolar remodeling. Heat shock protein 27 (HSP27) is reported to be involved in the periodontal remodeling. OBJECTIVE: To investigate the expression of HSP27 in the rat periodontium on the tension side during the orthodontic tooth movement.METHODS: The rat models of orthodontic tooth movement were constructed using NiTi coil spring with the force value of 50 g. The periodontal samples were collected at 1, 3, 5, 7, 10 and 14 days after exerting force to detect the expression level of HSP27 in the periodontium on the tension side.RESULTS AND CONCLUSION: Immunofluorescence staining results revealed that the expression level of HSP27 in the periodontal tissues began to increase at the 1st day after exerting force, reached the highest level at the 3rd day, and then fell to the initial level at the 5th day. To conclude, HSP27 is evoked by orthodontic force early, and it may play a role in periodontal remodeling.

Objective To observe the effect of short wave diathermy on the functioning of dendritic cells ( DCs) derived from the spleens of hepatitis B virus ( HBV) transgenic mice. Objective To investigate the effect of docetaxel combined capecitabine chemotherapy and psychological intervention after radical mastectomy. Methods 80 cases of radical mastectomy for breast cancer treated in our hospital from January 2015 to January 2017 were treated with docetaxel combined with capecitabine chemotherapy. According to different nursing methods, the patients were divided into 2 groups. The observation group was treated with psychological nursing, while the control group received routine nursing care. The anxiety and depression scores in two groups before and after nursing were observed and compared. Results There were no significant difference in the scores of anxiety and depression between the two groups before and after treatment. After the intervention group, patients in observation group were significantly improved, and better than the control group patients; The nursing satisfaction rate in the observation group was 97.5%,which was significantly higher than that in the control group (72.5%) (P<0.05). Conclusion After radical mastectomy of breast cancer with docetaxel combined with capecitabine chemotherapy and psychological clinical observation of dry pre found in breast cancer resection and chemotherapy as the basis, to patients with psychological intervention can improve the clinical situation of patients, promote their mental status improved, improve nursing satisfaction rate of patients, so it is worthy of reference.
10.Expression of heat shock protein 70 in periodontium during orthodontic tooth movement

BACKGROUND:Extensive research has been concentrated on the reconstruction of periodontium under orthodontic force. However, there are no reports regarding the expression of heat shock protein 70 (HSP70) in rat periodontium during orthodontic tooth movement. OBJECTIVE:To investigate the expression of HSP70 in rat periodontium during orthodontic tooth movement. METHODS:Rat orthodontic tooth movement models were established using NiTi extension spring with the force of 50 g. The orthodontic periodontium was col ected at 1, 3, 5, 7, 10 and 14 days after exerting force to detect the expression of HSP70. RESULTS AND CONCLUSION:According to immunofluorescence staining, the expression of HSP70 in the periodontal tissues was increased on day 1 after exerting force, and reached the highest level on day 5, then decreased to the initial level on day 14. These results suggest that the expression of HSP70 is evoked by orthodontic force, and it may play a role in periodontal remodeling.
