1.Traditional Chinese Medical Syndrome Patterns of Dengue Fever Patients:An Analysis of 210 Cases

Objective To investigate the distribution of traditional Chinese medical syndrome patterns of dengue fever, thus to standardize its clinical diagnosis and treatment and to enhance its therapeutic effect. Methods A prospective clinical trial was carried out in 210 dengue fever patients who were admitted from July to October of 2014. The clinical data of four physical examinations were collected and analyzed for the analysis of the distribution of syndrome patterns. Results ( 1) The traditional Chinese medical syndromes of 210 dengue fever patients were characterized by fever and aversion to cold, heaviness of limbs, poor appetite, headache and heaviness of head, abdominal fullness and discomfort, yellowish urine, yellowish and greasy fur, sluggish pulse. ( 2) The syndrome patterns of dengue fever were classified into blockage of damp-heat, disease involving both defensive phase and qi phase, heat attacking qi phase, heat attacking blood phase, toxicity invading pericardium, and sudden loss of yang-qi. The incidence of six patterns was in decreasing sequencing. ( 2) The results of laboratory examination showed that the decrease of white blood cell ( WBC) , neutrophil percentage and platelet count was obvious, and the haematocrit ( PLT) became disordered. The results of recheck showed taht the increase of calcitonin and C-reactive protein were not obvious, and the damage of vital organs was less. Conclusion Dengue fever can be classified into the damp-heat pestilence in traditional Chinese medical field. The syndrome patterns of dengue fever are dominated by blockage of damp-heat, and disease involving both defensive phase and qi phase, and correspondingly, the therapeutic methods should be focused on clearing heat, resolving dampness, and strengthening spleen.
2.The correlation between fraction of exhaled nitric oxide and lung function in asthmatic children

Objective To investigate the correlation between exhaled nitric oxide and lung function (FEV1)in asthmatic children.Methods Fifty three stable asthmatic children aged 5 to 14 years old were recruited from ShengJing Hospital of China Medical University.According to whether the patients were treated with inhaled corticosteroid(ICS)therapy regularly,they were divided into two groups:steroid group and non-steroid group,then fraction of exhaled nitric oxide(FeNO)and lung function were measured.Results In non-steroid group,the levels of FeNO(mean 40.450±25.428 part by billion)were significantly higher than those in the steroid group(mean 19.879±13.845 part by billion),and they were statistically significant.(P = 0.003).The mean FEVI in non-steroid group was(95.152±8.993)%,and the mean FEVI in non-steroid group was(91.350±11.690)%,and there were no significant differences between two groups (P =0.189).Significantly negative correlation was found between FeNO and FEV1 in steroid group(r =-0.465,P = 0.039),but there was no significant correlation between them in steroid group(r = 0.058,P =0.747).Conclusion The levels of FeNO were higher in non-steroid group than those of the steroid group in the stable asthmatic children.FeNO is a good biomarker to evaluate the airway inflammation of asthmatic children.
3.Effect of Extracorporeal Membrane Oxygenation on Clinical Outcome of Patients with Acute Left Main Coronary Artery Disease

Objective To evaluate the effect of extracorporeal membrane oxygenation (ECMO) combined with intraaortic balloon counterpulsation (IABP) on patients with acute left main coronary artery disease. Methods 3 patients suffered from acute left main coronary artery disease and timely supported by ECMO combined with or without IABP were reviewed. Results With the assistance of ECMO and/or IABP, all of the 3 patients had successfully undergone coronary angiography and coronary revascularization. The first case was performed percutaneous transluminal coronary angioplasty (PTCA) with stents placement; the second was performed coronary artery bypass grafting (CABG); the third case was performed PTCA and CABG. All patients were weaned off ECMO. 2 patients were discharged, whereas the other one died 1 month after CABG. After 6~24 months follow-up, 2 survivors had good quality of life. Conclusion Efficient ECMO support with or without IABP might offer opportunities for the following revascularization on patients with acute left main coronary artery disease.
4.Influence of platelet endothelial cell adhesion molecule-1 on diabetic retinopathy in patients with type 2 diabetes

Background To study diabetic retinopathy (DR) related risk factors is very important in the prevention of DR.Platelet endothelial cell adhesion molecule-1 (PECAM-1) is an important mediator that mediates high blood glucose-induced vascular diseases in diabetic patients.However,its mechanism is still below understood.Objective This clinical study was to investigate the effect of serum level changes of PECAM-1 on DR in type 2 diabetic patients.Methods Fifty-four patients with type 2 diabetes were enrolled from the endocrinology department of the Third People' s Hospital of Nantong City.Fundus examination was performed using the ophthalmoscope and fundus fluorescence angiography (FFA) on all the patients,and these patients were grouped as the non-DR (NDR)group (18 cases),non-proliferative DR(NPDR) group (20 cases) and proliferative DR group (PDR) (16 cases) based on the DR staging criterion of the Chinese Medical Association (1987 version).Eighteen age-and gender-matched normal subjects served as the normal control group.Peripheral blood was collected,and serum PECAM-1 levels were assayed using ELISA.Serum HbA1c levels were detected using the high performance liquid colorimetric(HPLC) method.The correlation of serum PECAM-1 level with serum HbA1c level was analyzed.All results were compared among the groups.Results The serum PECAM-1 levels were (10.907 ± 2.792),(7.024 ±2.377),(5.231 ± 1.816) and (3.817 ± 1.045) μg/L,respectively,in the PDR group,NPDR group,NDR group and normal control group,showing a significant difference among the 4 groups (F =12.630,P =0.02).Serum PECAM-1 content was significantly higher in the PDR group when compared with the NPDR group,NDR group and normal control group (P<0.05).The serum HbA1c levels were (12.596±3.148)%,(9.118±3.356)%,(5.491±1.017)% and (4.992 ± 0.725)% in the PDR group,NPDR group,NDR group and normal control group,respectively,with a significant difference among these 4 groups (F =7.130,P =0.015),and those in the PDR group and NPDR group were significantly elevated in comparison with the NDR group and normal control group (P<0.05).Significantly positive correlations were seen between serum PECAM-1 level and HbA1 c level in the PDR group,NPDR group and NDR group (r=0.799,P<0.01 ;r =0.647,P<0.01 ;r =0.685,P<0.01).Significantly more patients with a disease course of ≥ 10 years were in the NPDR group in comparison with the PDR group (P =0.023).Conclusions Increase of serum PECAM-1 level is closely associated with blood glucose level,and it is an important factor in the pathogenesis and development of DR.These results imply that control of blood glucose is crucial for the prevention of DR in patient with type 2 diabetes.
5.Ultrasonic diagnosis of nontramatic hiatal gastric hernia of esophagus in middle-aged and elderly people

Objective To discuss the clinical values of transabdominal ultrasonic diagnosis of nontraumatic hiatal gastric hernia of esophagus in middle-aged and elderly people.Methods Thirty-two cases were examined with fasting and filing methods in half-sitting,standing,supine and supine right anterior oblique positions.The findings were contrasted with X-ray barium meal examination,endoscopy and operation.Results Twenty-nine cases were of sliding hiatal gastric hernia of esophagus,3 of impacted hernia.The ultrasonic diagnosis coincidence rate was 100%.The signs and symtoms of acoustic images were widening of esophageal hiatus diaphragmatic muscle(≥1.37 cm),echoes of gastric mucosa were found in superior phrenic hernail sac and inside hernial sac.The display rates were 100%,100%,15.6%respectively.Conclusions The transabdominal ultrasonic diagnosis of nontraumatic hiatal gastric hernia of esophagus in middle-aged and elderly people is brief and accurate.
6.Effect of Fox01 expression on islet β-celis apoptosis in diabetic rat

SD rats were injected with streptozotocin and fed with high fat diet,used as type 2 diabetic model. Transcription factor Fox01 [in nucleus (15.00±1. 15 vs 6.45±0. 62) %, P<0.05], Caspase-3 [(23.73 ±1.48 vs 6.30±2.20)% ,P<0.01] expressions in pancreatic istet β cells and the positive rate of islet β cells apoptosis[(22.29±1.84 vs 6.25±2.42) %, P<0.01] in diabetic rats were higher than those of control rats. The islet cells which highly expresed Fox01 (in nucleus)and caspase-3 were just the apoptotic islet cells. Therefore,Fox01 may be involved in regulating apoptosis of islet β cells in type 2 diabetes.
7.Repairing of Spinal Cord Injury with Schwann Cell Neurilemma Channel in Rats

ObjectiveTo apply the Schwann cell neurilemma channel(SCNC) in promoting the regeneration of injuried spinal cord.MethodsSpinal cords of rats after laminectomy at T8/9 were divided in 3 groups randomly: 10 rats were transplanted with SCNC(Group A);10 rats only made models(Group B);10 rats were as normal control group(Group C).All rats received behaviour survey and inclined plane test every week 8 weeks after operation.6 weeks after operation,they were measured with cortical somatosensory evoked potential(CSEP).And 8 weeks after operation,all specimens were studied histologically.ResultsCSEP could be recorded in group A and C,but not in group B.There were marked differenties between group A or C and group B in behavior survey,inclined plane test.Amount of nerve fibers regenerated through the injury gap in group A was significantly larger than that in group B.ConclusionSCNC could facilitate axonal regeneration and promote the repairing of injuried spinal cord.
8.Determination of Paraben and Its Metabolite in Sediments by Ultra Performance Liquid Chromatography-Triple Quadrupole Tandem Mass Spectrometry

A new method for simultaneous determination of six benzoic acid esters and their metabolite p-hydroxybenzoic acid (p-HB) in sediments was developed.The samples were extracted with methanol and water,and then the extraction was purified by Oasis MCX solid phase column.The separation of targets was conducted by passing through a UPLC?HSS T3 column with a methanol-0.1% acid aqueous solution as the mobile phase under gradient elution.The qualitative analysis of the analytes was measured by Ultra performance liquid chromatography-Triple quadrupole tandem mass under multiple reaction monitoring mode (MRM) and the internal standard method was applied for quantitative analysis.The results indicated that seven target substances had a good linear relationship in the range of 0.1-500 μg/L with correlation coefficients above 0.9995 and the detection limits of 0.75-1.23 ng/g.The recoveries ranged from 65.5% to 88.3% and the relative standard deviation was 7.4%-13.6%.This method was suitable for the measurement of benzoic acid esters with the advantages such as simple processing,good selectivity and high sensitivity.
9.Relationship of CD4+ CD25hi regulatory T (Treg) cells to disease progression in HIV-infected patients.

OBJECTIVEAnalyzing the relationships between peripheral blood CD4+ CD25hi regulatory T (Treg) cells and peripheral blood immune status or plasma HIV-lviral load in HIV-infected individuals,so as to determine whether Treg were related to the progression of HIV-infected disease.
METHODS116 HIV-infected patients in different stages and 21 healthy control individuals were included in this study. The CD4+ and CD8+ T cell counts were determined by a standard 4-color flow cytometry technique. The Treg cells were examined with 3-color immune staining flow cytometry. The plasma HIV-1 viral load was detected by real time PCR.
RESULTSThe frequencies of Treg cells decreased in HIV-infected individuals with high CD4+ T cell counts( > 300/microl) compared with normal controls. With the progression of disease the frequencies of Treg cells were raised gradually, until were increased in HIV-infected individuals with low levels of CD4+ T cell counts ( < 100/microl). In addition, the frequencies of Treg cells were inversely related to CD4+ T cell counts and CD4+ /CD8+ ratio, data showed a statistically significant (respectively, r = -0.564, P < 0.001; r = -0.377, P < 0.001). Furthermore, the proportions of Treg cells were closely related to plasma HIV-1 RNA viral load (r = 0.514, P < 0.001).
CONCLUSIONCD4 CD25hi Treg cells should be a kind of important cells participating the immunopathogenesis of AIDS. It may play different roles in different stages of HIV-infected disease. The exact mechanism of Treg cells in the progression of the HIV-infected disease needs to be investigate further.

10.Establishment and optimization of detection of the hepatitis B immune complexes.

OBJECTIVETo establish and optimize methods to detect the immune complexes (IC) of hepatitis B virus directly.
METHODSA C1q solid phase ELISA, mouse anti-HBs MAb solid phase ELISA and the complement consumption assay were established to detect the IC and these methods were optimized.
RESULTSAll the three methods were highly sensitive, specific and reproducible. The C1q used for coating tended to lose its activity easily at room temperature. Although strict requirements are needed for the raw and processed materials for complement consumption assay and the process of manipulation is complex, it can quantitatively detect IC. Comparing to the C1q solid ELISA and complement consumption assay, the mouse anti-HBs MAb solid phase ELISA has its own merits: convenience and stability.
CONCLUSIONMouse anti-HBs MAb solid phase ELISA is the best way to detect IC directly.
