1.Influence and Reply of Staff Rotation on Instruments in A Laboratory Department

Under the background that the job division and complication of the laboratory instrument are highly improved, It puts forward the new demand for the use and maintenance of the laboratory instrument. Nowadays, for the lack of training, Many staff in medical laboratory can not correctly use and maintain the instrument in their rotation to various departments. The problems are solved by the following ways: making an appropriate staff rotation plan, ensuring all operations were carried out strictly according to the instrument guide book, establishing the holder appointment system and individual responsibility system, ensuring the reports were sent out by the new rotating personnel independently only after strict training and qualified examination. Through these methods, the accuracy of the equipment and the quality of testing have greatly increased.
2.Study on Rules for Acupoint Selection of Acupuncture and Moxibustion for Treatment of Knee Osteoarthritis Based on Data Mining

Objective To analyze the rules of acupoint selection in treating knee osteoarthritis with acupuncture and moxibustion therapy based on data mining; To provide references for clinical treatment.MethodsArticles about treating knee osteoarthritis with acupuncture and moxibustion were retrived from China National Knowledge Infrastructure (CNKI), Wanfang academic periodical full-text database, Chinese Journal Full-text Database (VIP) and SinoMed in China from Jan. 1st 2006 to Jan. 1st 2016. The database of acupuncture and moxibustion prescription was set up. The Apriori association rules, system cluster analysis and factor analysis were adopted in order to analyze the rules of acupoint selection for knee osteoarthritis.ResultsTotally 190 articles were included, covering 228 acupuncture- moxibustion prescriptions, 65 acupoints and the total frequency for 1736 times. The usual acupoints in the lower limbs were Dubi (ST35), Neixiyan (EX-LE 4), and Yanglingquan (GB34). The main maridians were Stomach Meridian of Foot-Yangming, Spleen Meridian of Foot-Taiyin, and Gallbladder Meridian of Foot-Shaoyang, accounting to for 1626 times (93.66%). Cluster analysis achieved other four groups of acupoint selection based on syndrome differentiation.ConclusionLocal acupoints and dialectical point selection were important principle in treating knee osteoarthritis combined with acupoint selection based on syndrome differentiation.

4.Roles of endoscopic biopsy and third ventriculostomy in the diagnosis and treatment of pineal region tumors in children

Objective To investigate the roles of endoscopic biopsy and third ventriculostomy in the diagnosis and treatment of pineal region tumors in children. Methods Endoscopic biopsy and third ventriculostomy were performed in 9 pediatric patients with pineal region tumors. Results Successful third ventriculostomy, confirmed by MRI, was performed in 9 cases of children with obstructive hydrocephalus. No complications were found in all patients. Conclusion Endoscopic biopsy and third ventriculostomy are effective neuroendoscopic procedures in minimally invasive preferential management of pineal region tumors.
5.The research on affinity of hospital separation carbapenems-resistant Acinetobacter

Objective To study the protein fingerprinting of carbapenems resistant and susceptible Acinetobacter baumannii (Ab) strains ,investigate the clinical value of affinity distance in carbapenems resistant strains .Methods A total of 22 carbapenems resistant Ab strains and 18 carbapenems susceptible Ab strains were collected ,and bacterial protein fingerprinting was detected by surface-enhanced laser desorption ionization time of flight mass spectrometry (SELDI-TOF-MS) ,differentially expressed proteins was analyzed by Biomarker Wizard 3 .1 .Cluster analysis of differential expressed proteins was conducted on SPSS 19 .0 .Results The protein expression pattern of carbapenems resistant and sensitive Ab strains had significant difference (P< 0 .05) .Cluster anal-ysis showed carbapenems resistance Ab was given priority to with type A ,followed by type B .Conclusion The results of cluster a-nalysis carbapenems resistance of Ab protein fingerprinting could determine the distance of affinity relationship of Ab .It could pro-vide a theoretical basis for the infection and clinical epidemiology of Ab .
6.Comparison of the hemostatic effect of total knee arthroplasty in intravenous and intra-articular injection of tranexamic acid

Objective To find a more suitable approach for the application of tranexamic acid(TXA)on total knee arthroplas‐ty (TKA) .Methods Totally 60 patients who met the inclusion criteria from January 2014 to August 2015 in the First Affiliated Hospital of Shihezi University were selected and divided into two groups according to the different route of administration .Group A (n=30) was intravenously injected with 100 mL TXA ,and group B(n=30) was locally injected with 100 mL TXA .Three hours drainage tubes occlusion were carried out after operation in the two groups .The intraoperative and postoperative dominant blood loss ,hidden blood loss indexes and the amount of total blood loss were recorded ,and coagulation indexes and D‐2 polymer were reg‐ularly monitored ,the incidence of thrombosis and postoperative adverse events were also observed .Results The amount of total blood loss in group B[(895 .41 ± 239 .02)mL] was lower than that in group A[(1 020 .89 ± 210 .83)mL] ,and the difference was statistically significant (P<0 .05);the total drainage volume in group B[(294 .33 ± 54 .25) mL] was lower than that in group A [(373 .33 ± 48 .02)mL] ,and the difference was statistically significant (P<0 .05) .After operation ,there was no significant differ‐ence between the two groups in coagulation indexes ,D‐2 polymer and the amount of hidden blood loss (P>0 .05) .No blood trans‐fusion ,symptomatic deep venous thrombosis and fatal pulmonary embolism occurred in the two groups .Conclusion The hemostatic effect of local application of TXA is better than that of intravenous injection in patients′initial TKA .
7.Perioperative anaesthetic management of bilateral lung transplantation for idiopathic pulmonary arterial hypertension

Objective To explore the key points of anaesthetic management of bilateral lung transplantation treating severe idiopathic pulmonary arterial hypertension.Methods Clinical data of 20 cases with severe IPAH treated by bilateral lung transplantation were analyzed retrospectively, 2 males and 18 females, ASA physical status Ⅲ or Ⅳ.The hemodynamic changes were observed at various points (30 min after double lung ventilation, 15 min after ECMO, during one-lung ventilation, 30 min after pulmonary artery block, at the opening of pulmonary artery and at the end of surgery) and pre-and post-operative changes of cardiovascular morphology were analyzed with heart ultrasound.Results Compared with 30 min after double lung ventilation, PaO2 and SVV were significantly increased and PAP, PAWP and PaCO2 were decreased 15 min after extracorporeal membrance oxygenation (ECMO) (P<0.05).PAP and PAWP were significant higher and SVV was lower 30 min after pulmonary artery block than 15 min after ECMO (P<0.05).Opening of pulmonary artery had significantly increased SVV and reduced PAP and PAWP compared with pulmonary artery block (P<0.05).ECMO was removed in 4 cases at end of the surgery.The remaining 16 cases assisted by ECMO were transferred to ICU, 3 cases underwent thoracotomy for exploration and hemostasis 1 day after the operation while 13 cases suffered from different degrees of left heart dysfunction postoperatively, 4 of which had acute heart failure.Of the 4 cases, 2 were treated by right femoral vein-right femoral artery bypass (V-A ECMO bypass) and 2 underwent right femoral vein-right internal jugular vein bypass (V-V ECMO bypass).Moreover, 3 cases performed tracheotomy 5 days, 6 days and 13 days day after the operation, respectively.All the patients were cured and discharged.Their cardiovascular morphology was improved.Conclusion The key successful factors of IPAH treated by bilateral lung transplantation are to improving perioperative heart function, keeping stable hemodynamics during induction of anesthesia and the operation and using ECMO properly and postoperative support.
8.Genetic polymorphisms of TNF-α and IL-6 and their correlations with susceptibility to colorectal cancer among Chinese Han people in Shandong province

Objective To study the correlations between genetic polymorphisms of TNF-α as well as IL-6 and susceptibility to colorectal cancer among Chinese Han people in Shandong province.Methods Single nucleotide polymorphisms (SNPs) of TNF-α-238G/A,-308G/A and IL-6-174G/C,-572G/C,-597G/A in 490 patients with colorectal cancer were analyzed by using gene chip.Concentrations of TNF-α and IL-6 in serum samples were measured by ELISA.A case-control study was conducted to analyze the correlations between SNPs of TNF-α-238G/A,-308G/A as well as IL-6-174G/C,-572G/C,-597G/A and susceptibility to colorectal cancer.Chi-square test or t test was used for statistical analysis.Relative risks were estimated based on the values of odds ratio (OR) and 95% confidence interval (95%CI).Results The frequency of TNF-α-308AA in patients with colorectal cancer was significantly higher than that in healthy subjects (x2 =6.15, P<0.05, OR=2.08, 95%CI=1.17-3.71), while the frequency of IL-6-572CC in patients with colorectal cancer was significantly lower than that in healthy subjects (x2 =4.97, P<0.05, OR=0.73, 95%CI=0.55-0.96).The frequency of TNF-α-308AA in patients with colon cancer (OR=2.31, 95%CI=1.17-4.55), tubular adenocarcinoma (OR=2.32, 95%CI=1.28-4.21), high (OR=2.05, 95%CI=1.01-4.15) or moderately differentiated adenocarcinoma (OR=5.88, 95%CI=1.79-19.30) was significantly higher than that in healthy subjects.The levels of serum TNF-α in TNF-α-308AA carriers with colorectal cancer were significantly higher than those in TNF-α-308G carriers with colorectal cancer (t=2.13, P<0.05) as well as those in healthy TNF-α-308AA carriers (t=2.13, P<0.05).The levels of serum IL-6 in colorectal cancer group were significantly higher than those in control group (t=6.74, P<0.001).Conclusion The SNPs of TNF-α-308 and IL-6-572 are associated with the occurrence and development of colorectal cancer in Chinese Han people in Shandong province.
9.Observation of Preventive Effect of Lonicera japonica Alcohol Extract on Mice with Liver Injury by Metab-olomics Method

OBJECTIVE:To study the preventive effect of Lonicera japonica alcohol extract on mice with liver injury based on metabolomics method. METHODS:30 mice were equally randomized into a normal control (isometric normal saline) group,a model(isometric normal saline)group and a group of L. japonica alcohol extract(2 g/kg). The mice were given drugs by ig once a day for 14 consecutive days. On the 8th day of administration,the models were established by giving 0.2% dimethyl sulfoxide (DMN,10 ml/kg)ip once a day for 7 consecutive days. Gas chromatography-mass spectrum(GC-MS)was used to analyze 24 h total ions chromatogram of the urine sample on the 1st,3rd,5th and 7th day of administration of drugs and DMN. The change in endogenic small molecule metabolites in urine was observed. Principal component analysis was employed to explore the change in the metabolite chromatogram and underlying biomarkers in urine. RESULTS:The contour of the chromatogram changed to a largest extent 1 to 5 d after given DMN,but showed an obvious trend towards regression 7 days thereafter. DMN resulted in increase in the contents of 8-phenyl-8-azbicyclo-[4,3,0]non-3-ene-7,9-dione,2-(6-heptynyl)-1,3 dioxolane,bis-(O-methyloxime)-4-ketoglu-cose,and decrease in the contents of malonic acid,2-(4- chlorophenylthiomethoxyl)ethyl,tetrahydro-2-furanacetaldehyde,D-ga-lactose,erythro-pentonic acid and galacturonic acid,in endogenic small molecule metabolites in mouse urine,for which Lonicera japonica alcohol extract can improve that. CONCLUSIONS:Previous administration of L. japonica alcohol extract ig has preven-tive effect to some extent on the physiological and metabolic conditions of mice with liver injury induced by DMN.
10.Value of pathological features in defining the longitudinal margin of the clinical target volume in radiotherapy of esophageal squamous-cell carcinoma

Objective To study the characteristics of microscopic spread of esophageal squamous-cell carcinoma (ESCC) and the influence of clinicopathological features on it to help define the clinical target volume (CTV) margin in radiotherapy.Methods Sixty-four surgical specimens of ESCC were observed for longitudinal microscopic spread per centimeter both proximally and distally from the tumor.The shrinkage ratio of each specimen was calculated and used for tissue incision.Results The further the distance beyond the tumor, the lower the incidence there was of microscopic spread.Positive rates of microscopic spread in group 3 cm of proximal and distal were 4.8% and 6.9%, respectively, and in group 4 cm were both 3.6%.Tumors longer than 5 cm in length,with poorer differentiation, lymph nodes metastasis and more aggressive phase had higher positive rates (79.3% vs 45.7%,77.4% vs 45.5%,76.0% vs 51.2%,70.5% vs 40.0%,χ2=7.52,6.86,3.91,5.36;P=0.006,0.009,0.042,0.021).Differentiation and tumor length were main factors contributing to microscopic spread (χ2=0.19,4.82;P=0.020,0.017).Conclusions To cover 95% of the microscopic spread,a margin of 3.0 cm proximal and 4.0 cm distal beyond gross tumor volume is needed and as to 90%, a margin of 3.0 cm both proximal and distal is needed.Moreover, the influence of pathological features should be taken into account.
