To explore the effects of Western medicine including Ca(2+) channel antagonist and angiotensin-receptor antagonist combined with Songling Xuemaikang Capsule (SXC), a compound traditional Chinese herbal medicine for calming liver and suppressing liver yang, on blood pressure indexes derived from 24-hour ambulatory blood pressure monitoring (ABPM), and to evaluate the effects and stability of SXC in lowering blood pressure.

OBJECTIVETo investigate the effect of folic acid on homocysteine (Hcy)-induced chemokine secretion and NADPH oxidase activity in human monocytes.
METHODSHuman monocytes from healthy volunteers were incubated with Hcy (100 micromol/L) with or without folic acid (5 micromol/L) for 24 h; MCP-1 and IL-8 were assessed by ELISA. DCFH-DA was added to monitor intracellular ROS production on confocal microscopy. A cytochrome c reduction assay was used to measure NADPH oxidase activity.
RESULTSThe Hcy-induced secretion of MCP-1 and IL-8 was significantly reduced by folic acid [(1.88 +/- 0.51) ng/ml vs. (4.36 +/- 0.72) ng/ml vs. (2.40 +/- 0.60) ng/ml and (4.9 +/- 1.9) ng/ml vs. (12.7 +/- 1.5) ng/ml vs. (7.2 +/- 1.9) ng/ml, all P < 0.05]. The Hcy-induced production of ROS was also significantly attenuated by folic acid. Moreover, the Hcy-induced NADPH oxidase activity increase was significantly inhibited by cotreatment with folic acid.
CONCLUSIONFolic acid may attenuate oxidative stress induced by Hcy by reducing NADPH oxidase activity in monocytes.

3.Investigation of the nursing document quality in Harbin general hospitals

Objective To find out the defects of nursing documents and the interrelated causes and to propose measures for improvement through investigating the status of nursing documents quality of 10 general hospitals in Harbin, with the aim of providing valuable reference for sustainable improvement of nursing documents. Methods Using stratified sampling method, five secondary general hospitals and five tertiary general hospitals were randomly selected in Harbin. Nursing documents of general surgery and cardiac care were investigated, 909 nursing documents from November 2008 to May 2009 were researched, including 262 documents of surgical department and 265 cases of medical department in tertiary hospitals. Surgical department 178 and medical department 204 in secondary hospitals. Combined literature method, spot field investigation and personal interview , the status were researched, the data were analyzed. Results The nursing documents quality assessment of the objective hospitals showed that: the average scores of surgical departments and medical departments in tertiary hospitals were 97. 20 and 97.26, respectively; scores of surgical departments and medical departments were 97.64 and 98.22 in secondary hospitals. The medical documents qualities of the secondary hospitals were better than the tertiary hospitals'; The surgical nursing documents qualities had statistical difference. Critical care nursing records had the best quality. The main problems are: scratch, sticky,coating, the wrong date, the content of records are incomplete, illegible handwriting, the wrong records of the dynamics of blood pressure, and so on. Conclusions In general hospital of harbin city, the overall quality of nursing documents are good, but there are still some problems. The main causes of nursing documents defects are the shortage of nurses, awareness of the law, indifferent sense of duty, nurses professional and cultural qualities which need to be improved, lack of health care communication and insufficient administrative supervision.
4.Treatment of clavicular fractures using intramedullary nailing or K-wire versus plating fixation: a meta-analysis.

OBJECTIVETFo compare the efficacy and complications rate of intramedullary (IM) nailing or K-wire versus plating fixation for clavicular fractures.
METHODSPubmed, Embase, Cochrane Library databases, CNKI, VIP and Wangfang databases were searched to find all randomized or quasi-randomized controlled trials of clavicle fractures using plating versus IM nailing or K-wire. The methodologic quality of the studies was assessed. After independent study selection by 2 authors ,data were collected and extracted independently. Outcomes of postoperative shoulder functional measurement, the efficacy and information of the operation and complications rate were meta-analyzed using RevMan 5 software.
RESULTSNine hundreds and seventy-six patients in 10 randomized controlled trials (RCTs) and 3 quasi-RCTs were involved in the meta-analysis,of which 5 studies compared the K-wire and the plating fixations and 8 studies compared the IM nailing and the plating fixations. The overall odds ratio(OR) (with 95% CI) of the operation efficacy for K-wire versus the plating was 3.79 (1.93, 7.46). The overall weighted mean difference (with 95% CI) of Constant Shoulder score for plating versus IM fixation was -1.39 (-3.43, 0.65) in 6 studies. The overall OR of the plating versus IM nailing was 9.34(2.70, 32.32) for the overall major complications in 5 studies and 5.04 (1.52,16.77) for the revision rate in 5 studies.
CONCLUSIONThe current limited evidences suggested that the IM fixation could reduce the incidences of the overall major complications and the revision surgery, while the post-operative efficacy of the plating was superior to the K-wire. More high quality RCTs are still needed in the future.

5.Expressions and clinical implications of kruppel-like factor 6 and APC in human colorectal carcinoma.

OBJECTIVETo investigate the expressions and clinical implications of kruppel-like factor 6(KLF-6) and APC in human colorectal carcinoma.
METHODSThe expressions of KLF-6 and APC in tumor and normal tissues from 32 patients with colorectal carcinoma were investigated by RT-PCR and immunohistochemical technique.
RESULTSThe expression rates of KLF-6 and APC mRNA were 37.5% and 34.3% in tumor tissue, 96.9% and 93.8% respectively in normal tissues (both P< 0.05). The expression rates of KLF-6 and APC protein were 28.1% and 25.0% in colorectal carcinomas, 81.3% and 84.43% respectively in normal tissues (both P< 0.05). There was a significant correlation between the expressions of KLF-6 and APC in colorectal carcinomas (P < 0.05). The expressions of KLF-6 and APC were significantly correlated with tumor differentiation, depth of infiltration, lymph node metastasis and clinical stage (P< 0.05).
CONCLUSIONDown-regulations of KLF-6 and APC might play an important role in the carcinogenesis, development, metastasis of human colorectal carcinoma.

6.Gentiopicroside,originated from Gentiana macrophylla Pall,possesses anti-arthritic efficacy in adjuvant-induced arthritis rats

OBJECTIVE This work aimed to investigate the anti-rheumatoid arthritic effect of gentio-picroside from Gentiana macrophylla Pall using an animal model of adjuvant induced arthritis. METH-ODS Adjuvant arthritis was induced in fifty SD male rats,which were randomly divided into five groups (n=10):control(0.5% CMC-Na)group,AIA(rats with CFA)group,dexamethasone(1 mg·kg-1)group, gentiopicroside(50 mg·kg-1)group,and gentiopicroside(100 mg·kg-1)group.Rats were administered intragastrically with drugs or CMC-Na once a day for a period of 2 weeks.Paw swelling,arthritic index, histological changes were assessed to evaluate the anti-arthritic effect.Weight growth,spleen and thymus indexes were also investigated in.RESULTS Gentiopicroside at dose of 100 mg·kg-1significantly inhibited the secondary paw swelling(P<0.05)and arthritis index(P<0.05),decreased synovial inflammatory infil-tration, synovial hyperplasia and bone erosion. Furthermore, gentiopicroside showed no immunosup-pressive adverse effects in body weight, index of spleen and thyums compared with dexamethasone administration (P<0.05, P<0.01). CONCLUSION Gentiopicroside possessed anti-arthritic efficacy in AIA rats without immunosuppressive effects.
7.Causes of chronic cough in children in Chongqing, China: an analysis of 202 cases.

OBJECTIVE:
To investigate the constitution of causes of chronic cough in children.
METHODS:
The clinical data of 202 children with chronic cough who were hospitalized in the Affiliated Children's Hospital of Chongqing Medical University from May 2015 to November 2017 were retrospectively analyzed.
RESULTS:
As for the causes of chronic cough in the 202 children, 81 (40.1%) had post-infection cough (PIC), 71 (35.1%) had cough variant asthma (CVA), 43 (21.3%) had upper airway cough syndrome (UACS), 3 (1.5%) had foreign body aspiration, 1 (0.5%) had gastroesophageal reflux cough, 2 (1.0%) had Tourette syndrome, and 1 (0.5%) had congenital respiratory disorders. Of the 202 children, 119 (58.9%) had chronic cough caused by a single factor and 83 (41.1%) had chronic cough caused by multiple factors. There was a significant difference in the constitution of causes of chronic cough among the children with different ages (<1 year, ≥1 year, ≥3 years and 6-14 years) or natures of cough (wet cough and dry cough) (P<0.01).
CONCLUSIONS
The top three causes of chronic cough in children are PIC, CVA and UACS. There are significant differences in the main causes of chronic cough between children with different ages and between those with different natures of cough.

8.The long-term outcome and prognostic factors analysis of multimodality treatment for 283 patients with thymoma

Objective To study the long-term outcome and prognostic factors in patients with thymoma treated with multimodality therapy. Methods 283 patients with pathologically confirmed thymoma were enrolled into this study. Overall survival rate and local control rate were calculated. Cox regression model was used to analyze the prognostic factors. Results The 5-year overall survival rate was 94.3%, 86.3%, 71.6%, 39.4% and 80.4% for patients with stage Ⅰ,Ⅱ,Ⅲ, Ⅳ disease and all patients, respectively. The 10-year overall survival rate was 84.3%, 75.4%, 56.6% , 29.6% and 68.4% for stage Ⅰ,Ⅱ,Ⅲ, Ⅳ disease and all patients. The Masaoka staging, resection extent, and gender were independent prognositic factors in multivariate analysis. Conclusion The Masaoka staging and resection extent are independent prognositic factors in patients with thymoma treated with multimodality therapy.

10.The impact of centralized volume-based procurement policy on the pharmaceutical industry:Review based on the SCP paradigm

Objective:To systematically review the impact of Chinese centralized volume-based procurement(VBP)policy on the pharmaceutical industry.Methods:Relevant literature published between January 1,2019 and July 31,2023 were retrieved from the database of CNKI,WanFang data,PubMed,and Web of Science.A thematic analysis framework was constructed for data collation and analysis applying the Structure-Conduct-Performance(SCP)paradigm.Results:Forty-one studies were included,with 10(24.4%),24(58.5%),and 23(56.1%)reporting the dimension of structure,conduct,and performance,covering 10 secondary themes.Literature shows that,in terms of market structure,the market share of bid-winning enterprises increased after VBP policy,and the concentration ratio of pharmaceutical industry promoted;as for enterprise conduct,the R&D investment of both bid-winning and bid-non-winning enterprises increased under policy implementation,and the selling expenses on policy-covered products significantly reduced while policy-uncovered products did not;as for industry performance,the impact of VBP policy on operational performance varies among enterprise,and the initially positive effect on promoting innovative output of bid-winning enterprises was observed.Conclusions:The VBP policy has shown positive effects in optimizing market structure,guiding enterprise conduct,and improving industry performance.To consolidate and expand policy effects,it is recommended to further strengthen the comprehensive management of drug prices,expand the boundaries of the VBP market,and strengthen policy monitoring and tracking research.
