2. Clinical research on acupoint application on pulmonary function and quality of life in patients with pneumoconiosis

OBJECTIVE: To explore the effects of acupoint application on pulmonary function and quality of life in patients with pneumoconiosis. METHODS: By random number table method,74 pneumoconiosis patients were randomly divided into two groups: acupoint application group and control group,37 cases in each one. The control group was given conventional treatment. Based onconventional treatment,the acupoint application group was given acupoint application with Chinese herbal medicine on the first day of Chu Fu,Zhong Fu,Mo Fu every year for three years. The changes of pulmonary function was observed in each group. The quality of life of patients was investigated by the World Health Organization Quality of Life-BREF. RESULTS: Before treatment,the pulmonary function indicators such as vital capacity,forced vital capacity(FVC),forced expiratory volume in first second(FEV1),FEV1/ FVC% 、peak expiratory flow( PEF),maximal voluntary ventilation and quality of life score between the two groups had no statistical significance( P > 0. 05). Compared with the same group before treatment,all indexes of pulmonary function,except FEV1/ FVC% and PEF,and life quality score were improved in patients in the control group after treatment( P < 0. 05). After treatment,all indexes of pulmonary function and life quality score were improved in patients in acupoint application group( P < 0. 01). The remaining 5 lung function indexes,except PEF,and the quality of life of the patients in acupoint application group were higher than those in the control group( P < 0. 05). CONCLUSION: Acupoint application therapy can improve lung ventilation function and improve quality of life in patients with pneumoconiosis.
3.Effect of Huatan Tongluo Prescription on Vulnerability of Carotid Atherosclerotic Plaque in Hypertension

ObjectiveTo evaluate the effect of Huatan Tongluo prescription on the vulnerability of atherosclerotic plaques in the carotid arteries of patients with hypertension of phlegm-stasis combination syndrome. MethodA total of 132 eligible patients were randomly divided into an observation group (66 cases) and a control group (66 cases). The control group received oral atorvastatin calcium tablets and enteric-coated aspirin tablets, while the observation group received Huatan Tongluo prescription in addition to the treatment received by the control group. The treatment duration was 6 months. A carotid artery ultrasound examination was performed to record the number of plaques, the maximum plaque area, the maximum plaque cross-sectional thickness, and the intima-media thickness (IMT) of the carotid artery. Crouse score, plaque vulnerability score, and phlegm-stasis combination syndrome score were assessed. Blood lipid levels [total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C)], inflammatory markers [neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), interleukin-6 (IL-6), and high-sensitivity C-reactive protein (hs-CRP)], vascular endothelial function [endothelin-1 (ET-1), von Willebrand factor (vWF), and nitric oxide (NO)], and relevant proteins [pentraxin 3 (PTX3) and galectin-3 (Gal-3)] levels were measured. Safety evaluation was conducted, and comparisons were made in terms of carotid artery stenosis rate, plaque regression efficacy, and traditional Chinese medicine (TCM) syndrome efficacy. ResultCompared with the results before treatment, both groups showed significant reductions in IMT, plaque number, maximum plaque area, and maximum plaque cross-sectional thickness (P<0.05). After treatment, the observation group exhibited more significant reductions in the above indicators compared with the control group (P<0.05). After treatment, Crouse scores, plaque vulnerability scores, and phlegm-stasis combination syndrome scores in both groups were lower than those before treatment (P<0.05). After treatment, the observation group had lower scores in these indicators than the control group (P<0.05). In terms of blood lipid levels, both groups showed decreases in TC, TG, and LDL-C levels, and an increase in HDL-C levels after treatment compared to those before treatment (P<0.05). The observation group exhibited greater improvements in these lipid parameters than the control group (P<0.05). Inflammatory markers NLR, MLR, IL-6, and hs-CRP significantly decreased in both groups after treatment compared with those before treatment (P<0.05). The observation group showed more significant reductions in these markers than the control group after treatment (P<0.05). After treatment, both groups demonstrated decreases in levels of ET-1, vWF, PTX3, and Gal-3, along with an increase in NO levels compared with those before treatment (P<0.05). The observation group showed more significant improvements in these markers than the control group after treatment (P<0.05). After treatment, the observation group had a lower carotid artery stenosis rate than the control group (P<0.05). The plaque regression efficacy rate was 51.72% (30/58) in the observation group, and the total effective rate of TCM syndrome was 84.48% (49/58), both of which were higher than 18.64% (11/59) and 52.54% (31/59) in the control group (χ²=10.061, 13.799, P<0.05). No adverse reactions related to the Huatan Tongluo prescription were observed during the treatment period. ConclusionIn addition to statin therapy, Huatan Tongluo prescription can effectively reverse carotid artery atherosclerotic plaques in patients with hypertension and carotid artery stenosis, reduce plaque vulnerability, exhibit lipid-lowering and anti-inflammatory effects, and improve vascular endothelial function. The treatment demonstrates favorable clinical efficacy and safety. Therefore, it is very worthy of clinical promotion and application.
4.Expression and Significance of Serum Mir-132 in Patients with Obstructive Sleep Apnea with Cognitive Impairment

【Objective】To explore the relationship between the miR- 132 expression in serum and cognitive deficits of OSA. 【Methods】 66 Chinese adults age 30 to 60 years old were enrolled and categorized into two groups based on Montreal Cognitive Assessment(MoCA)scores: OSA patients with cognitive impairment(OSAI,n=36),OSA patients without cognitive impairment(OSAN,n=30),and thirty adults without OSA as healthy control group(HC,n=30). Out- of- center cardiopulmonary sleep testing (OCST) and MoCA assessment were performed and the relative expression of miR-132 in serum was detected by PCR.【Results】No significant difference was observed in age,education,gender and hypertension(P>0.05). The relative expression level of miR-132 was significantly up-regulated in OSAI patient′s serum compared to the OSAN and HC patients (P<0.001),and had a positive correlation with MoCA score (r=- 0.726,P<0.001). ROC analyses showed that the areas under the curve(AUC)were statistically significant from the line of identity in OSA with cognitive impairment(AUC=0.935,95% CI:0.890- 0.981,P<0.001)and in OSA(AUC=0.787,95% CI:0.695-0.879,P<0.001).【Conclusions】Elevated serum miR-132 expression levels are closely related to the diagnosis of OSA and its cognitive dysfunction. Detection of serum miR- 132 may be a potential indicator of cognitive dysfunction and diagnosis in OSA patients.
5.Clinical significant of predicting the risk of dysglycemia and dyslipidemia based on body mass index,waist circumference and waist-to-hip ratio

Objective To investigate the clinical significant and the difference of predicting the risk of dysglycemia and dyslipidemia by body mass index (BMI),waist circumference (WC) and waist-to-hip ratio (WHR) in order to look for the best predicting index.Methods Five thousand and thirty residents were participated to this study who were from Jinshan New Area and nearby of Jinshan,Shanghai,including 2004 males and 3026 females.They were divided into obesity group and non-obesity group based on the index of BMI,WC and WHR respectively.The ROC was made based on the above indies in different gender groups.Results The ROC area of BMI was the biggest with 0.641 (P <0.05) for male and 0.617(P <0.05) for female.The cut off value was 24.67 or 23.88 based on the male or the female.The same trend was seen in terms of WC with 88.5 cm for the male and 84.5 cm for the female.The small cut off value was seen in terms of WHR and there was no significant between male and female.Among male people,the cut off values was 26.01 in terms of BMI and 88.5 cm in terms of WC,0.89 cm in terms of WHR.Among the female people,there was the less predicting significant in terms of WC or WHR.The area under the curve had no significant differences,and the BMI predicted abnormal blood glucose of no value,the area under the curve is only 0.513 (P > 0.05).Conclusion BMI is proved the best predictor for the risk of dyslipidemia.There are significant among BMI,WC and WHR in terms of predicting dysglycemia in different gender.BMI is proved without significant regarding of predicting dysglycemia in females.
6.Clinical effect of alpha lipoic acid on early diabetic nephropathy

Objective To investigate the clinical application of alpha lipoic acid on early diabetic nephropathy(DN).Methods Seventy patients with early diabetic nephropathy were randomly divided into two groups.Patients in control group were treated with regular glycemic control methods,while patients in treatment group were received alpha lipoic acid 300 mg static drop treatment for two weeks beside regular treatment.High sensitivity C reaction protein,microalbuminuria and urine creatinine were measured before and after the treatment.Microalbuminuria and urine creatinine were re-measured three months later.Results At the end of two weeks treatment,the levels of high sensitivity C reaction protein in the treatment group decreased from (3.92 ± 4.56) mg/L to (2.24 ± 2.49) mg/L,and there was significant difference (t =2.803,P < 0.05).Microalbuminuria decreased from (127.18 ± 76.91) mg/L to (84.50 ± 122.87) mg/L,and there was significant difference (t =2.501,P < 0.05).The ratio of microalbuminuria and urine creatinine in treatment group decreased from(28.95 ± 30.49) mg/mmol to(13.76 ± 20.83) mg/mmol,and there was significant difference (t =3.616,P < 0.05).While there was no significant difference in the control group before and after regular waglycemic control.At three months after treatment,there were no significant difference regarding of microalbuminuria,the ratio of microalbuminuria and urine creatinine between treatment and control groups (P > 0.05).Conclusion Alpha lipoic acid might be with ability of decreasing the levels of microalbuminuria of diabetic nephropathy in the short term,but further study is needed to verify whether alpha lipoic acid can decrease the level of microalbuminuria in the long term.
7.Relationship between serum cardiac troponin I levels and risk of subseqent heart events in unstable angina

Objective To investigate cardiac troponin I (cTnⅠ) for detecting minor myocardial damage and evaluating the prognostic value in unstable angina (UA) Methods Serum cTnⅠ, creatine kinase isoenzyme MB (CK MB) and a part in cardiac troponin T (cTnT) were measured in 106 (UA) and 44 stable angina (SA) patients as well as 68 cases with normal coronary angiography, 72 healthy subjects Relative risk factors and occurrence of heart events were analyzed Results 61 of 106 patients with UA had elevated cTnⅠ levels (57 5%), but 8 of 44 cases with SA had positive results (18 18%) Among them, only 3 cases had slightly elevated CK MB levels The incidences of heart events were significantly higher in patients with elevated cTnⅠ levels than in patients with normal one The higher cTnⅠ levels was and the more heart events occurred cTnT values was similarly found to be an indicator of increased risk of heart events Conclusion cTnⅠ is a higher sensitive and specific marker for assessing minor myocardial damage cTnⅠ is a strong independent indicator of risk stratification in UA
8.New octadecanoid derivatives from the seeds of Ipomoea nil.

Four new octadecanoid derivatives (1-4) including a pair of enantiomers (1/2), along with 12 known analogues (5-16), were isolatedfrom the seeds of Ipomoea nil. Their structures were determined by detailed spectroscopic analyses and comparison with reported data of structurally related compounds, with the absolute configurations of 1 and 2 being assigned by an in situ dimolybdenum ECD method. Our bioassays revealed that these isolates did not show ABTS radical scavenging activity while 10 and 13 displayed better α-glucosidase inhibitory activity than the positive control acarbose (IC 167.7 ± 1.55 μmol·L), with IC of 92.73 ± 3.12 and 11.39 ± 2.18μmol·L, respectively.

9.Application of Multiple Kits in Special Parentage Testing Cases.

OBJECTIVES:
To analyse the genetic polymorphism of 21 autosome STR loci in Han population of Shandong Province and the cases with loci mutation or allelic loss typed by Goldeneye® DNA identification system 25A.
METHODS:
Totally 40 autosome STR loci types of 273 unrelated individuals in Han population of Shandong Province were typed by Goldeneye® DNA identification system 25A and 22NC, and the genetic polymorphism of 21 STR loci in those was analysed. Meanwhile, six cases with loci mutation were analysed by adding the tests with Goldeneye® DNA identification system 22NC, 20Y and 17X. Another three cases with allelic loss were tested by AmpFℓSTR® Identifiler® Plus PCR and analysed by gene sequencing.
RESULTS:
The genetic parameters of 21 autosome STR loci in Han population of Shandong Province were obtained. When STR loci were added up to 40, five of those with loci mutation met the identification requirements, and the results of X-STR or Y-STR types were consistent with that of STR loci. There was another duo case with one suspected loci mutation, biological source of six STR loci genotypes could not be found in the genotypes of supposed father. The Y-STR genotype of two individuals was identical that indicated both of them came from same paternal line. However, the fatherhood was excluded according to the autosome STR loci system. For two cases with allelic loss on D18S51, base mutation or loss were found in the primer binding domain of mother and child by gene sequencing. Another mother-child case with allelic loss on D13S317 was certified by AmpFℓSTR® Identifiler® Plus PCR kit.
CONCLUSIONS
The 21 autosome STR loci in Han population of Shandong Province have high polymorphism, which can be used in routine cases of paternity identification. For some duo cases with loci mutation, Goldeneye® DNA identification system 25A cannot satisfy the identification requirements, thus more autosome STR loci should be added properly. For the cases with allelic loss, the problem can be resolved by gene sequencing or using different merchant kits.

10.New di-spirocyclic labdane diterpenoids from the aerial parts of Leonurus japonicus.

Phytochemical investigation on the ethanol extract of a well-known medicinal herb Leonurus japonicus, led to the separation of 18 labdane type diterpenoids (1-18). Through comprehensive spectroscopic analyses and quantum chemical calculations, these compounds were structurally characterized as six new interesting 5,5,5-di-spirocyclic ones (1-6), two new (7 and 8) and six known (13-18) interesting 6,5,5-di-spirocyclic ones, a new rare 14,15-dinor derivative (9), and three new ones incorporating a γ-lactone unit (10-12). An in vitro neuroprotective assay in RSC96 cells revealed that compounds 7 and 12 exhibited neuroprotective activity in a concentration-dependent way, comparable to the reference drug N-acetylcysteine.
