1.A study on the polymorphisms and haplotypes at the apoAI-c Ⅲ gene cluster from the patients with NIDDM

The polymorphisms and haplotypes identified by restriction fragment length polymorphisms (RFLPs) at the apolipoprotein A1-C3 gene cluster (ApoAl-C3) were studied in 75 Han-nationality-Chinese patients with NIDDM, 30 of which were complicated with hypertriglyceridemia. Fifty unrelated healthy subjects matched for age and body weight were chosen as controls. The results showed that after the RFLPs hydrolyzed by the restriction endonucleases SstI and MspI were hybridized with the human ApoAl gene cDNA fragment(l. 0 kb),S2 and M2 tnutational sites consistent with other reports were found . Significant frequency difference in genotype M1M1 was observed between the simple NIDDM and control group(62% vs 42%). In the NIDDM with hypertriglyceridemia group,both of the S2S2 and M2M2 genotypes had a frequency of 10% and the haplotype S2-M2 had a higher frequency than that in controls. This suggests the association of S2M2 haplotype with NIDDM complicated by hypertriglyceridemia.
2.Predictive value of combined determination of ICA、64KDAb and IAA in the first-degree relatives of type I diabetics

Objective To study the predictive value of islet cell antibody (ICA),64KD protein antibody(64KDAb) and insulin auto-antibody (IAA) in the first-degree relatives of type 1 diabetics.Methods The determination of islet autoantibody distribution in type 1 diabetes patients,first-degree relatives without diabetes and normal individuals was made.Results The results revealed that ICA and 64KDAb positive rate in type I diabetes patients and their first-degree relatives was significantly higher than normal individuals.The positive rate of 64KDAb is higher than that of ICA and IAA. Conclusion These results suggest that 64KDAb's sensitivity is higher than ICA and IAA's,so 64KDAb is more valuable as the marker of predicting type 1 diabetes.The combined determination of ICA,64KDAb and IAA might improve the value for predicting diabetes.
3.Clinical observation of large dose of cytidine diphosphate choline in cardiopulmonary cerebral resuscitation

ObjectiveTo explore the treatment efficacy and feasibility of large dose of cytidine diphosphate choline (CDPC) in cardiopulmonary cerebral resuscitation.MethodsOne hundred and thirtytwo patients with sudden cardiac arrest successfully treated by the cardiopulmonary cerebral resuscitation were divided into treatment group ( 75 patients) and control group (57 patients) by random number tables.Two groups of patients were given adrenaline resuscitation drugs,reducing the intracranial pressure drugs,hypothermia,hormones and other basic treatment.Control group used CDPC 0.25 g with glucoseinjection 300 ml to do intravenous drip (once a day,21 days as a period of treatment),and treatment group added CDPC to 2.00 g.The patients were treated by basic traditional analeptic.The Glasgow coma scale (GCS) was carried out in two groups in admission to hospital and 3,7,14,21 days after treatment,and the changes of lysophosphatidic acid (LPA) and phosphatidic acid (AP) level were monitored.ResultsThe GCS score was significantly higher in treatment group 7,14,21 days after treatment(P < 0.05 or < 0.01 ),but GCS score had no significant difference before and after treatment in control group(P > 0.05).The level of LPA in treatment group was significantly decreased 14 days after treatment [ (2.98 ± 0.28) μmol/L ] and 21 days after treatment[ (1.85 ± 0.21)μmol/L],and had significant difference compared with that admission to hospital [ (3.75 ± 0.22) μ mol/L ] (P < 0.05 or < 0.01 ).In control group,there was no significant difference before and after treatment (P >0.05).The level of AP in treatment group 21 days after treatment was significantly decreased compared with that in control group [ ( 3.45 ± 0.23 ) μ mol/L vs.( 5.20 ± 0.21 ) μ mol/L ](P < 0.01).ConclusionLarge dose of CDPC can improve neurological function of cardiopulmonary cerebral resuscitation,which is a safe,effective and easy way to be popularized.

5.Level of serum leptin in newly diagnosed type 1 diabetic patients and the effect of short-term insulin therapy on it

Objective To observe the changes of leptin levels in newly diagnosed type 1 diabetic patients and the effect of insulin therapy on serum leptin levels,so as to investigate the relationship between insulin and leptin.Methods Twenty-nine type 1 diabetic patients were enrolled from out-patient clinic and ward of our hospital.Twenty healthy people in our hospital were selected as normal controls.All blood samples were obtained after an overnight fasting for 14 hours.Fasting serum levels of glucose,hemoglobinA 1c(HbA 1c),triglycerides,cholesterol,insulin,C-peptide and leptin were analyzed in all subjects.The type 1 diabetic patients received insulin injection subcutaneously,and levels of glucose,insulin,C-peptide and leptin were measured 2 weeks later.Results In newly diagnosed type 1 diabetic patients,leptin levels were lower than those in normal controls (P0.05).It was found by correlation analysis that in control group,serum leptin levels were correlated with sex,BMI and serum insulin levels,while in newly diagnosed type 1 diabetic group,serum leptin levels were related to sex and serum insulin levels.Conclusion Serum leptin levels of patients with newly diagnosed type 1 diabetes are lower than those in control group.It is likely that short-term insulin injection subcutaneously can not affect serum leptin levels.

7.Pharmacokinetics of propofol administered by target-controlled infusion in patients with obstractive jaundice

Objective To compare the pharmacokinetic profile of propofol administered by target-controlled infusion (TCI) during anesthesia in patients with and without obstructive jaundice. Methods Twenty-four ASAⅠorⅡpatients aged 40-65 yrs weighing 50-75 kg undergoing elective surgery under general anesthesia were divided into 3 groups (n = 8 each) : group A control (serum total bilirubin 171.1?mol?L-1) . Each group received propofol by TCI using Graseby 3500 infusion pump, based on pharmacokinetic parameter set published by Marsh. The target plasma concentration of propofol was set at 3?g?ml-1 . TCI of propofol was started from the induction of anesthesia and maintained until the end of the surgery. Arterial blood samples were taken at 0.5, 1, 2, 4, 6, 8 min after TCI of propofol was started and every 15 min during maintenance of anesthesia and at 2, 4, 6, 8, 10, 20, 30, 40, 50, 60, 90, 120, 180, 240 and 300 min after TCI was terminated. Plasma concentrations of propofol were determined by high-performance liquid chromatography (HPLC) with fluorescence detector. NONMEM was used to analyze the pharmacokinetic parameters. Results The 3 groups were comparable with respect to sex ratio, age and body weight. The pharmacokinetic profile of propofol given by TCI was best described by three-compartment open pharmacokinetic model in the majority of patients and by two-compartment open pharmacokinetic model in a few patients. There were no significant differences in the pharmacokinetic profile of propofol among the 3 groups. Conclusion Obstructive jaundice does not affect the pharmacokinetics of propofol.

Objective To detect the activity of α1 antitrypsin(AAT) with initial velocity of enzymatic reaction in order to detect the activity of samples in the process of separating and purifying plasma protein ,chromogenic substrate assay was optimized.Methods The effect of trypsin concentration and reaction time on enzymatic reaction was acquired by the kinetic monitoring mode of the microplate reader .Initial velocity was calculated to confirm the largest concentration of trypsin which was saturated by substrate .AAT was incubated with trypsin and absorbance produced by enzymatic reaction of remaining trypsin and substrate could reflect the activity of AAT .A standard curve was established with △D fitting with the activity of AAT standard.The activity of related samples was detected and the precision and accuracy of the method were evaluated . Results Trypsin concentration was 0.0625 mg/ml.Within 20 minutes, enzymatic reaction was with initial velocity .The range of the standard curve was 200-1200 IU/ml.Correlation coefficient was more than 0.99.The activity of Cohn Ⅳ, samples of pre-processing and elution were (720.59 ±18.63), (601.84 ±19.18),and (568.09 ±24.83)IU/ml, respec-tively.The relative standard deviation was less than 10%. Sample recovery rate was 90%-110%.Conclusion The optimized chromogenic substrate assay greatly improves accuracy and precision .The method can be used for the detec-tion of AAT activity of samples in laboratories and workshops .
10.Induction of apoptosis and cell cycle arrest in human endometrial cancer cells by adiponectin

Two human endometrial carcinoma cell lines, HEC-1-A and RL95-2, were treated with adiponectin,and then changes of cell count, apoptosis and cell cycle arrest were measured. The expression of phospho-AMPK(Thr172) and AMPK was determined by Western Blot. The results showed that adiponectin may exert direct anti-proliferative effects on HEC-1-A and RL95-2 cells by inducing cell cycle arrest and apoptosis, which may be mediated by AMPK pathway.
