1.Ultrastructural Changes of Cochlea Nerve Following Gunshot Wound in the Maxillofacial Region

This experiment was done by using the Sweden trauma model. Ten dogs, divided into two experiment groups, were hit in the lelf maxillofacial region with a small steel sphere (1.03g, 1400m/s). The specimens from the cochlea nerves were examined under electron microscope immediately (the first group) and at 6 hour (the second group) after trauma, respectively. Another four uninjured dogs were used as a crontrol (the third group). Both experiment groups were found to have mitochondrion swell, crista irregularity and break in axons of the cochlea nerves after trauma. The mechanism of the hearing loss following maxillofacial wound by high velocity missile is discussed.

50.0%).ESBLs isolates reached up 63.5%.CONCLUSIONS E.coli has a high detection rate and serious drug resistance in urinary infections.Conventional ESBLs tests can help to appropriate use antibiotics and lower the occurrence of drug resistance.Clinical doctors are advised to restrict the usage of antibiotics in order to lower the occurrence of drug resistance strains,as well as to prevent the spread of ESBLs.
3.Application of allogeneic amniotic membrane for myringoplasty in the treatment of traumatic perforation of tympanic membrane

OBJECTIVE To investigate the clinical efficacy of using allogeneic amniotic membrane for myringoplasty in the treatment of traumatic perforation of tympanic membrane.METHODS 27 cases of traumatic perforation of tympanic membrane underwent myringoplasty with allogeneic amniotic membrane between July 2003 and February 2008 were analyzed retrospectively.The perforation healing rate and hearing improvement were studied.RESULTS All cases were followed-up for more than 6 months. The perforation healing rate was 96.3%and PTA gain was(12?3.7) dB.CONCLUSION The virtue of myringoplasty with allogeneic amniotic membrane is the plenty of service,convenient operation,little damage and good histocompatibility and effect.
4.Reliability and validity of the Chinese-version Hypertension Self-care Profile

Objective To test the reliability,validity and Chinese adaptation of Chinese-version Hypertension Self-care Profile (HBP SCP).Methods HBP SCP was translated into a Chinese version and had some Chinese adaptation,the reliability and validity of HBP SCP was tested in 377 patients with hypertension.Results Item-total correlations was 0.396~0.881 and the determination coefficient for each scale was 5.890~20.874.The Cronbachs'α was 0.950 for the total scale,the test-retest correlation was 0.918.The content validity index for the scale was 0.83~1.00,2 and 3 factors were extracted by principal components analysis,which contributed 58.934%,67.224% and 66.601% of the variance.Conclusions The Chinese-version of HBP SCP has good psychometric quality and can be used as a measurement tool for Chinese patients with hypertension.

6.Comparison of SPECT and CT in detecting skull base invasion in nasopharyngeal carcinoma

Objective To investigate the detecting ability of single photon emission computed tomography (SPECT) and CT in skull base invasion in nasopharyngeal carcinoma. Methods Sixty three patients with nasopharyngeal carcinoma were examined by whole body and skull base SPECT and CT of nasopharynx and skull base before radiotherapy. The results were double blind compared and evaluated. Results The overall positive rates of skull base invasion detected by SPECT and CT were 63.5% and 25.4%. In patients with headache, cranial nerve palsy and both, they were 87.9%,93.3%,92.3% and 42.4%,46.7%,46.2%. In patients with T 1+T 2 and T 3+T 4 lesions, they were 37.5%,90.3% and 0.0%,51.6%. 8.Effects of cytokines on the expression of CD40 and CD40 ligand in cultured human monocytes/macrophages

Objective: To investigate the effect of c ytokines (IFN-γ,TNF and IL-1) on the expression of CD40 and CD40 ligand (CD4 0L) in monocytes/macrophages. Methods: The mRNA expression of C D40 and CD40L was measured by RT-PCR and the CD40,CD40L expression on the mono cytes/macrophages were detected by flow cytometric analysis. Results: IFN-γ,TNF and IL-1 could not only significantly up-regulate the mRNA levels of CD40 and CD40L in cultured monocytes/macrophages, but also increase t he expression of CD40 and CD40L. Antioxidant VitE could reduce the expression o f CD40 and CD40L induced by IFN-γ,TNF and IL-1. Conclusion: IFN-γ,TNF and IL-1 can stimulate high expression of CD40 and CD40L . Antio xidant VitE can partially inhibit the expression of CD40 and CD40L induced by cy tokines in cultured monocytes/macrophages.
9.Significance of magnetic resonance diffusion tensor imaging for evaluation of upper motor neuron disease in patients with amyotrophic lateral sclerosis

BACKGROUND: Magnetic resonance diffusion tensor imaging (MRDTI) may non-wounded detect damage of fiber in white matter and becomes an effectively way to evaluate upper motor neuron(UMN) impairments.OBJECTIVE: To investigate the clinical significance of MRDTI on amyotrophic lateral selerosis(ALS).DESIGN: Case contrast observation.SETTING: Department of Neurology, Renmin Hospital of Wuhan University.PARTICIPANTS: Twenty ALS patients were selected from Department of Neurology, Renmin Hospital of Wuhan University from April to December 2005. There were 11 males and 9 females, and their ages ranged from 33 to 73 years with the mean age of (51±10) years. All subjects met the diagnostic criteria of ALS set by World Neurology League.Other 15 healthy subjects were collected as control group. There were 8 males and 7 females, and their ages ranged from 31 to 73 years with mean age of (50±11) years. All subjects provided the confirm consent.METHODS: Based on level of upper and lower motor neuron impairments, ALS patients were divided into UMN impairment group (n =16) and lower motor neuron group (n =4). Functional scores of ALS, illness developing velocity and pyramidal sign scores were performed, respectively. All subjects were scanned with DTI at axial view. Regions of interest [subcortical white matter of precentral gyrul and postcentral gyrul (Pre-CG/Post-CG), centrum semiovale and frontal white matter (CS/FWM), peripheral lateral cerebral ventricle, posterior limb of internal capsule (PIC), cerebral peduncle (CP), genu corpus callosum and splenium corpus callosum (GCC/SCC) and dorsal thalamus (DT)] were selected to measure fractional anisotropy (FA) and apparent diffusion coefficient(ADC).MAIN OUTCOME MEASURES: Correlations among FA, ADC, functional score of ALS, illness developing velocity and pyramidal sign scores.RESULTS: Twenty patients and 15 subjects in the control group were involved in the final analysis. ① FA was reduced and ADC increased in the posterior limb of the internal capsule in patients with UMN signs compared to healthy volunteers (t =3.452, 2.670; P < 0.01, 0.05). Nonparametric tests revealed that there was a trend toward reduced FA in the posterior limb of the internal capsule in B group compared to controls (U =11, P =0.057). ② In UMN impairment group, FA in the posterior limb of the internal capsule was positively correlated with the ALS rating scale (r =0.577, P <0.05) and negatively correlated with pyramidalsign scores (r = -0.789, P < 0.01 ),CONCLUSION : The impairment of pyramidal tracts can be noninvasively evaluated by diffusion tensor MR in vivo, thus providing useful information in diagnosing and further understanding MND.
10.Comparison between SPECT and MRI in detecting skull-base invasion in nasopharyngeal carcinoma

Objective To investigate the ability of single photon emission computed tomography (SPECT) and MRI in detecting skull-base invasion in nasopharyngeal carcinoma. Methods Sixty-one patients with nasopharyngeal carcinoma received whole body and skull-base tomography SPECT, and nasopharynx and skull-base MRI before radiotherapy. The results were double-blind compared and evaluated. Results The overall positive rates of skull-base invasion detected by SPECT and MRI were 51% and 46% (P=0.508). In paitents with headache, cranial nerve palsy or both, the rates were 83% and 86% (P=1.000) ,80% and 80% (P=1.000), 88% and 94% (P=1.000), respectively. In patients with T1+T2 and T3+T4lesions,the rates were 22% and 0(P=0.031) ,74% and 82% (P=0.250) ,repectively. In patients with N0+N1and N2+N3lesions,they were 50% and 48% (P=1.000) ,53% and 40% (P=0.500) ,respectively. The conformation rate between SPECT and MRI was 85%. Binary Logistic regression analysis showed that T stage was a risk factor for positive SPECT(χ2=4.23,P=0.040, OR=3.04). Headache tended to be a risk factor for both positive SPECT and positive MRI (χ2=3.13, P=0.077, OR=4.54;χ2=3.64,P=0.056,OR=12.00). Conclusions The detection sensitivity of SPECT in skull-base invasion in nasopharyngeal carcinoma is equivalent to that of MRI. The consistency between SPECT and MRI is good. Moreover, there is a good correlation between SPECT and symptoms, signs and stage. SPECT of skullbase tomography is necessary for patients with severe headache, negative CT and those who can not receive MRI. When SPECT result is positive,skull-base should be considered to be invaded and should be defined as gross tumor volume in radiotherapy planning.
