1.Curative Effect of Acupuncture and Rehabilitation Training in Treating Hemiplegia After Stroke

Patients of post-stroke hemiplegia were divided into acupuncture and rehabilita tion training group (n = 50), acupuncture group (n = 32), and rehabilitation training group (n= 31), and treated with acupuncture and/or rehabilitation training. The patients' moving ability and capability of daily life were estimated before and after treatment. The total effective rate in the acupuncture and rehabilitation training group was higher than those in the acupuncture group and the rehabilitation training group (P< 0.01), and there was no difference between the acupuncture group and the rehabilitation group (P > 0.05). The method of acupuncture combining rehabilitation training is an effective therapy for treating post-stroke hemiplegia.
2.Changes in expression of NLRP3 inflammasomes in spinal cord of rats with neuropathic pain

Objective To evaluate the changes in the expression of NOD-like receptor family pyrin domain-containing 3 (NLRP3) inflammasomes in the spinal cord of rats with neuropathic pain.Methods Fifty-four healthy adult male Sprague-Dawley rats,weighing 250-300 g,were divided into 2 groups (n=27 each) using a random number table:sham operation group (S group) and neuropathic pain group (NP group).Neuropathic pain was produced by chronic constriction injury in anesthetized rats.The sciatic nerve was exposed,and 4 loose ligatures were placed on the sciatic nerve at 1 mm intervals with 4.0 chromic catgut.The mechanical paw withdrawal threshold (MWT) was measured at 1 day before operation and 1,4,7 and 14 days after operation.After measurement of MWT at each time point after operation,the animals were sacrificed,and the L4_6 segments of the spinal cord were removed for determination of the expression of NLRP3,apoptosis-associated speck-like protein containing a CARD (ASC),caspase-1 and interleukin-1beta (IL-1β) protein and mRNA (by Western blot or by fluorescent quantitative real-time polymerase chain reaction).After measurement of MWT at day 14 after operation,the L4-6 segments of the spinal cord were removed for determination of the co-expression of NLRP3 with glial fibrillary acidic protein (GFAP) (by double immunofluorescence staining).Results Compared with S group,the MWT was significantly decreased,and the expression of NLRP3,ASC,caspase-1 and IL-1β protein and mRNA was up-regulated at each time point after operation in NP group (P<0.05).The expression of NLRP3 and ASC protein and mRNA peaked at day 4 after operation,the expression of caspase-1 and IL-1β protein and mR-NA peaked at day 7 after operation,and the MWT reached the lowest value at day 7 after operation (P<0.05).NLPR3 inflammasomes were mainly expressed in the astrocytes and neurons.Conclusion The expression of NLPR3 inflammasomes in the spinal astrocytes and neurons is up-regulated in the rats with peripheral nerve damage,and the change may be involved in the development and maintenance of neuropathic pain.
3.The effect of octreotide on lung injury after ischemia-reperfusion in the rabbit liver

Objective To research the effect of octreotide on the lung injury of far place organ after ischemia-reperfusion in rabbit liver.Methods Prings maneuver rabbit hepatic ischemia-reperfusion models were established. 24 adult New Zealand rabbits were random divided into three groups: group Ⅰ (sham operative group) , group Ⅱ (ischemia-reperfusion by physiological saline group) and group Ⅲ (octreotide preconditioning group). To group Ⅲ, we injected octreotide of 20μg/kg to abdominal cavity and octreotide of 30(μg/kg to skin following, and octreotide was dissolved into 2ml with 0. 9% physiological saline. To group Ⅰ and Ⅱ, octreotide were replaced with the same amount of physiological saline. The changes of MAP, HR in every group were recorded at the time before ischemia (T1 ) , 30min (T2) after ischemia, 30min(T3) , 60min(T4) , 120min(T5) , 240min(T6) after reperfusion. The tumor necrosis factor-alpha (TNF-a) and interleukin-lbeta (IL-1β) in the plasma in every group at T1, T2, T3, T4 , T5, T6 were detected. These rabbits were killed 240 min after reperfusion, then the lung's hepatocellular ultrastructures of every group were observed under electromicroscope, and the apoptosis of lung was detected by TUNEL. Results The MAP, HR of group Ⅱ and group Ⅲ were lower than that of group Ⅰ at T2 to T4. Moreover, group Ⅱ were lower than that of group Ⅲ (P <0.05). The TNF-a, IL-1β of group Ⅱ (fromT2) and group Ⅲ ( from T3) were higher than that of group Ⅰ ( P < 0.05) , and group Ⅲ were lower than group Ⅱ after ischemia (P <0. 01). Through electromicroscope, we found that the injury of the lungs hepatocellular ultrastructure in group Ⅲ was slighter than that in group Ⅱ . We detected the apoptosis of the lung organizes by TUNEL under 5 fields of light microscopes, and found that the apoptosis counts of group Ⅱ (55. 82 ±4. 19) and group Ⅲ (32. 17 ±3. 10) were more than that of group Ⅰ (3. 96 ±0. 87), and group Ⅲ were less than thatof group Ⅱ (P < 0. 01). Conclusion Octreotide can protect the lung injury of far place organ after ische-mia-reperfusion in rabbit liver.
4.Expression and role of NGX6 in tumor

NGX6 is a recently discovered tumor-suppressor gene,which can inhibit tumor invasion and metastasis via suppressing tumor angiogenesis and lymphangiogenesis.The expression of NGX6 gene has nega-tive correlation with many tumors,such as nasopharyngeal carcinoma,lung cancer,colon cancer and so on. Therefore,the measurement of NGX6′s level has a great influence on the prognosis of patients with tumor.
5.Mdr 1a gene knockout and drug transport

The mdr 1-type P-glycoprotein can confer multidrug resistance to tumor cells by actively pumping a wide variety of drugs from the cells. To counteract this drug resistance, P-glycoprotein-blocking agents are currently administered to patient during chemotherapy. However, this may also affect the normal physiological functions of the mdr 1-type P-glycoprotein. The P-gp is a transmembrane efflux transporter found on the luminal side of the capillary cells that comprise the blood-brain barrier. Recent insights recognize P-glycoprotein, as an important element of the blood-brain barrier (BBB), play an important role for mdr 1a P-glycoprotein in the blood-brain barrier. Studies in mice, which lack P-gp in the BBB 〔mdr1a (-/-) mice〕, have contributed significantly to these insights. In addition, the potential of new therapeutic approaches in the near future as well as their limitations will be clearified.
6.Risk factors and control strategies of nosocomial infection in intensive care patients in the neurological department of internal medicine

Objective To explore the risk factors and control strategies of nosocomial infection in intensive care patients in the neurological department of internal medicine.Methods 422 intensive care patients in the neuro-logical department of internal medicine were retrospectively analyzed.Results The incidence rate of neurology ICU infection was 24.41%,which was significantly higher than 4.54%of the same period of nosocomial infection rate,the difference was statistically significant(χ2 =10.36,P<0.05).In the intensive care unit of the nervous system,the respiratory infection was mainly caused by respiratory infection,the incidence rate was 57.28%,urinary system was 25.24%,and digestive system was 9.71%.Neural medical intensive care unit patients aged 60 years old,time was more than or equal to 14 days in hospital,impaired consciousness,invasive procedures,use of antibiotics and a history of diabetes mellitus in hospital infection incidence increased significantly,and the differences were statistically signifi-cant(χ2 =7.65,10.76,16.87,9.43,9.25,9.87,all P<0.05).Conclusion Older patients,hospitalized time lon-ger,impaired consciousness,invasive procedures,antibiotic treatment and a history of diabetes mellitus were neural medical intensive care unit patients occurred nosocomial infection risk factors,clinical should strengthen nursing,to improve the work of prevention and monitoring to minimize nosocomial infection.
7.Value of acoustic perceptual method for analysis of compensatory articulation errors in postoperative patients with cleft palate

Objective To establish an acoustic perceptual method analyzing the compensatory articulation errors in children with operated cleft palate via the formants of Chinese pure vowels. Methods The first three formants which represent vocal transmission character in Chinese pure vowels of 84 subjects with operated cleft palate, were measured by Computerized Speech Signal Processing System (CSSPS). The Chinese vowel graph of postoperative patients with cleft palate was stated by the first formant frequencies (F1) and the second formant frequencies (F2) of the Chinese pure vowels between the two groups. Results Values of F1 and F2 of vowels except [a] in the poor articulation group (Group A) were significantly different from those in the good articulation group (Group B) (P<0.05 or P<0.01). As compared with that in Group B, the vowel graph demonstrated the decreased perceptual distances in Group A. These findings indicated that there might still be the backward movements of tongue, perverted mandibular movements and disharmonious lip movements in addition to the velopharyngeal insufficiency (VPI) in Group A. Conclusion The speech articulation of children with repaired cleft palate should be gained by correcting the aberrant compensatory articulation errors in the condition of velopharyngeal competence. Computerized Speech Signal Processing System (CSSPS), which is regarded as the content of objective quantitative measurement, is a precise, simple, reliable and atroumatic technique for children with cleft palate to analyze pathological compensatory articulation errors.

10.Factors Influencing of Outcome in Cauda Equina Syndrome

Objective: We examined the cause、level of pathology、onset of symptoms、time taken to treatment and outcome of 14 patients with cauda equina syndrome(CES) to determine how these factors influenced the eventual outcome.Methods:14 patients with CES treated at our hospital between 1991 and 1997 underwent laminectomy followed by bilateral decompression.The mean time of followup was 35 months(range 22~48months).Logistical regression analysis was used to manage the data.Results:Out of 14 patients,11 had satisfactory recovery at 2 years postdecompression;3 patients were left with some residual dysfunction.Conclusion:There was a clear correlation between delayed decompression and the poor overall outcome,as well as the presence of perineal anaesthesia and significant sphicter dysfuncation.
