1.Acute aggravation of chronic tinnitus: a study of the hearing characteristic and prognosis.

OBJECTIVETo investigate the hearing change when chronic tinnitus acute aggravated and the affect of prognosis due to hearing change.
METHODSThe pure tone threshold and acoustic immittance were used for every case in the acute aggravation of chronic tinnitus (AACT) group, and for some of AACT group members the auditory brainstem response (ABR), distort-product otoacoustic emission (DPOAE), electrocochleogram (EcochG), glycerin test and MRI were used at the same time for further diagnosis. For the chronic tinnitus, the patients accepted the intervention of tinnitus retraining therapy (TRT) both for the AACT group and the control group. As for the recent hearing loss appeared in the AACT group members, the patients were treated the cases as the sudden deafness, and then the hearing characteristic of the AACT group was analyzed and the effect of the chronic tinnitus both for the AACT group and control group after the TRT for 3, 6, 9 and 12 months was evaluated.
RESULTSThree kinds of hearing changes appeared when chronic tinnitus acute aggravated. Based on the dated high frequencies hearing loss ranged 4-8 kHz, recent hearing loss appeared in 1-2 middle frequencies near the impaired high frequencies; Based on the dated single middle frequency hearing loss, another middle frequency hearing impaired near the dated hearing loss frequency, the sawtooth-like audiogram changed to groove-like audiogram; Hearing fluctuation in low frequency area, hearing threshold increased 10-30 dB in 125-1000 Hz but kept unchanged in the high frequency area. The recent local frequency hearing loss healed after the vasodilator and neurotrophic drugs therapy, but the dated hearing loss can't rehabilitate. Along with the rehabilitation of recent local hearing loss, 25 of 32 cases chronic tinnitus victims get to habituation; this finding showed that the experience of tinnitus aggravation and relief accelerated the habituation. Until the 12 months after the TRT, the curative effect difference between AACT group and control group showed statistical significance.
CONCLUSIONSWhen the chronic tinnitus acute aggravated, 3 kinds of local area hearing impaired. Timely diagnosis and effective treatment had brought not only hearing rehabilitation but also promotion to habituation of chronic tinnitus.

2.Clinical features of auditory neuropathy under pure tone audiometry and acoustic immitance examination

Objective To analyze the clinical characteristics of auditory neuropathy under pure tone audiometry and acoustic immitance examination.Methods Seventeen patients (32 ears) diagnosed as having auditory neuropathy were examined for audiology features by pure tone audiometry and acoustic immitance.Results Bilateral and symmetrical hearing loss was found in 15 patients and low-frequency sensorineural hearing loss was noted in 26 ears.Seventeen ears with a course of disease less than 5 years presented light or moderate dysaudia,and those with more than 5 years presented grave loss of aural comprehension.16 patients (31 ears) in the acoustic immitance examination showed type A tympanogram,and absence of stapedius muscle reflex was found in 15 patients (30 ears).The threshold of acoustic stapedius reflex increased in 2 patients (2 ears).Conclusion Auditory neuropathy primarily presents bilateral and symmetrical hearing loss,low-frequency hearing loss at the initial stage and total-frequency hearing loss finally,absence of stapedius muscle reflex and type A tympanogram,and absence of loudness recruitment phenomenon.
3.Clinical analysis of coronary artery disease in elderly patients with sleep disordered breathing.

OBJECTIVETo understand the prevalence of sleep disordered breathing (SDB) in elderly patients with coronary artery disease (CAD) and explore the relations between SDB and CAD.
METHODSSixty-two elderly patients with and 18 without CAD identified by coronary angiography underwent examinations by polysomnography (PSG). Left ventricular ejection fraction (LVEF) was measured by 99Tc equilibrium radionuclide angiography.
RESULTSIn the 62 elderly patients with CAD, 53.2% had SDB, a rate significantly higher that (22.2%) in the 18 non-CAD patients. The CAD patients with SDB had higher respiratory disturbance index (RDI) and body mass index (BMI) and lower arterial saturation of oxygen (SaO2) during sleep, with longer duration of low SPO2 (less that 90%). The incidence of hypertension was higher in CAD patients with SDB than in those without SDB. No significant correlation was found between the severity of coronary artery disease and RDI (r=-0.16, P>0.05).
CONCLUSIONThe elderly patients with CAD have higher incidence of SDB, and appropriate interventions should be administered in those with severe SDB.

4.Relationship between Laterality of Auditory Brainstem Response and Speech Comprehension in Children with Autism Spectrum Disorders : A Preliminary Retrospective Controlled Study

[Objective] To explore the relationship between laterality of auditory brainstem response and speech comprehension in children with autism spectrum disorders preliminarily.[Method] Review hearing diagnosis data in clinical audiology center from September 2013 to August 2015.We compare auditory brainstem responses in children with autism spectrum disorders (ASD group) and with other disorders (control group),and we further divide children with autism spectrum disorders into two groups (high ability group and low ability group) in terms of their speech comprehension and compare their auditory brainstem responses.[Result] The latencies of waves Ⅲ and Ⅴ,and interpeak latencies Ⅰ to Ⅲ and Ⅰ to Ⅴ of the ASD were significantly prolonged than the control group.And the laterality of waves Ⅲ and Ⅴ,and interpeak latencies Ⅰ to Ⅲ and Ⅰ to Ⅴ present in the ASD group,but not in the control group.Meanwhile,the laterality of waves Ⅲ and Ⅴ,and interpeak latencies Ⅰ to Ⅲ and Ⅰ to Ⅴ present in the high ability group,but not in the low ability group.[Conclusion] Laterality was not a common phenomenon in auditory brainstem response,and it may be associated with the development of speech comprehension in children with autism spectrum disorders.
5.Clinical significance of common leukemia gene mutations in patients with acute promyelocytic leukemia.

This study was aimed to explore whether multiple common gene mutations of leukemia synergistically involved in acute promyelocytic leukemia (APL) pathogenesis, and to investigate their relevance to clinical features, cytogenetics and molecular risk stratification. 84 specimens of admitted de novo APL patients from February 2005 to October 2010 were collected, the gene mutations of bone marrow mononuclear cells and clinical features of mutation-positive patients were analyzed by genomic DNA-PCR. The results indicated that the prevalence of mutations was 60.7% (51/84), in which the mutations with the highest incidence were found as FLT3-ITD, reaching 27.4% (23/84). Next, there were 12 cases WT1 mutation, 9 for FLT3-TKD, 7 for TET2, 5 for N-RAS, 4 for ASXL1, 2 for EZH2 mutation and 1 positive case in MLL-PTD, IDH1 and CBL mutation respectively. No mutation was found in other JAK1, DNMT3, c-Kit, NPM1, IDH2, RUNX1 and JAK2 (V617F) common leukemia-related genes. Combined analysis with clinical data demonstrated that the patients with FLT3-ITD mutation displayed higher white blood cell counts, while the patients with N-RAS mutation showed lower platelet counts. Overall survival of these patients was obviously shorten as compared with patients with wild-type. This difference between mutant and wild-type of all above mentioned cases was statistically significant (P < 0.05). The difference between APL with simple t (15;17) and additional abnormal karyotype was not statistically significant. It is concluded that the FLT3-ITD mutation is recurrent genetic change in APL, and together with N-RAS mutation indicates poor prognosis. Additional abnormal karyotype does not associate with prognosis of APL.

6.Effectiveness and Safety of a Novel Approach for Management of Patients with Potential Difficult Mask Ventilation and Tracheal Intubation: A Multi-center Randomized Trial.

Background:Patients with potential difficult mask ventilation (DV) and difficult intubation (DI) are often managed with awake intubation, which can be stressful for patients and anesthesiologists. This prospective randomized study evaluated a new approach, fast difficult airway evaluation (FDAE). We hypothesized that the FDAE approach would reduce the need for awake intubation.
Methods:After obtaining informed consent, 302 patients with potential DV/DI undergoing elective surgeries were randomly assigned to the FDAE group (Group E) and the control group (Group C). In Group E, patients were gradually sedated, and adequacy of manual mask ventilation during spontaneous breathing was assessed at various sedation levels. Awake intubation was applied in those with inadequate mask ventilation. In Group C, DI was evaluated under local anesthesia. However, the care team could intubate under general anesthesia if the vocal cords were visible. The primary outcome was the rate of awake intubations in both groups and the induction efficiency assessed by the induction time. The secondary outcome was the incidence of serious complications.
ResultsThe rate of awake intubation was significantly lower in Group E than that in Group C (5.81% vs. 36.05%, χ = 42.3, P < 0.001). The induction time was much shorter in Group E than in Group C (11.85 ± 4.82 min vs. 18.71 ± 7.85 min, t = 5.39, P < 0.001). There was no significant difference in the incidence of intubation related complications between the two groups. Patients in Group E had a much lower incidence of recall (9.68% vs. 44.90%, χ = 47.68, P < 0.001) of the induction process and higher satisfaction levels than patients in Group C (t = 15.36, P < 0.001).
ConclusionsThe FDAE significantly reduces the need for awake intubation and improves the efficiency of the intubation process without comprising safety in patients with potential difficult mask ventilation and DI.
