Objective To assess the short-term effect of cardiac resynchronization therapy (CRT) by speckle tracking strain imaging(STI). Methods Sixteen chronic heart failure patients(CHF) with CRT were enrolled. The time to peak systolic longitudinal strain and the time to peak systolic radial strain were derived from the LV apical views, and the time to peak systolic circumferential strain was derived from the parasternal short axis. The standard deviation for time to peak longitudinal, radial and circumferential strain in the 18 segments (LS-SD18, RS-SD18, CS-SD18) and the maximal temporal difference of any two segments(LS-dif, RS-dif, CS-dif) were calculated as a strain-derived dyssynchrony index. The difference of LS-SDIS, RS-SD18, CS-SD18 between 1 month,3 months after CRT and baseline (△LS-SD18, △RS-SD18, △CS-SD18) was calculated. The correlation between △LS-SD18,ARS-SD18,△CS-SD18 and the reduction of end-systolic volume at follow-up(△ESV%) were analyzed. Results The LS-SD18, RS-SD18,CS-SD18 and RS-dif were decreased significantly 1 month,3 months after CRT.△RS-SD18 was correlated with △ESV% 3 months after CRT(r = 0. 694). Conclusions The strain-derived dyssynchrony index is great significient for short-term effect of CRT.

Objective To evaluate the tissue motion tracking of mitral annular displaeement(TMAD) method in the assessment of short-term effect of cardiac resynchronization therapy (CRT). Methods Twenty six chronic heart failure (CHF) patients with CRT were detected by TMAD. The maximal displacement of the six points of mitral annulus(DS), the standard deviation of the time to regional peak systolie displacement of the six points (Td-sd) and the maximal temporal difference of any two points(Tds-diff) were compared before and one month after CRT. And the correlation between mean displacement and left ventricular ejection fraetion(LVEF),left ventricular end systolic volume(LVESV), fraction area change (FAC) was analyzed. Results Compared with the pre-operatively, the displacement and the mean displaeement of the six points of mitral annulus were significantly increased(P<0.01),Tds diff and Td-sd were decreased(P<0.05) one month after CRT. TMAD correlated with LVEF,LVESV and FAC (r =0.419, r = 0.529, r = 0.567, P <0.05) one month after CRT. Conclusions TMAD is a new technology to evaluate the short-term effect of CRT.

Sepsis and septic shock have high morbidity and mortality. The key point is that huge amount of nitric oxide (NO) is releasing into circulation, inducing marked dilation of blood vessels, and continuously decreased of blood pressure. The central link is inflammation and oxidative stress. Methylene blue can effectively inhibit NO, remove excessive NO and block the signal transduction pathway named inducible nitric oxide synthase-NO-soluble guanylate cyclase-cyclic guanosine monophosphate (iNOS-NO-sGC-cGMP). Meanwhile, it also play a role in inhibiting oxidative stress and inflammation. This paper reviews the mechanism of methylene blue in the treatment of sepsis and septic shock, as well as its treatment time window, optimal dose, administration mode and potential adverse reactions, to provide new ideas for clinical treatment.

Objective:To determine whether human papillomavirus(HPV L1)C-terminal conserved sequence antibodies with cross-reactive major capsid proteins of different types of HPV L1 have the ability to degrade HPV6 infection. Methods:Condyloma specimens were collected,HPV6 infection cases were identified from the collected samples, and virus was extracted.Polypeptide anti-sera were diluted in different proportions, and then co-cultured and neutralized with the resulting virus, then removed to contact mono-layer-cultured human immortalized keratinocytes and tested by HPV6 disease using PCR.Content of HPV6 DNA in human immortalized keratinocytes was exposed, and the presence of HPV6 L1 protein in this cells was tested by ELISA.Results:Human immortalized ke-ratinocytes infected with HPV6 virus neutralization at different dilution concentrations, the PCR products of their DNA extracts were electrophoresis and showed positive bands of HPV6 specificity zone at 280 bp of the gel,and the intensity of positive bands gradually decreased with increasing antiserum concentration.Protein extracted from human immortalized keratinocytes exposed to anti-serum neutralizing virus was tested by ELISA,and the amount of HPV L1 protein showed the same gradient trend as the above PCR test results, and the difference were statistically significant. Conclusion:It is preliminarily proved that HPV6 L1 conserved sequence polypeptide antisera can partially degrade the infection ability of the virus, and it has the value of studying more HPV neutralization types.

Objective To evaluate the efficacy of auricular-plaster therapy indiabetes patients with insomnia, and provide suggestions for treatment. Methods The relevant randomized controlled trials and quasi-randomized controlled trials about using auricular-plaster therapy in diabetes patients with insomnia were searched in Cochrane Library, JBI, EMbase, Chinese Biological Medical Database ( CBM ) , Wanfang Data, VIP, China National Knowledge Internet(CNKI) and Pub Med. Information was selected. Retrieval time was limited from the foundation of those database to January 1st 2016. We use the Review manager 5. 3 analysis software to conduct Meta-analysis.Results 10 primary studies were included, which involved in 1 315 patients. Meta-analysis was processed based on different intervention methods and various outcome indicators. ①Different intervention methods:the effective rate of the auricular-plaster therapy group were higher than the control group, with significant differences [ OR= 7. 56, 95%CI ( 3. 92, 14. 58 ) , P<0. 000 01 ]; The effective rate of the comprehensive group ( including auricular-plaster therapy) were higher than the control group, with significant differences [OR=3.45, 95%CI(2.60,4.57), P=0.009];②Different results of outcome indicators, comprehensive interventionhas significant improvementonthe sleep quality of patients [MD=-1.96,95%CI(-2.33,-1.59),P<0.000 01];Moreover, comprehensive intervention could control postprandial blood glucose within twohours, with significant differences [MD=-1.86,95%CI(-1.94,-1.78),P<0.000 01].Conclusions Theauricular-plaster therapy can improve the sleep quality and sleep effect of diabetes patients with insomnia. However, because of the large literature difference, the results of the meta-analysis should be critical treated.

Objective:To investigate the gap between the health education and the best evidence among nurses and patients with postdialysis fatigue (PDF) , find out the problems and analyze the reasons, and provide a reference and basis for future clinical nursing decisions among the dialysis room nurses. Methods:By searching the evidence-based guideline websites and integrating the best evidence comprehensively, a health knowledge questionnaire for dialysis room nurses and patients with PDF was made. In December 2018, a total of 7 nurses and 56 patients in the dialysis room of Dongzhimen Hospital, Beijing University of Chinese Medicine were selected by purpose sampling method, and they were investigated by the health knowledge questionnaire for dialysis room nurses and patients. Results:The score of the PDF knowledge questionnaire for the 7 nurses was (56.43±4.37) , with a highest score of 64 and a lowest score of 51, and the standard-reaching rate was 28.57% (2/7) . The score of the PDF knowledge questionnaire for the 56 patients was (52.41±9.54) , with a highest score of 70 and a lowest score of 20, and the standard-reaching rate was 28.57% (16/56) . Patients and nurses received verbal health education and small lectures in the department, and there was no unified education content. Conclusions:There is no unified and scientific education content nor standardized education model. Evidence-based health education should be carried out for nurses to improve the awareness of nurses and patients in this regard, thereby reducing patients' fatigue.
