1.Efficacy and safety of permissive hypercapnia ventilation in mechanically ventilated newborns: a meta-analysis

Objective To evaluate the efficacy and safety of permissive hypercapnia ventilation in mechanically ventilated newborns.Methods PubMed,Embase,the Cochrane Library,China National Knowledge Infrastructure (CKNI),Wanfang Data and Chinese BioMedical Literature Database (CBM) were searched up until March 31,2013.Randomized controlled trials (RCTs) comparing permissive hypercapnia (PHC) group with normocapnia (NC) group in mechanically ventilated newborns were included.The primary outcomes included the incidence of ventilator associated lung injury (VALI),bronchopulmonary dysplasia (BPD),intraventricular hemorrhage (IVH),periventricular leukomalacia (PVL),patent ductus arteriosus (PDA),neonatal necrotizing enterocolitis (NEC),neurodevelopmental injury and the mortality rate.Secondary outcomes included the duration of ventilatory support and the length of hospital stay.The Cochrane Handbook 5.1.0 was used to evaluate the methodological quality and RevMan 5.1 software from Cochrane Collaboration was used for meta-analysis.The fixed effects model or the random effects model was adopted according to the result of heterogeneity.Results (1) A total of 325 articles were searched,and eight RCTs involving 605 newborns (302 newborns in PHC group while 300 newborns in NC group) which met the inclusion criteria were selected.In seven studies,the partial pressure of carbon dioxide (PCO2) was controlled at < 65 mmHg (1 mmHg=0.133 kPa) and pH at ≥ 7.2 in PHC group.In one study,PCO2 was > 52 mmHg and pH>7.2,without descripition of the upper limit of PCO2.(2) Four articles described the method of random allocation in detail; three described allocation concealment; all eight studies used blinding method for research subjects; two used blinding method for outcome assessment; all eight studies reported complete data; and three articles described the source of other bias.(3) All eight studies reported the incidence of VALI (I2=56%,P=0.03).The random effects model was used for the meta-analysis,and there was significant difference between PHC group and NC group (RR=0.52,95%CI:0.29-0.93,P=0.03).According to the gestational age,the eight studies were divided into two subgroups.One subgroup,including three studies with an average gestational age of 25 weeks (I2=0%,P=0.46),showed no significant difference in the incidence of VALI between PHC and NC group (RR=1.05,95%CI:0.72-1.54,P=0.78).The other subgroup,including five studies with gestational age of >27 weeks (I2=0%,P=0.68),showed significant difference in the incidence of VALI between the two groups (RR=0.27,95%CI:0.14-0.50,P<0.01).The in-hospital mortality and duration of ventilation showed significant difference between the two groups (in-hospital mortality:RR=0.40,95%CI:0.22-0.74,P<0.01; duration of ventilation:difference in means=-0.75,95%CI:-1.04--0.46,P<0.01).There was no significant difference in the incidence of BPD,IVH,PVL,PDA,NEC and neurodevelopmental impairment between the two groups (all P>0.05).Conclusions PHC ventilation in mechanically ventilated newborns can decrease the incidence of VALI,the in-hospital mortality and the duration of ventilation,while its protective efficacy against BPD is not remarkable.It does not increase the risk of IVH,PVL,PDA,NEC and neurodevelopmental injury,when the PCO2 is < 65 mmHg and pH ≥ 7.2.
2.Analysis of epidemiological characteristics of acute pesticide poisoning during 2007-2013 in Jinan, China.

OBJECTIVETo investigate the epidemiological characteristics of acute pesticide poisoning in Jinan, China and to provide effective proposals for the prevention and control of pesticide poisoning.
METHODSReported cases of acute pesticide poisoning from 2007 to 2013 in Jinan were collected and subjected to statistical analysis using SPSS 17.
RESULTSA total of 4852 cases of acute pesticide poisoning were reported from 2007 to 2013 in Jinan. The fatality rate was 14.10%. The incidence of non-occupational poisoning accounted for 94.23% (4 572/4 852). The incidence of occupational poisoning showed a decreasing trend from 2007 to 2013, with most cases poisoned by insecticides (236 cases, accounting for 82.86% of all cases of occupational poisoning). Among non-occupational poisoning cases, fatality rate was higher in cases aged 60 years or over than in those under 60 years; fatality rate was higher among male cases (16.11%) than among female cases (13.79%); herbicides had a significantly higher lethal rate (22.25%) than insecticides (14.02%) (P < 0.05).
CONCLUSIONMore attention should be paid to acute pesticide poisoning in Jinan, and effective management measures should be taken immediately.

3.Nucleic acid testing results by two different nucleic acid screening systems in blood screening: A comparative study

【Objective】 To explore the role and significance of nucleic acid testing(NAT) in blood transfusion. 【Methods】 The NAT data from March 2015 to December 2019 were collected and analyzed by inquiring the monthly analysis table of NAT-yielding results and the information management system of Qiao Blood Station (shinow9.5). The NAT capability of Kehua and Roche nucleic acid screening systems were compared. 【Results】 A total of 19 8348 samples were screened by Kehua and Roche nucleic acid screening systems, 67 reactive samples were detected, including 65 HBV DNA reactive samples, 2 HIV RNA reactive samples, and no HCV RNA reactive sample. 151 096 samples and 47 252 samples were detected by Kehua system and Roche system, with the resolution ratio at 44.55% vs 56.25% (P>0.05) and the reactive rate at 0.032% vs 0.038 % (P>0.05), respectively. The effective resolution ratio were 42.86%, 45.45%, 50%, 40% and 57.14% each year from 2015 to 2019, and the reactive rates were 0.041%, 0.042%, 0.027%, 0.021% and 0.038%, respectively. There was no statistical significance among each year by the effective resolution ratio and the reactive rate (P>0.05). The reactive yield at resolution was the highest (77.42%, 24/31) in minipool with CT <33 and the lowest(13.64%, 3/22) in minipool with CT≥40, mostly(73.13%, 49/67) remaining in CT<35. 【Conclusion】 Both Kehua and Roche screening system can detect NAT reactive samples in enzymatic non-reactive samples. The lower the CT value of mini pool, the greater the resolution probability of reactive samples. NAT can further guarantee the safety of blood transfusion.
4.Genetic Polymorphisms of 19 STR Loci in Populations of Three Culture Region in Shandong.

OBJECTIVES:
To analyse the genetic polymorphisms of 19 autosomal STR loci in Han population of east, middle-northwest and southwest-south Shandong and to explore its genetic relationships among the population of these three regions.
METHODS:
STR loci of 1 044 unrelated Han individuals in three Shandong regions were typed with a Goldeneye® DNA ID System 20A kit. The allele frequency and population genetics parameters of 19 autosomal STR loci were statistically analysed by Modified-Powerstates software. The genetic distances among the population in three regions were calculated by Arlequin v3.5 software. The phylogenetic tree was conducted using MEGA v4.0 software.
RESULTS:
Fifteen of 19 autosomal STR loci were detected with the H values greater than 0.7, PIC values greater than 0.7, and DP values greater than 0.9 in the populations of all three Shandong regions. Among the populations in these three regions, the genetic distance between the populations in middle-northwest and southwest-south Shandong was closest (Fst=0.000 16), followed by east and southwest-south Shandong (Fst=0.0003 6). The genetic distance between the populations in east and middle-northwest Shandong was the farthest (Fst=0.000 66, P<0.05).
CONCLUSIONS
The 19 autosomal STR loci show good genetic polymorphisms in Han population of three Shandong regions, and 15 of them are high. There are genetic differences between the populations in east and middle-northwest Shandong.

AIM: To compare the efficacy of pars plana vitrectomy(PPV)combined with internal limiting membrane(ILM)peeling or inverted ILM flap for the treatment of macular hole retinal detachment(MHRD)in high myopia.METHODS: A retrospective clinical study. A total of 38 cases(38 eyes)with MHRD in high myopia were treated with PPV combined with ILM peeling or PPV combined with inverted ILM flap in our hospital from January 2020 to June 2021. They were divided into control group(PPV combined with ILM peeling)and observation group(PPV combined with inverted ILM flap)according to the surgical method. Follow-up to 3mo after surgery, the surgery time, best corrected visual acuity(BCVA), the rate of macular hole closure and retinal reattachment were compared between two groups.RESULTS: The surgery time of two groups were compared and there were no statistically significant difference(30.71±4.55min vs. 35.20±5.44min, P=0.384). The BCVA in the two groups were significantly improved at the last follow-up(all P<0.01). But there was no difference in the postoperative BCVA(LogMAR)of the two groups(1.39±0.24 vs. 1.46±0.27, P=0.700). The rate of macular hole closure in the observation group was higher than that of control group(100% vs. 71%, P=0.024). There was no difference in the rate of recurrent retinal detachment of the patients in two groups(0 vs. 10%, P=0.492).CONCLUSION: Both surgeries can improve the BCVA, while PPV combined with inverted ILM flap has higher rate of macular hole closure.
7.Observation on effect of acupuncture at Yuji (LU 10) on the pulmonary function of patients with bronchial asthma and immediate efficacy of relieving asthma.

OBJECTIVETo observe the immediate efficacy difference of relieving asthma for patients with the a cute attack of bronchial asthma between acupuncture and salbutamol and make the comparison of the efficacy at different time points during the needle retaining.
METHODSFive hundred and seventy-seven cases were randomized into two groups, an acupuncture group (289 cases) and an inhalation group (288 cases). All the patients received oxygen intake and anti-infection treatment with azithromycin. At the acute attack of asthma, acupuncture was applied immediately at bilateral Yuji (LU 10) in the acupuncture group. The needle was inserted obliquely towards the palm, 20-35 mm in depth, remained for 60 min. The needle was rotated once every 5 min. In the inhalation group, the salbutamol aerosol was provided, 200 microg, less than 8 sprays in 24 h. The changes in the indices of the pulmonary function and the symptom score of TCM were observed at the acute attack, right after acupuncture 5 min, 30 min and 60 min of the needle retaining in the acupuncture group, as well as 5 min after spraying in the inhalation group separately. The differences between groups were compared.
RESULTSThe indices of the pulmonary function and the symptom scores of TCM were all improved remarkably right after acupuncture in the acupuncture group and 5 min after spraying in the inhalation group (all P < 0.05), and the results in the inhalation group were superior to those in the acupuncture group (all P < 0.05). There were no remarkably differences between the results in 5 min of the needle retaining in the acupuncture group and those immediately after acupuncture. The indices were improved remarkably after the needle retaining for over 30 min in the acupuncture group as compared with those at the moment of acupuncture (all P < 0.05), which was similar to the effect of the inhalation group (all P > 0.05). The differences in the above indices were not significant statistically in 60 min of the needle retaining in the acupuncture group as compared with those in the needle remaining for 30 min (all P > 0.05).
CONCLUSIONAcupuncture at Yuji (LU 10) acts on asthma relieving for the acute attack of bronchial asthma. It achieves the immediate effect quickly and the efficacy is the best in 30 min of needle retaining, which is equal to salbutamol aerosol.

8.Efficacy of preoperative intravitreal injection of Conbercept combined with 25G+ pars plana vitrectomy in the treatment of proliferative diabetic retinopathy

AIM: To explore the efficacy of preoperative intravitreal injection of conbercept combined with 25G+ pars plana vitrectomy(PPV)in the treatment of proliferative diabetic retinopathy(PDR).METHODS: The clinical data of 154 patients(176 eyes)with PDR admitted to our hospital from January 2019 to June 2021 were collected for retrospective analysis. According to the treatment methods, 80 patients(92 eyes)in combined treatment group were treated with preoperative intravitreal injection of conbercept combined with 25G+PPV, and 74 patients(84 eyes)in control group were given 25G+PPV only. The postoperative clinical efficacy and levels of adipokines [adiponectin(APN), retinol binding protein 4(RBP4)] before and after surgery were compared between both groups of patients.RESULTS: The combined treatment group showed better clinical efficacy than the control group at 1mo after surgery(P<0.05). Both groups had lower RBP4 levels at 3mo after surgery(P<0.05), with the combined treatment group showing a lower level than the control group(P<0.05). Serum APN levels significantly increased in both groups after surgery(P<0.05), with the combined treatment group having a higher level than the control group(P<0.05). The combined treatment group had lower incidence rates of retinal proliferation and postoperative complications after than the control group 3mo of follow-up(P<0.05).CONCLUSION: Preoperative intravitreal injection of conbercept combined with 25G+PPV is beneficial in improving the therapeutic effect of PDR and reducing the incidence rates of complications, which may be related to the regulations of the expressions of adipokines.
9.Characterization of Effective Components in Guizhi Jia Gegentang for Intervention of Influenza Virus Pneumonia Based on Serum Pharmacochemistry of Traditional Chinese Medicine

ObjectiveTo characterize the efficacy components of Guizhi Jia Gegentang(GGT) in intervening influenza virus pneumonia by ultra-performance liquid chromatography-quadrupole-electrostatic field orbitrap high resolution mass spectrometry(UPLC-Q-Exactive Orbitrap MS). MethodBALB/c mice were randomly divided into normal group and GGT group(36 g·kg-1·d-1) with six mice in each group. GGT group was continuously administered GGT extract for 5 d, while the normal group was administered an equal amount of ultrapure water. Serum and lung tissue were collected after administration, and UPLC-Q-Exactive Orbitrap MS was used to characterize the prototypical and metabolic components of GGT in serum and lung tissue of mice. The components existed simultaneously in the serum and lung tissue of mice from the GGT group were defined as its functional components, and the targets of efficacy components were searched by SwissTargetPrediction database, and GeneCards database was used to query the target of influenza virus pneumonia, and then the intersection was taken to obtain potential targets of GGT for intervening in the disease. Protein-protein interaction(PPI) network analysis of potential targets was performed by STRING database, and Kyoto Encyclopedia of Genes and Genomes(KEGG) pathway enrichment analysis on potential targets was performed by Metascape. ResultA total of 29 prototypical components and 28 metabolic components of GGT were detected in the drug-containing serum of mice, of which 11 prototypical components and 4 metabolic components were detected in the lung tissue of mice. The main metabolic pathways included reduction, hydroxylation, methylation, glucuronidation and sulfation. The results of PPI network and KEGG analysis showed that these functional components may act through their effects on targets such as albumin(ALB), epidermal growth factor receptor(EGFR), steroid receptor coactivator(SRC), Toll-like receptor 4(TLR4), nuclear transcription factor(NF)-κB and adhesion junction. ConclusionThe 11 prototypical components and 4 metabolites present simultaneously in the drug-containing serum and lung tissue of mice may be the potential therapeutic components of GGT in interfering with influenza viral pneumonia, and act through interfering with inflammatory metabolic pathways. This study can provide a reference for the mechanism study of GGT in the treatment of influenza viral pneumonia.
10.Association between social support and outcomes as successful smoking cessation in males from the rural areas.

OBJECTIVETo examine the association between social support and successful smoking cessation outcomes in males from rural areas.
METHODSA community-based case-control study was conducted with 642 adult male cases who appeared to be successful spontaneous smoking quitters as cases, together with other 700 adult males who failed quitting smoking to serve, as controls. Social Support Rating Scale (SSRS) was used to assess the individual social support. Multiple linear method was used to evaluate the relationship between social support and the outcome of successful cessation in smoking.
RESULTSAfter adjusting the potential confounders as age, education, marital status, profession, age of initial smoking and number of pack-years on smoking, the adjusted mean of subjective social support in successful quitters was significant lower than that in the failed ones (P<0.001). The differences of adjusted means between objective social support and its utility were not statistically different in the two groups among the population who recognized that cigarettes should always be provided when people interact with each other (P=0.124; P=0.763). However, the adjusted means of social support and the related three dimensions did not show significant differences in the two groups among the population in disapproval of the above said social norms (P>0.05).
CONCLUSIONData from our research indicated that social support would negatively affect the successful spontaneous cessation of smoking among people under unhealthy smoking culture.
