Objective: To investigate the frequency , distribution and features of lymph node metastasis in lung cancer, and to provide evidence for lymph node dissection. Methods: 348 patients with lung cancer were retrospectively studied, all patients received R_3 surgery plus systemic lymph node dissection according to the mapping system developed by Naruke. Results: Total 3 689 groups of lymph nodes were dissected . The metastatic rates of N_1 and N_2 were 23.4% and 16.5%, respectively. N_1 or N_2 metastasis was not found in Tis tumor. Objective To analyze the clinical characteristic,treatment and prognosis of traumatic macular holes resulted from ocular contusion.Methods The clinical data of 47 cases with traumatic macular hole was retrospectively reviewed.The general condition of the patients was summarized,optical coherence tomography and multifocal electroretinogram (mfERG) were used to evaluate anatomic and functional outcomes.The patients were divided into observation group and surgery group by the treatment they received,and the prognosis was evaluated.Results Traumatic macular hole occurs mainly in male.In the observation group,the mean diameter of macular hole was(490.0±86.9) μm.During the 12 month follow up,the holes in 7 cases (33.3％) were closed spontaneously,Vision and diameters of 14 cases (57.1％) maintained stable for a long time,the vision of 1 case (3.3 ％) declined mildly and the diameter of 1 case (3.3％) enlarged slightly.Visual acuity was improved significantly at last follow up (Z=-2.40,P＜ 0.05).The amplitudes of N1 wave of mfERG increased both in central fovea and macular area(t=13.30,5.06;P＜0.05).These data suggests that the macular function was recovered well.In the surgery group,the mean diameter of macular hole was (643.3 ± 125.0) μm and statistically larger than that of the observation group (t=-4.76,P＜0.05).At the last follow-up,visual acuity were not improved significantly (Z=-1.79,P＞0.05).The amplitudes of N1 wave in 6 cases (23.1 ％) improved merely and the difference was not statistically significant (t =1.98,P ＞ 0.05).These data suggests that the macular function was recovered slightly only in a few patients.Conclusions A part of the patients with smaller diameters of macular holes may close spontaneously,and they may get better visual acuity.Vitrectomy may help to close the macular holes in some severe cases,but the improvement of functional outcomes is not significant.

Objective:To retrospectively analyze the survival outcomes of the surgical management of primary small cell carcino-ma of the esophagus. Methods:The medical records were reviewed for patients diagnosed with esophageal carcinoma and underwent esophagectomy from January 2000 to December 2009 at the Department of Thoracic Surgery of the Henan Cancer Hospital. We fo-cused on the clinical data of patients with small cell carcinoma of the esophagus. The Kaplan-Meier approach with log-rank test was used for survival analysis. Results:A total of 5,062 patients underwent esophagectomy with curative intent at the Department of Thorac-ic Surgery of the Henan Cancer Hospital;among which, 57 (1.1%) were diagnosed with small cell carcinoma of esophagus. The most common surgical approach was trans-left thoracic incision esophagectomy. Cervical esophagogastrostomy was performed for all pa-tients. The most common chemotherapy regimen was EP. The overall 5-year survival rate was 12.5%, and the median survival time was 45 months. Among the various stages, the 5-year survival rate and survival time were 25% and 50 months for Stage I, 5.9% and 43 months for Stage II, and 4.3%and 43 months for StageⅢ. Subgroup analysis showed that cases treated with surgery alone had poorer overall median survival time compared with those cases that underwent surgery plus chemotherapy (23.2 months vs. 60.7 months, re-spectively;P<0.01). Even for Stage I patients, thesurgery plus chemotherapysubgroup was associated with a significantly longer me-dian survival time than the surgery alone subgroup (81.9 months vs. 22.3 months, P<0.01). Conclusion:For patients with primary small cell carcinoma of the esophagus, surgery alone cannot provide the optimal prognosis. Surgery combined with systemic chemother-apy can improve the survival time.

Objective To investigate the efficacy of the hospital intensive treatment by auto-CPAP on elderly patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and to analyse the possible factors related to the effectiveness.Methods Subjects were selected from elderly OSAHS patients over 60 years old.The eligible subjects were divided into intensive treatment group and non-intensive treatment group (non-ITG).Patients in intensive treatment group were intensively treated using auto-CPAP for 3 consecutive days,assisted with targeted health education and guidance,and making proper adjustments to parameters of the auto-CPAP according to treatment outcomes at the previous day.But those in non-intensive treatment group were just treated with auto-CPAP for only 1 day,supplemented by regular health education and guidance.The apnea-hypopnea index (AHI),minimum oxygen saturation (SaO2min),mean oxygen saturation (SaO2mean) and the time spent at SaO2 lower than 90％ (tSaO2＜90％),the occurrence of clinical symptoms and adverse effect,Epworth sleepiness scale(ESS) were compared between both groups.The correlation analyses were also conducted.Results ESS (3.58±3.76 vs.6.84 ± 3.22),AHI [(7.85±6.53) time/h vs.(10.42±7.27) time/h] and tSaO2＜90％[(5.65±15.43) min vs.(15.26±33.14) min] were lower(t=6.902,2.760,2.765,allP＜0.05),while SaO2mean [(96.57±1.53)％ vs.(94.63±1.38)％] and SaO2min [(88.24±4.43)％ vs.(83.28±5.06)％]were higher(t=-9.870,-7.740,both P＜0.05)in intensive treatment group than in non intensive treatment group.The occurrences of clinical symptoms and adverse effect (except for skin allergy) were decreased in intensive treatment group versus non-ITG (all P＜0.05).The AHI after intensive treatment was decreased in females versus males [(7.85±5.19) times /h vs.(11.27±7.78) times/h,t=2.133,P＜0.05].BMI,age,gender and the state of OSAHS were correlated to AHI,SaO2 mean,SaO2 min and tSao2＜90％ after intensive treatment (all P＜0.05).Conclusions The hospital intensive treatment can improve the clinical results and decrease the occurrences of clinical symptoms and adverse effect for elderly OSAHS patients,and is expected to improve therapy compliance,and its curative efficacy is correlated to BMI,age,gender and state of OSAHS before therapy.

Objective To investigate the diagnosis and treatment of Hashimoto's disease. Method The clinical data of 51cases of Hashimoto's disease admitted to our department from 1986 to 2001 were collected and analyzed. Result The preoperative misdiagnosis rate is 78.4%. 37 of 51 cases undergone surgical treatment and the results were satisfied. Follow up data, which were collected from 30 patients, showed that 6 patients(16.2%) developed hypothyroidism . Conclusion The examination of antithyroglobulin (TGA) and antimicrosomal (TMA) and the frozen section examination during operation are helpful to the diagnosis. Appropriate operation is a safe and effective treatment for patients with Hashimoto's disease who have received effectiveless thyroxine treatment.

Objective To explore the effect of surgical comprehensive treatment(SCT) in treating primary liver cancer(PLC).Methods The clinical data of 108 patients with PLC undergoing SCT in XiangYa Hospital from January 1999 to September 2002 were reviewed retrospectively. The SCT including surgical resection ,radiofrequency ablation(RFA),transcatheter hepatic chemo-embolization(TACE),percutaneous ethanol injection(PEI),TACE via drug delivery system(DDS),cryosurgery,biological treatment,etc.Results The postoperative mortality was 0.9%, and the postoperative complications was 18.7%,intrahepatic recurrent rate and remote metastasis within 1 year after SCT were 56.1%,17.9%.The 1-,2-,3-year survival rate after SCT were 79.46%,61.83%,36.61%,respectively.Conclusions Liver resection combined with other treatment methods can cut down the rate of intrahepatic recurrence and remote metastasis, and can improve the tumor-free survival rate and long-term survival.

〔Abstract〕 Based on elaborating the application of WeChat public platform in various industries, the paper overviews the function and characteristics of WeChat public platform, taking Shanxi Provincial Peoplel's Hospital as an example, it introduces the application of We-Chat public platform in outpatient and inpatient departments, points out that WeChat service could be carried out in hospital in the futrue.

Objective To study the variation of blood concentration of tetramine in human body with acute tetramine intoxication treated with three different protocols and the levels in human body after discharge from the hos-pital. Methods The blood concentration of tetramine was determined by gas chromatography-mass spectrometry (GC-MS). All 101 patients of acute tetramine intoxication were divided into 3 groups (routine comprehensive treat-ment group,hemoperfusion group,blood transfusion group) according to their blood concentration of tetramine and clinical symptoms. The patients were followed up to monitor the tetramine levels in a year. Results The level of tet-famine in blood was decreased from 33.0(1.7～115.0)μg/L to 18.0(0.3～47.6)μg/L in routine comprehensive treatment group. The total decrement was 45.5 %. The level was decreased from 108.0 (54.0～290.0)μg/L to 26.0 μg/L in hemoperfusion group. The total decrement was 75.9%. The decrement was 20.0%～45.0% after each he-moperfusion. The level was slightly high after 24 h of hemoperfusion. The total decrement in blood transfusion group was 33.5%～60.0%. The level was <0.3μg/L in all 25 out-patients 1 year after their intoxication. Conclusion Routine comprehensive treatment,hemoperfusion,blood transfusion are effective in the treatment of acute tetramine intoxication. The degradation of tetramine in human body is slow.

Objective The imaging quality and irradiation dose were compared between scatter filtering template performance and conventional grid in digital radiography.MethodsImages of contrast-detail phantom and ROC statistic phantom corrected with scatter filtering template method and grid were obtained,respectively.The image quality factors(IQF)and sihnal detection probability values(Pdet)and imaging dose were analyzed by ANOVA,SNK and t test statistics for the two methods of scatter removing.ResultsThe difference of IQF and signal detection probability of images corrected by scatter filtering template technique and conventional snd Was statistically significant.ConclusionsIn digital radiography,STFF can effectively filter scattering,improve image quality without increasing the exposure.With the same image quality.it can reduce the entrance surface dose by about 30% than filter grid.In order to effectively filter scattering,improve the quality of X-ray image,and reduce the patient dose.STFF is an alternative method.

Objective:To explore the relationship between social support and negative emotion in parents of children with congenital heart disease before the operation.
Methods:Social support rating scale (SSRS), self-rating anxiety scale (SAS), and Center for epidemiological survey-depression scale (CES-D) were used to assess the association between social support and anxiety and depression in 226 parents.
Results:The mean SSRS score was signiifcantly lower than the normal. There was signiifcant difference in social support of parents in monthly family earning and knowledge about surgery. Parents with monthly family income≤1000 yuan had lower scores than the other groups. Parents who knew nothing about the surgery approach had lower scores. Social support was significantly correlated to negative emotion (anxiety and depression). Social support could predict anxiety and depression.
Conclusion:Social support is widely and significantly correlated to negative emotion in parents of children with congenital heart disease before the surgery.
