1.Clinical and histopathological analysis of paediatric patients with orbital space -occupying lesions

AIM:To explore the histopathological classification and incidence of orbital space-occupying lesions in children in order to provide references for the diagnosis of orbital space-occupying lesions in children.
METHODS:A total of 290 paediatric patients with orbital space - occupying lesions diagnosed by pathological examinations at the Second Hospital of Tianjin Medical University from January 1998 to December 2012 were retrospectively reviewed.
RESULTS:In 290 paediatric patients with orbital space-occupying lesions, venous hemangioma was the most common (58 cases,20. 0%), the other commons were as follows: hemangio lymphangioma, dermid cyst, neurofibroma, optic nerve glioma, rhabdomyo sarcoma and so on. Capillary hemangioma, venous hemangioma, optic nerve glioma had the predominant age incidence. On the other hand, optic nerve glioma, venous hemangioma, rhabdomyosarcoma had the predominant sex incidence.
CONCLUSION: The majority of paediatric patients with orbital space - occupying lesions are congenital and embryonic lesions, the most common of which is venous hemangioma. Some lesions had the predominant age and sex incidence.
2.Analysis in stressor of patients with maintained hemodialysis

Objective To observe the stressor characteristic of patients with maintained hemodialy-sis (MHD). Methods The Chinese version of bemodialysis stressor seale(HSS) was adopted among 436 bemodialysis participants and the results underwent analysis. Results Among 436 patients psychological stressors> physiological stressers > social stressors, the most severe stressors included the requirement of long-term dialysis, the rest showed sequence from high to low: medical cost, concerning about the future,changes in family responsibility and decrease in sexuality. The slightest stressors included decreased ability to procreate, being limited in some styles of clothing, arterial and venous stick, reversal in family role with the children and changes in body appearance. Significant differences existed in patients with different age,gender, education background, working status and economic status, it manifested as follows: the more elder-ly, the more severity of stress, male patients had more severe stress than the female, patients with higher e-ducation background showed lower stress level, patients who kept working had lower level of stress, pa-tients who had better economic status showed lower stress level. Conclusions MHD patients often expe-rience high stress levels. Some measures should be taken to help MHD patients to cope better with stress.
4.Clinical features of interstitial lung involvement in primary Sj(o)gren's syndrome

Objective To analyse the clinical features and risk factors of interstitial lung disease in primary SjSgren's syndrome (pSS).Methods A retrospective study was employed.All 130 pSS patients who were hospitalized during 2004 to 2006 were enrolled into this study.Thirty-seven clinical and laboratory variables were used as the research factors and interstitial lung involvement as the related factor.The data were analyzed with the Logistic regression model.Results of 130 patients,22(16.9%) had interstitial lung involvement.Twelve patients were asymptomic when pulmonary involvement was detected.Variables in the univariate analysis which were significantly associated with lung fibrosis were age,exocrine glands(including parotid,sublingual gland and submandibular gland) swelling,oral ulceration and fever.Positive rate of antiU1RNP antibody in group of lung damage was significantly higher than non-lung damage group [5/22(22.7%)vs 6/108 (5.6%),P=0.021].In the Logistic regression model.variables which were significantly associated with pulmonary involvement were exocrine glands swelling (OR=3.739,95%CI 1.069~16.079,P<0.05).oral ulceration (OR=3.739,95%CI 1.069~16.079,P<0.05)and fever (OR=3.067,95%CI 1.198~20.067,P<0.05).Conclusions This study indicates that some of the pulmonary damages of pSS are subclinical.Exocrine glands swelling,oral ulceration and fever are the risk factors for interstitial pulmonary involvements in pSS.
5.Experimental study on union culture of mesenchymal stem cell with fast setting calcium phosphate scaffold

[Objective]To examine the effects of fast setting calcium phosphate scaffold on human mesenchymal stem cells(MSCs) adhesion and proliferation.[Method]Scaffolds were self-hardened in molds.Three ml human bone marrow aspirates,taken form the iliac crest of normal donors were suspended in low glucose DMEM supplemented with 10% fetal bovine serum.Atfer 72 hours the nonadherent cells were removed by changing medium,and the adherent layer cultured until it reached 80% conference.Cultured cells at passage 3 were used for the experiments.MSCs were seeded on calcium phosphate scaffold.The cell-scaffold constructs were cultured with complete media for 1 week.Scanning electron microscope(SEM)and the terazolium-based colorimetric assay(MTT test)examinations were performed.[Result]When cells were cultured in scaffolds in vitro,the number of cells on scaffold increased with time.SEM showed that the scaffold had a three-dimensional interconnected pore structure.The cells appeared to infiltrate into the macropores of the scaffold and the extracellular matrix components were observed.[Conclusion]This study demonstrates that the fast setting calcium phosphate scaffold has good biocompatibility,cells are able to adhere and proliferate on scaffold specimens,and it is a good substrate candidate for tissue engineered bone substitute.
6.Research Advance in Environmental Exposure and Health Impact of Di(2-ethylhexyl) phthalate

Asaprimarilyused plasticizerin polyvinyl chloride,d(i2-ethylhexy)lphthalate(DEHP)is extensively used in consumer products,flooring and wall coverings,food contact applications,and medical devices.Because DEHP has been detected in waters,soils and air,and its metabolites have also been detected in human urinary,sufficient evidences show that DEHP is ubiquitous in the environment worldwide.DEHP is already in the blacklist of priority pollutants in water in China.Many researches indicate that DEHP is one kind of endocrine disrupting chemicals in animals,with carcinogenicity,teratogenicity and mutagenicity.Based on the researches and review on DEHP,this paper principally discussed the environmental level,metabolism and biological markers of DEHP,as well as the research progress on human health effects.
7.Achillis Tendon-Sliding Procedure and Subtalar Joint Capsula Release by the Same Cut and Individualization in Treatment of Congenital Clubfoot

Objective To explore achillis tendon-sliding procedure and subtalar joint capsula release by the same cut and individualization treatment used in the correction of congenital clubfoot.Methods Forty eight cases (76 feet) of congenital clubfoot who were first visit were treated by achillis tendon-sliding procedure and subtalar joint capsula release by the same cut,meantime,footplate fascia release and transfer of the anterior tibial tenden were made by individualization.Results All cases were followed-up,the average time follow-up was 1 year and 6 months,excellent were 71% and good were 15.7%.Five cases were recurrence and its rate was 6.94%.Conclusions Achillis tendon-sliding procedure and subtalar joint capsula release by the same cut can solve rotation of displacement between calcaneus and talus and individualization treatment can also solve high arch deformity and adduction of foot.This technique is a simple,safe and effective treatment for congenital clubfoot.
8.Effect of Yangxue Qingnao Granule on the Expression of CD11b in CA1 Region of Hippocampus of Vascular Dementia Rats.

OBJECTIVETo observe the effect of Yangxue Qingnao Granule (YQG) on the expression of CD11b in CA1 region of hippocampus of vascular dementia rats, and to explore its regulation on microglias.
METHODSTotally 144 SD rats were randomly divided into the sham-operation group, the vascular dementia model group (model), and the YQG treated group (treated). The vascular dementia rat model was prepared by modified Pulsinelli's four-vessel occlusion. Rats in the sham-operation group and the model group were administered with normal saline -(at the daily dose of 10 mL/kg) by gastrogavage, while those in the treated group were administered with YQG (0.32 g/mL, at the daily dose of 10 mL/kg) by gastrogavage. All administration was performed once per day for 8 successive weeks. The expression of CD11b in CA1 region of hippocampus of vascular dementia rats was detected at week 1, 2, 4, and 8, respectively.
RESULTSCompared with the sham-operation group, the expression of CD11b in CA1 region of hippocampus of vascular dementia rats were significantly enhanced in the model group at each time point (P < 0.01). Compared with the model group, the expression of CD11b in CA1 region of hippocampus of vascular dementia rats significantly decreased in the treated group at each time point (P < 0.01), especially at week 2.
CONCLUSIONObvious activation and proliferation of microglias could be seen in CA1 region of hippocampus of vascular dementia rats, and YQG could inhibit activation and proliferation of microglias.

9.The application of full-size three-dimensional printing model combined with three-dimensional digital demonstration in facilitating patient's preoperative understanding of laparoscopic partial nephrectomy

Objective To evaluate the feasibility and effectiveness of full-size three-dimensional individual printed model (3D-IPM) for improving the patient's understanding of laparoscopic partial nephrectomy (LPN) preoperatively.Methods Between June 2015 and June 2016,37 patients with cT1 renal tumors underwent retroperitoneal LPN.The 3D individual digital models (3D-IDMs) were created using 3D medical image reconstructing and guiding system (3D-MIRGS) and the full-size 3D-IPMs were fabricated correspondingly.For each patient and his/her closest accompanying immediate family member (CAIFM),two preoperative conversations with a single surgeon were held by using patient's CT films,the entity of 3D-IPM combined with 3D-IDM demonstration separately.The preoperative levels of comprehension to the renal anatomy,mass characteristics,the upcoming PN procedure,potential complication risks and postoperative prognosis were evaluated using a self-made scoring questionnaire in the patient and CAIFM groups.Results All the fabrications of full-size 3D-IPMs were technically successful.Both in patient and CAIFM groups,the total preoperative comprehending score (total-PCS) elevated significantly by presenting 3D-IPM combined with 3D-IDM demonstration than CT films (42.7 vs.31.5 in patient group,P < 0.05;44.6 vs.33.1 in CAIFM group,P < 0.05).Sub-PCSs in all 5 aspects also showed a uniformed climbing pattern with the assistance of 3D-IPM combined with 3D-IDM demonstration rather than CT films.Conclusion The application of 3D-IPM combined with 3D-IDM demonstration can improve the preoperative comprehension of the patient and CAIFM to LPN with more intuitionistic and verisimilar presentation.
