Alzheimer's disease(AD),the most common form of dementia,it is lack of effective cure or preventive treatment.Dementias in the elder are an increasing medical,social and economic problems and current treatments are only mildly effective.Recently,amyloid-beta protein(A?) has become a major therapeutic target.A? vaccine treatment can improve cognition in the patients with AD,but adverse events,such as meningencephalitis were observed in clinical study.The passive A? immunotherapy in humans is effective with possible safety.However,patients need to be monitored carefully.
2.Insulin and Alzheimer's disease

Recently,the incidence of Alzheimer's disease has been significantly increasing.However,the pathogenesis of Alzheimer's disease remains unknown.It is presumed that insulin and insulin receptor may be involved in the pathogenesis of Alzheimer's disease.The relationship between insulin and cerebral glucose metabolism,the relationship between insulin,insulin receptor,insulin resistance and pathogenesis of Alzheimer's disease as well as the possible mechanism are reviewed in this paper,and the progress of treatment of AD is also described.

Unlike other vulnerable groups, rehabilitation is the most urgent problem to be resolved for the disabled. Good working onrehabilitation for the disabled is the foundation to improve the basic living condition and enhance the ability to participate in the society, andcommunity-based rehabilitation is the key to achieve "Rehabilitation for All" for the disabled. The goal "Rehabilitation for All" can beachieved only if "rehabilitation in community and service in family" be reached.
5.Effect of astaxanthin intervention on contrast-induced acute kidney injury in experimental rats

Objective To explore the protective effect and mechanism of astaxanthin (AST) on the acute kidney injury induced by iohexol in rats.Method Thirty rats were randomly divided into five groups:control group (Ctrl);iohexol group (CM);astaxanthin group (AST,100 mg/kg),low astaxanthin dose group (LAST+CM,50 mg/kg) and high astaxanthin dose group (HAST+CM,100 mg/kg),6 in each group.The rats in AST,LAST+CM,HAST+CM groups were administrated with AST by oral gavages using an intubation needle for 10 consecutive days.The rats in Ctrl and CM groups rats in Ctrl,CM groups were given with dissolvant instead in equal volume.Except for the Ctrl and AST groups,on day 8,rats were given indomethacin,L-NAME and iohexol in their femoral vein under chloral hydrate anesthesia to build a contrast induced-nephropathy (CIN) model.At the end of the experiment (72 h after CIN induction),all rats were sacrificed.The Scr level,BUN level,renal histology,renal tissue activities in superoxide dismutase (SOD),catalase (CAT),glutathione peroxidase (GPx),Glutathione (GSH) and level of malondialdehyde (MDA) were performed.Apoptosis of renal cells was detected by Bcl-2,Bax and Caspase-3 p17 with Western blot.Results Compared with Ctrl group,the levels of Scr,BUN were significantly increased in CM group (all P < 0.01);while compared with CM group,the indicators were decreased in treatment groups (P < 0.01).Renal tubular structure damage,medulla congestion,loss of brush border,vacuolar degeneration,apoptosis and proteinaceous casts were observed in the CM group,and the renal injury scores were higher compared with Ctrl group (P < 0.05),however,administrated with AST could significantly improve the changes (P < 0.05).Oxidative stress indicators showed that MDA level were increased while SOD,GPx,GSH activities were significantly decreased at CM group (all P < 0.05),and the indicators above were ameliorated in treatment groups (all P < 0.05).Western blot showed that the expression of Bcl-2 was down-regulated while the Bax,Caspase 3 p17 was up-regulated respectively at CM group (P < 0.05),while the HAST+CM group could prevent the changes.Conclusions Iohexol can results in oxidative stress increased in kidney,which activate Caspase-3 p17 signal path,down-regulated Bcl-2 expression,up-regulated Bax expression respectively,and lead to cell apoptosis.AST can ameliorate the changes,especially with high AST dose,which suggest that the possible protection mechanism is by ameliorating oxidative stress and inhibiting apoptosis pathways.

The new revision of Declaration of Helsinki in 2013 has adjusted the structure, added or deleted some provisions on the content and modification, and modificated on the details of some words.Revision of the new version has improved the architecture, perfected the content of the declaration, strengthened the protection of the subjects, increased demand for researchers, and cleared the duty of country, research institutions and bidders.The enlighten-ment to our country are:constantly revised involving human clinical trials of the relevant laws and regulations , to pro-tect rights and interests of the subjects;National obligations for protection of the subjects, clinical trials explore the establishment of national compensation system;Ethics committee responsibilities and clearly positioning, exploring to establish an independent ethical review body.
7.Relationship Between Bilirubin Blood Lipid Comprehensive Index and Fibrinogen to Severity of Coronary Lesions in Patients With Coronary Artery Disease

Objective: To investigate the relationship between Bilirubin blood lipid comprehensive index and ifbrinogen (FIB) to severity of coronary lesions in patients with coronary artery disease (CAD).
Methods: A total of 324 patients with angiography (CAG) conifrmed diagnosis were divided into 2 sets of groups.①By CAG examination, the patients were divided into 2 groups: CAD group,n=262 and Non-CAD group,n=62.②By Gensini scoring system, the patients were divided into 4 quartile groups: 1st quartile group,n=58, 2nd quartile group,n=110, 3rd group, n=80 and 4th quartile group,n=76. The blood levels of total bilirubin (TBIL), total cholesterol (TC), LDL-C, HDL-C, TG and ifbrinogen were measured and bilirubin blood lipid comprehensive index, TC/(HDL-C+TBIL) and LDL-C/(HDL-C+TBIL) were calculated respectively.
Results:①By CAG examination, compared with Non-CAD group, CAD group had increased TC, LDL-C, ratios of TC/(HDL-C+TBIL), LDL-C/(HDL-C+TBIL) and FIB,P<0.05; decreased TBIL and indirect bilirubin (IBIL),P<0.05.②By Gensini scoring system, the level of TBIL, IBIL, ratios of LDL-C/(HDL-C+TBIL), TC/(HDL-C+TBIL), HDL-C and FIB were different among 4 groups,P<0.05.③ Logistic regression analysis indicated that with excluded interactional factors, the bilirubin blood lipid comprehensive index was the independent risk factor for CAD[TC/(HDL-C+TBIL): OR: 1.668, 95% CI:1.065~2.784,P=0.028; LDL-C/(HDL-C+TBIL ): OR: 1.786, 95% CI: 1.021~2.879,P=0.030].④Correlation analysis presented that TC/(HDL-C+TBIL), LDL-C/(HDL-C+TBIL) and FIB were positively related to Gensini scoring system (r=0.423,P<0.01), (r=0.417,P<0.01) and (r= 0.293,P <0.01) respectively.
Conclusion: Bilirubin blood lipid comprehensive index and ifbrinogen were positively related to severity of coronary lesions in CAD patients.

Benign anastomotic stenosis after esophagogastrectomy could reduce the patients' quality of life,even resulting in severe malnutrition and death.The endoscopic treatment includes dilatation,stent insertion,locoregional injection,and a relatively new technique radial incision and cutting.This article reviewed the progress in endoscopic treatment of benign anastomotic stenosis after esophagogastrectomy.
9.Bifidobacterium: Re-evaluation from Microecological Point of View

OBJECTIVE To re-evaluate Bifidobacterium from microecological point of view. METHODS With the development of microecology,it is paid more and more attention to the medical field.The genesis and development of diseases were closely related with microbe imbalance.As a kind of common microbial population in intestinal tract,Bifidobacterium are one of the most important sources for the exploitation of microecological preparation.Bifidobacterium were re-evaluated by the reference review method. RESULTS This review is focused on the current situation,bioactivity,clinical application,and prospective of Bifidobacterium. CONCLUSIONS Bifidobacterium have good application prospects.
10.The effects of infrasound on the ultrastructure of the rat pallium

Objective To further investigate the threshold value and the dose-effect relationship of infrasound on the rat pallium, and to observe the effects on the pallium's ultrastructure. Methods Rats were exposed to 16 Hz, 90 dB infrasound for 2 hours daily. With a transmission electron microscope, changes in the ultrastructure of the cortex of the rats' parietal lobes were observed 2 hours, 3 days and 7 days after the treatment. Results There were no changes in the ultrastructure of the pallium in rats which had been exposed for 7 days or 14 days. Denaturation appeared when the rats had been exposed for 21 days, 28 days or 35 days. Denaturation become more serious as the exposure time increased beyond 21 days. Denaturation and damage gradually returned to normal with time after exposure. Conclusion 16 Hz, 90 dB infrasound exposure over an extended time may cause denaturation and other changes in the ultrastructure of the pallium in rats. Such changes can return to normal with time.
