OBJECTIVE:To observe the therapeutic effects of Yiganzhuanyintang(YGZYT)Ⅰ,Ⅱ,prepared in this hospital,on HBV and liver function in patients with chronic hepatitis B.METHODS:100 patients with chronic hepatitis B were divided into two groups:treatment group,YGZYT Ⅰ,Ⅱ(dissolved in 150ml water,bid po)and control group,liver function protecting agents.The therapeutic course was 6 months.RESULTS:In treatment group,ALT was obviously decreased and negative turn-over rates of HBeAg and HBV-DNA were 44.7% and 42.1% respectively with significant differences from those in control group(P

Objective:To study the mechanical properties of a new titanium alloy with lower elastic modulus for prosthodontic use.Methods:Samples of a newly developed Ti-Nb-Zr-Sn alloy with low elastic modulus were prepared.The microstructure,microhardness profile of the specimen cross section were measured by microhardness test meter and metallurgical microscope.Results:The data of the tensile strength(MPa),yield strength(MPa),elongation(%),elastic modulus(GPa) and microhardness(Hv)of Ti-Nb-Zr-Sn alloy were 681.7,410,6,42.1 and 453 respectively.The microstructure was with fine crystal grains.Conclusion: The microstructure and microhardness profile of the Ti-Nb-Zr-Sn alloy may meet the requirement for dental applications.
3.Comparison of endoscopic and conventional great saphenous vein harvesting in coronary artery bypass grafting

Objective To observe the clinical effects of endoscopic great saphenous vein harvesting(ESVH) in coronary artery bypass grafting. Methods Endoscopic great saphenous vein harvesting was performed in 89 patients undergoing coronary artery bypass grafting(CABG) from April 2004 to May 2005.An incision 2 cm in length was made on the knee joint and the great saphenous vein was harvested using the VasoView-5 system.Clinical data of the 89 patients(Endoscopic Group) were compared with another 38 patients undergoing conventional great saphenous vein harvesting for CABG from April 2003 to March 2005(Conventional Group) in respect of complications of the leg, the recovery time to walking,and the patency rate at 6 postoperative months.Results The number of harvested vein grafts in the Endoscopic Group was 2~3(mean,2.6).Complications of the leg were significantly less in the Endoscopic Group(6/89) than in the Conventional Group(8/38)(?~2=4.197,P=0.040).The occurrence of pain and numbness of the leg was 7/89 in the Endoscopic Group and 36/38 in the Conventional Group(?~2=89.740,P=0.000).The occurrence of swelling of the leg was 9/89 in the Endoscopic Group and 30/38 in the Conventional Group(?~2=59.299,P=0.000).The recovery time to walking was significantly shorter in the Endoscopic Group(2.3?0.9 d) than in the Conventional Group(3.4?1.6 d)(t=-4.952,P=0.000).There was no significant difference in the patency rate at 6 postoperative months between the Endoscopic Group(96.0%,48/50) and the Conventional Group(95.3%,19/20)(?~2=0.000,P=1.000).Conclusions Use of endoscopic vein harvesting in coronary artery bypass grafting decreases the incidence of postoperative leg-wound infections,post-operative pain,and length of hospital stay.
4.A study of cytotoxicity of membrane material of cardiac assist pump

Objective To study the cytotoxicity of the membrane material of heart assist pump. Methods The membrane materials tested incladed polyurethane (group A) and medical polychloroethylene (group B),and a blank control group with none of the material. L-929 cells (the mouse fibroblasts) were cultured on either material in groups A and B,and 6 cell cultures were without any material. After 24,48 and 72 hours,the growth of cell in groups A,B and the control was observed,and the number was counted. Educts of two materials used in groups A and B was respectively added to culture medium,then the OD was measured every 24h and proliferation rate was determined by MTT test. Results L-929 cells grew very well in groups A and B with less death. The cell counts showed no difference between 2 groups at each time. Objective:
To analyze the effect of different stress levels noninvasive mask BiPAP ventilation in the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with type Ⅱ respiratory failure.
Methods:
From January 2015 to June 2017, 120 patients of AECOPD with typeⅡrespiratory failure in Jincheng People's Hospital were randomly divided into three groups by the random number table, with 40 cases in each group.The suction pressure of A group was set to 10-14cmH2O.The suction pressure of B group was set to 15-19cmH2O, while the suction pressure of C group was set to 20-25cmH2O.The clinical curative effect of three groups was compared.
Results:
The PaO2 level of three time points among the three groups were compared by repeated measurements of variance.The results showed that after treatment for 24h, the PaO2 levels of A group, B group and C group were (57.15±5.03)mmHg, (60.28±5.21)mmHg and (58.38±6.06)mmHg, respectively.After treatment for 72h, the PaO2 levels of A group, B group and C group were (63.02±6.08)mmHg, (80.16±5.28)mmHg and (70.07±5.23)mmHg, respectively.After treatment for 168h, the PaO2 levels of A group, B group and C group were (70.11±60.28)mmHg, (86.35±5.01)mmHg, (79.26±5.16)mmHg, respectively.The results showed that the difference in PaO2 of different groups was statistically significant (
6.Bedside Chest Radiography in the Intensive Care Unit

Objective To evaluate the applied value of bedside chest radiography in the intensive care unit ( ICU ) . Methods One hundred and ninety-three cases with 507 chest radiographs (CXRs) in the ICU were analysed retrospectively. The appearances of cardiopulmonary abnormalities were analysed.Results 97.4% of the CXRs were satisfied for diagnosis. 83.9% of the CXRs showed cardiopulmonary abnormalities, including atelectasis, pneumonia, pulmonary edema, ARDS, pneumothorax, mediastinal and subcataneous emphysema,pleural effusion,pericardial effusion and pulmonary thromboembolism, and 6.2% out of these positive cases was discovered accidentally. The chest radiograph has moderate accuracy (77.9%) in visualizing cardiopulmonary abnormalities at the initial CXR including atelectasis,pneumonia,pulmonary edema and pleural effusion. Misdiagnosis occurred mainly on the patients with a small quantity of pneumothorax and pulmonary thromboembolism. About one-third cases of cardiogenic edema and ARDS were indistinguishable.Conclusion Bedside chest radiographs are of significant value for the patients in the ICU.
7.The Evaluation of Monitoring Devices in the Intensive Care Unit with Bedside Chest Computed Radiography

Objective To assess the contribution of the portable chest computed radiography (CR) in evaluation of monitoring devices of the patients in the intensive care unit (ICU). Methods One hundred and sixty-two cases with 387 chest radiographs in the ICU were analysed retrospectively. The location of the catheters of monitoring devices and complications were observed.Results The malposition of the catheters was detected in 47 cases(16.9%),including the endotracheal (ET) tubes too deep at the position, the central venous catheters placed into the internal jugular veins,and the position of the thoracic drain tubes to be deep not enough causing the drain to fall.The complications after operation of monitoring devices were not common,including pneumothorax caused by ventilatory assistance,atelectasis and pneumonia caused by malposition of the ET tubes,totally in 11 cases.10 cases with cardiopulmonary abnormalities were discovered accidentally in all 162 cases(6.2%) when evaluation of monitoring devices.Conclusion Bedside chest CR not only can show the catheter position and the complications of the monitoring devices ,but also the cardiopulmonary abnormalities of patients in the ICU.
8.Changes of resting heart rate in patients with essential hypertension and its clinical significance

Objective To study the relationship between resting heart rate(RHR)and essential hypertension(EH).Methods 215 patients with EH and 82 healthy subjects underwent RHR detection.The relationship between RHR and the state of EH was analyzed.Results RHR in EH group was significantly higher than that in normal group[(78.9±9.1)bpm vs(69.5±10.9)bpm,P<0.01)],RHR in different stages of EH was significantly different(F=21.280,P<0.01,respectively),and increased with grades(P<0.05 orP<0.01).RHR in EH patients complicated with left ventricle hypertrophy(LVH)was higher than those with no LVH[(83.9±9.1)bpm vs (75.9±8.9)bpm,P<0.01].There was positive correlation between RHR and LVH(r=0.8097,P<0.01).Conclusion RHR is related to the development and progress of EH,and it may be one of the index for estimating the state of EH.
9.The effect of Glutamine on the shape of residual intestine and colon in rats with short bowel syndrome

Objectives:To investigate the effect of glutamine on the shape of residual intestine and colon in rats with short bowel syndrome. Methods:23 male Sprague-Dawley rats,underwent a 80% small bowel resection,were randomly divided into three groups:food group(n=8) rats,fed rat chow and water libitum after operation;TPN group(n=8),infused with Gln-supplied TPN;and normal control group.On seventh day after operation,rats were weighted and remaining jejunum、remaining ileum and colon were harvested for histological observation(light microscopy and electron microscopy). Results:There was significant difference in rat average weight between food group and Gln group after operation.Jejunal mucosal villus height(VH) and mucosal thickness(MT) and ileal mucosal VH in food group were significantly increased than those in control group.Jejunal mucosal VH and MT in control group were significantly higher than in TPN group.Ileal mucosal crypt depth(CD) and MT in control group were also significantly higher than in TPN group.Jejunal and ileal mucosal VH、CD and MT in Gln group were significantly higher than in TPN group.Colonic MT in food group was significantly higher than in control group.Colonic MT in Gln group was significantly bigger than in TPN group. Conclusions:After 80% intestinal resection,the remaining intestine can develop the adaptation,but the adaptation is incomplete.TPN therapy can maintain body weight,but only TPN can not result in the adaption.Gln-supplied TPN can stop the remaining itestinal mucosal atrophy,and promote the remaining intestinal adaptation and colonic mucosal hypertrophy.
10.The Posterior Contour Sign of the Lumbar Intervertebral Disc----A Sign to Show the Posterior Border of Disc in Plain Film

Objective:To eveluate and study the anatomic basis of the posterior contour sign of lumbar intervertebral disc.Methods:The most protruding length of the soft tissue mass behind intervertebral spase on lumbar plain film were measured in 100 cases and compared with CT or MR findings.Results:41 posterior contour sign of the interlumbar disc were fined in 39 cases,of which there were 25 in L4/5(61.0%),15 in L3/4(15.6%)and 1 in L2/3(2.4%).The protruding was less than 0.2 cm in 26 and larger than 0.2 cm in 15,there were 10 lumbar disc protruding confirmed by CT or MR ,7 of which the pesterior contour ≥0.2 cm,and 3>0.3 cm,none was ≤0.2 cm.Conclusion:The adipose tissue in the posterior border of lumbar body and disc was the anatomic basis of the sign,and it was an importent sign for diagnosing of lumbar disc hernia on plain film.
