1.Pharmacokinetics of magnesium isoglycyrrhizinate after a single and multiple intravenous dose in healthy volunteers

AIM: To study the pharmacokinetics of magnesium isoglycyrrhizinate (MgIG),which was diluted by 5%glucose injection in a total volume of 250 mL, after a single and multiple intravenous dose in 10 healthy volunteers. METHODS: MgIG 100 mg once daily for 9 d in the multiple-dose regiment. Plasma MgIG concentrations are measured using high performance liquid chromatography (HPLC). Waters HPLC instrument was used with the Hypersil ODS2 C18 (5 μm, 300 mm×4.6 mm) column. The mobile phase was composed of 0.23 mol·L-1 phosphate buffer (pH=7.4):acetonitrile (79∶21).Flow rate was 1.0 mL·min-1 and column temperature was maintained at 40°C. The UV detector was set at 250 nm. The concentration time curves of MgIG were fitted to a two-compartment open model. RESULTS: The pharmacokinetic parameters obtained from the single-dose study were as follows: cmax=(29±s 4) mg·L-1;t(1)/(2)α=(1.72±0.27) h;t(1)/(2)α=(23±3) h and AUC0-72=(448±75) mg·h·L-1. The steady-state pharmacokinetic parameters were: cssmin=(13±3) mg·L-1;cssmax=(42±6) mg·L-1;cav=(21±4) mg·L-1;t(1)/(2)α=(1.6±0.4) h;t(1)/(2)β=(24±4) h and AUCss0-24=(513±108) mg·h·L-1.CONCLUSION:The distribution and elimination rate of MgIG were not changed after multiple intravenous administration.
2.Application of continuous subglottic irrigation and aspiration in patients with artificial airway

Objective To explore the effect of continuous subglottic irrigation and aspiration in patients with artificial airway. Methods One hundred and twenty-eight patients with mechanical ventilation were randomly divided into observation group and control group with 64 cases in each group.The observation group was treated by continuous subglottic irrigation and aspiration,and the control group received traditional nursing.The two groups were observed and compared in terms of incidence of aspiration and ventilator-associated pneumonia.Result The incidences of ventilator associated pneumonia(VAP)and aspiration in the observation group were significantly lower than that in the control group(P<0.05).Conclusion Continuous subglottic irrigation and aspiration can effectively prevent aspiration and VAP among patients with artificial airway.
3.The Mechanism of Formation and Transformation of Deficient Kidney and Deficient Spleen

The results of this study showed that no deficiency of the spleen or kidney were seen in the normal control group animal. About 1/3 of the anemic -rat control group revealed, earlier or later, spleen deficiency, without kidney deficiency. For the group of rats with iron deficiency, deficiency of the kidney appeared after 17 days, with many rats suffered from spleen deficiency, in which 43 % of the animals changed from deficiency of the spleen to deficiency of both spleen -kidney or kidney deficiency alone. The difference between the iron -deficiency group and animic control group of spleen deficiency was that the serum iron was markedly lower in the former group, whereas the serum iron in the deficiency of both the kidney and spleen - kidney rats was even lower than the spleen deficiency group. When deficiency of the kidney was induced by iron deficiency, the abnormality in the membranous protein of RBC is even more serious. Conclusion: Iron might be one of the crucial biochemical basis of the relations between the kidney and spleen.

6.PhaseⅠClinical Trial of Xuesaitong Saline Injections in Healthy Volunteers

Objective To assess the safety and tolerance of single-dose and continuous intravenous drip of Xuesaitong saline injection in Chinese volunteers,thus to establish the effective clinical dosage.Methods Thirty-one healthy volunteers were randomized into 4 single-dose groups(100,200,400 and 600 mg respectively)and a multiple-dose group(400 mg,qd ? 14 d).Subjective symptoms,objective signs,vital signs including blood pressure,heart rate,respiration were observed,and routine blood tests,routine urine tests,hepatic function,renal function,clotting function,electrocardiogram were monitored after medication.Results In the single-dose groups,there were no abnormal findings in the vital signs and the lab examinations 0h,1h,8h,24h after medication.Dizziness without association with Xuesaitong occurring in one volunteers of the 200 mg group.It could subside after taking a rest.In the multiple-dose group,no significant changes were found in the vital signs and the lab examinations 4 d,8 d,12 d,15 d after the first dosing.Rash occurred in 2 volunteers,being considered potentially related to the drug and disappearing in one week.Conclusions Xuesaitong is safe at a dose of 100 ~ 600 mg in a single administration or at a dose of 400 mg once day for 14 days for male or female Chinese volunteers,400 mg being suggested for the clinical use.However,Further studies are needed to evaluate the safety of xuesaitong in the target population and allergic reaction should be observed.
7.Application of Color Doppler Flow Imaging in Portal Vein Chemotherapy by Ultrasound Guided Percutaneous Transhepatic Portal Injection

To evaluate the role of color Doppler flow imaging in the portal vein chemotherapy by ultrasound guided percutaneous transhepatic portal injection, the blood supply of liver tumor and portal hemodynamics were studied in 15 patients with primary hepatic carcinoma and one patient with secondary hepatic carcinoma. The continuous wave in the lesion and forward continuous wave in the portal vein seen by pulsed Doppler in nine cases, were indications for this therapeutic method. The distribution of the drugs in the liver could be observed by color Doppler flow imaging when injection was being done. The therapeutic efficacy of portal vein chemotherapy in liver tumor may be evaluated by color Doppler folw imaging and puncture biopsy.
8.Treatment of open tibial fractures of type ⅢB with unreamed locking intramedullary nails and skin flap grafting

Objective To investigate the methods for emergency treatment of open tibial fractures of type ⅢB. Methods From February 1999 to February 2002, 16 cases of open tibial fractures of type ⅢB (age range: 19 47 years; average age: 27 years; the skin defect area: 4.5cm ?3cm-14cm ?6cm) were treated with administration of antibiotics, debridement, stabilization by unreamed locking intramedullary nails, and finally transfer or transplantation of cutaneous or musculo cutaneous flaps. Autografting from the iliac crest was done for 4 cases with bone defects. Results The follow ups averaged 9 months (ranging from 6 months to 14 months). The survival rate of flaps was 100%and all were free of deep infection. All fractures healed within 6 months in 14 patients and within 10 months in 2 patients. According to the Johner Wruchs method, the functions of ankle and knee were excellent in 14 cases, and good in 2 cases. Conclusion Emergency management of open tibial fractures of type ⅢB with unreamed locking intramedullary nails and skin flap grafting results in good outcome.

Objective To investigate the clinical presentation, hormonal profile and metabolic abnormalities in subgroups of women with PCOS and explore a reasonable classification for PCOS. Methods A cross-sectional study of 192 women with PCOS (14-38 years of age) was performed.
