2.Piezosurgery-assisted sliding genioplasty

Objective To evaluate the value of the clinical application of piezosurgery-assisted sliding genioplasty.Methods A total of 9 patients (7 women,2 man,aged 19-40 years) with microgenia were performed operation by intraoral V incision with subperiosteal exposure of the mandibular mentum area.Note relaxtion and preservation of the mental nerves.A horizontal osteotomy of cortical bone on anterior mandible was underwent by the conventional electric bone saw and internal plate bone was sectioned with the piezoelectric device.The segment was divided with preservation of the lingual soft tissue attachment and was advanced in the mandibular mentum area.These was fixed in the reposition by titanium plate and screw for preventing displacement.Results The piezoelectric device produced selective sectioning of the mineralized bone structures,bone was cut precisely without damage to vital anatomic structures and dental pulp,and was much easier to control and less aggressive,creating only minimal damage to soft tissues.The operative effects were satisfactory during 3- 6 months of follow-up.Stable chin contours were created and became nature.Iatrogenic problems with bleeding,in fection,mental nerve injury had been avoided.Conclusions Piezosurgery is a safe and precise technique to reduce the complication rate and to improve the treatment outcomes of genioplasties.
3.Correction of bent cartilaginous vault

Objective To seek a method to correct the bent cartilaginous vault.Methods This study included 18 patients (12 women,6 men,aged 18-45 years) suffering from the bent cartilaginous vault with the various degrees of nasal obstruction from January 2005 to December 2009.A open surgical approach was adopted to allow correction of the dorsal and caudal deviations of the nasal septum without weakening its structural support to the dorsum or nasal tip.The approach depended on full mobilization of deviated cartilage,removal of the deviated part of the quadrangular cartilage,followed by straightening of a caudal septal extension graft and its fixation in the corrected position.Rhinoplasty was performed to correct bent cartilaginous vault.Results A satisfactory result was gained in all clinical cases,except a septum still to remain light tilt (not affect the breathing function).Follwing-up for 3-24 months,all cases dorsum of nose was straighten and nasal septum situated mediately.All patients indicated cosmetic satisfaction of nasal dorsum and tip and improvement in nasal obstruction.No nasal deformity and septum tresis was found in all patients.Conclusions ()pen rhinoplasty is a helpful technique in the treatment of the bent cartilaginous vault.Septal surgery is necessary in the patients with cartilage framework,not only to improve breathing but also to achieve a straight,symmetrical and external nose as well.
4.Lengthening the short nose with bilateral septal spreader graft and columellar strut

Objective To investigate a safe and effective method of lengthening the short nose.Methods Thirty-nine cases of short nose were corrected by the bilateral septal spreader graft and columellar strut using autologous cartilage,combined with expanded polytetrafluoroethylene (e-PTFE)to reconstruct the supporting structures to improve the forward and upward strength of the nasal tip in order to increase the nasal length and to correct the over rotation of nasal tip.The shield and cap grafts were also used for the patients whose nasal tip were too low with vertical dome division technique.Results All the patients showed preserved nasal length after surgery with well-proportioned nasal features and the nasal lip angles were normal.There was no evidence of postoperative nasal shortening.No operative or postoperative complications were observed by follow-up for 6 months to 1 year,except for 2 cases with graft cartilage being apparent on nasal tip.All the patients were satisfied with the surgical results achieved.Conclusions It is necessary to provide powerful forward and upward strength to lengthen nose and correct the over rotation of nasal tip effectively and safely.The application of bilateral septal spreader graft and columellar strut using autologous cartilage and combined with e-PTFE represents a valuable tool for the short nose and proper cartilage grafts of nasal tip can improve the height of the nasal tip and further correct the over rotation of the nasal tip.
5.Clinical application and Fuzzy evaluation of autologous platelet-rich plasma mixed with fat graft in facial tissue depression

Objective To evaluate the clinical effect of the facial soft tissue defect treated with platelet-rich plasma (PRP) mixed with purified fat.Methods 35 patients with facial soft-tissue defects were treated by PRP mixed with centrifuged fat tissue.Using Fuzzy judgment method the clinical effectiveness of therapeutic outcomes were evaluated on fullness,injection of frequency and satisfaction during 3 to 6 months of follow-up.Results The therapeutic outcomes showed that the reconstructed face became a relative symmetrical and natural appearance with not evidently fat resorption and stable after operation.Both patients and surgeons were satisfied with the results.The score of effectiveness was highest in Fuzzy judgment.Conclusions The efficacy of PRP is significant in enhancing fat graft,reducing fat necrosis and resorption.The fat tissue mixed with PRP is a reliable reconstruction option for the facial soft tissue defect.
6.The role of colonoscopy in diagnosis of gastrointestinal graft-versus-host disease and cytomegalovirus colitis after allergenic hematopoietic stem cell transplantation

0. 1). All the GI-GVHD and CMV colitis patients presented with a variety of colonic mucosal lesions. Besides the tortoiseshell-pattern mucosa and deep ulcer were characteristic lesions in GI-GVHD and CMV colitis respectively, the remaining mucosa lesions including edema, reddish patchy, erythma, erosion and superficial ulcer could not differentiate GI-GVHD from CMV colitis. Three GI-GVHD cases presented with pseud-omembrane, and 1 CMV colitis patient with herpes-like mucosa. Oozing bleeding of terminal-ileum mucosa and ileocecal valve inflammation could easily be found in GC patients. 63. 8% tissue samples were taken biopsies from rectosigmoid in GI-GVHD, and 70. 0% and 43. 8% in CMV colitis and GC patients respectively. Conclusion The positivity of peripheral blood CMV-DNA can not distinguish GI-GVHD from CMV colitis in allo-HSCT patients. GI-GVHD and CMV colitis manifest with a variety of lesions in colonoscopy, the tor- toiseshell-pattern mucosa in GI-GVHD and deep ulcer in CMV colitis are characteristic lesions. The patients of GI-GVHD complicated with CMV colitis readily present oozing bleeding of terminal-ileum mucosa and ileo-cecal valve inflammation. Colonoscopy and tissue biopsy of left-colon can diagnose the most of GI-GVHD and CMV colitis, but it's better to undertake pan-colon as well as terminal ileum examination for more accurate diagnosis.
7.The Investigation of the Body Donation Will and Influence Factors of People in Yunnan Province

Objective:To investigate the mainstream view and statusof people on body donation in Yunnan province, and provide references. Methods:Random sampling questionnaire survey was used, the obtained data was statistically analyzed. Results:Only 48. 0% of the people understand the body donation, 20. 6% of people know body donation ways and procedures, 27. 8% of people have heard of body receiving station, 11. 4% of people know the body receiving station in yunnan province, 17. 2% of the people to understand body donation of relevant laws and regulations. 47. 4% of people that are willing to donate his body. Conclusion:Make perfect laws and reg-ulations, respect for the body, giving donors and their families allowance, body donation monument, to carry out education and publicity of the body donation form to the whole society support and participate in the good atmos-phere of body donation has great benefits.
8.The Investigation on the Will and Its Influence Factors of the Yi, Dai and Han People in Yunnan Province about Body Donation

Objective To understand the mainstream view of ethnic minorities and Han nationality in Yunnan province on Body donation, understand the present status and possible issues about body donation in Yunnan province, so as to provide reference for Yunnan province on body donation. Methods We conducted questionnaire survey by random sampling. Results Yi,Dai,Han on the body donation knowledge and Donation rate was low,and the Yi, Dai was lower than Han.promoting development of medicineandthat is the continuation of lifebecame the main factors affecting the willing to donate body,there was significant difference between the three ethnic groups ( <0.05) .After death should be laid to rest,family and public pressure,fear of being free to use,religious beliefbecome the main factors affecting the unwilling to donate body, three nations also had significant difference ( <0.05) .Conclusions Most people think that the whole society should pay close attention to body donation, that body donation is a very meaningful thing. Joint effort should be made by both the government and civil society for body donation programs in Yunnan Province.

10.Endoscopic presentation of gastrointestinal graft-versus-host disease after allogeneic hematopoietic stem cell transplantation

Objective To investigate colonoscopic presentation and explore biopsy style of lower gastrointestinal graft-versus-host disease (GI-GVHD) after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods The endoscopic findings including mucosa erythema,edema,erosion,ulcer,tortoiseshell-pattern and sloughing were observed in 36 patients with GI-GVHD and the rate of apoptotic cell yields in colon and end-ileum was calculated.Results Mucosa lesions were found in almost all of the patients both in colorectal and end-ileum (97.2% vs 94.1%,P =0.609).Mucosa erythema was more often seen in end-ileum (47.2% vs 79.4%,P =0.007) and tortoiseshell-pattern was mainly in colorectal mucosa (63.9% vs 5.9%,P =0.000).Mucosa edema,erosion and oozing bleeding were the same prevalence in large intestine and end-ileum (97.2% vs 94.1%,80.6% vs 79.4%,47.2% vs 47.1%,P > 0.05,respectively).Sloughing was found in 76.5% (26/34) GI-GVHD patients,and it was almost the same prevalence in large intestine and end-ileum (52.8% vs 47.1%,P >0.05).Almost all of the colorectal mucosa sloughing located in the tortoiseshell-pattern mucosa.Rates of apoptotic cell in rectal,colonic and end-ileal mucosa were 88.9%,91.3% and 75.9%,respectively,and the rates were 88.2% and 93.9% in ileum plus rectum and ileum plus colon respectively,showing that biopsy only in ileum was not sufficient for the pathologic diagnosis of GI-GVHD (93.9% vs 75.9%,P =0.070).Conclusion Endoscopic presentations of GI-GVHD after allo-HSCT are not the same between colorectal and end-ileal mucosa.Sloughing with GIGVHD feature is not rarely seen in lower GI.Tortoiseshell-pattern mucosa should also be pathognomonic feature of colorectal GVHD in endoscopy.Pathologic tissue should not only be biopsied in end-ileum,but also in colorectal mucosa in the same time.
