1.Awareness rate of PCV13 and influencing factors of vaccination intention among parents of infants and young children in Huangpu District , Shanghai

Objective To understand the awareness rate and willingness of parents of school-age infants and young children in Huangpu District, Shanghai to receive the 13 valent pneumococcal conjugate vaccine (PCV13) and its influencing factors, and to provide basic data for formulating community health education policies. Methods A stratified random sampling method was used to conduct a full coverage questionnaire survey on the PVC13 awareness rate and vaccination willingness of 1030 parents of infants and young children in 10 communities. Results A total of 1000 questionnaire surveys were completed, with an accurate response rate of 97.08%. The awareness rate of PCV13 was 85.50%, and the awareness rate of complications was less than 80%. The PCV13 vaccination rate was 55.60%. 38.74% of the reasons for not being vaccinated were concerned about vaccine side effects, and 32.21% were concerned about vaccine quality. The higher the education level of parents and the per capita annual income of the family, the higher the awareness rate of pneumococcal vaccine knowledge and the willingness to receive vaccination. Parents with registered residence in other places had low willingness to vaccinate PCV13. Conclusion The public has a higher awareness of PCV13 and a higher willingness to receive vaccination. It is necessary to strengthen the scientific popularization of complications of pneumococcal pneumonia and vaccine safety, strengthen vaccine safety supervision and disclosure of regulatory results, and strengthen PCV13 science popularization for parents of infants and young children with registered residence outside the city.
2.Association of Toll-like receptor 7, CTLA-4 gene polymorphisms with severe asthma

Objective To explore the association of Toll-like receptor 7, CTLA-4 gene polymorphisms and severe asthma. Methods From February 2018 to March 2020, 175 asthma patients admitted to the respiratory department of our hospital were selected as the research subjects (109 cases of mild disease and 66 cases of severe disease), and 248 cases of healthy people who were included in the outpatient physical examination of our hospital during the same period were selected as the normal control group. Toll-like receptor 7 and CTLA-4 gene polymorphisms in the above groups were determined, and the relationship between Toll-like receptor 7 and CTLA-4 polymorphisms and severe asthma was evaluated by calculating the odds ratio (OR) and 95% confidence interval(CI). The relationship between the genotypes of Toll-like receptor 7 and CTLA-4 polymorphisms and severe asthma were evaluated by logistic regression analysis. Results The proportion of TLR7 rs3853839 CC genotype, CTLA-4 rs231725 AA genotype, TLR7 rs3853839 C allele frequency and CTLA-4 rs231725 A allele frequency in severe asthma group and mild asthma group were higher than those in normal control group(P<0.05). The proportion of TLR7 rs3853839 CC genotype, the proportion of CTLA-4 rs231725 AA genotype, the frequency of TLR7 rs3853839 C allele, and the frequency of CTLA-4 rs231725 A allele in the severe asthma group were higher than those in the mild asthma group(P<0.05). TLR7 rs3853839 CC genotype (OR=10.32, 95%CI=5.59-23.89), CTLA-4 rs231725 AA genotype (OR=13.21, 95%CI=3.58-20.25), TLR7 rs3853839 C allele frequency (OR=11.32, 95% CI=4.25-21.14) and CTLA-4 rs231725 A allele frequency (OR=13.24, 95% CI=6.59-20.21) could increase the susceptibility to severe asthma(P<0.05). TLR7 rs3853839CC genotype, TLR7 rs3853839C allele frequency, CTLA-4 rs231725AA genotype and CTLA-4 rs231725A allele frequency were risk factors for severe asthma(P<0.05). Conclusion TLR7 rs3853839 CC genotype, TLR7 rs3853839 C allele frequency, CTLA-4 rs231725 AA genotype and CTLA-4 rs231725 A allele frequency are associated with the occurrence of severe asthma.
3.Molecular traceability of the pathogen of the first local dengue infection outbreak in Hubei Province in 2019

Objective To identify the etiology of the first local dengue infection outbreak in Hubei Province in 2019, and to determine the serotype and genotype of the virus and trace its source. Methods Serum samples were collected from dengue fever cases in the acute phase. The IgM and IgG antibodies were detected by enzyme-linked immunosorbent assay (ELISA),and the serotype was determined by fluorescence quantitative PCR. C6/36 cells were used to isolate virus and obtain virus E gene and complete genome sequence for systematic evolution analysis to trace the possible source of infection. Results The pathogen of the outbreak was identified as dengue serotype I infection,and five virus strains were isolated. Sequence analysis showed that the virus belonged to genotype I of dengue I, and had the highest homology with the strain isolated in Guangzhou, 2019. Conclusion The first local dengue infection outbreak in Hubei Province in 2019 was caused by genotype I of the type I dengue virus.
4.Epidemic characteristics and prediction of varicella in Chongqing in 2014-2020

Objective To analyze the epidemic characteristics of varicella in Chongqing from 2014 to 2020, and to provide evidence for the development of scientific and effective varicella control strategies. Methods Data on the outbreak of varicella and vaccination in Chongqing from 2014 to 2020 were collected through the China Disease Prevention and Control Information System, and descriptive epidemiological methods were used for statistical analysis. Results A total of 181 551 cases of varicella were reported in Chongqing from 2014 to 2020, with an average annual incidence rate of 83.79 per 100 000. The incidence rate of varicella increased from 39.95 per 100 000 in 2014 to 81.88 per 100 000 in 2020 (P < 0.001). The incidence of varicella was seasonal, with the peak periods occurring from May to June and from October to December each year. The average annual incidence rate in municipal districts was 88.90/100 000, higher than 67.42/100 000 in counties and 82.50/100 000 in autonomous counties. The average annual incidence rate of varicella in males (87.13/100 000) was higher than that in females (80.38/100 000). The incidence of varicella was mainly distributed in people under 15 years old, with 143 508 cases (79.10%) reported, and the highest incidence age was 5-9 years old (37.00%). Among the affected occupations , 133 733 cases (62.6%) were students , 39 274 cases (18.40%) were children in nursery care, and 17 963 cases (8.4%) were scattered children. The actual number of doses of varicella vaccine from 2014 to 2020 was 2 302 522 doses, with the coverage rates of one-dose and two-dose vaccines being 75.56% and 32.17%, respectively. ARIMA predicted that there would be 2 604, 811, 756, 1 226, 2 405, 3 904, 2 410, 1 211, 2 034, 6 878, 10 887, and 8 955 cases of varicella from January to December 2021. Conclusion The incidence of varicella in Chongqing is on the rise, with obvious seasonal, regional and population distribution characteristics. It is necessary to strengthen the prevention and control of varicella epidemic, strengthen the prevention and control measures of key groups and key institutions in the high incidence season, strengthen the publicity of varicella vaccine, and improve the vaccination rate of two-doses of varicella vaccine for eligible children.
5.Epidemiological characteristics and risk factors of nontuberculous Mycobacterium infection in elderly patients with ramus in Huai'an Area

Objective The epidemiological characteristics and risk factors of infection of non-tuberculous mycobacterial (NTM) by elderly bronchiectasis patients in Huai 'an area were analyzed, and the theoretical basis for prevention of NTM infection by elderly bronchiectasis patients in Huai 'an area was provided. Methods Among the 371 elderly patients with bronchiectasis admitted to our hospital from January 2020 to June 2022 were selected and divided into control group and NTM group according to whether they had NTM or not. The NTM strains were isolated and identified. Clinical data of patients were collected from the medical record system. Independent risk factors of NTM infection in elderly patients with ramus were analyzed by univariate analysis and logistic regression, including gender, age, previous smoking status, number of ramus, pulmonary cavities, hypoproteinemia, and CD4+T cell level. Results A total of 108 cases NTM infection (29.11%) among the 371 patients with branch enlargement. There was no statistical significance in cough, phlegm, hemoptysis and fever between the two groups (P>0.05). The proportion of chest tightness and shortness in NTM group was significantly higher than that in control group (P<0.05). Six mycobacterium species were isolated from 108 patients with NTM infection, including 57 intracellular, 22 abscess, 14 avium, 7 Kansas, 5 terrapin and 3 flavus. There were significant differences in age, sex, smoking history, place of residence, number of brandished lobes, hypoproteinemia, pulmonary cavities, and number of CD4+T lymphocytes between the two groups (P<0.05). Multivariate logistic regression analysis showed that smoking history >20 years (OR=1.692), number of branchial dilated lobe ≥5 (OR=2.671) and thin-walled cavity (OR=2.458) were independent risk factors for NTM infection in elderly patients with branchial dilated lobe (P<0.05). Conclusion Elderly patients with bronchiectasis in Huai 'an area have a high risk of NTM infection, which are mainly Mycobacterium intracellular and mycobacterium abscess. The main risk factors are the number of bronchiectasis lung ≥5, smoking history >20 years, and thin-walled cavity. Patients should actively quit smoking, improve the body immunity, and prevent NTM infection in patients with bronchiectasis.
6.Epidemiological characteristics and drug resistance analysis of pathogenic bacteria of infective endocarditis in Nanjing area in 2015-2020

Objective To analyze and discuss the epidemiological characteristics and drug resistance of pathogenic bacteria of infective endocarditis in Nanjing in the past five years, and to provide reference for the prevention and treatment of infective endocarditis. Methods A total of 220 infective endocarditis cases in Nanjing area from 2015 to 2020 were included in the analysis. The distribution characteristics of pathogens were collected and analyzed. Information on drug resistance was also collected, and the potential relationship between pathogen distribution and drug resistance was explored. Statistical analysis was performed using SPSS 19.0. Results The ratio of male to female patients included in this study was 143:77, and most of them were aged from 41 to 60 years old. There were 195 patients with underlying cardiac diseases (88.64%). A total of 1132 pathogenic bacteria were detected in the blood samples of all 220 patients, among which 1 007 were mainly gram-positive bacteria (88.96%): 725 strains of Streptococcus viridans (725/1 132, 64.05%), 124 strains of Staphylococcus aureus (124/1 132, 10.95%), 96 strains of Staphylococcus epidermidis (96/1 132, 8.48%), and 62 strains (62/1 132, 5.48%) of others (Staphylococcus human, etc.); the main gram-negative bacteria ( a total of 125 strains , 11.04%) were 73 strains of Pseudomonas aeruginosa (73/1 132, 6.45%) and 52 strains of Escherichia coli (52/1 132, 4.59%). Among the strains isolated from subjects of different ages and genders, the distribution difference was consistent with the total trend, and there was no significant difference (all P>0.05). Among gram-positive bacteria, Streptococcus was sensitive to antibiotics such as penicillin, oxacillin, and vancomycin. Staphylococcus aureus and S. epidermidis were sensitive to vancomycin and resistant to oxacillin and penicillin. Gram-negative bacteria were less resistant to amikacin and meropenem. All these differences were statistically significant (all P<0.05). Conclusion For patients with infective endocarditis, antibiotics should be selected reasonably according to the type of pathogenic bacteria and the corresponding drug resistance characteristics.
7.Analysis of CD4+ T cell count of different genotypes of HIV infected people in Xi'an in 2017 -2021

Objective To study the difference of CD4+ T cell count among different genotypes of HIV infected people in Xi'an from 2017 to 2021. Methods A total of 1 623 newly diagnosed AIDS patients in the AIDS prevention and control information system in Xi'an from 2017 to 2021 were selected. The genotypes of all the patients were sequenced, and the differences of CD4+T cell counts among different genotypes were analyzed. Results From 2017 to 2021, the main genotype of HIV infected people in Xi'an was CRF01_ AE(921/1623)、CRF07_ BC(145/1623)、CRF08_ BC (557/1623), the gene cluster is mainly CRF01_ AE (cluster 1) (185/ 1623) and CRF01_ AE (cluster 2) (1438/1623), where CRF01_ The average CD4+ T cell count of AE genotype was (146.26 ± 11.63)/μ L,CRF07_ The average CD4+ T cell count of BC genotype was (254.69 ± 15.49)/μ L,CRF08_ The average CD4+ T cell count of BC genotype was (217.96 ± 12.89)/μ L,CRF01_ The average number of CD4+ T cells in AE (cluster 1) was (185.58±12.79)/ μ L,CRF01_ The average number of CD4+ T cells in AE (cluster 2) was (179.90 ± 15.96)/ μ 50. There was significant difference in CD4+ T cell count among patients with different gene subtypes and gene clusters (P<0.05). Conclusion From 2017 to 2021, the main genotype of HIV infected people in Xi'an was CRF01_ AE, the gene cluster is mainly CRF01_ AE (Cluster 2), there were significant differences in CD4+T cell counts among patients with different gene subtypes and gene subsets, which could serve as a reference target for AIDS treatment in this Municipality.
8.Seasonal characteristics and etiologic features of the cluster of fever in Pudong New Area of Shanghai in 2013-2021

Objective To understand the seasonal and etiological characteristics of the epidemic situation of cluster fever in Pudong New Area, Shanghai, and to provide a scientific basis for the formulation of prevention and control strategies. Methods The epidemic data of cluster fever in Pudong New Area of Shanghai from 2013 to 2021 were collected, and the data were analyzed by concentration degree and circular distribution methods. Results From 2013 to 2021, there were 109 cases of cluster fever reported in Pudong New Area, with primary schools reporting the most cases, 77 cases (70.64%). The M value was 0.59. Circular distribution results showed that angle dispersion index r value was 0.54, the average angle was 357.32°, and the average angle of Rayleigh’s test Z value was 32.07. The peak day was December 28, and the peak period was from October 25 to March 3 of the following year. The cluster of fever in Pudong New Area was mainly caused by influenza virus. Most cases were single pathogen infection, and some were double pathogen infection. Influenza A virus had the highest detection rate in both single pathogen infection and double pathogen infection. Conclusion Primary schools are the focus of prevention and control of the cluster of fever in Pudong New Area. The epidemic situation of cluster fever has a strong seasonal characteristic, and the peak of incidence is from late October to early March of the next year. Respiratory pathogens such as influenza virus are the main causes of the epidemic situation.
9.Etiological and epidemiological analysis of viral diarrhea surveillance in Hubei Province in 2017-2019

Objective To understand the pathogen composition and epidemic characteristics of viral diarrhea in Hubei province. Methods Stool samples of adults and children from sentinel monitoring of viral diarrhea in Hubei province from 2017 to 2019 were collected. Real-time RT-PCR method was used to detect the nucleic acids of rotavirus, norovirus, adenovirus, astrovirus and sapovirus. The detection results of the 5 pathogens were statistically analyzed. Results The total positive rate of 5 diarrheal viruses in 1 957 samples was 36.84%, and the number of cases of mixed infection accounted for 19.00%, among which rotavirus accounted for 9.50%, norovirus 13.29%, astrovirus 4.14%, adenovirus 14.10% and sapovirus 3.37%. In Hubei province, rotavirus and norovirus were still prevalent in autumn and winter. The epidemic seasons of adenovirus and astrovirus were similar, mainly occurring in April and May, while the infection rate of sapovirus was relatively high in November and January of the following year. The main infected population of the five diarrheal viruses was still children aged 5 years and younger, which was significantly higher than other age groups. Conclusion The main pathogens of viral diarrhea in Hubei from 2017 to 2019 are adenovirus, norovirus and rotavirus, and there are some mixed infections. It is necessary to strengthen the monitoring of major pathogens and take effective and scientific protection measures for high incidence seasons and key populations.
10.Infection and strain analysis of superficial fungi in 498 cases in Shiyan area

Objective To understand the clinical characteristics, susceptibility factors, and the composition and distribution of pathogenic fungi in Shiyan area. Methods From January 2018 to December 2018, at the outpatient department of Taihe Hospital, a total of 498 patients with superficial mycosis were confirmed by clinical features and microbial identification. Clinical data was collected and statistical analysis of these patients was carried out. Results Among 498 patients, the top three diseases were 123 cases of tinea corporis (24.70%), 110 cases of onychomycosis (22.09%), and 54 cases of body ringworm (10.84%). The top three pathogenic fungi were Trichophyton rubrum (232 strains, 46.59%), Trichophyton interdigitale (71 strains, 14.26%) and Candida albicans (41 strains, 8.23%). There were some differences in the incidence of patients of different genders, ages, and jobs. Conclusion Tinea cruris was the main superficial mycosis in Shiyan area and Trichophyton rubrum was the main pathogen of superficial mycosis. The epidemic trend is similar to that in most regions of the country, but it has its own characteristics.
