Chinese Journal of Trauma 1993;0(05):-
Objective To explore remote effect of decalcification bone matrix (DBM) in treatment of delayed union and nonunion of long bone fractures. Methods From 1986, 96 cases of delayed union and nonunion of long bone fractures were treated with DBM. Of all, 38 cases of nonunion of long bone were treated with surgical implantation, 37 delayed union and 21 nonunion with percutaneous injection. Results All 96 cases were followed up for 4-16 years (average 7.5 years). Of 37 cases of delayed union treated with percutaneous injection, 35 attained bone union but 2 resulted in nonunion of tibial fractures with a union rate of 94.6%. Of 21 cases of bone nonunion treated with percutaneous injection, 17 got bone union but 4 did nonunion (3 tibial nonunion and 1 humeral nonunion) with a union rate of 80.1% and union time for 3-8 months (average 4.5 months). Of 38 cases treated with implantation of DBM, 36 had bone union but 2 nonunion at 1/3 inferior to the tibia with a union rate of 94.7%. Conclusions Either percutaneous injection or surgical implantation of DBM can attain satisfactory effect even to autograft. After prepared by dehydration, degrease, decalcification and irradiation disinfection, DBM is characterized by safer transplantation and less immunity than other bone transplantations and can be preserved for a long time so as to be applied both in the peacetime and the wartime.

Objective To explore the assessment of precision, accuracy, and quality control(QC) procedures needed to satisfy the external quality assessment criteria with "operational process specifications" (OPSpecs) derived from an analytical quality-planning model. Methods The routine operating specifications were presented in the form of an OPSpecs chart, which described the operational limits for imprecision and inaccuracy when a desired level of quality assurance was provided by a specific QC procedure. Results OPSpecs charts could be used to compare the operational limits for different QC procedures and to select a QC procedure that was appropriate for the precision and accuracy of a specific measurement procedure. The inaccuracy and imprecision observed for a measurement procedure was plotted on the OPSpecs chart to define the current operating point, which was then compared with the operational limits of candidate QC procedures. Conclusion The general design approach is applicable in all instrument systems.

Objective To determine the effect of peripheral nerve regeneration through NGF ·HAMM (Nerve growth factor and Human Ammion membrane Matrix) grafts. MethodsA 1.2 cm defect of the left sciatic nerve were made and then repaired randomly by means of autogenous nerve bridging (group N),NGF · HAMM microporous conduit bridging (group NH) or HAMM microporous conduit bridging (group H) in 60 adult Wistar rats. There were 20 animals in every group. At 1,2 and 3months post operation, the following items as qualitative histology, quatitative histomorphometry, electrophysiology and functional assessments of end-organ reconnection were determined. ResultsAt 2and 3 months post operation,Motor nerve conduction velocity (MNCV) of the nerve repaired with HAMM were lower than that of the nerve repaired with autograft and NGF · HAMM. At 3 months post operation, there was no significant difference between NH group and N group in nerve conduction velocity (P>0. 05). At 2 and 3 months post operation, both NGF · HAMM conduit and nerve autograft groups showed more nerve fibers, bigger axon diameter and thicker meylin sheaths than H group.Conclution The combination of HAMM and NGF world enhance nerve regeneration and myeliogeny,as well as could cooperatively promote the functional recovery of injured nerves and limbs. Our results indicate that the NGF · HAMM microporous conduit is a promising tool for use as a nerve guide tube in peripheral nerve regeneration.

Objective: To study the inhibitory effect of retrovirus-mediated antisense human telomerase RNA (hTR) gene on hepatocelluar carcinoma, so as to explore an effective way to inhibit telomrerase activity in the treatment of hepatocellular carcinoma. Methods: Sense and antisense hTR gene were transfected into the packaging cell line PT67 by electroporation, and the stablely transfected cells were selected with G418. The recombinant retroviral supernatant was collected and transfected into HepG2 cells. After G418 selection, PCR was used to verify the integration of the hTR gene. Cell growth curves were drawn using MTT assay and the expression of PCNA was determined by immunofluorescence. TRAP-PCR-ELISA was adopted to detect the telomerase activity; cell cycle and apoptosis were evaluated by flow cytometry (FCM).Results: The expression of hTR gene could be amplified in HepG2-hTR-EcoRⅠ and HepG2-hTR-BamHⅠ cells, but not in untransfected HepG2 cells. The antisense hTR complementary to the template region of telomerase inhibited growth and proliferation of HepG2 cells. The expression of neutrophil proliferating cell nuclear antigen (PCNA) was decreased. Telomerase activity in the antisense hTR-treated group was (2.31?0.16),which was significantly lower than those of the other 2 groups(P

Objective Porphyromonas gingivalis(Pg)is one of the main putative pathogens of periodontal disease, espcially of adult periodontitis. This in vitro study was aimed at observing the inhibition actions of 3 kinds of antimicrobial drugs on Pg. Methods 106 stains of Pg were isolated from subgingival plaques with adult periodontitis. The agar diffusion test was used to detect the inhibition actions of metronidazole, clindamycin and tinidazole on 106 stains of Pg. Results Their MIC was respectively:0. 25mg/L,0. 6rng/L. The MIC90 of tinidazole<MIC90 of clindamycin. The results showed:all of metroidazole, tinidazole and clindamycin can be used effectively against Pg,clindarnycin and tinidazole showed more potent antibacterial activity than metronidazole. The different concentration of Pg-liquid (l05 CFU/ml, 106 CFU/ml and 108 CFU/ml) had not influenced the results. Couclusion This study is significant to antibiotical therapy of periodontal diseases.

Objective To explore imaging features of the lateral atlanto-dental interval (LADI) of atlanto-axial joint and its value in the diagnosis of atlanto-axial joint dislocation/subluxation. Methods Two hurdred and forty healthy volunteers without atlanto-axial joint dysfunction and 32 patients with torticollis and/or functional impairment at atlanto-axial joint were enrolled into this study. All subjects were examined with multi-slice CT in both neutral and max rotation positions to the left/right. Clinical endpoints including VBLADI, variance range of the VBLADI and asymmetric conditions were evaluated. x2 test is used to evaluate the variance of incidence of bilateral LADI asymmetry in different age groups, Pearson-test(2-tailed) is used to evaluate the correlation between VBLADI and rotary function of atlanto-axial joint, precise test of fourfold table is used to compare normal group with patient groups. Results ( 1 ) In the normal control group: asymmetry of bilateral LADI were observed in 204 among 240 (85.00%) healthy volunteers including 60 youngsters( < 15 years) and 180 adults( ≥ 15 years). The median and P95% of |VBLADI| were 0.850 mm,2.450 mm in the young ( < 15 years) and 0.700 mm,2.051 mm in the adult(≥15 years),there were 2 and 10 volunteers whose |VBLADI| ≥P95% in the young and in the adult respectively. There was no correlation between VBLADI( -2.146 to 2.114 mm,Median 0.000 mm) and LA(22.949° to 44.649°, Median 34.500°), RA (25.284° to 45.334°, Median 35.300° ), VLRA ( - 11.643°to 8.623 °, median 0.000 ° ) respectively in normal people ( r = - 0.030, - 0.005,0.026, P > 0.05 ). ( 2 ) In the study with 32 patients including 25 youngsters( < 15 years) and 7 adults( ≥ 15 years). Seventeen of 32 had atlanto-axial lateral subdislocation (AALSD),in 16 youngsters( < 15 years), the median of the |VBLADI| was 3.350 mm and 14 cases |VBLADI| ≥2.450 mm;1 adult patient(≥ 15 years) had |VBLADI| ≥ 2.051 mm; 7 of 32 patients had atlanto-axial rotatory dislocation (AARD) including 4 youngsters |VBLADI| <2.450 mm,median 1.200 mm,3 adults |VBLADI| <2.051 mm,all of the 7 patients with AARD had unilateral parasol LADI in neutral position;8 of 32 patients had atlanto-axial complicated dislocation(AACD) including 5 youngsters |VBLADI| ≥2.450 mm,3 adults |VBLADI| ≥2.051 mm. There are more cases whose |VBLADI| ≥P95% in group AALSD and group AACD than in abnormal group( P<0.01 ), but there is no difference between group AARD and normal group( P = 0.738 ).Conclusion The imaging finding of "asymmetry of bilateral LADI" may be either a physiological variation or a pathological condition, clinical manifestations and other imaging findings should be evaluated to make a diagnosis.
