1.Astragalus polysaccharide′s synergistic enhancement on Carvedilol′s improvement effects on heart function of rat with dilated cardiomyopathy

Objective To study astragalus polysaccharide′s synergistic enhancement on Carvedilol′s improvement effects on dilated cardiomyopathy′s heart function.Methods SD rat model of dilated cardiomyopathy was established and divided into CON group (no treatment),CAR group (only Carvedilol treatment),APS group (only astragalus polysaccharide treatment) and C+A group (Carvedilol and astragalus polysaccharide treatment) according to their treatment methods.The left ventricular ejection fraction (LVEF),Tei index,high sensitive C reactive protein (hs-CRP),tumor necrosis factor (TNF)-α,interleukin (IL)-1,IL-6,IL-8,CD3+,CD4+ and CD8+ T lymphocyte subsets in four groups were detected and compared.Results The LVEF in C+A group and CAR group were significantly higher than those in CON group and APS group respectively (P<0.05,respectively),while Tei index in C+A group and CAR group were significantly lower than those in CON group and APS group respectively (P<0.05,respectively).The LVEF in C+A group was significantly higher than that in CAR group (P<0.05),while Tei index in C+A group was significantly lower than that in CAR group (P<0.05).The hs-CRP,TNF-α,IL-1,IL-6 and IL-8 in C+A group and APS group were significantly lower than those in CON group and CAR group respectively (P<0.05,respectively),while CD3+,CD4+ and CD8+ T lymphocyte subsets in C+A group and APS group were significantly higher than those in CON group and CAR group respectively (P<0.05,respectively).The hs-CRP,TNF-α,IL-1,IL-6 and IL-8 in C+A group were significantly lower than those in APS group respectively (P<0.05,respectively),while CD3+,CD4+ and CD8+ T lymphocyte subsets in C+A group were significantly higher than those in APS group respectively (P<0.05,respectively).Conclusion Astragalus polysaccharide has significant synergistic enhancement on Carvedilol′s improvement effects on dilated cardiomyopathy′s heart function,which may be related to the enhancement of T lymphocyte immunity and the decreasing of inflammatory reaction.
2.Predictive value of revised Geneva score,ECG score and D-dimer in pulmonary embolism

Objective To evaluate the value of revised Geneva score, Daniel ECG score and age-adjusted D-dimer for predicting pulmonary embolism(PE).Methods A total of 91 cases suspected as pulmonary embolism were collected, and 52 cases were diagnosed as pulmonary embolism by computed tomographic pulmonary angiography(CTPA) results.Receiver operating characteristic(ROC) curves and diagnostic test evaluation indexes were used to evaluate the probability of PE predicted by the revised Geneva score,Daniel ECG score, age-adjusted D-dimer and combination of these two clinical scores and age-adjusted D-dimer.Results The confirmed PE was 21.4% with a low probability(revised Geneva score 0~3 points),55.4% in intermediate probability(4~10 points),85.7% in high probability(score≥11 points).The prevalence of PE was 33.3% with a low clinical probability (Daniel ECG score<2 points)and 70.7% with a high clinical probability(Daniel ECG score≥2 points).The sensitivity and specificity of age-adjusted D-dimer in predicting pulmonary embolism were 92.3%,69.2%.The area under curve of the ROC curve(AUC)in the revised Geneva score and Daniel ECG score has no significant difference(Z=0.979).The negative predictive value of the revised Geneva score, Daniel ECG score combined with D-dimer in pulmonary embolism were 100.0% and 87.5%.Conclusion All of revised Geneva score, Daniel ECG score and age-adjusted D-dimer have certain predictive value on pulmonary embolism , and the revised Geneva score combined with age-adjusted D-dimer can be more safely exclude pulmonary embolism.

4.The electrophysiological characteristics of ventralis intermedius nucleus and its role in localizations of Vim-thalamotomy for Parkinson's disease patients

Objective To investigate the electrophysiological characteristics of the ventralis intermedius nucleus (Vim) in order to find an easy and safe way to confirm the target in Vim-thalamotomy. Method In microelectrode-guided selective Vim-thalamotomy for 23 Parkinson's disease patients, the background activity, amplitude and discharge frequencies of Vim were compared with its surrounding structures. The response of kinesthetic neuron and tremor cell to microstimulation was also compared. Result There were differences in backgroud activity and discharge amplitude for Vim, ventralis lateralis nucleus (VL), ventralis caudalis nucleus (VC), and internal capsule. Based on the response to active or passive movement of contralateral limb tremor cells were divided into two subgroups, which were different in localization. Contralateral tremor showed different response when the two subgroups of tremor cells were mircrostimulated. Conclusion The anterior border of Vim was easily found by microrecording. Only by combining microstimulation with microrecording could the posterior border of Vim and its interior and lateral ordination of target were identified exactly and safely. Kinesthetic neurons and tremor cells which responded to the movement of contralateral limbs should be destroyed.
5.Comparison of three methods for measuring C reaction protein

Objective To compare the results of three methods for measuring C reaction protein.Methods 100 patients were collected from our hospital,and three different methods for measuring C reaction protein were used to analyze the level of C reaction protein.Results Correlation analysis showed that the correlation between immunoturbidimetric assay and immunochromatography was higher.The differences of three methods assayed the C reaction protein were significant (P <0.05 )among normal people by the variance analysis,but had not statistical significance among abnormal people (P >0.05).The differences among all of test peo-ple were significant(P <0.05).Conclusion Detection of C reaction protein was important for diagnose inflammatory diseases.
6.Effects of the different hypodermic injection site of insulin Oil blood glucose fluctuations in type 2 diabetic patients

Objective To observe the fluctuations of blood glucose in type 2 diabetic patients treated by insulin hypodermic injection in different sites.Methods 30 patients with type 2 diabetes were randomly divided into three groups:abdominal,edltoid region and lateral femoral subcutaneous injection group.After half a year of insulin therapy,dynamic glucose monitoring for consecutive(71±10)hours was conducted,and(847±80)results of glucose Level were obtained.Results The average glucose had no difference in three group,but diurnal maximum and maximum fluctuation amplitude in patients of abdominal subcutaneous injection group were significantly lower than the Other groups(P<0.05).The time percentage of glycemic fluctuations within 3.6~10.0mmol/L and diurnal minimum was higher compared to other groups(P<0.05).Conclusion The site choice of insulin subcutaneous injection can influence the fluctuations of blood glucose in type 2 diabetes mellitus and abdominal subcutaneous tissue was the most ideal of the three subcutaneous injection sites.
7.Investigation on the psychological health states of practical nurse

Objective To investigate the psychological health states of practical nurse and to effectively conduct clinical psychological guidance.Methods 368 practical nurse were determined by SCL-90 and compared with normal control.Results There were significant differences between practical nurse and normal control,such as compulsion,sensitive interpersonal,depression,anxiety,terror crankiness(P<0.05~P<0.01),and no remarkable difference in other factors(P>0.05).Conclusion Nurse's gross mental health level were worse than normal control,especially in compulsion,sensitive interpersonal,depression,anxiety,terror crankiness. So, mental health education should be enhanced for them.

9.Clinical Observation of Alteplase Combined with Butylphthalide in the Treatment of Acute Ischemic Stroke

OBJECTIVE:To discuss clinical efficacy and safety of alteplase combined with butylphthalide to the patients with acute ischemic stroke. METHODS:98 patients with acute ischemic stroke in our hospital were selected and divided into ob-servation group and control group according to random number table,with 49 patients in each group. Control group was addition-ally given Butylphthalide and sodium chloride injection 100 ml,ivgtt,bid,on the basis of routine treatment as controlling blood glucose,blood pressure,etc.;observation group additionally received Alteplase for injection 5 mg added into NS 10 ml,iv+Al-teplase for injection 45 mg added into NS 100 ml,ivgtt,qd,on the basis of control group. Both groups were treated for 2 weeks. Clinical efficacies of 2 groups were compared as well as cerebral infarction area,NIHSS score,ability score of daily liv-ing,the levels of IL-6,IL-8,IL-10,CRP,24 h urine protein,Scr and creatinine clearance rate before and after treatment. The occurrence of ADR was also recorded. RESULTS:1 patient of observation group and 2 patients of control group withdrew from the study due to severe hemorrhage. Total effective rate of observation group(95.83%)was significantly higher than that of con-trol group(80.85%),with statistical significance(P<0.05). Before treatment,there was no statistical significance in above in-dexes between 2 groups (P>0.05). The cerebral infarction area and NIHSS score were reduced significantly in observation group after treatment,ability score of daily living were increased significantly in observation group after treatment,and the im-provement of observation group was significantly better than control group,with statistical significance (P<0.05). 24 h urine protein and Scr of observation group was significantly lower than those of control group,with statistical significance(P<0.05). the level of IL-6 in observation group 1 week after treatment and the levels of IL-6,IL-8,IL-10 and CRP 2 weeks after treat-ment were significantly lower than in control group,with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Alteplase combined with butylphthalide show signifi-cant therapeutic efficacy,can effectively reduce the level of serum inflammatory factors,control brain tissue ischemia and cere-bral infarction area,and improve neurologic function and pro-tect renal function in patients with acute ischemic stroke.
