1.Application of whole brain angiography via radial approach in young patients for the diagnosis of ischemic cerebrovascular disease

Objective To assess the feasibility and safety of transradial puncture route for digital subtraction angiography (DSA)in young patients with ischemic cerebrovascular disease. Methods One hundred young patients with cerebrovascular disease who underwent whole brain angiography at the Department of Neurology,the 81 st Hospital of PLA were enrolled. They were divided into either a radial group (n =50)or a femoral group (n = 50)using a computer random number method. The time of puncture,success rate of puncture,success rate of selective angiography,exposure time,and incidence of complications were compared and analyzed. Results (1)The puncture time of the radial group was significantly longer than that of the femoral group,they were 3. 00 ± 0. 50 min and 1. 50 ± 0. 25 min respectively. There was significant difference (t = 18. 97,P < 0. 05). There were no significant differences in the success rate of puncture (98% vs. 100%),success rate of selective angiography (100% vs. 100%),and exposure time (5. 3 ± 2. 2 vs. 4. 8 ± 1. 7 min)between the two groups (all P > 0. 05). (2)The incidence of complications of the radial group (4%,n = 2)was significantly 1ower than that of the femoral group (18%,n = 9 ). There was significant difference between the two groups (χ2 = 5. 01,P < 0. 05 ). Conclusion Transradial route puncture for whole brain DSA in young patients with cerebrovascular disease is safely and feasible.
2.New features in drug resistance of Haemophilus inlfuenzae isolated from children with infection diseases in Suzhou during 2011-2012

Objectives To investigate the changes and features of drug resistance in Haemophilus inlfuenzae (Hi) isolated from children with infection diseases in Suzhou. Methods One thousand two hundred and twenty-two Hi strains isolated from clinical specimens were collected from January 2011 to June 2012. Antimicrobial susceptibility was tested by Kirby-Bauer me-thod, andβ-lactamase was analyzed by chromogenic nitroceifn method. Then strains were divided into four groups according to their speciifc resistance patterns:①β-lactamase positive strains (except for cefuroxime-resistant strains),②β-lactamase negative ampicillin-resistant (BLNAR) strains,③cefuroxime-resistant strains, and④other resistant strains. Results The resistance of iso-lated Hi from children in Suzhou area to ampicillin, co-trimoxazole, cefaclor, tetracycline, chloramphenicol, azithromycin, cefu-roxime, ampicillin/sulbactam was 32.7%, 76.7%, 27.4%, 14.3%, 10.2%, 8.4%, 6.9%and 4.3%, respectively, and the detection rate in four groups was 29.0%, 2.6%, 6.9%, 41.2%, respectively. Multi-resistant strains were mainly distributed in Group①and Group②, accounting for 67.5%and 81.3%of each group. Azithromycin, tetracycline and chloramphenicol showed high antimicrobial activity against BLNAR and cefuroxime-resistant Hi strains. The positive rate of resistance toβ-lactamase was 30.8%. The rates of resistance to cefuroxime, cefaclor, ampicillin/sulbactam and cotrimoxazole inβ-lactamase-positive ampicillin-resistant strains are signiifcantly different from those inβ-lactamase-negative ampicillin-resistant strains. Conclusions The resistance feature of Hi isolated from children in Suzhou shows signiifcant changes, including new appearance of cefuroxime-resistant strain, a rapid increase in resistance to azithromycin, and a large proportion of multidrug-resistant strains. The rapid increase in BLNAR and the emergence of cefuroxime-resistant strains have become the new resistance pattern of Hi in this area.
3.WU Mianhua's Experience in the Treatment of Tumor-Related Complications Using Hehuanhua (Flos Albiziae)

This paper summarized professor WU Mianhua's experience in the treatment of tumors using Hehuanhua (Flos Albiziae). He believes that the occurrence and development of tumors are closely related to emotions, and thereafter proposed the concept of "masses usually due to constraint, and should be treated by regulating the spirit".Hehuanhua is a light and aromatic flower that enters the heart, liver and spleen meridians, and is good at relieving constraint and regulating the spirit, harmonizing the center with aroma, which can be used to treat all kinds of tumor-related complications. Hehuanmi is flower buds, picked before the flowers fully blooms, have the effect of clearing and lightening upward, opening up the heart and mind, and unblocking liver qi. Commonly used formulas include self-made Ningxin Anmian Decoction (宁心安眠汤) to treat tumor-related insomnia, self-made Aitongxiao Formula (癌痛消方) to treat tumor-related pain, self-made Erhua Jieyu Formula (二花解郁方) to treat tumor-associated depression, and self-made Shugan Tiaohe Decoction (疏肝调和汤) to deal with peri-menopausal state after the treatment of tumors. Hehuanhua is an inflorescence, which captures the aromatic essence and has the function of removing the filth and turbidity, and can wake up the spleen and harmonize the stomach; therefore, it is often used in self-made Tiaomu Hezhong Decoction (调木和中汤) to treat chemotherapy-related digestive tract adverse reactions or combined with rose flowers to treat chemotherapy-induced oral ulcers.
4. Therapeutic effect of early directional catheterization on malignant middle cerebral artery infarction

Objective:
To investigate the efficacy and safety of cuboid stereotactic aspiration of necrotic brain tissue for treating malignant middle cerebral artery infarction in the elderly patients.
Methods:
Sixteen elderly patients with malignant middle cerebral artery infarction were selected from June 2017 to January 2019 in our hospital. Patients were followed up for 6 months to evaluate the efficacy of stereotactic aspiration of necrotic brain tissue using the modified Rankin Scale (mRS).
Results:
The 30-day mortality was 18.75%. Among the 16 elderly patients, 6 (37.5%) had an mRS score of 3 (defined as moderate disability), 6 (37.5%) had an mRS score of 4 (defined as moderate to severe disability), 1 (6.25%) had an mRS score of 5 (defined as severe disability), and 3 (18.75%) had an mRS score of 6. The probability of 6-month favorable outcome, defined as an mRS score of ≤3, was 37.5%, and the 6-month mortality was 18.75%.
Conclusions
It is a simple, minimally invasive, effective and safe method to treat malignant middle cerebral artery infarction in the elderly patients with cuboid stereotactic aspiration of necrotic brain tissue, which needs to be confirmed by further randomized controlled studies.
