1.Study on the risk values of glycated hemoglobin in diabetes mellitus and pre-diabetes

Objective To study the risk values of glycosylated hemoglobin ( HbA1c) screening patients with diabetes mellitus(DM) and pre-diabetes(PreD) in different age and sex.Methods HbA1c results from10 840 in Taizhou Hospital in 2012 were retrospectively collected , and CHAID analysis of decision tree was used in preliminary hierarchy .Age was further divided into five groups according to the preliminary results , then one-way ANOVA and linear regression were utilized to determine the differences between age groups and the prediction intervals (PI) of HbA1c,respectively.The upper limit value of 95%PI of HbA1c was set as diabetes risk values ( DRVs).By comparison of the sensitivity of DM patients screening by DRVs in 1 889 cases whose HbA1c were between 6.0%to 6.5%and the upper limit value of traditional HbA1c reference intervals to verify the screening value ofDRVs .Then the DRVs of HbA1c were utilized to screen DM in the population whose HbA 1c is were 6.0%-6.5% in 2008,and followed their re-examination records toverify the screening value of the DRVs toin PreD patients. Results (1) On the basis of the initially root of decisiontree by age , the populationwas divided into 9 groups ( P =0.000, F =231.462).By calculatingthe 95% confidence interval(CI) of each group and merging reasonably,it was finally divided into 5 groups:20-30years,31-40years,41-50years,51-60years and >60years (F=434.342, P=0.000).(2)Using the method of linear regression , the 95% PI of HbA1c of the 5 groups showed as following:20-30 years, males 4.6%-5.9%, females 4.6%-5.9%.31-40 years, males 4.8%-6.0%, females 4.7%-6.0%.41-50 years, males 4.9%-6.2%, females 4.8%-6.1%.51-60 years, males 5.0%-6.3%, females 4.9%-6.2%.>60 years, males 5.1%-6.4%, females 5.0%-6.3%.The traditional HbA1c reference interval was4.8%-6.3%.(3) The screening sensitivity of DM by the upper limit value of traditional HbA1c reference interval and DRVs in group which HbA1c was between 6.0%-6.5% were 34.5%and 48.1%,respectively.(4) Reviewing of the group with HbA1c between 6.0%-6.5% and not diagnosed as DM in 2008, 49 hadnegative DM screening results by using the upper limit value of traditional HbA1c reference interval but were positive in our setting DRVs , and 13 have developed to DM now , which accounted for 26.5%.Conclusion HbA1c DRVs need to be set hierarchically between different age and sex groups as a supplement to the traditional reference interval , which has a great significance for the diagnosis and treatment of DM and also forthe screening and intervention of PreD patients.
2.The comparative study of the clinical effect for different surgical approach to treat children with supracondylar fractures

Objective To investigate the clinical effect of tongue-shaped flap cut into the triceps and triceps both sides of the road approach road into the way of two surgical treatments of supracondylar fractures in children.Methods 90 patients with pediatric supracondylar fractures were chosen.50 cases of tongue-shaped flap to triceps road cut into were set as A group,40 patients taking brachial three muscles on both sides of the first approach were set as the B group.The clinical effects of the two groups were compared.Results The operative time,blood loss,cast immobilization time in B group was superior to group A ( P < 0.05 ),there was statistically different.The pain score improvement in group B was significantly better than group A before and after treatment(P <0.05),the difference was statistically significant.After treatment,A group of patients with excellent results was in 22 cases,good in 24 cases,the fine rate was 92%.B group with excellent results was in 23 cases,good was in 16 cases,the good rate was 97.5%.The superior quality of the treatment effect in B group was better than A group( P < 0.05),the difference was statistically significant.Conclusion The operative time,blood loss,time cast immobilization,pain improvement,the treatment effects for triceps both sides of the approach for the treatment of supracondylar fractures in children are better than the triceps tongue-shaped flap cut into the road,and there is high clinical value.
3.Expression of MIP-3? gene regulated by irradiation via Egr-1 promotor in Lewis lung cancer

Objective To construct the expression vector of early growth response-1 promoter-macrophage inflammatory protein 3? (pEgr-1-MIP-3?, pEM) to study MIP-3? expression in transfected Lewis lung cancer cells. Methods MIP-3? cDNA was inserted into plasmid with Egr-1 promoter by directional cloning. The vectors were transfected into Lewis lung cancer cells with liposomes. The expression of MIP-3? in transfected Lewis lung cancer cells induced by electron ray irradiation at different doses were confirmed by RT-PCR and Western blotting. Results The recombinant vector was sequenced, and the result indicated the correct insertion in the expression vector. After electron ray irradiation at different doses, MIP-3? expression in the transfected Lewis lung cancer cells increased significantly. Conclusion Electron ray can enhance the expression of MIP-3? in Lewis lung cancer cells. This has laid the experimental foundation for the studies of pEM for radio-genetic therapy in vivo.
4.MRI Appearances of Liver Carcinoma after Gamma Knife Therapy

Objective To study the value of unenhanced and dynamic contrast-enhanced MR imaging in follow-up of liver carcinomas after gamma knife therapy.Methods 57 patients examined by MR imaging after gamma knife therapy were analyzed retrospectively,and compared with outcome of AFP.Results Lesions treated with gamma knife therapy on precontrast imaging showed 4 kinds of appearances:(1) Slightly low signal intensity on T1WI and slightly high signal intensity on T2WI.(2)Slightly low signal intensity on both T1WI and T2WI.(3)Slightly high signal intensity on T1WI,isointensity and slightly high signal heterogeneous intensity on T2WI.(4)Slightly high signal intensity on both T1WI and T2WI.The necrotic lesions showed sustained ring-shaped or honeycomb enhancement or patching enhancement in hepatic tissue around the lesions with lesions unenhancing.Recurrent lesions or new lesions were enhanced in arterial phase,followed by rapid wash-out in the delay phase.The rate of completely necrosis was 93.5% in small liver carcinoma group(

6.How to relieve pre-operative parental anxiety before anesthesia: Chinese parental attitudes

Objective To assess Chinese parental requirement for relieving their preoperative anxiety,and their attitudes regarding the measures reducing their anxiety and to investigate the possible factors associated with increased anxiety.Methods One hundred and four pairs of pediatric patients (aged 2-12 yr,of ASA physical status Ⅰ or Ⅱ) undergoing elective surgery under general anesthesia and their parents were enrolled in this study.The general data of the pediatric patients and their parents and parental requirement for relieving their preoperative anxiety,and their attitudes regarding the measures reducing their anxiety were obtained using a questionnaire.Parental and children's anxiety? levels were assessed using the State-Trait Anxiety Inventory (STAI) and modified Yale Preoperative Anxiety Scale (m-YPAS),respectively.Results Parental anxiety levels were positively correlated with the children's anxiety levels (r =0.254).Parental desires to relieve their own anxiety under the personal direction of medical staff were significantly higher than the desires for the other measures; parental desires to relieve their own anxiety through parent-present induction of anesthesia (PPIA) were significantly lower than the desires for the other measures.The factors influencing parental attitudes included gender,educational level,preoperative anxiety level,and preoperative children' s anxiety level.Conclusion To relieve Chinese parental anxiety before surgery,the anesthesiologists should be aware that the majority of parents have a high desire to receive verbal instructions from medical staff before surgery was performed on their children and should provide enough information related to anesthesia and surgery; when PPIA is applied,parental desire for PPIA should be taken into full account; the anesthesiologists offer support and comfort for the parents properly.

8. Preparation of flavonoids from Glycyrrhiza uralensis by two-dimensional reversed-phase liquid chromatography

Objective To develop a systematic chromatography separation method for flavonoids from Glycyrrhiza uralensis Fisch. (GU). Methods A new method for the separation of effective parts and monomers of flavonoids from GU by two-dimensional reversed-phase liquid chromatography was developed using the self-developed preparation chromatography plant system with independent intellectual property rights. Flavonoids compounds were enriched with specific adsorption materials. The separation conditions of the chromatography were optimized by the chromatographic separation expert system software, and the loading weight of samples and the enrichment times of a separation were investigated. Results The process of chromatography separation of flavonoids from GU had good precision and reproducibility with C18 as separation and enrichment solid phase and the methanol/water and acetonitrile/water as mobile phase of one-dimensional and two-dimensional chromatography system. The dilution solution which used for one-dimensional and two-dimensional enrichment chromatography was water. The flow rate of gradient elution and dilution enrichment solution was 21 mL/min. The sample loading amount of chromatography separation was 300 mg each time. A total of 16 flavonoids parts contained stable chemical composition were obtained by the repeatable separation method after three times of enrichment. Nine pure compounds were obtained and identified by NMR and MS, which were liquiritin, liquiritigenin, formononetin, echinatin, 7,4'-dihydroxyflavone, 4'-O-[β-D-apio-D-furanosyl-(1→2)-β-D-glucopyranosyl] liquiritigenin, isoliquiritigenin, glycyrol, and glycycoumarin. Conclusion The study can provide a certain reference value for the systematic separation and cognition for flavonoids from GU.
9.The Research on Expression and Purification of a Chimeric Anti-p185 Antibody

The Her-2 proto-oncogene encodes a 185kDa transmenbrane glycoprotein p185 which has intrinsic tyrosine kinase activity. It is overexpressed in several malignant human tumors like breast cancer. A chimeric antibody by assembling a single-chain Fv antibody and a human IgG1 Fc fragment was constructed. This chimeric antibody reacts with tumor surface antigen p185c-erbB-2 specifically. In order to put the antibody into clinical application, two steps purification method was used to attain the antibody’s purity more than 95%. Both the lyophilized pharmaceutical formulations of the antibody were found. The formulations can keep the stability and activity of the antibody for at least one year. These results were the foundation of the chimeric antibody for cancer therapy.
10.Primary and metastatic hepatic neuroendocrine carcinoma: a clinicopathological study

Objective To study the clinicopathological features of primary and metastatic hepatic neuroendocrine carcinoma.Methods The records of 35 patients with primary hepatic neuroendocrine carcinoma and 35 patients with metastatic hepatic neuroendocrine carcinoma were retrospectively reviewed.These patients served as the primary group(priNET,n=35)and the metastasis group (metNET,n=35),respectively.Results There were significant differences between the two groups of patients in gender,site,size and number of tumor(P<0.05).Although there was no significant difference between the two groups in the distribution of the tumors in the two lobes of liver (P>0.05),priNET had more tumors localized to one lobe of liver while metNET had more tumors involving both lobes of liver(P<0.05).Conclusions Gender,size,site and number of tumor may play an important role in the differentiation of primary or metastatic hepatic neuroendocrine tumor.
