Objective To evaluate the association between stress hyperglycemia and clinical out-comes among patients with and without diabetes mellitns admitted to medical intensive care unit (MICU).Methods We reviewed the medical ICU records of 1016 consecutive patients admitted to Wu Han Univer-sity Ren Min Hospital. According to admission glucose level, 126 patients with debates and 890 patients without known debates were divided into group 1 ( <6 mmol/L), group 2(6 ~ 8mmol/L), group 3(8 ~10mmol/L), and group 4( > 10mmol/L). The primary end point of the study was in-hospital mortality. Secondary end points included length of hospital stay, length of ICU stay, the ratio of mechanical ventila-tion, blood transfusions, antibiotics and blood glucose treatment. Results The hospital mortality among patients without a history of diabetes mellitus was 4. 7%, 12. 1%, 20. 5%, 30. 3% in group 1, 2, 3, 4,respectively. The ratio of mechanical ventilation, blood transfusions, and antibiotics increased progressively as glucose values increased. Conclusions Hyperglycemia was a common disease, and it may be an impor-tant marker of poor clinical outcome and mortality in patients without history of diabetes.

Objective:To explore the role of nuclear transcription factor and the effect of nasal tolerance on EAN.Methods:75 male Wistar rats were divided into five groups: Group NS, EAN, EANL, EANM, EANH. The expression of IFN-?, TGF-?1, IL-10 mRNA and P65 protein were detected in spleen by using of RT-PCR and western blot. Results:The expression of IFN-? mRNA were the highest at early phase, those of TGF-?1 and IL-10 augmented significantly at recovery phase. In Group EANM and EANH, the severity of clinical signs and the infiltration of inflammatory cells decreased. IFN-? mRNA and P65 protein expression was lower than that in Group EAN, however, IL-10 and TGF-?1 mRNA increased markedly. At the same time, the expression of P65 protein were positively correlated with those of IFN-? mRNA, negatively correlated with those of TGF-?1 and IL-10 mRNA.Conclusion:NF-?B may play a vital role in EAN. Nasal administration of antigen may effectively prevent Wistar rat from inducing EAN in some degree, which may associate with the inhibition of NF-?B.

Rat models of diabetes were established by injecting streptozocin intraperitoneally.According to random number table, three groups were divided:normal group, diabetic group, and fisetin-treated group. After 24 weeks, all rats were sacrificed.Biochemical parameters of blood and urine samples were tested. The pathological changes were observed by paraffin sections staining with HE.The expression of extracellular matrix proteins was analyzed via PAS and Masson staining.Location of p300 protein expression was analyzed by immunohistochemistry. The protein expressions of p300 and MMP-2 were determined by Western blotting. The mRNA expressions of MMP-2 were analyzed via real-time PCR.The biochemical parameters and kidney pathological images in fisetin-treated group were better than those in diabetic group. The expression of extraeellular matrix proteins was lower than that in diabetic group.Immunohistochemistry analysis showed that among three groups the expression of p300 was mainly in glomeruli, and was also expressed in cell nucleus and cytoplasm and the coloration of fisetin-treated group was weakened as compared with diabetic group.Western blotting analysis showed that the expression of p300 protein in fisetin-treated group was lower than that in diabetic group(P<O.05).The expressions of MMP-2 mRNA and MMP-2 protein were higher than those in diabetic group (P < 0.05).It is suggested that fisetin may attenuate diabetes associated abnormalities in the kidney of rats, owing probably to inhibiting the expressions of p300 and enhancing the expressions of MMP-2.

Objective To study the fecel-oral and blood transfusion of TT virus. Methods Paired feces and serum samples from 6 patients with type B and/or C hepatitis were tested for TTV DNA and its titers by PCR with seminested primers.Genotypes were determined after their sequences were compared with the original N22 and TA278 clone. Results TTV DNA was detected in sera from all patients, while it was detected in feces from 3 patients, including 2 with high viral titers in serum. The detection of fecal TTV DNA was dependent on the viral titers in serum.TTV isolates in 3 pairs of feces and serum had identical sequence of 222 base pairs.Their genotypes were 1a,1b and 2,respectively.Conclusion The excretion of TTV into feces indicates that TTV would be transmitted not only parenterally but also nonparenterally by a fecal-oral route.

Journal of China Pharmaceutical University 2017;48(3):348-354
To study the effect of antioxidants R-(+)-lipoic acid (R-LA) on cells growth, proliferation and related mechanisms in human HepG2 cells lines.MTT was used to measure cells growth and proliferation.Reactive oxygen species (ROS) kit was used to analyze ROS level.Cell apoptosis and cell morphological changes were observed by flow cytometry and Hoechst 33258 test.Protein expression levels of apoptosis, autophagy and related pathway were analyzed through Western blot, including Bax,Bcl-2,caspase 3,PARP,ATG5,ATG7,LC3,Beclin1,mTOR,P70S6K,P38,P53,ERK and Akt etc. Results showed that cell growth and proliferation were inhibited in a dose-and time-dependent manner after being treated by lipoic acid.R-LA could increase ROS production,pro-apoptosis proteins Bax levels, activated caspase family and PARP.Meanwhile,R-LA could up-regulate the levels of autophagy-related proteins including ATG5,ATG7,Beclin1 and LC3,and down-regulate phosphomTOR and P70S6K levels.Signal pathway results showed that R-LA could up-regulate phospho-P38 and phospho-JNK levels, and decrease phospho-Akt and phospho-ERK.When adding 3-methyladenine,autophagy was inhibited.Thus,R-LA might activate autophagy and induce apoptosis by P38/AMPK-JNK,PI3K/AKT and Ras/ Raf/MEK/ERK pathways.
