1.The predictive value of five factors to the maternal difficulty airway

Objective To research the most commonly used five method to evaluated the difficulty airway , and compare which methods were more suited for the pregnant woman in general anesthesia. Methods 214 patients with full-term pregnancy who requested emergency or elective caesarean-section were assigned. During the pre-anesthetic visit,we evaluated patients from Mallampati score, thyromental distance, body mass index (BMI), inter-incisor gap, and upper lip bite test. After endotracheal intubation ,patients were divided into 2 groups based on Cormack classification. Results Five ways sensitivity descending order were upper lip bite test (79.5%)、Mallampati score (76.9%)、BMI (56.4%)、inter-incisor gap (51.3%)、thyromental distance (35.9%); specificity descending order were upper lip bite test (93.1%)、Mallampati scores (86.3%)、inter-incisor gap (85.1%)、thyromental distance (76.6%)、BMI (62.3%). Conclusions In pregnant women ,Mallampati score and the upper lip bite test are the better indicators to predict difficult airway.
2.Feature of airway inflammation in asthmatic rats complicated with chronic obstructive pulmonary disease and effect of Chinese herbs for reinforcing Shen and supplementing qi on it.

OBJECTIVETo observe the airway inflammatory change in asthmatic rats complicated with chronic obstructive pulmonary disease (COPD) and to assess the intervention effects of Chinese herbs for reinforcing Shen and supplementing qi (CH) on it.
METHODSEighty-four Norway rats were randomized into 7 groups, the normal control group (A), the COPD model group (B), the asthma model group (C), the combined COPD and the asthma model group (D), and the three CH treated groups (E, F and G, combined model rats administered by low-, moderate- and high- dose CH, respectively), 12 rats in each group. Changes of symptoms, pathologic changes of the lung tissue, airway reactivity, and serum levels of interleukin-4, interleukin-6, interleukin-8 (IL-4, IL-6, and IL-8) and interferon-gamma (IFN-gamma) in rats were observed.
RESULTSSymptoms were alleviated in the three CH treated groups. Similar pathological features were shown in group B and D, showing inflammatory cell, mainly lymphocyte, infiltration in bronchial and lung tissues, with cilia denudation, partial alveolar wall rupture, alveolar cavity expansion, and accompanied with evident eosinophilic infiltration. These inflammatory exudation in group E-G was alleviated, while in group C, it developed showing a trend similar to that in group D. Airway resistance raised along with the concentration of Mch used. In group D, the serum level of IL-4 was higher than that in group B, and level of INF-gamma was lower than that in group A, B and C (all P <0.05). CH showed a lowering effect on serum levels of IL-4 and -8, and a dose-dependent rising effect on IFN-gamma.
CONCLUSIONSIL- 4 significantly increased and INF-gamma decreased in rat model of combined COPD and asthma, its mechanism is similar to that of Th1/Th2 imbalance in asthma. Chinese herbs for reinforcing Shen and supplementing qi could improve the symptoms and inhibit the airway inflammation in the combined COPD and asthma model rats, its mechanism might be related with the alleviation of TH1/TH2 imbalance.

3.Effect of epimedium herb on RANTES and monocyte chemotactic protein-3 expression in lung tissue of asthmatic rats.

OBJECTIVETo observe the effects of epimedium herb (EH), a Chinese herb for replenishing Shen, on regulated on activation, normal T call expressed and secreted (RANTES) and monocyte chemotactic protein-3 (MCP-3) expression in lung tissue of asthmatic rats, for further exploring the action mechanism of EH in treating asthma.
METHODSBrown Norway rats were randomly divided into six groups: the normal control group (A), the allergic asthma model group (B), the group of model rat treated with dexamethasone (C), and the three groups of model rat treated with low (0.125 g/mL), medium (0.5 g/mL), high (2.0 g/mL) dose of EH (D, E and F). RANTES and MCP-3 mRNA expressions in lung tissue were tested with Real-time PCR and the serum levels of tumor necrosis factor alpha (TNF-alpha), interleukin-4 (IL-4), and interleukin-5 (IL-5) were measured by enzyme linked immunosorbent assay (ELISA).
RESULTSAs compared with the model group (B), RANTES, MCP-3 expression and TNF-alpha were lower in the 4 treated groups (C, D, E and F, P < 0.05 or P < 0.01); IL-4 was lower in Group C, E and F (P = 0.007, P = 0.047, P = 0.033), while that in Group D was higher than that in Group C (P = 0.012). As for level of IL-5, lowering was shown only in Group C and F (P = 0.003, P = 0.005).
CONCLUSIONApplying EH in the attack stage of asthma can alleviate the airway inflammation by down-regulating the expression of RANTES and MCP-3 in lung tissue.

4.Separation and purification of active fraction with macroporous resigns

AIM: To purify the water-extract of Sinisa recipe with macoroporous resigns. METHODS: Screening the purification condition by means of the content of active principle, fingerprint chromatography, the yield of dry extracts,et al. of the purified product. RESULTS: The purified product by type HP20 macoroporous resign is in good agreement with the active fraction of Sinisa in the composition of specific principles. CONCLUSION: HP20 type macoroporos resign is considered as optimum for active fraction of Sinisa recipe in purifying efficience.
5.Moxibustion therapy improving delayed memory deficits via promoting neurogenesis and angiogenesis of hippocampus in a vascular dementia rat model

Objective:To investigate the alteration of delayed memory and its relationship with neurogenesis and angiogenesis in vascular dementia (VD) rats after moxibustion therapy. Methods: Two hundred adult male SPF Wistar rats were chosen for the experiment. Thirty-six rats were randomly selected as the sham operation group. Except for rats in the sham operation group (n=36), the others were made into VD models by bilateral common carotid arteries occlusion (BCCAo). After modeling, the 108 survived rats were randomly divided into 3 groups: a model group, a neural stem cells (NSCs) plus endothelial progenitor cells (EPCs) moxibustion group and a NSCs moxibustion group. Co-transplanted implant was transplanted into the rats in the NSCs plus EPCs moxibustion group, and the rats in the NSCs moxibustion group were transplanted by NSCs only. The NSCs plus EPCs moxibustion group and the NSCs moxibustion group received suspended moxibustion therapy at Baihui (GV 20), Dazhui (GV 14) and Shenting (GV 24), (each group was divided into 3 subgroups by the treatment course as 1, 2 and 3 courses). Every group was measured by Morris water maze to evaluate its delayed memory after 3 treatment courses and the rat’s brain was taken out after perfusion of 4% paraformaldehyde one day after 1, 2 and 3 treatment courses, respectively. Marker protein expression was detected by laser confocal microscope to analyze the effect on neurogenesis and angiogenesis. Results: VD rats showed delayed memory in Morris water maze test 3 d after ischemic injury. After 3 courses of moxibustion therapy, VD-induced delayed memory deficits were improved in the NSCs plus EPCs moxibustion group and the NSCs moxibustion group. The expressions of nestin, doublecortin (DCX) and CD34 increased significantly in the two moxibusiton groups after every treatment course (all P<0.05), which might contribute to the neurogenesis and angiogenesis in hippocampus. In addition, compared with the rats in the NSCs moxibustion group, the expressions of nestin, DCX and CD34 increased significantly in the NSCs plus EPCs moxibustion group (P<0.05). Conclusion: Moxibustion can reverse VD-induced delayed memory deficits, which may be related to the promotion of neurogenesis and angiogenesis.
6.Primipara' s fatigue status during pregnancy and analysis of the related factors

Objective To investigate primipara' s fatigue status during pregnancy and the major related factors, provide basis for relevant intervention measures. Methods A total of 150 primiparas from a top-notched hospital in Beijing were investigated by using General Fatigue Self-assessment Scale (FSAS) ,Self-Efficacy Scale (GSES), Trait Coping Style Questionnaire (TCSQ) and Pregnancy Pressure Scale (PPS). Results Primipara's fatigue status was at a mild level, and could be relieved by sleep or rest; Age, gestational age and weight gain during pregnancy had different degrees of relevance with the score of FSAS( P <0. 05 ) ; Primipara' s fatigue was negatively relate to the general self-efficacy ( P <0. 05 ) ; Primipara' s fatigue was positively relate to positive coping,pregnancy pressure(P <0. 05 ). Conclusions The health education and pregnancy counseling for the primipara should be strengthened. And we should pay attention to primipara with fast weight gain, old gestational age and pregnancy complications. Further more, primipara should be encouraged to adopt positive coping, cultivate self-efficacy and reduce pregnancy stress by using early period intervention, which could help to reduce fatigue during pregnancy.
7.Coblation treatment for laryngeal papilloma in adult

Objective To evaluate the effect and feasibility of coblation treatment for laryngeal papilloma in adult. Methods A total of 18 patients with laryngeal papilloma treated by coblation from April 2008 to June 2010 was retrospectively analysed. There were 4 cases of multiple laryngeal papilloma and 14 cases of single laryngeal papilloma. All of these patients were treated with CoblatorTM Reflex #7070 under general anesthesia without tracheotomy. Results The volumes of blood loss during surgery varied from l ml to 10 ml, 2 ml on average. There was no postoperative bleeding or other complications. Follow-ups ranged from 6 months to 33 months after surgery, median 18 months. Recurrence occurred 4 months and 8 months after surgery in two patients. They were treated with coblation for the second time and followed up for 4 months and 12 months respectively. Recurrence occurred again in one patient. Conclusion Coblation under suspension laryngoscope for laryngeal papilloma in adult was an ideal way due to less bleeding and less damage.
8.On functional area of Back-shu based on relationship between Back-shu points and Jiaji points.

OBJECTIVETo prove the existence of functional area of Back-shu and provide a new thinking for clinical practice.
METHODSThe consistency between the Back-shu points and Jiaji points at the same level of spinal column was discussed from the origin and development, mechanisms and clinical application of Back-shu and Jiaji points.
CONCLUSIONThe points at the same level of spinal column has same origin of development, mechanisms and clinical functions, so as to prove the existence of functional area of Back-shu.

9.Effects of icariin on Bcl-2 and Bax protein expressions and eosinophils apoptosis in bronchial asthmatic mice.

OBJECTIVETo study the effects of icariin on Bcl-2 and Bax protein expressions and eosinophils apoptosis in bronchial asthmatic mice.
METHODS48 female Balb/c mice were randomly divided into 6 groups, i.e., the normal control group, the model group, the Dexamethasone group, the low dose icariin group, the middle dose icariin group, and the high dose icariin group, 8 mice in each group. Bronchial asthma in mice were induced by intraperitoneal sensitization and challenged with nebulized ovalbumin (OVA). The mice of each treatment group were administrated with different doses of icariin by peritoneal injection from the first asthma sensitization (the 3rd week after the modeling) to the day before killing once every other day, while mice in the normal control group were administrated with physiological saline. The mice were killed after 6 weeks of treatment. The apoptosis of eosinophils and the Bcl-2 and Bax protein expressions of the lung tissues were detected by TUNEL and immunohistochemical assay respectively.
RESULTSAs compared with the model group, the apoptosis ratio of eosinophils were higher in the rest four treatment groups (P<0.05). The Bcl-2 protein positive areas in the lung tissues and the airway wall were significantly lowered (P<0.05). The Bax protein positive area significantly increased (P<0.05).
CONCLUSIONIn bronchial asthmatic mice, icariin could enhance the apoptosis of eosinophils and lessen their infiltration by decreasing the expression of Bcl-2 protein and increasing the expression of Bax protein in lung.

10.Comparison of the effect of rh-endostatin on intratumoral and myocardial micrangium in mice.

OBJECTIVETo compare the effect of rh-endostatin on micrangium in tumor and myocardial tissue in nude mice.
METHODSNude mice were randomized into 4 groups (10 mice in each group), blank control group (without tumor burden, received NS 100 µl×d(-1) injection), drug control group (without tumor burden, received rh-endostatin 400 µg×d(-1) injection), model group (with tumor burden, received NS 100 µl×d(-1) injection) and treatment group (with tumor burden, received rh-endostatin 400 µg×d(-1) injection) for 28 days. The tumor volume and body weight of the mice were measured before and after administration. The expression of CD34, MMP-2, MMP-9, HIF-1α and VEGF in the myocardium and tumor were detected by immunohistochemistry. The vascular structure was observed by immunoenzymatic CD34 and Masson double staining.
RESULTSThe increase of tumor volume of the treatment group [(48.18 ± 37.31) mm(3)] was significantly lower than that in the model group [(113.80 ± 73.27) mm(3)). The changes of body weight was not significant different among the four groups. After treated with rh-endostatin, the expressions of MMP-9 and VEGF in tumors were significantly down-regulated, but the expressions of MMP-2 and HIF-1α in the tumor were not. The microvessel density (MVD) in the tumors of treatment group was significantly decreased compared with that of model group. The proportion of tumor vessels covered by collagen in the treatment group was increased compared with that of the model group. However, MVD and micrangium in myocardium were not changed significantly.
CONCLUSIONRh-endostatin can decrease the expression of MMP-9, VEGF and MVD, inhibit the tumor growth and normalize tumor micrangium in tumor but not weaken the MMPs and MVD of mature micrangium in myocadium.
