1.Preliminary study on the effects of an artificial liver support system in the treatment of medicamentous liver insufficiency.

OBJECTIVESTo study the effectiveness of an artificial liver support system.
METHODSThirty-two patients with medicamentous liver insufficiency were treated with an artificial liver support system in addition to the routine medicinal therapy. Thirty patients treated with routine medicinal therapy only served as controls.
RESULTSThe clinical symptoms (e.g. hepatic encephalopathy) and the laboratory indices (serum total bilirubin and prothrombin time) of the treatment group patients were obviously improved compared with those of the control group patients (P < 0.05). The cure rate and hospitalization days were 90.6% (26/32) and 47 days respectively in the treatment group, and 43.3% (13/30) and 72 days in the control group (P < 0.05).
CONCLUSIONUsing an artificial liver support system combined with routine medicinal therapy is more effective than using medication alone.

2.Quality evaluation and stability investigation of asarone submicro emulsion injection.

The content of the asarone submicro emulsion injection was determind by HPLC method, and thereby a quality evaluation method was established based on indexes of pH value, particle size, peroxide value, methoxy aniline values, free fatty acid, lysophosphatidylcholine, visible foreign substances, insoluble particle, sterility, bacterial endotoxin and impurities, etc. The results showed that the injection exhibited uniform physical appearance and all the products were in milkwhite liquid. The content of the three batches products were respectively 102.9%, 100.8%, 97.70% of the labeled amount, with mean particle size of 210-250 nm, and other indexes all met with the standards. The reserved samples showed no obvious change in terms of detection indexes and indicated good stability after the accelerated stability test and long-term stability for 12 months. The quality evaluation method established in this study could be applied to quality control and stability investigation of asarone submicron emulsion injection, which laid a basis for further clinical research and application.

3.Impact of directly sequenced core and non-structrural protein 5B regions on hepatitis C virus genotyping

Objective To explore the sensitivity and accuracy of directly sequenced core and non-structrural protein (NS)5B regions for hepatitis C virus (HCV)genotyping.Methods Fifty-one serum samples from chronic hepatitis C patients were collected in the study.Reverse transcription-polymerase chain reaction was used to amplify core and NS5B regions.Genotypes or subtypes were determined by the phylogenetic analysis of directly sequenced core and NS5B regions.Results Among the 51 samples,49 (96.1 %)were successfully typed by phylogenetic analysis of directly sequenced core region.There were overall five genotypes determined in the area,including 1b (61 .2%,30/49 ),2a (20.4%,10/49 ),2b (2.0%,1/49),3a (4.1 %,2/49 )and 6a (12.2%,6/49 ).The positive rate of HCV genotying was 88.2% (45/51 )on the basis of NS5B region.HCV genotypes 1b,2a,2b,3a and 6a were found in 62.2% (28/45),20.0% (9/45 ),2.2% (1/45 ),4.4% (2/45 )and 11 .1 % (5/45 )of the patients, respectively.Conclusion The HCV genotyping based on core regions,compared with that based on NS5B,shows the advantages of primer design,amplification efficiency and accuracy,suggesting that it has the priority to be used in the epidemiological and clinical study of HCV genotyping.
4.Causes and prevention strategies of crisis events in neurosurgery nursing

Objective To analyze the causes of crisis events in neurosurgery nursing, put forward prevention strategies so as to avoid nursing risks and ensure nursing safety. Methods We analyzed 38initiatively reported nursing crisis events in neurosurgery nursing and look for their causes. Results The factors,which caused the nursing crisis events, consisted of diseases (31.59%) , nursing staff (31.59%) , patients (23.68), environments (5.26%) and drugs (7.89%). Conclusions Crisis events in neurosurgery nursing are caused by multiple factors. A series of nursing prevention measures, such as establishing the warning assessment system, improving visibility, allocating nurses dynamically, maintaining environmental safety,paying attention to training and standardizing material regulation, should be taken to control the occurrence of the crisis events.
5.Investigation on the status of neurosurgery nurses in Beijing

Objective To investigate the constitution and status of neurosurgery nurses in Beijing,and to provide evidence for the standardization of the hierarchical use of neurosurgery nurses.Methods A serf-designed questionnaire based on characteristic of neurosurgery nursing work was used to investigate 144 neurosurgery nurses' status and their working content.The data Were allalyzed with software SPSS 13.0.Results 82.64%of the nurses had a primary title,and 76.38%of them had a technical secondary school or junior college education background,40.28%of the nurses had been worked for less than 5 years;Nurses with different education backgrounds.title or length of service had no significant difference in"routine work"(P>0.05);There were significant differences between nurses in"professional work",such as"writing nursing document"、"rehabilitation training","operating precision iustnment",etc,and in guiding clinical practice(P<0.05).Conclusions Hidden danger exists in neurosurgery nursing work. All nurses are participating in basal nursing. Nurses with considerable working experience,professional level and age bracket become the main force in specialized nursing,teaching and nursing research.
6.Investigation on neurosurgical nurses' consent to health education and its influential factor

Objective To investigate on neurosurgical nurses' consent to health education and its influential factor , in order to adopt eligible healthy education and decline the influential factors which affect the effectiveness of healthy education. Methods Ninety neurosurgical nurses in grade three A hospital were investigated with self-designed questionnaire. Results 74.44%~82. 22% nurses ratified face to face or using instruction handbook to do the healthy education for neurosurgical patients. There was different attitude between higher seniority and lower seniority nurses (x2=9. 61,P=0.002) for health education, and between nurses with different educational background(x2=8.773, P=0.003.87.78% nurses thought that deficiency of the nursing human resources was the main factor which affected the effectiveness of health education. Conclusions From nurses' point of view, there is some difficulty to carry out health instruction for patients in neurosurgery department. It is more suitable to adopt face to face instruction and select materials with the characteristics of repeated reading or convenient for looking up. Insufficience of nursing human resources is still the main factor which affects the effectiveness of health education.

8.Mutation analysis of the HBV reverse transcriptase in nucleos(t)ide-treated patients with chronic HBV infection

Objective To characterize genotypic resistance within HBV RT region in chronic hepatitis B (CHB) patients with nucleos(t) ide analogue (NA) treatment.Methods Serum samples of 229 CHB patients with NA treatment were obtained.Full-length HBV RT sequences were amplified,sequenced and analyzed,on the following NA resistant (NAr) mutations belonging to different NAr pathways.Results Among 229 HBV isolates,14.41% (33/229) and 85.59% (196/229) were genotype B and C,respectively;and the patients with HBV genotype C may be more susceptible to develope resistant mutations than patients with HBV genotype B (x2 =2.95,P < 0.05).NAr mutations were detected in 63 CHB patients.Mutations were not found at rtI169,rtT184,rtA194 or rtS202.RtM204 mutations were detected at the highest frequency among 63 mutants (40/63,63.49%) and found to display 11 combination mutation patterns,in which rtM204I were associated with rtL80I/V and rtL180M,and rtM204V were associated with rtL180M,respectively.Conclusions There are complicated mutation patterns in the HBV RT region for chronic hepatitis B (CHB) patients with nucleos(t) ide analogue (NA) treatment.RtM204V/I mutation was the highest.
9.Clinical and pathological characteristics of septum pellucidum tumor and choice of surgical approaches for its resection.

BACKGROUNDTumor involving the septum pellucidum is uncommon. Surgery as the main therapeutic procedure for this lesion is a challenge to neurosurgeons. We analyzed the clinical characteristics and pathological features of septum pellucidum tumor in 41 patients and compared the curative effects of frontal transcortical, trans-sulcal and interhemispheric transcallosal approaches.
METHODSClinical characteristics and the pathological features of septum pellucidum tumor were investigated retrospectively in 41 patients. The differences in postoperative residual rates, extents of tumors and resection of normal brain tissues after use of the three approaches in these patients were analyzed statistically.
RESULTSSeptum pellucidum tumor is more likely to attack young or middle-aged persons. The tumor mainly presents itself as a central neurocytoma or cerebral low-grade glioma in pathology and manifests as intracranial hypertension clinically. No difference was found in the extent of tumor resection but significant difference in the extent of normal brain tissue resection and in postoperative disability rate among the three approaches. The transcortical approach brought about the most serious injury to brain tissue and the highest disability rate, Whereas the frontal transcallosal approach the lightest injury and the lowest disability rate. The injury to brain tissue and the disability rate brought about by the front trans-sulcus approach were between the above two approaches.
CONCLUSIONSOperation is still regarded the major treatment for septum pellucidum tumor. Transcallosal and trans-sulcus approaches are fit with the concept of minimally invasive surgery, and transcallosal approach is the first choice for septum pellucidum tumor.

10.VEGF-induced tubulogenesis of endothelial cells from human brain malignant glioma in the three dimentional model.

OBJECTIVETo compare the tubulogenesis capability of malignant glioma-derived microvessel endothelial cells (GDMEC) from human brain with that of ECV304 cells in a three dimentional model and to explore the significance of GDMEC in the study on angiogenesis.
METHODSThe GDMEC were isolated from malignant gliomas of human brain and purified by selective binding to the monoclonal antibody against CD105 bound to the magnetic MACS MicroBeads. GDMEC and endothelial-like cell line ECV304 were compared with their capabilities of formatting tubule-like structure (TLS) in the three dimentional collagen matrix, with or without inducement by various concentration of vascular endothelial growth factor (VEGF).
RESULTSThe obtained GDMEC had a high purification (98%) and could be successfully cultured in vitro. GDMECs formed more TLS than ECV304 cells of the same number and at the same time points. VEGF could induce rapid formation of TLS in a dose-dependent manner, however, ECV304 cells were less response to VEGF stimulation.
CONCLUSIONSGDMEC could maintain their endothelial characteristics and potential capability of angiogenesis. They were more response to VEGF than ECV304, therefore, more suitable for in vitro studies on tumor angiogenesis.
