2.Dynamic hip screw, proximal femoral nail antirotation and InterTan nail for intertrochanteric fractures

BACKGROUND: The extramedulary fixation system including dynamic hip screw (DHS) is commonly used in treatment of Intertrochanteric fracture. However, in patients with unstable intertrochanteric fracture, extramedulary fixation system often leads to the failure of fracture fixation. Intramedulary fixation system including both proximal femoral nail antirotation (PFNA) and InterTan nail has been widely used in the treatment of unstable intertrochanteric fractures. OBJECTIVE:To compare the therapeutic effects of extramedulary fixation system containing DHS, PFNA and InterTan nail in the treatment of intertrochanteric fracture. METHODS:Literatures were searched in Wanfang, PubMed, Embase, Medline, the Cochrane library to screen literatures published from January 1990 to November 2014. Relevant studies addressing extramedulary fixation system containing DHS, PFNA and InterTan nail were screened. RESULTS AND CONCLUSION: 346 articles were screened, and 13 of them were in accordance with the inclusion criteria. 1 271 patients with different types of intertrochanteric fracture were assessed in this study. Compared to DHS group, patients treated with PFNA and InterTan nail had shorter operation time and less blood loss. No significant difference in rehabilitation time and Harris score was detected among three kinds of fixation methods. Additionaly, PFNA and InterTan nail had a similar effect. These findings verify that compared with DHS, PFNA and InterTan nail can optimize the surgery, but cannot elevate postoperative outcomes.
3.Effect of Sancao prescription extracts on lung adencarcinoma cell line(A549)

AIM: To investigate the ethanolic and aqueous extracts from Sancao prescription (Spica prunellae, Oldenlandia diffuse (willd) Roxb, Herba agrimoniae) on the proliferation of human lung adenocarcinoma cell line (A549). METHODS: 95% ,60% and 30% ethanolic extract and aqueous extract were prepared from Sancao pre-scription. The MTT assay was used to determine the inhibitory action against the proliferation of A549. RESULTS: IC_(50) of 60% ethanolic extract over A549 was one of the lowest in extracts. Combination of 60% and 90% ethanolic extract showed the synergistic antitumour activity. CONCLUSION: Ethanolic extract of Sancao prescription has and effect on human hung adenocarcinoma(A549).
4.Near-infrared light combined with tacrolimus ointment for the treatment of facial glucocorticoid-dependent dermatitis: a randomized controlled trial

Objective To evaluate the efficacy and safety of near-infrared light combined with tacrolimus 0.1% ointment in the treatment of facial glucocorticoid-dependent dermatitis.Methods A total of 68 patients with facial glucocorticoid-dependent dermatitis were enrolled from Department of Dermatology of Yantai Yuhuangding Hospital between December 2014 and December 2015,and randomly and equally divided into treatment group and combination group by a random number table.The treatment group was treated topically with tacrolimus ointment twice a day for 4 weeks.Besides the treatment with tacrolimus ointment,the combination group was irradiated with near-infrared light once a week for 4 sessions.After 4-week treatment,improvement in clinical manifestations such as itching and burning sensation was evaluated,so was the therapeutic effect.Results The combination group showed significantly higher response rate (85.3% [29/34]) compared with the treatment group (61.8% [21/34],x2 =4.84,P < 0.05).Additionally,response rates for itching and burning sensation,erythema,scales and papules were all significantly higher in the combination group than in the treatment group (x2 =4.84,6.35,8.42 and 5.52,respectively,all P < 0.05).Conclusion Near-infrared light combined with tacrolimus ointment is effective and safe for the treatment of facial glucocorticoid-dependent dermatitis.
5.Determination of Anticancer Activity Components of Flavonoids from Hedyotis diffusa by HPLC

OBJECTIVE:To establish the method for the simultaneous determination of anticancer activity components of flavonoids from Hedyotis diffusa,i.e. quercetin,kaempferol. METHODS:HPLC was applied to determine the contents and performed on Alltima C18(250 mm?4.6 mm,5 ?m) column. Mobile phase consisted of methanol(A)-0.5% glacial acetic acid,(gradient elution). The detection wavelength was aet at 350 nm. RESULTS:The linear range of quercetin was 0.006 2~0.244 0 ?g(r=0.999 8)and that of kaempferol 0.007 8~0.310 6 ?g(r=0.999 9). The average recovery of quercetin was 101.84%(RSD=1.79%,n=6) and that of kaempferol 99.04%(RSD=2.90%,n=6). CONCLUSIONS:The method is simple,accurate and reproducible for the quality control of H. diffusa.
6.Mechanism of Chromium Accumulation by Fused Yeast

The mechanism of chromium accumulation and microstructure transformation of the fused yeast were studied in this paper.The result showed that the process of Cr6+ reduction and adsorption was accom-panied by the H+ consumption.The main adsorptive groups on the strain surface included amino,hydroxyl,phosphate group and amide,among which phosphate group played vital role in the chromium accumulation.The removal rate of chromium and reduction rate of hexavalent chromium declined 70% and 46%,respec-tively,when phosphate group was masked.During the adsorption process the chromium ions complexed on the surface of fused yeast was transported into the cell wall and combined with inclusion to form steady spe-cies and this took 90 min to reach the equilibrium.The biosorption and reduction of Cr on the cell surface would alter microstructure of cell surface,reduce cell membrane potential and increase cell membrane per-meability.
7.Total thyroidectomy for bilateral multinodular goiter

Objective To evaluate the safety and rationality of total/near total bilateral thyroidectomy(TBT) for patients with bilateral multinodular goiter(BMG). Methods From January 2003 to December 2006,311 BMG cases were preoperatively divided into two groups, 130 cases in group A underwent TBT, and 181 cases in group B were treated with subtotal/partial bilateral thyroidectomy. Results There were 6 and 2 eases in group A and group B respectively diagnosed by intraoperative frozen biopsy as BMG, but identified as papillary carcinoma by final pathology. Hence the 6 cases in group A avoided reoporation, while the 2 cases in group B underwent a resection of the remnant gland. Transient hoarseness developed in 3 (2.42%, 3/124) and 3 (1.68%, 3/179) eases in group A and group B respectively (P =0.48). Transient hypocalcemia developed in 11 (8.87% ,11/124) and 9(5.03% ,9/179) cases in group A and group B respectively(P =0.16). There was no postoperative goiter recurrence in group A, but recurrence developed in 12 cases (6.70%,12/179) in group B(P=0.02). Conclusions Total bilateral thyroidectomy is safe and rational for the management of bilateral thyroid goiter.
8.Research Progress in Acupuncture Treatmentfor Male Infertility

The incidence of male infertility increased year by year, which seriously affects family welfare and social development. This article reviews and sorts out recent years’ studies on acupuncture treatment for male infertility. The results show that acupuncture treatment has produced a good clinical effect on this disease. Some progresses have been made in clinical acupuncture point selection, operating methods, complex treatment and therapeutic mechanism. The clinical effect and the mechanism of action of acupuncture treatment for this disease have been deeply explored from symptom differentiation and classification, acupoint selection and prescription, acupoint stimulation mode, and combined acupuncture and moxibustion therapy so as to provide new ideas for clinical acupuncture treatment and basic study.
9.Antimicrobial resistant mechanisms of carbapenem-resistant Klebsiella pneumonia infection in newborns

Objective To investigate the antimicrobial resistant and transmission mechanisms of carbapenem-resistant K.pneumonia (CR-KP) infection of newborns.Methods A retrospective study was conducted on totally 37 non-repetitive CR-KP which were isolated from patients hospitalized between April 2011 and October 2013.Resistance genes were identified by PCR and sequencing.Plasmid was analyzed by pulsed-field gel electrophoresis (PFGE).Conjugation experiments were performed to determine the transferability of beta-lactamase.Multilocus sequence typing (MLST) was used to determine the genotypes and homology of these isolates.Out-membrane proteins were examined by PCR and sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE).Results Thirty-seven CR-KP isolates were tested.The resistant rates of imipenem, meropenem, ertapenem were 89.2% (33/37), 83.8% (31/37) ,97.3% (36/ 37), respectively.All the 37 CR-KP exhibited 100% (37/37) sensitivity to tigecycline, colistin, levofloxacin and amikacin, while resistance to most of the other antibiotics.By PCR, 67.6% (25/37) isolates were blaNDM-1 positive, 35.1% (13/37) isolates were blaIMP-4 positive and 2.7% (1/37) isolate were blaIMP-8 positive, including two isolates carrying both blaNDM-1 and blaIMP-4.PFGE results showed that the isolates carried 2-4 plasmids and both blaNDM-1 and blaIMP-4 were transferable by plasmids.MLST assigned them to sequence type (ST) 20, ST17, ST54, ST705, ST290,which showed that there were infectious outbreaks caused by NDM-1-producing and IMP-4-producing respectively among newborns.SDS-PAGE result indicated that there was no absence of outer membrane proteins OmpK35 and OmpK36.Conclusions The main resistant mechanisms of CR-KP causing infection in newborns were those the isolates carried carbapenemase of blaNDM-1 or blaIMP-4 and the K.pneumonia with two kinds of carbapemenase were detected.
10.Postoperative complications of complex proximal humeral fractures after treated with proximal humeral internal locking system plate fixation

BACKGROUND:Proximal humeral internal locking system fixation for complex humeral fractures via deltoid splitting approach provides good clinical results, but certain complications stil existed.
OBJECTIVE:To explore the postoperative complications and the related risk factors for displaced three-part and four-part fractures of proximal humerus treated with proximal humeral internal locking system fixation via deltoid-splitting approach, and to propose the corresponding countermeasures.
METHODS:106 cases with displaced three-part and four-part fractures of proximal humerus were retrospectively analyzed. The relationship between postoperative complications and the related risk factors was analyzed with Logistic regression analysis.
RESULTS AND CONCLUSION:A total of 81 patients were fol owed-up for 12 to 30 months. The mean Constant score at 12 months after operation was (76.57±4.70) points. The postoperative complications occurred in 31 patients (38.3%) of which impingement syndrome involved in 16 cases (19.8%), head-shaft angle loss in six cases (7.4%), head-shaft angle loss combined with screws cut-out in two cases (2.5%), pure screws cut-out in two cases (2.5%), humeral head necrosis in two cases (2.5%), fat liquefaction in five cases (6.2%). Single factor analysis showed that there were significant differences in the superiorly located greater tuberosity, superiorly located plate and Neer classification between impingement group and un-impinged group (P<0.05). There were statistical y significant differences in age, postoperative medial cortical defects and Neer classification between head-shaft angle loss group and un-loss group (P<0.05). By means of logistic regression analysis, the superiorly located greater tuberosity, superiorly located plate and Neer classification were the individual predictors for postoperative impingement syndrome;postoperative medial cortical defect and Neer classification were the individual predictors for postoperative head-shaft angle loss.
