ObjectiveTo evaluate the impact of hyperglycemia in patients admitted to medical intensive care units(MICU)on their prognosis.MethodsMedical records of 1016 patients,including 126 cases of known diabetes,359 cases with newly diagnoses hyperglycemia,and 531 normoglycemic cases,admitted to MICU in Wuhan University People's Hospital during September 2006 to September 2007 were reviewed to compare their case-fatality,length of hospital stay,length of MICU stay,mechanical ventilation,blood transfusion and antibiotics use.ResultsIn.hospital case-fatality was 22.8%(82/359)in patients with newly-diagnosed hyperglycemia.significantly higher than that in patients with known diabetes of 12.7% (16/126)and in normoglyeemic cases of 5.1%(27/531)(P＜0.01),and that Was higher in those with diabetes than those with normal blood glucose level(P=0.002).Proportions of patients who needed mechanical ventilation,blood transfusion,antibiotics use was higher in newly diagnosed hyperglycemia group than those in the other two groups.ConclusionsHyperglycemia represents an important mal'ker of poor clinical outcomes and mortality in patients admitted tO MICU.Patients with newly diagnosed hyperglycemia had a significantly higher case-fatality and poorer outcomes than those with known diabetes or normoglyeemia.

Objective To study the clinical effect of Xianglian in Mixture (Mixture of Nutgrass Galingale Rhizome and Golden Thread) in treatment of premature beat accompanied anxiety disorder.Methods Totally 100 patients of premature beat accompanied anxiety disorder were randomly assigned into two groups.The 50 patients in the treatment group were treated with Xianglian Mixture and the 50 in the control group were treated with Rytmonorm and Alprazolam for 4 weeks.The TCM symptom scores,premature beats in Holter,scores of Hamilton anxiety scale (HAMA) and the therapeutic effect on all these indices of both groups were observed.Results After treatment,the TCM symptom scores,premature beats in Holter,scores of HAMA of both groups were decreased significantly (P0.05).Conclusion Xianglian Mixture can reduce premature beats and relieve anxiety as well.

Objective To investigate the reliability of CT-guided percutaneous lung biopsy pathological results in diag?nosing Non-small cell lung cancer (NSCLC). Methods Patients (n=91) were selected whose radiology examination re?vealed pulmonary masses but were failed to be confirmed with pulmonary lesion through sputum or bronchoscopy. Then, they were diagnosed as non-small cell lung cancer by CT-guided percutaneous lung biopsy. After that, they received surgical op?eration and biopsies were taken during operations for pathological analysis. Pathological results between from percutaneous lung biopsy (pathological types and using different analysis tools) and from surgical operation were compared. Results The coincidence rate of pathological type is 86.81%(79/91) using pathological results through percutaneous lung biopsy or oper?ation. Compared with the pathological results of operation, the coincidence rate of pathological type using percutaneous lung biopsy show no statistical significance [Squamous cell carcinoma 88.57%(31/35) vs Grandular cell carcinoma 86.27%(44/51) vs adeno-squamous carcinoma 80.00%(4/5),χ2=0.310,P>0.05];the coincidence rate of percutaneous lung biopsy pathologi?cal results using different approaches demonstrate statistical significance compared with surgical biopsy pathological results (χ2=9.698, P<0.05). The coincidence rate of pathological results obtained by Smear and biopsy [94.4%(51/54)] is higher than that of surgical biopsy pathological results using smear alone [60.0%(6/10)] and by biopsy alone [81.4%(22/27)]. Con?clusion Using CT-guided percutaneous lung biopsy, it can produce reliable results on pathological type of non-small cell lung cancer. But the reliability can be affected by different analysis approaches that were hired to exam samples from CT-guided percutaneous lung biopsy.

Objective To evaluate the association between stress hyperglycemia and clinical out-comes among patients with and without diabetes mellitns admitted to medical intensive care unit (MICU).Methods We reviewed the medical ICU records of 1016 consecutive patients admitted to Wu Han Univer-sity Ren Min Hospital. According to admission glucose level, 126 patients with debates and 890 patients without known debates were divided into group 1 ( <6 mmol/L), group 2(6 ～ 8mmol/L), group 3(8 ～10mmol/L), and group 4( > 10mmol/L). The primary end point of the study was in-hospital mortality. Secondary end points included length of hospital stay, length of ICU stay, the ratio of mechanical ventila-tion, blood transfusions, antibiotics and blood glucose treatment. Results The hospital mortality among patients without a history of diabetes mellitus was 4. 7%, 12. 1%, 20. 5%, 30. 3% in group 1, 2, 3, 4,respectively. The ratio of mechanical ventilation, blood transfusions, and antibiotics increased progressively as glucose values increased. Conclusions Hyperglycemia was a common disease, and it may be an impor-tant marker of poor clinical outcome and mortality in patients without history of diabetes.

Objective To investigate the effect of matrix metalloproteinase(MMP-2) in the attack of exprimental autoimmune myositis(EAM) rats, and the effects of methylprednisolone on expressions of gene mRNA and protein of MMP-2. Methods The expression levels of MMP-2 in the peripheral blood, spleen lymphocytes and muscles were detected by using RT-PCR and immunohistochemistry techniques and was compared among EAM group,EAM with methylprednisolone treatment group (EAMM) and control group. Results ⑴ The degree of episode in EAMM group was lower than that of EAM group , and the infiltration of the inflammatory cells and the necrosis of the muscles were also mild in EAMM group. ⑵There was significant upregulation of the expression of the MMP-2 gene mRNA in the lymphocytes of peripheral bood and spleen of EAM rats as compared with that of the control,Elevation of the expression of MMP-2 protein in rats muscles tissues of EAMM group was observed obviously. However, the expression of the mRNA and the protein of MMP-2 was suppressed significantly in EAM group compared with control group. Conclusions The upregulation of the expression of MMP-2 may be correlated with the onset of EAM. Methylprednisolone may relieve the degree of the pathology of EAM by the downregulation of MMP-2 .

Objective:To explore the role of nuclear transcription factor and the effect of nasal tolerance on EAN.Methods:75 male Wistar rats were divided into five groups: Group NS, EAN, EANL, EANM, EANH. The expression of IFN-?, TGF-?1, IL-10 mRNA and P65 protein were detected in spleen by using of RT-PCR and western blot.Results:The expression of IFN-? mRNA were the highest at early phase, those of TGF-?1 and IL-10 augmented significantly at recovery phase. In Group EANM and EANH, the severity of clinical signs and the infiltration of inflammatory cells decreased. IFN-? mRNA and P65 protein expression was lower than that in Group EAN, however, IL-10 and TGF-?1 mRNA increased markedly. At the same time, the expression of P65 protein were positively correlated with those of IFN-? mRNA, negatively correlated with those of TGF-?1 and IL-10 mRNA.Conclusion:NF-?B may play a vital role in EAN. Nasal administration of antigen may effectively prevent Wistar rat from inducing EAN in some degree, which may associate with the inhibition of NF-?B.

Objective: To evaluate the mechanical properties of Rene95 alloy component made by laser engineered net shaping (LENS). Methods: The component was fabricated from Rene95 alloy powder by LENS, then the mechanical properties of the component, Rene95 alloy casting and dental Ni-Cr alloy casting were studied. Results: The tensile strength(MPa), yield strength(MPa), elongations(%) and the microhardness(Hv) of Rene 95 alloy component made by LENS were 1 354.27?63.44, 1 175.29?34.32, 16.53?5.85, and 464.33?10.95, those of Rene95 alloy casting 1 430.28?55.73, 1 145.52?29.78, 10.31?4.25 and 457.56? 11.38, those of dental Ni-Cr alloy casting 612.05?18.46, 426.06?12.70, 6.21?1.21 and 260.14? 6.08, respectively.Conclusion: The mechanical properties of Rene95 alloy component made by LENS were same as those of Rene95 alloy casting, better than dental Ni-Cr alloy casting.

Abstract?AlM: To observe the clinical efficacy and safety of glaucoma filtration surgery combined with the application of 5 - fluorouracil for clinical treatment to offer the reference.?METHODS:A total of 91 cases of patients with glaucoma treated in our hospital were randomly divided into three groups, 30 cases of group A were given mitomycin in surgery, 31 cases of group B were given 5-fluorouracil, 30 cases of group C were given cyclosporine. They were given 12 -month-follow-up after operation, the operation success rate were recorded, the incidence of 1 -year-adverse reactions, visual acuity, intraocular pressure, low intraocular pressure, shallow anterior chamber, corneal damage index were recorded.?RESULTS: lntraocular pressure ofgroups A, B, C were respectively (20.5±8.6)mmHg,(15.2±5.5)mmHg,(20.0± 8. 1 ) mmHg, by the analysis of variance, group B was significantly lower than other two groups,P<0. 05;patients of groups A, B, C whose visual acuity increased ratios after 1y were respectively 22. 5%, 38. 1%,and 23. 1% , by the Chi. square test, the increased ratio of group B was higher than other two groups , P<0. 05; The operation success rates after 1y of operation of groups A, B, C were respectively 67. 5%,88. 1%,71. 8%,by the Chi square test, the increased ratio of group B was higher than other two groups, P<0. 05;The general incidence of adverse reaction were respectively 22. 5%,9. 5%,20. 5%,by the Chi square test , the increased ratio of group B was lower than other two groups, P<0. 05.?CONCLUSlON:Glaucoma filtration surgery combined with the application of 5-fluorouracil can improve the operation success rate, decrease the occurrence rate of adverse reaction, and safety can be improved.

The effect of electroacupuncture in Qiansanli and Housanli to a rat's change in infarction tissue, necrosis of neuron and vessel, and the number of synapse.ResultsElectro acupuncture could improve histopathologic indices, increase the number of synapse, and promote the recover of synapse's structure.ConclusionElectro acupuncture can produce a marked protecting effect on the brain in Wistar rats focal cerebral ischemia, regulate the plasticity of cerebral neuron.
