1.The clinical effect of SMA Concentrator and Tension Band Fixation for the Treatment of Patellar Comminuted Facture

Objective To compare the clinical effect between SMA concentrator and tension band internal fixation for the treatment of patellar comminuted facture. Methods Retrospective analysis of 132 cases of patellar fracture surgery patients,72 cases of them with internal fixation by SMA concentrator,the other 60cases with tension band internal fixation. Results All patients were followed up for 6 to 16 months. The recovery rate of sma concertrator was 95.8% and that of tension hard internal fixation was 88. 3%. Conclusion Using SMA concentrator to treat patellar comminuted fracture was a better treatment.
2.Efficacy and Health-Related Quality of Life for small cell lung cancer patients after prophylactic cranial irradiation

Objective To analyze the survival,brain metastases and health-related quality of life (HRQOL) for small cell lung cancer patients after prophylactic cranial irradiation(PCI).Methods From Aug 2007 to Apr 2011,42 small cell lung cancer patients were eligible for analysis.Overall survival rate was estimated by the Kaplan-Meier method.The HRQOL scores before and after PCI were compared by the nonparameter wilcoxon test.Results The median survival time from the start of PCI was 23 months(95% CI,15.05-30.95),progression free survival time was 17 months (95% CI,14.33-19.67),1-,2-and 3-year survival rates were 85.5%,45.8% and 36.6%,respectively.The median time from PCI to brain metastases was 15 months(95% CI,12.44-17.56),1-and 2-year brain metastases rates were 77.6% and 54.1%.PCI had a little influence on HRQOL scales except for cognitive function.Conclusions For patients with limited stage small cell lung cancer who achieve complete or nearly complete remission after initial treatment as well as patients with extensive stage who respond to initial chemotherapy,PCI is effective in decreasing the rate of brain metastasis and improving survival,while the adverse effects is acceptable.There is no significant impact on HRQOL scales during short term.

[Objective]To construct arecombinant eukaryotic expression plasmid carrying human BMP-7 gene which was transfected into bone marrow stromal cells(BMSCs) in vitro and expressed.[Method]Clone human BMP-7 cDNA from a Chinese woman placenta was recombinanted with plasmid pcDNA3.1,to construct eukaryotic expression carrier of recombinaut pcDNA3.1-hBMP-7.The BMSCs were isolated from rabbit bone marrow and cultured in vitro.They were divided into three groups: pcDNA3.1-hBMP-7 transduced group;pcDNA3.1 transduced group;untransduced group.The expression of hBMP-7 was detected by RT-PCR,Immuneohistochemistry.ALP,Collogen,Osteocalcin production were detected.[Result]The pcDNA3.1-hBMP-7 transduced BMSCs expressed hBMP-7 at both mRNA and protein levels.ALP activity was detected in pcDNA3.1-hBMP-7 transduced cells from day 2 to day 10,peaking on day 8.Collogen,osteocalcin production were elevated significantly too(P
4.Effect of noxious stimulation factor on γ-aminobutyric acid distribution in dog spinal cord during propofol anesthesia

Objective To evaluate the effect of the noxious stimulation factor on γ-aminobutyric acid (GABA) distribution in dog spinal cord during propofol anesthesia.Methods Sixteen healthy mongrel dogs of both sexes,aged 12-18 months,weighing 10-12 kg,were randomly divided into 2 groups (n =8 each):noxious stimulation group (S group) and control group (C group).Anesthesia was induced with propofol 7 mg/kg.The animals were mechanically ventilated after tracheal intubation.Right femoral artery was cannulated for mean arterial pressure (MAP) and pulse rate monitoring.Anesthesia was maintained with propofol infusion at a constant rate of 70 mg· kg-1 · h-1.5 % formalin 300 μl was subcutaneously injected into the central region of tails in group S,while the equal volume of normal saline was injected instead of formalin in group C.MAP and pulse rate were recorded before injection of formalin or normal saline (T1) and after injection of formalin or normal saline (T2).The dogs were scarified by decapitation at 50 min of continuous propofol infusion and cervical 2-3 segments of the spinal cord were removed for determination of GABA level in different regions of the spinal cord (frontal horn,posterior horn,intermediate zone,frontal funiculus,posterior funiculus and lateral funiculus) by HPLC.Results MAP and pulse rate were significantly higher at T2 than at T1 in S group (P < 0.05).There were no significant differences in GABA level among the different regions of the spinal cord in C group (P > 0.05).Compared with C group,GABA level in the frontal horn and posterior horn was significantly increased (P < 0.05),and no significant change was found in the other regions of the spinal cord in S group (P > 0.05).Conclusion The noxious stimulation factor can induce an increase in GABA level in the frontal horn and posterior horn of dog spinal cord during propofol anesthesia.
5.Reliability of PainVision method for assessment of postoperative pain in patients undergoing gynaecological laparoscopy : a comparison with visual analog scale

Objective To evaluate the reliability of PainVision method for assessment of postoperative pain in patients undergoing gynaecological laparoscopy.Methods Twenty ASA physical status Ⅰ-Ⅱ patients,aged 30-45 yr,undergoing gynaecological laparoscopy under fentanyl-propofol-cisatracurium anesthesia,were studied.Pain intensity was simultaneously assessed using PainVision method and visual analog scale (VAS) at 12,24 and 48 h after surgery.PainVision was a new method for quantitative measurement of pain intensity using a painless electrical stimulation (PainVision PS-2100 device).Pain degree was calculated from two parameters,current perception threshold and pain compatible electrical current by using PainVision.The former parameter was defined by the lowest electrical current detected ; the latter parameter defined by the electrical current judged as being compatible with the intensity of ongoing pain.Results There was a significant positive correlation between pain degree calculated by PainVision method and VAS scores,and the correlation coefficient was 0.902 (P < 0.01).Conclusion PainVision method can be applied for assessment of postoperative pain in patients undergoing gynaecological laparoscopy.
6.Qualitative Analysis of Composition in Water Extracts from Hugu Capsule by HPLC

Objective To analyze and identify the chemical composition in water extracts from Hugu capsule and provide evidence for its pharmacological study and quality control. Methods A HPLC-DAD method was used. The separation was performed on Kromasil C18 column with acetonitrile and 1%glacial acetic acid as mobile phase by gradient elution at a flow rate of 0.8 mL/min. The chemical composition in water extracts from Hugu capsule were identified by comparison with the related chromatographic fingerprint. Results Thirteen characteristic peaks were found in the HPLC chromatographic fingerprint, and four peaks were identified. Conclusion The HPLC-DAD fingerprint expressed the general character of the chemical composition in water extracts from Hugu capsule. It can be used for qualitative analysis of water extracts from Hugu capsule, and improve the quality of Hugu capsule.

In order to overcome the shortcomings of traditional X-ray inspection taking passive protection mode, this paper combines the automatic control technology, puts forward a kind of active protection X-ray equipment. The device of automatic detection of patients receiving X-ray irradiation part, intelligent adjustment in patients and shooting device between automatic tracking radiation protection device height. The device has the advantages of automatic adjustment, anti-radiation device, reduce the height of non-irradiated area X-ray radiation and improve the work efficiency. Testing by the professional organization, the device can decrease more than 90% of X-ray dose for patients with non-irradiated area.

8.The comparative study of the performance of display devices in medical imaging system

ObjectiveTo analyze the effects of three different display devices on the interpretation of imaging data in medieal imaging system.MethodsThe luminance-meter L100 and the test patterns provided by the American Association of Physicists in Medicine(AAPM)were used to assess the performance of three display devices(the spherical,flat panel and liquid crystal display devices).The corresponding luminance response was compared with the reference criteria provided by AAPM Task Group 18(AAPM TG18).DR chest photography was taken on CDRAD2.0 contrast-detail phantom using the following experimental parameters:100 mA,80 kVp,and the exposure times for 6,10,12,and 16 ms.After each exposure,the surface dose of phantom was recorded and the image quality factor(IQF)was calculated.Statistical analysis of IQF was performed using ANOVA. ResultsThe maximum luminance values in the spherical,flat panel and liquid crystal display devices were 170,59 and 231 cd/m2 and the luminance ratio was 257, 99 and 350 respectively.There was a significant difference in the IQF bewteen the liquid crystal display device and other two display devices.Conclusion The liquid crystal display device has the better image quality.
9.Relationship and clinical significance between mutated BRAF with prophylactic cen-tral-neck nodal dissection in papillary thyroid carcinoma

Objective:To evaluate the molecular diagnosis marker of papillary thyroid carcinoma (PTC),the relationship between lymphatic metastasis of central neck compartment PTC,and the opera-tion indication of prophylactic central neck dissection.Methods:We conducted a retrospective study, including 275 PTC patients and detected their BRAF mutation rates during 201 2 and 201 4 and explored the risk factors of the central node lymphatic metastasis by Logistic regression model.Results:Of the 275 PTC patients,224 (81 .5%)were female and 51 (1 8.5%)were male.BRAF mutational rates were 53.8% (1 48 /275)and lymphatic metastasis 57.8% (1 59 /275).Multivariate analysis showed calcifica-tion (ORadjusted =1 .47,95%CI:1 .1 0 -1 .98,P =0.01 ),tumor diameter (ORadjusted =1 .48,95%CI:1 .04 -2.30,P =0.048)and age (ORadjusted =1 .48,95%CI:1 .04 -2.30,P =0.048)were associa-ted with lymphatic metastasis.In stratified analysis,BRAF mutation (ORadjusted =3.1 9,95%CI:1 .1 8 -9.43,P =0.023 )in clear boarder group and BRAF mutation (ORadjusted =4.84,95% CI:1 .68 -1 3.84,P =0.003)in calcification group were more likely to have lymphatic metastases.Conclusion:Central neck metastasis takes up a high ratio in papillary thyroid cancer patients,BRAF mutation in pa-pillary thyroid carcinoma is a characteristic molecular event.Furthermore,patients with calcification un-der ultrasound detection,lower age group and longer tumor diameter are more susceptible to suffer central neck metastasis.Especially for stratified analysis,non-calcified BRAF mutation or BRAF mutation with clear border under ultrasound detection are more susceptible to suffer central neck metastasis,and radical prophylactic central neck dissection should be carried on for these patients.
10.Preliminary investigation of stereotactic body radiation therapy for medically inoperable stage Ⅰ / Ⅱ non-small cell lung cancer

Objective To evaluate the therapeutic efficacy and treatment-related toxicity of stereotactic body radiation therapy(SBRT)in patients with medically inoperable stage Ⅰ/Ⅱ non-small cell lung cancer(NSCLC). Methods SBRT was applied to 30 patients, including clinically staged T1 ,T2(≤5cm)or T3(chest wall primary tumors only), N0, M0 ,biopsy-confirmed NSCLC. All patients were precluded from lobotomy because of physical condition or comorbidity. No patients developed tumors of any T-stage in the proximal zone. SBRT was performed with the total dose of 50 Gy to 70 Gy in 10 - 11 fractions during 12 - 15 days. prescription line was set onthe edge of the PTV. Results The follow-up rate was 100%. The number of patients who completed the 1-, and 2-year follow-up were 15, and 10, respectively. All 30 patients completed therapy as planned. The complete response(CR), partial response(PR)and stable disease(SD)rates were 37%, 53% and 3%, respectively. With a median follow-up of 16 months(range,4-36 months), Kaplan-Meier local control at 2 years was 94%. The 2-year overall survival was 84% and the 2-year cancer specific survival was 90%. Seven patients(23%)developed Grade 2 pneumonitis, no grade > 2 acute or late lung toxicity was observed. No one developed chest wall pain. Conclusions It is feasible to deliver 50 Gy to 70 Gy of SBRT in 10 - 11 fractions for medically inoperable patients with stage Ⅰ / Ⅱ NSCLC. It was associated with low incidence of toxicities and provided sustained local tumor control.The preliminary investigation indicated the cancer specific survival probability of SBRT was high. It is necessary to perform similar investigation in a larger number of patients with long-term follow-up.
