1. Evaluation on the operative situations of radical hysterectomy in 440 cervical cancer patients

Objective: To analyze the clinical and operative situations of radical hysterectomy in 440 cervical cancer patients. Methods: The clinical and operative situations were retrospectively evaluated in 440 cervical cancer patients. All the patients received radical hysterectomy and pelvic lymphadenectomy by the Wertheim-Okabayashi procedure. Results: The average age of the patients was 43.9 (21-78) years old and the mean body mass index (BMI) was 23.2(15.8-47.3) kg/m2. Histologic classification included: 353 cases (80.2%) of squamous cell carcinoma, 56 (12.8%) cases of adenocarcinoma, 20 (4.6%) cases of adeno-squamous carcinoma, and 11 (2.5%) cases of other types. According to International Federation of Gynecology and Obstetrics (FIGO) staging standard, 314 (71.4%) cases were at stage I and 126 (28.6%) cases were at stage II, respectively. The mean operation time was 221 (90-410) min and the average blood loss was 697 (80-2 300) mL. One hundred and forty eight patients received blood transfusion with an average volume of 825 (200-2 300) mL. 87 patients (19.8%) had lymph node metastasis, and the average number of positive lymph nodes was 2.1(1-5). 83 patients (18.9%) had lymph-vascular space involvement (LVSI). The patients stayed in hospital for 26(9-57) days. Post surgery complications occurred in 145 women (32.9%). They included urine retention, wound infection, lymphocyst, and deep-vein thrombosis (DVT). One hundred and 25 patients had more than 100 mL of post-void residual urine volume (PVR), and 73 patients got fever after surgery. Conclusions: Radical hysterectomy and pelvic lymphadenectomy performed in our hospital achieved better clinical outcome in the treatment of cervical cancer.
2.Correlation between asymmetric dimethylarginine and endothelial dysfunction in patients with uremia

Objective To investigate the correlation between asymmetric dimethylarginine (AD-MA) and endothelial dysfunction in patients with uremia.Methods Uremic patients who did not receive hemodialysis were defined as A group (n =40) ; uremic patients who had received hemodialysis were divided into B group (n =45) ;healthy people were defined as C group (n =20) ;and chronic kidney disease (stage 2 ~ 4) patients were defined as D group (n =20).The diameter of intima-media thickness,and endothelium-dependent or independent dilation (EDD or EID) of radial artery in right forearm were detected with diasonography.The levels of ADMA were determined by high-performance liquid chromatography.Results Compared to C group,the levels of ADMA in A,B and D groups were significantly increased [C:(0.78 ±0.19) μmol/L,A:(1.51 ±0.16) μ mol/L,B:(1.13 ±0.14) μmol/L,D:(0.92 ±0.11) μmol/L; P <0.05].Compared to A group,the levels of ADMA were significantly decreased in B group (P <0.05).EDD and EID were decreased significantly in A,B and D groups compared to C group [EDD:C:(13.52±1.73)% vs A:(7.32 ±0.54)%,B:(9.02 ±0.86)%,D:(10.13 ±1.25)%,P <0.05;EID:C:(14.45±1.85)% vsA:(10.37 ±1.51)%,B:(9.54±1.39)%,D:(11.17±1.56)%,P <0.05].EDD in B group was significantly lower than A group (P <0.05).In group A,a negative correlation was found between EDD and the level of ADMA (r =-0.81,P =0.020).Conclusions ADMA level was significantly increased in uremic patients.A close correlation existed between ADMA and endothelial dysfunction of radial artery.
3.Effect of hypoxia and hyperglycemia on retinal pigment epithelial cells co-cultured with human mesenchymal stem cells

AIM: To evaluate cell proliferation, apoptosis and migration of human retinal pigment epithelial cells ( RPE) when co - cultured with human marrow mesenchymal stem cells ( hMSCs ) in condition of hypoxia and hyperglycemia so as to explore possible mechanisms of diabetes aggravating choroidal neovascularization ( CNV) preliminarily.
METHODS:Both hMSCs and RPE cells were co-cultured in a transwell system. The experiment was divided into four groups: 21% O2 with 5. 56mmol/L glucose ( control group, A ), 21% O2 with 30mmol/L glucose ( hyperglycemia and normoxia group, B ) , 5% O2 with 5.56mmol/L glucose ( normoglycemia and hypoxia group, C ) and 5% O2 with 30mmol/L glucose ( hyperglycemia and hypoxia group, D) . Cell Counting Kit-8 (CCK-8) was used to detect the proliferation of RPE cells in each group at 12, 24 and 48h respectively. Flow cytometry was performed to observe apoptosis of RPE cells at 24h. Additionally, we assessed migration
capabilities of RPE via transwell assay under the condition of hyperglycemia and hypoxia by co-culturing of hMSCs.RESULTS:In this co-culturing system, at 12, 24 and 48h, group B (1. 61±0. 41, 1. 80±0. 34;1. 91±0. 35), C (1.34±0. 46, 1. 94±0. 40, 2. 14±0. 41) and D (1. 98±0. 47, 2.26±0.42, 2. 55±0. 40) showed significantly higher proliferation rate than group A (0. 92±0. 45, 1. 27±0. 32, 1.59±0. 41, P<0. 05). The migration capabilities of RPE in group B (149. 5±9. 19), C (140±9. 90) and D (170. 5±7. 78) increased dramatically compared with group A ( 114. 5±7.78, P<0.05) at 24h, whereas there was no significant difference of apoptosis ratio among four groups (P>0. 05).
CONCLUSION:By coexistence with hMSCs, the synergy of hyperglycemia and hypoxia can improve migration and proliferation of RPE cells, and have no effect on apoptosis of RPE cells within short period.
4. Studies on the robot-assisted single port laparoscopic surgery of gynecological malignant tumors

Objective To explore the technique of single port laparoscopy assisted by the third generation SI class Da Vinci robot in treatment of gynecological malignant tumors. Methods The clinical data were retrospectively analyzed of 8 patients with robot-assisted single port laparoscopy of gynecological malignant tumor from June to August 2018 in Changzheng Hospital. Of the 8 patients [aged 34–51 (43±6) years and BMI of 20.1–26.3 (23.2±3.1) kg/m2], 4 were early endometrial cancer and another 4 were early cervical cancer. With the help of porous instruments of ordinary robot system and Lagiport single-site multi-channel platform, the third generation SI class Da Vinci robotic surgical system was used to complete the robot assisted transumbilical single port laparoscopic surgery. Four patients with early endometrial cancer received total hysterectomy + double-adnexy resection + sentinel lymph node biopsy, and another 4 with early cervical cancer received radical resection of cervical carcinoma. Results Robot assisted single port laparoscopic surgery was successfully performed in all 8 patients with no additional auxiliary puncture port or laparotomy. The operation time was (208±68) min, and the intraoperative blood loss was (155±71) ml. No injury of ureter, bladder, rectum and obturator nerve and blood transfusion occurred during operation. No postoperative complications occurred such as blood transfusion, persistent fever, wound exudation, pelvic and abdominal infection and deep venous thrombosis. Four patients with endometrial cancer were discharged 4 days after operation, and another 4 with cervical cancer were discharged 7 days after operation. All patients were satisfied with the wound after operation. Conclusion The third generation SI-class Da Vinci robot can complete gynecological malignant tumor surgery with the help of ordinary robot instruments and Lagiport single-site multi-channel platform, which is one of the effective surgical methods for gynecological malignant tumor.
5.Establishing scientific evaluation view to promote traditional Chinese medicine

Traditional Chinese Medicine(TCM) makes great contributions to the prosperous growth and people's health.But understanding deviation and imperfect evaluation system of TCM affect the healthy development of TCM.Clinic practice is the motive power of TCM,and curative effect is the key of TCM researches,and the scientific evaluation system is the safeguard for a healthy development of TCM.So we should focus on clinical researches of stubborn diseases and emergency cases to satisfy social demand and upgrade the position of TCM in the medical system.At the same time,functional disease must be explored to show the advantage of TCM.Our mission is to establish a scientific objective evaluation system to accurately understand TCM and take it as the turning point to give an impetus to theoretical breakthrough of the basic studies to promote an overall and healthy development of TCM.

7.Protection of valsartan combined with simvastatin on the kidney in early diabetic nephropathy rats

Objective To explore the protection of valsartan combined with simvastatin on kidney in early diabetic nephropathy rats. Methods Diabetic nephropathy rats model were induced by streptozocin (STZ) ,the experimental rats were randomly divided into 5 groups: control (group C), diabetic nephropathy (group D) ,diabetes treated with valsartan (group X) ,diabetes treated with simvastatin (group Z) ,and diabetes treated with combined valsartan and simvastatin ( group L). Blood glucose (BG), HbA1c, blood cholesterol ( TC), trigalloylglycerol ( TG ), blood ureanitrogen ( BUN ), serum creatinine (SCr) , urinary albumin excretion rate (UAER) were measured, and the podocyte ultrastructure was observed by transmission electronic microscopy. Results The levels of BG, HbA1c,TC,TG and UAER in group D increased significantly compared togroup C(BG:[20.3 ±3.2]mmol/L vs [6.1 -±0. 4]mmol/L;HbA1c:[7.18 ±0.47]% vs [3.37 ±0. 15]% ;TC: [2. 69 ±0. 35] mmol/L vs [1.28 ±0. 24] mmol/L;TG: [3.09 ±0. 37] mmol/L vs [1.18 ±0. 25]mmol/L) (P < 0. 05 ). Creatinine clearance rates (Ccr) in group D ( [0. 89 ± 0. 19] ml/min ) decreased significantly compared to group C( [1.27 ±0. 33] ml/min) ,as well as group X,Z and L( Ps < 0. 05 ). UAER in group D was significantly higher than that in group C ( [19. 87 ±3. 85] μg/24 h vs [3. 67 ± 1.01] μg/24 h) (P < 0. 05 ), as well as group X, Z and L ( P < 0. 05 ), and the improvement in group L was particularly significant ( P < 0. 05 ). The projections of podocyte in group D severely syncretized, there were slightly improvement in group X, Z and L compared to group D, and the improvement in group L was remarkable. Conclusion The treatment with valsartan, simvastatin and their combination will effectively protect the kidney in early diabetic nephropathy rats,and the effect of using the combination therapy is much better.

9.The X-ray findings of spondylometaphyseal dysplasia

Objective To explore the clinical,radiological findings and pathogenic factor of spondylometaphyseal dysplasia.Methods Five cases were reported and the relevant documents were studied retrospectively.Plain X-ray film was performed in all patients.Results There were 4 male and 1 female,age ranged from 4 to 15 years with average of 9 years.The main features of 5 cases included delayed bone age,stature short,short trunk,waddling gait noted,scoliosis in 2 cases,kyphoscoliosis in 1 case,severe genu valgum in 2 cases.The main X-ray appearance of 5 cases is multiple irregularities of long bones metaphyses associated with platyspondylia,epiphysis is normal.Type Ⅰ SMD in 2 cases,Type Ⅱ SMD in 1 case,Type Ⅲ SMD in 2 cases.Conclusion When we meet children with delayed bone age and stature short,and muhiple irregularities of long bones metaphyses associated with platyspondylia were seen in X-ray plain film.we should think about spondylometaphyseal dysplasia.
10.Transurethral seminal vesiculoscopy combined with finasteride for recurrent hematospermia.

OBJECTIVETo evaluate the clinical effectiveness of transurethral seminal vesiculoscopy (TUSV) combined with finasteride in the treatment of recurrent hemospermia.
METHODSThis study included 32 patients with recurrent hematospermia, with the disease course of 3 months to 4 years. After administration of finasteride at 5 mg/d for 2 weeks, the patients underwent TUSV for both exploration of the causes and treatment, followed by medication with finasteride at the same dose for another 2 weeks. Postoperative follow-up was conducted for observation of the outcomes and complications.
RESULTSTUSV was successfully accomplished in all the 32 cases, which revealed 16 cases of seminal vesiculitis, 10 seminal calculi, 1 seminal vesicle cyst, 2 seminal vesicle polyps, and 3 seminal vesicle abscess. The operative time was 20 to 51 (31.0 +/- 5.2) minutes. Postoperative complications included 1 case of acute epididymitis and 3 cases of breast discomfort within the first 4 weeks. No incontinence, urethral stricture, rectal injury, retrograde ejaculation, and sexual dysfunction occurred postoperatively. All the patients but 1 were followed up for 6 months to 2 years. Twenty-nine of the cases were cured, and 2 experienced recurrence.
CONCLUSIONTransurethral seminal vesiculoscopy combined with finasteride is safe and effective for the treatment of recurrent hemospermia.
