4.Identification of the Chemical Constituents of the Ethyl Acetate Extraction Portion of Polygonum bistorta

OBJECTIVE:To study the chemical constituents of the ethyl acetate extraction portion of Polygonum bistorta,pro-vide scientific basis for the basic research of its effective substances. METHODS:Taking the ethyl acetate extraction portion of P. bistorta 95% ethanol extract,positive phase and reversed phase column chromatography were used for repeated isolation and purifi-cation,and the structures of the purified compounds were identified on the basis of physicochemical properties or spectral data. RE-SULTS:11 monomer compounds were isolated and identified as β-sitosterol(1),dihydromyricetin(2),quercetin(3),kaempferol (4),epicatechin(5),chlorogenic acid(6),rutin(7),gallic acid(8),1,2,3,4-tetrahydro-8-hydroxy-4-isopropyl-1-methylnaph-thalene-6-carboxylic acid (9),protocatechuic acid (10) and ellagic acid (11). CONCLUSIONS:The literature is confirmed that compound 2 is isolated from P. bistorta for the first time and compound 9 is isolated from Polygonum for the first time. The related research results have provided experimental references for further clarifying the effective substances of P. bistorta.

Facing current situation of integrative medicine (IM), authors put forward that clinical and diagnosis program of IM could be carried out from clinical path, pathogenesis, treatment theory and philosophy, and so on, but with different integration degrees. Meanwhile, formulation of concrete program should be disease-targetedly set up, and adjusted from person to person, from place to place, from time to time. As for settled IM program , authors could evaluate it from whether Chinese medicine and Western medicine have formed complementary, synergistic, excitatory actions, and toxicity attenuation; whether more problems could be solved in efficacy, safety, practicability, and economy than previous single mode.

OBJECTIVE:To observe the effects of Xiaoer Reganning oral liquid(XRN) on clinical symptoms and signs of children with acute upper respiratory tract infections.METHODS:161 children with acute upper respiratory tract infections were randomly divided into three groups.The patients were treated with XRN(XRN group,60 cases),Shuanghuanglian oral liquid(SHL group,41 cases) and Wei C Yingqiao granules(WCG group,60 cases),3~4 times every day for 3 days.The changes of clinical symptoms of coughing rhinorrhea,lassitude,anorexia and dyssomnia,and signs of pharyngeal congestion and tonsillar swelling were studied on the patients before and after treatment for three days,and the results of three groups were compared.RESULTS:The improvement of the clinical symptoms and signs in XRN group was better than that in the control groups(P
7.Study on Antipyretic Action of Xiaoerreganning Oral Solution on Acute Upper Respiratory Tract Infection of Children

Objective: To evaluate the antipyretic action of Xiaoerreganning Oral Solution on acute upper respiratory tract infection of children. Methods: 161 children with acute upper respiratory tract infection were randomly divided into 3 groups. 60 patients at different age ranges in Xiaoerreganning Oral Solution group were treated with Xiaoerreganning Oral Solution at different dosags, 3~4 times every day. The treatment course was 3 days. Patients' body-temperatures before and after treatment were recorded. The temperature differences in various time points before and after treatment were computed, and compared with the results of Shuanghuanglian Oral Solution group and Wei C Yingqiao Tablets group.Results: All three drugs could reduce the patients' body-temperatures gradually, but the antipyretic action of Xiaoerreganning Oral Solution was the strongest. 8.Short-term intensive insulin treatment and sensitivity of sulfonylureas

Objective To research the effects of short-term intensive insulin treatment on regaining the sensitivity of sulfonylureas in diabetes patients. Methods Thirty patients from outpatient and emergency department,including 12 male and 18 female,who took regular-dose sulfonylureas but was high blood glucose level,were selected to suspend the sulfonylureas treatment and were given the BIAsp30 to control the blood glucose level for three months,then they were stopped the BIAsp30 and took the same sulfonylureas used before.Results The average fasting blood glucose(FBG) was(9.4?7.5)mmol/L and the average postprandial 2 h blood glucose(PG2h)(or random blood glucose) was(14.2?7.2)mmol/L in 3 months before stopping the sulfonylureas.The average FBG was(5.7?0.7)mmol/L and PG2h was(7.2?1.4)mmol/L at the beginning of the insulin getting the blood glucose under control.The average FBG was(6.0?0.8)mmol/L and PG2h was (7.8?1.2)mmol/L during the insulin treatment.The average FBG was(6.1?0.6)mmol/L and PG2 h was(7.7?1.3)mmol/L at the end of the insulin treatment.The average FBG was(6.5?0.5)mmol/L and PG2h was(8.1?(0.8))mmol/L when continuing the sulfonylureas treatment in one months.It increased significantly to compare the blood glucose before the treatment of insulin to that after the treatment of insulin(P
9.A preliminary efficacy evaluation for percutaneous endoscopic necrosectomy in treating infectious pancreatic necrosis

Objective To evaluate the therapeutic effect of percutaneous endoscopic necrosectomy (PEN) in treating infectious pancreatic necrosis (IPN).Methods A retrospective review of clinical data of 6 patients with IPN who received PEN in Changhai Hospital, Second Military Medical University from Dec 2015 to Sep 2016 was performed.Clinical parameters were recorded, including basic information, severity evaluation and therapeutic methods and times.In addition, vital sign parameters and inflammatory marks before and after PEN treatment were compared.Results There were 4 patients with severe acute pancreatitis (SAP) and 2 patients with moderately severe acute pancreatitis (MSAP) in these 6 patients with IPN.Mean APACHEⅡ score was 12 (10~15), and mean MCTSI scores was 9.3(8~10).All 6 patients received a total of 13 times PEN treatments, with a mean of 2.2(1~3) times.Each patient was treated with a mean of 2.5(1~4) drainage tubes placed in the peripancreatic abscess after PEN treatment, and the mean time for drainage was 139 d(106~183 d).Besides, the mean hospitalization time was 116 d (48~223 d).All the patients′ condition was improved significantly after PEN treatment, including reduced heart rate, body temperature and inflammatory markers, without bleeding or other serious complications.Only 1 patient had pancreatic fistula after treatment, and no patients needed open abdominal drainage surgery.Patients with higher MCTSI scores likely required more times of PEN and more drainage catheters, longer length of drainage and hospital stay.Conclusions PEN was safe and effective for treating patient with IPN, but those with higher MCTSI scores were associated with more PEN treatments, more drainage tubes, and longer time of drainage and hospitalization.
10.Facial skin lesions in male patients with liver cirrhosis: role of serum sex hormones and correlation with impaired liver function.

OBJECTIVETo investigate the relationship between serum sex hormone levels, liver function, and pathogenic mechanisms related to cutaneous lesions involving the facial skin in male patients with liver cirrhosis.
METHODSFifty male cirrhotic patients with facial skin lesions, including spider angiomas, angiotelectasis and special type rash, (mean age: 48.1 +/- 12.2 years) were randomly selected for study and enrolled as the case group. Thirty cirrhotic male patients without facial skin lesions (mean age: 44.5 +/- 11.7 years) were enrolled as the control group. Serum levels of luteinizing hormone (LH), follicular stimulating hormone (FSH), prolactin (PRL), estradiol (E2), progesterone (PRGE), and testosterone (T) were detected and compared between cases and controls by the t-test. All patients were sub-categorized according to severity of cirrhosis (Child-Pugh classification) and comparisons between cases and controls were carried out by single factor analysis of variance. Logistic regression modeling was used to evaluate whether the presence of skin lesions is related to changes in markers of liver impairment, including alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBil), serum albumin (Alb), prothrombin time (PT-SEC), creatinine (CREA), platelet count (PLT), and alcoholism.
RESULTSIn the cases with spider veins, LH level was significantly elevated (t = 2.01) and T level was significantly decreased (t = -2.20) (both, P less than 0.05 vs. controls). In the cases with telangiectasia, the LH level (t = 3.76, E2 (t = 2.08) and E2/T ratio (t = 2.98) were significantly elevated and T level was significantly decreased (t = -3.77) (all, P less than 0.05 vs. controls). In the cases with special type rash, FSH level was significantly elevated (t = 2.03) and T level was significantly decreased (t = -2.01) (both, P less than 0.05 vs. controls). In the case group, E2 levels decreased as severity of liver damage increased, while in the control group, E2 levels increased as severity of liver damage increased; however, the difference in average E2 values of the two groups did not reach statistical significance (P more than 0.05). In both cases and controls, the T levels were decreased as the severity of liver damage increased (F = 3.70, P less than 0.05). Multivariate logistic regression analysis showed that increased incidence of facial skin lesions is associated with alcoholism (odds ratio (OR) = 4.46, 95% confidence interval (CI) = 1.45-13.7, P less than 0.05) and elevated serum levels of AST (OR = 11.87, 95% CI = 1.24-113.1, P less than 0.05).
CONCLUSIONAlcoholism, impaired liver function, and perturbed levels of circulating sex hormones are associated with cirrhosis-related facial lesions and may play important roles in the pathogenesis of cutaneous lesions in patients with cirrhosis.
