1.The effect of octreotide acetate on plasma ETX and serum inflammatory cytokine of the rabbit with hepatic Ischemia-reperfusion Injury

Objective To observe the effect of octreotide acetate on plasma ETX and serum inflammatory cytokine of the rabbit with hepatic ischcmia reperfusion injury. Methods Pringle's maneuver rabbit hepatic ischemia-repeffusion models were established. 24 adult New Zealand rabbits were random divided into equal 3 groups: sham operative group(group A), iacbemia reperfusion group(group B)and octreotide acetate preconditioning group(group C). Endotoxin (ETX) in the plasma, tumor necrosis factor-alpha (TNF-α) and intcdeukin-1beta (IL-1β) were detected in every rabbit at the time before iachemia (T1), 30min after ischemia (T2), 30min (T3) and 120min (T4) after reperfusion. Results From T2 to T4, the ETX in group B and C were higher than that in group A (P < 0.05), the ETX of group C were lower than that in group B (P<0.05). From T2 to T4, the TNF-α of group B and C were higher than that of group A(P<0.05). From T3 to T4 the TNF-α of group C were higher than that of group A(P<0.05). From T2 to T4,the IL-1β of group B and group C were higher than that of group A(P<0.05), and the IL-1β of group C were lower than that of group B (P<0.05). Conclusion Octreotide acetate can decrease plasma ETX and down-regulate inflammation factors, such as TNFαand IL-1β, in serum of the rabbit with hepatic iacbe-mia-reperfusion injury, which may be the protective mechanism of oetreotide acetate on rabbit hepatic isehemia-reperfusiun injury.
2.Detection of micrometastases in the axillary lymph nodes in patients with breast cancer by reverse transcriptase polymerase chain reaction and southern hybridization

Objective This study was to detect micrometastases in the axillary lymph nodes in primary breast cancer patients and investigate its clinical significance. Methods The cytokeratin 19 gene expression was examined by RT-PCR and Southern hybridization methods. The sensitivity was compared with that of immunohistochemistry(IHC) method.Results The sensitivity was 1∶5?105, 1∶106 and 1∶5?104 by RT-PCR, Southern hybridization and IHC methods,respectively. Thirty-five of 170 routine pathology negative lymph nodes (206%)in 52 primary breast cancer patients contained CK-19 positive signals. Twenty-one out of 33(636%) routine negative lymph nodes were found micrometastases positive in 17 patients who proved to be with lymph node metastases by routine pathologic examination.Eleven out of 35(314%) metastasis negative patients by routine pathology were CK-19 gene positive.Hence the micrometastasis found by our method is an independent parameter.Conclusion RT-PCR method, using CK-19 as a biological marker,was sensitive and specific in detecting micrometastases in primary breast cancer,it could be a new prognostic parameter for breast cancer patients.
3.Effects of remifentanil on the proliferation and apoptosis of human liver carcinoma cell line HepG2 in vitro

Objective To evaluate the effects of remifentanil on the proliferation,the cell cycle and apoptosis of human liver carcinoma cell line HepG2 in vitro.Methods Human liver carcinoma cells HepG2 were cultured in vitro.The HepG2 cells of the test group were incubated in the RPMI-1640 medium with remifentanil at different concentration(0.001,0.01,0.1,1,10,100,200 μmol/L).The HepG2 cells of the control group were incubated in the RPMI-1640 medium for 48 hours.The level of the cell proliferation was evaluated with methylthiazolyldiphenyl-tetrazolium bromide (MTT) method.The cell cycle was detected with flow cytometry (FCM).The morphological change of apoptosis cell was observed by fluorescence microscopy after staining by Hoechst33258.Results Remifentanil inhibited the proliferation of the HepG2 cells with a dose-dependent effect.Compared with control group,the cell proliferation capability was apparently decreased in the test group (P < 0.05) when the concentration of remifentanil was over 1 μmol/L (P <0.05).However,no significant difference in cell proliferation was found when remifentanil was 100 and 200 μmol/ L.The ratio of G0/G1 phase of HepG2 cells was significantly enhanced and the ratio of S phase of HepG2 cells was significantly decreased when remifentanil was over 1 μmol/L.The fluorescent microscopy stained by the Hoechst33258 showed part of HcpG2 cells apoptosis in test group,and the apoptotic rate was significantly increased when remifentanil was over 1 μmol/L (P < 0.05).Conclusions The data suggest that remifentanil would inhibit the proliferation of HepG2 cells and induce apoptosis when remifentanil was over 1 μmol/L.
4.The protective effect of octreotide on myocardial injury after hepatic ischemia-reperfusion injury in rabbit

Objective To study the protective effect of octreotide on myocardial injury after hepatic ischemia-reperfusion in rabbit. Methods Pringle's maneuver rabbit hepatic ischemia-reperfusion model was established. 24 adult New Zealand rabbits were random divided into equal 3 groups: sham operative group ( group A) , ischemia-reperfusion group( group B) and octreotide preconditioning group ( group C ). The levels of CK-MB( MB isoenzyme of creatine kinase) and LDH ( 1actate dehydrogenase), superoxide dismutase (SOD) and malondialdehyde (MDA) of each group were measured at the time before ischemia (T1) , after ischemia for 30 mins ( T2 ) and after reperfusion for 60 mins ( T3 ), 120mins ( T4 ), 240 mins ( T5 ). The SOD and MDA in myocardial tissue of each group were measured after reperfusion for 240 mins. The changes of ultrastructure in the myocardial cell were observed by transmission electron microscopy after reperfusion for 240 mins. Results There was no significant difference in the levels of CK-MB and LDH in serum of each group before ischemia ( P >0. 05). The CK-MB and LDH of group B and C were higher than that of group A ( P <0.05) after ischemia for30 mins. The CK-MB and LDH of group C were lower than that of group B in this period( P <0. 05 ). The highest time point of LDH and CK-MB were after reperfusion for 120 mins and 240 mins. The contents of MDA in group B and group C were higher than that in group A from after ischemia for 30 mins in plasma and after reperfusion for 240 mins in myocardial tissue ( P < 0. 05 ),and it in group C were lower than that in group B( P <0.05) .The contents of SOD in group B and group C were lower than that in group A from after ischemia for 30 mins in blood plasma and after reperfusion for 240 mins in myocardial tissue ( P <0. 05), and in group C were higher than that in group B( P <0. 05).The electromicroscope showed that the pathological change of myocardial ultrastructure of group C was slighter than that of group B. Conclusion Octreotide can stabilize myocardial cell membrane and reduce release of oxygen free radical and significantly relieve the injury of myocardial ultrastructure after hepatic ischemiareperfusion in rabbit. Octreotide preconditioning can relieve myocardial injury after hepatic ischemia-reperfusion in rabbit.
5.Neuroprotective effect screening and the mechanism of 10 kinds of coumarin derivatives.

The study reports the detection of neuroprotective effect of 10 kinds of coumarin derivatives and explores their possible mechanism. MTT method was used to screen the neuroprotective effect of 10 coumarin derivatives on neurotoxic agents (Aβ25-35 and rotenone) or OGD (oxygen-glucose deprivation). A compound with better protective effect was obtained. Then the effect of this compound on neurotoxic agents on PC12 was detected by the morphological observation. Furthermore, the effect of compound 3 on microglia with lipopolysaccharide (LPS) induced inflammation was detected. And the inflammatory factor was tested. Finally, direct free radical scavenging ability was detected. Compound 3 was found to be the best compound through three neurons toxic models. Not only compound 3 ameliorated cell viability reduced by three neurons toxic models, but also significantly inhibited the production of inflammatory factor (TNF-α and IL-1β). And its free radical scavenging ability is very good, especially the effect on superoxide anion, which is comparable with vitamin C. The significant scavenging effect of compound 3 on superoxide anion might be the mechanism of the neuroprotection. Compound 3 as a potential neural cell protective agent merits further investigation.
6.Data analysis of MRI misdiagnosis and missed diagnosis of breast lesions on clinical, radiologic and pathologic features

Objective To analyze the MRI data of misdiagnosed and missed diagnosed of breast lesions and their histopathological features.Methods Data from 241 breast lesions within 121 patients were recruited in this study.The data included MRI images,uhrasounds and X-ray images were retrospectively interpreted by two radiologist and each lesion was assessed according to the BI-RADS classification.The pathologic features of miss or error diagnosed lesions on MRI were analyzed.Results In 241 breast lesions (malignance 120,bcnign 121),4 lcsions were miss diagnosed on MRI.Thcy were 2 intraductal papillomatosis and 2 fibroadenoma.All was benign.Twenty three lesions were misdiagnosed on MRI.Sixteen were overestimation,including 3 chronic inflammations,3 sclerosing adenosis,2 fibroadenoma,4 fibrocystic changes with or without atypical ductal hyperplasia (ADH),2 intraductal papilloma,1 infiltration of pectoralis major muscle and 1 axillary lymphnode metastasis.Meanwhile,there were 7 lesions were underestimation.These lesions included 2 invasive ductal carcinomas,1 mucinous adenocarcinoma,2 DCIS and 1 blunt duct adenosis with ADH and focal cancerous,1 inflammatory breast cancer underwent chemotherapy.The sensitivity and specificity and accuracy of breast MRI were 95.83 % (115/120),72.73 % (88/121),84.23 % (203/241),respectively.MRI findings had no difference with respect to mammogram or ultrasound was 75.10 % (181/241).Conclusion MRI misdiagnosis and missed often occurs in smaller breast lesions,morphologic and hemodynamic malignant manifestation atypical,especially intraductal lesions.MRI diagnosis should be combined with physical examination,X-ray mammogram and ultrasound to improve diagnostic accuracy and reduce missed diagnosis.
7.Application value of CMIA in HCV infection validation in low risk population

Objective To detect the reactive samples of enzyme-linked immunosorbent assay (ELISA1) by chemiluminescence microparticle immunoassay (CMIA),and to analyze the application value of CMIA in HCV infection validation of blood donors.Methods Nucleic acid 3-item combined testing (NAT),another ELISA2,HCV antibody supplementary test(Western Blot test,WB) and CMIA test supplemented in blood samples of 102 ELISA1 anti-HCV reactive blood donors were retrospectively analysed.Results Among 102 blood donors of anti-HCV positive,32 cases (31.37%,32/102) were HCV RNA reactive samples,50 cases (49.02%,50/102) were ELISA2/WB reactive simultaneously.With CMIA NAT results as the reference standard,CMIA was poorly correlated with HCV RNA (Spearman correlation coefficient rs=0.395,P<0.01),and the consistency between them was weak by Kappa test (Kappa=0.270,P<0.01).With ELISA2/WB detection results as the reference standard,CMIA was highly correlated with the results(Spearman correlation coefficient rs=0.713,P<0.01),and which showed high consistency by Kappa test (Kappa=0.674,P<0.01).Conclusion CMIA as a detection method of protein label after HCV infection has great value in the HCV infection confirmation in low-risk population.
8.The true feelings of dying patients in China:a meta-synthesis of qualitative studies

Objective To conduct a systematic review on the need and experience of patients in end of life and provide evidence-based guidance for delivery of end-of-life care in hospital and community settings. Methods Two Chinese (CBM and CNKI) and four English relevant databases (PsycINFO,MEDLINE,EMbase and CINAHL) were in-cluded to retrieve literatures on Chinese dying patients' needs and experience between inception and August 2016. The JBI Critical Appraisal Tool for qualitative studies in Australia was used to evaluate the quality of studies. The searching results were synthesized by employing Integrating Methods. Results Seventy-five findings were extracted from 9 qualified studies. The 75 findings were sequently integrated into 5 categories and finally 2 synthesized re-sults were summarized from the 5 categories. Integration result 1:under the sufferings from both physical and psy-chological aspects,dying patients require support from both family and society. Integration result 2:although death is acceptable for patients,they still hope for dignity and goals of life. Conclusion Needs of patients in end-of-life should be understood and respected. Reasonable and effective measures need to be taken to meet their needs and improve their quality of life.
